Abstract
The invention concerns an orthosis comprising a compressive tubular part (10), made of mesh or fabric, with variable cross-section shaped to suit the profile of a lower limb, and whereof the structure of the mesh or fabric is selected, on the basis of the perimeter dimensions of the lower limb, so as to apply thereon a substantially constant immobilising pressure over the height of the limb, of the order of 30 to 40 mm Hg. The tubular part extends from the foot to the root of the thigh and comprises a highly elastic extensible internal region (12), linked along two generatrices to an inextensible or hardly extensible external region (14). In the upper part is also provided a thigh sleeve (22) open along a generatrix and provided with adjustable closure means (24).

La première intervention est le "stripping", qui est un arrachement sur fil de l'axe variqueux de la saphène interne ou externe. Elle est source de délabrements importants des branches collatérales de la saphène qui, par conséquent, vont saigner en provoquant des hématomes, sources de douleurs et d'inconfort pour les patients dans les heures et les jours qui suivent la chirurgie. L'autre technique couramment utilisée est la phlébectomie ambulatoire, qui est un geste chirurgical moins traumatisant que le stripping (mais qui peut aussi être réalisée dans le cadre d'un stripping). Cette technique réalise une exérèse minutieuse des veines superficielles sous anesthésie locale en ambulatoire. Les hématomes sont moins importants que dans le cas du stripping, mais les risques de saignement immédiat existent quand le patient se lève - risques liés à la méthode ambulatoire. Dans l'un et l'autre cas, pour réduire les saignements et atténuer leurs conséquences, le chirurgien applique immédiatement après l'intervention une compression très forte sur le membre opéré, pour limiter les saignements et l'inflammation post-opératoire. D'autres méthodes de chirurgie vasculaire provoquent des désagréments de même nature, par exemple les méthodes dites de "chirurgie conservatrice" dérivée de la "cure CHIVA" (cure Conservatrice Hémodynamique de l'Insuffisance Veineuse en Ambulatoire). L'un des buts de l'invention est de proposer une orthèse qui, mise en pla- ce immédiatement après la fin de l'opération, procure une compression contrôlée, régulière sur tout le membre inférieur, non dépendante de l'opérateur et suffisamment forte pour limiter les saignements et l'inflammation post-opératoire. Le manchon cuisse 22 est alors refermé dans la position voulue, par exemple au moyen de fermetures à bandes agrippantes 24 qui permettent d'en réajuster aisément le serrage, si nécessaire. La figure 8 illustre le profil de pression de contention - sensiblement cons- tant - procuré par l'orthèse de l'invention (en A), par rapport à celui - dégressif - procuré par un bas de contention classique (en B). Les profils en tiretés correspondent aux profils idéaux, tandis que les profils en trait plein illustrent les relevés réels. Ces derniers tiennent comptent, d'une part, de la morphologie du genou qui empêche localement l'application d'une pression régulière et, d'autre part, du fait qu'il est difficile de maintenir la pression en haut de cuisse en raison du diamètre important en cet région (loi de Laplace).

Claims
1.
