La présente invention concerne l'utilisation d'un vecteur comprenant un acide nucléique codant pour un facteur angiogénique pour la prévention, l'amélioration et/ou le traitement de l'hypertension artérielle pulmonaire.
Description
UTILISATION D'ADENOVIRUS RECOMBINANT DEFECTIF COMPRENANT UN ACIDE NUCLEIQUE CODANT POUR UN FACTEUR ANGIOGENIQUE POUR LE TRAITEMENT DE L'HYPERTENSION ARTERIELLE PULMONAIRE
La présente invention concerne l'utilisation d'un vecteur comprenant un acide nucléique codant pour un facteur angiogénique pour la prévention, l'amélioration et/ou le traitement de l'hypertension artérielle pulmonaire. Elle concerne également des compositions pharmaceutiques particulières permettant l'administration locale et efficace de ces vecteurs.
L'hypertension artérielle pulmonaire (HTAP) est une pathologie fréquente, fatale dans ses formes graves, actuellement dépourvue de traitement en dehors de la transplantation.
Elle est caractérisée par une élévation des résistances artérielles pulmonaires faisant obstacle à l'éjection du ventricule droit et compromettant le débit cardiaque. Plusieurs anomalies fonctionnelles et structurales de la paroi vasculaire pulmonaire participent au développement de l'HTAP incluant : une hypeφlasie des cellules musculaires lisses avec hypertrophie médiale et intimale, une accumulation de la matrice extra cellulaire, une raréfaction vasculaire avec réduction de la densité capillaire périphérique.
L'invention fournit pour la première fois une méthode efficace pour le traitement de l'hypertension artérielle pulmonaire. Cette méthode est basée sur l'utilisation de vecteurs comportant un acide nucléique codant pour un facteur angiogénique.
L'implication de différents facteurs angiogéniques tels que les facteurs de croissance du fibroblaste (FGF) et le facteur de croissance de l'endothélium vasculaire (VEGF) dans le développement de l'hypertension artérielle pulmonaire a été étudiée cependant, à ce jour, le rôle de ces facteurs n'a pu être élucidé.
Le premier facteur de croissance sélectif des cellules endothéliales, VEGF. a été identifié en 1989. Les études réalisées depuis ont démontré l'importance de VEGF dans les processus angiogéniques normaux et pathologiques. C'est un facteur angiogénique puissant dans la cornée de lapin et la membrane chorioallantoidienne de ooulet, deux modèles classiques d'angiogénèse in vivo. VEGF est également connu comme facteur de perméabilité vasculaire. C'est une glycoprotéine homodimérique de 34-36 kDa, liant l'héparine, et dont la structure comporte un peptide signal lui permettant d'être sécrétée. Le gène codant pour VEGF est composé de 8 exons. Quatre formes de peptides sont générées par épissage alternatif. Elles comprennent respectivement 121,165, 189 et 206 acides aminés chez l'homme.
Chez l'adulte, VEGF est exprimé dans de nombreux tissus normaux, en particulier le coeur et les poumons. Cette expression n'est pas forcément associée à une angiogénèse importante. Les différentes formes de VEGF reconnaissent deux récepteurs à activité tyrosine kinase de la famille fms : le récepteur Flt-1 et le récepteur Flk-1. Le récepteur Flt-1 (VEGFR)-l est un récepteur de haute affinité (10 pM). Le récepteur KDR ou flk-1 ou (VEGFR)-2 est un récepteur de basse affinité (750 pM) qui serait responsable des effets mitogènes du peptide. Un des aspects les plus remarquables de la régulation de l'expression de VEGF est sa sensibilité à des conditions hypoxiques. Il a été montré que des périodes relativement brèves d'hypoxie stimulent l'expression de VEGF par des cellules en culture in vitro, notamment les cardiomyocytes et les cellules musculaires lisses. Cependant le rôle de ce facteur dans le développement ou la prévention de l'hypertension artérielle pulmonaire n'est pas connu.
La famille des facteurs de croissance de l'endothélium vasculaire comprend également d'autres molécules utilisables dans la présente invention comme le PIGF (Placenta Growth Factor), le VEGF-B, le VEGF-C, le VEGF-D et le VEGF-E. Le VEGF et le VEGF-B sont , au sein de la famille des facteurs de croissance de l'endothélium vasculaire, les formes plus particulièrement préférées dans le cadre de l'invention.
Le VEGF-B est produit dans de nombreux tissus adultes, en particulier le coeur, le muscle squelettique et le pancréas. Deux formes de peptides sont générées par épissage alternatif et comprennent respectivement 167 et 186 acides aminés. Le
VEGF-B stimule la prolifération des cellules endothéliales mais il ne se lie pas au récepteur VEGFR-2.
La famille des facteurs de croissance du fibroblaste (FGF) comprend de nombreux représentants et, à ce jour, au moins 14 membres ont été identifiés (pour une revue voir Birnbaum et al. Médecine et Science 13, p 392 - 396, 1997). Bien que les formes de FGF-1 et FGF-2 soient exprimées dans les cellules de l'épithélium pulmonaire et au niveau des cellules vasculaires dans le poumon, aucune information n'est disponible sur l'expression de ces facteurs en relation avec l'hypertension artérielle pulmonaire, ni sur la capacité de ces facteurs à induire la prolifération des cellules endothéliales et les cellules musculaires lisses au niveau du poumon, ni sur l'expression de ces facteurs au niveau des cellules épithéliales bronchiales ou alvéolaires en relation avec différentes conditions environnementales et notamment les conditions d'hypoxie.
De manière inattendue, la demanderesse a maintenant mis en évidence que le transfert au niveau du poumon d'un acide nucléique codant pour un facteur angiogénique permet de réduire la pression artérielle pulmonaire et de prévenir l'hypertrophie ventriculaire droite associée à l'hypertension artérielle pulmonaire avec une efficacité jamais égalée jusqu'alors.
Pour étudier l'effet des facteurs angiogéniques dans la prévention et le traitement de l'hypertension artérielle pulmonaire hypoxique, la demanderesse a utilisé comme modèle, des rats en hypoxie chronique. Le transfert d'acides nucléiques codant pour des facteurs angiogéniques a été réalisé au moyen de vecteurs recombinants de type adenovirus administrés par instillation intratrachéale. Les résultats obtenus montrent que l'expression au niveau du poumon de facteurs angiogéniques tels que notamment le VEGF-B ou le FGF-1 permet de réduire la
pression-artérielle pulmonaire, de prévenir l'hypertrophie ventriculaire droite et le remodelage vasculaire pulmonaire. L'invention fournit ainsi, pour la première fois, une méthode de traitement efficace de l'hypertension artérielle pulmonaire.
Parmi les différents facteurs angiogéniques utilisables dans le cadre de la présente invention, on peut citer notamment : les membres de la famille des facteurs de croissance du fibroblaste (FGF), et plus particulièrement le FGF-1, FGF-2, FGF-4, et FGF-5, les facteurs de croissance de l'endothélium vasculaire et plus particulièrement le VEGF, le VEGF-B, le VEGF-C, le VEGF-D, le VEGF-D et le P.I.G.F (Placenta Growth Factor) et les facteurs de type angiopoiétine (angiopoiétine 1 et angiopoiétine 2).
Un premier objet de l'invention concerne l'utilisation d'un vecteur comprenant un acide nucléique codant pour un facteur angiogénique pour la préparation d'une composition pharmaceutique destinée à la prévention, à l'amélioration et/ou au traitement de l'hypertension artérielle pulmonaire.
Préférentiellement, le facteur angiogénique est un facteur de croissance des cellules endothéliales choisi parmi la famille du FGF ou du VEGF ou de l' angiopoiétine, ou une combinaison d'au moins deux facteurs choisis parmi au moins une de ces familles. A titre d'exemple de combinaisons avantageuses de facteurs angiogéniques on peut citer notamment la combinaison associant au moins le FGF- 1 et le VEGF, la combinaison associant au moins le FGF-1 et le VEGF-B, la combinaison associant au moins le FGF-1 et l' angiopoiétine 1, la combinaison associant au moins le VEGF et l'angiopoïétine 1 et la combinaison associant au moins le VEGF-B et l'angiopoïétine 1.
Selon un mode particulier, le facteur de croissance des cellules endothéliales est choisi parmi le FGF-1, le FGF-2, le FGF-4, le FGF-5, ou leurs variants.
Selon un autre mode, le facteur de croissance des cellules endothéliales est choisi parmi le VEGF, le VEGF-B, le VEGF-C, le VEGF-D, le VEGF-E ou leurs
variants^ De préférence, le facteur de croissance des cellules endothéliales est choisi parmi le VEGF ou le VEGF-B.
Au sens de la présente invention on entend par "variant" d'un polypeptide ou d'une protéine tout analogue, fragment, dérivé ou forme mutée qui est dérivé d'un polypeptide ou d'une protéine et qui retient au moins une fonction biologique dudit polypeptide ou de ladite protéine. Différent variants d'un polypeptide ou d'une protéine peuvent exister à l'état naturel. Ces variants peuvent être des variations alléliques caractérisées par des différences dans la séquence nucléotidique des gènes de structure codant pour la protéine ou peuvent résulter d'un épissage différentiel ou de modifications post-traductionnelle. Ces variants peuvent être obtenus par substitution, délétion, addition et/ou modification d'un ou plusieurs résidus acides aminés. Ces modifications peuvent être réalisées par toutes techniques connues de l'homme du métier.
Ces variants sont notamment des molécules ayant une plus grande affinité pour leurs sites de fixation, des séquences permettant une expression améliorée in vivo, des molécules présentant une plus grande résistance aux protéases, des molécules ayant une efficacité thérapeutique plus grande ou des effets secondaires moindres, ou éventuellement de nouvelles propriétés biologiques.
Parmi les variants préférés du FGF- 1 , on peut citer plus particulièrement les variants naturels du FGF-1, telles que les formes décrites dans le brevet US 4,868,1 13 et comprenant 154 acides aminés, 140 acides aminés ou 134 acides aminés. Parmi les variants préférés de VEGF, on peut citer les formes VEGF12,, VEGF ,6S, VEGF,89 et VEGF 20 . Parmi les variants préférés de VEGF-B, on peut citer les formes VEGF 186 et VEGF ,67 . A titre de variant plus particulièrement préféré, on peut citer les formes FGF-1 ( 21-154 ) et le VEGF 165 et les formes VEGF 186 et VEGF l67.
D'autres variants utilisables dans le cadre de l'invention sont notamment les molécules dans lesquelles un ou plusieurs résidus ont été substitués, les dérivés
obtenus-par délétion de régions n'intervenant pas ou peu dans l'interaction avec les sites de liaison considérés ou exprimant une activité indésirable, et les dérivés comportant par rapport à la séquence native des résidus supplémentaires, tels que par exemple un signal de sécrétion et/ou un peptide de jonction.
Dans le cas du FGF-1, de manière avantageuse, la séquence nucléotidique codant pour le facteur angiogénique contient également un signal de sécrétion permettant de diriger le FGF-1 synthétisé dans les voies de sécrétion des cellules infectées, de manière à ce que le FGF- 1 synthétisé soit libéré plus efficacement dans les compartiments extracellulaires et puisse activer ses récepteurs. Le signal de sécrétion utilisé peut être un signal de sécrétion hétérologue ou même artificiel. A titre d'exemple, on peut citer le signal de sécrétion de l'interféron β humain qui permet la sécrétion importante de FGF- 1.
La séquence d'ADN codant pour le facteur angiogénique utilisée dans le cadre de la présente invention peut être un ADNc, un ADN génomique (ADNg), ou une construction hybride consistant par exemple en un ADNc dans lequel seraient insérés un ou plusieurs introns. Il peut également s'agir de séquences synthétiques ou semisynthétiques. De manière particulièrement avantageuse, on utilise un ADNc ou un ADNg. En particulier, l'utilisation d'un ADNg peut permettre une meilleure expression dans les cellules humaines.
Avantageusement, la séquence codant pour le facteur angiogénique est placée sous le contrôle de signaux permettant son expression dans les cellules de l'épithélium pulmonaire. Préférentiellement, il s'agit de signaux d'expression hétérologues, c'est-à-dire de signaux différents de ceux naturellement responsables de l'expression du facteur angiogénique. Il peut s'agir en particulier de séquences responsables de l'expression d'autres protéines, ou de séquences synthétiques. Notamment, il peut s'agir de séquences promotrices de gènes eucaryotes ou viraux. Par exemple, il peut s'agir de séquences promotrices issues du génome de la cellule que l'on désire infecter. De même, il peut s'agir de séquences promotrices issues du
génome- d'un virus, y compris l'adénovirus utilisé. A cet égard, on peut citer par exemple les promoteurs El A, MLP, CMV, LTR-RSV, etc. En outre, ces séquences d'expression peuvent être modifiées par addition de séquences d'activation, de régulation, ou permettant une expression tissu-spécifique. Il peut en effet être particulièrement intéressant d'utiliser des signaux d'expression actifs spécifiquement ou majoritairement dans les cellules de l'épithélium pulmonaire, de manière à ce que la séquence d'ADN ne soit exprimée et ne produise son effet que lorsque le vecteur a effectivement infecté ces cellules ; à cet égard, on peut citer par exemple le promoteur du gène de la cytokératine 18.
L'acide nucléique codant pour un ou plusieurs facteurs angiogéniques est introduit dans un vecteur. Au sens de la présente invention on entend par "vecteur" tout moyen permettant le transfert d'un acide nucléique dans une cellule hôte, de préférence au niveau du poumon et plus particulièrement au niveau de l'épithélium pulmonaire. Le terme vecteur comprend les vecteurs viraux et non-viraux pour le transfert d'un acide nucléique dans une cellule in vivo ou ex vivo. Un type de vecteur pour la mise en oeuvre de l'invention peut être par exemple un plasmide, un cosmide ou tout ADN non encapsidé par un virus, un phage, un chromosome artificiel, un virus recombinant, etc. Il s'agit de préférence d'un plasmide ou d'un virus recombinant.
Parmi les vecteurs de type plasmidique, on peut citer tous les plasmides de clonage et/ou d'expression connus de l'homme du métier et qui comportent généralement une origine de réplication. On peut citer également des plasmides portant des origines de réplication et/ou des marqueurs de sélection perfectionnés tels que décrits par exemple dans les demandes WO96/26270 et WO97/ 10343.
Parmi les vecteurs de type virus recombinant, on peut citer préférentiellement les virus adénovirus, rétrovirus, virus de l'heφès, les lentivirus, les virus adéno- associés recombinants ou le virus SV40. La construction de ce type de virus recombinants défectifs pour la réplication a été largement décrite dans la littérature,
ainsi que les propriétés d'infection de ces vecteurs (voir notamment S. Baeck et K-.L March (1998), Circul. Research vol. 82, pp 295-305), T. Shenk, B.N. Fields, D.M. Knipe, P.M. Howley et al (1996), Adenoviridae : the viruses and their réplication (in virology). Pp 211-2148, EDS - Ravenspublishers/Philadelphia, P. Yeh et M. Perricaudet (1997), FASEB Vol. 11, pp 615-623.
Un virus recombinant particulièrement préféré pour la mise en oeuvre de l'invention est un adénovirus recombinant défectif.
Les adénovirus sont des virus à ADN double brin linéaire d'une taille de 36 (kilobases) kb environ. Il en existe différents sérotypes, dont la structure et les propriétés varient quelque peu, mais qui présentent une organisation génétique comparable. Plus particulièrement, les adénovirus recombinants peuvent être d'origine humaine ou animale. Concernant les adénovirus d'origine humaine, on peut citer préférentiellement ceux classés dans le groupe C, en particulier les adénovirus de type 2 (Ad2), 5 (Ad5), 7 (Ad7) ou 12 (Ad 12). Parmi les différents adénovirus d'origine animale, on peut citer préférentiellement les adénovirus d'origine canine, et notamment toutes les souches des adénovirus CAV2 [souche manhattan ou A26/61 (ATCC VR-800) par exemple]. D'autres adénovirus d'origine animale sont cités notamment dans la demande WO94/26914 incoφorée à la présente par référence.
Le génome des adénovirus comprend notamment une séquence inversée répétée (ITR) à chaque extrémité, une séquence d'encapsidation (Psi), des gènes précoces et des gènes tardifs. Les principaux gènes précoces sont contenus dans les régions El, E2, E3 et E4. Parmi ceux-ci, les gènes contenus dans la région El notamment sont nécessaires à la propagation virale. Les principaux gènes tardifs sont contenus dans les régions Ll à L5. Le génome de l'adénovirus Ad5 a été entièrement séquence et est accessible sur base de données (voir notamment Genbank M73260). De même des parties, voire la totalité d'autres génomes adénoviraux (Ad2, Ad7, Ad 12, etc) ont également été séquencées.
Pour leur utilisation comme vecteurs recombinants, différentes constructions dérivées des adénovirus ont été préparées, incoφorant différents gènes thérapeutiques. Dans chacune de ces constructions, l'adénovirus a été modifié de manière à le rendre incapable de réplication dans la cellule infectée. Ainsi, les constructions décrites dans l'art antérieur sont des adénovirus délétés de la région El, essentielle à la réplication virale, au niveau de laquelle sont insérées les séquences d'ADN hétérologue (Levrero et al., Gène 101 (1991) 195 ; Gosh-Choudhury et al., Gène 50 (1986) 161). Par ailleurs, pour améliorer les propriétés du vecteur, il a été proposé de créer d'autres délétions ou modifications dans le génome de l'adénovirus. Ainsi, une mutation ponctuelle thermosensible a été introduite dans le mutant tsl25, permettant d'inactiver la protéine de 72kDa de liaison à l'ADN (DBP) (Van der Vliet et al., J. Virol., 1975, 15(2) 348-354). D'autres vecteurs comprennent une délétion d'une autre région essentielle à la réplication et/ou à la propagation virale, la région E4. La région E4 est en effet impliquée dans la régulation de l'expression des gènes tardifs, dans la stabilité des ARN nucléaires tardifs, dans l'extinction de l'expression des protéines de la cellule hôte et dans l'efficacité de la réplication de l'ADN viral. Des vecteurs adénoviraux dans lesquels les régions El et E4 sont délétées possèdent donc un bruit de fond de transcription et une expression de gènes viraux très réduits. De tels vecteurs ont été décrits pas exemple dans les demandes WO94/28152, WO95/02697, WO96/22378). En outre, des vecteurs portant une modification au niveau du gène IVa2 ont également été décrits (WO96/ 10088).
Dans un mode préféré de mise en oeuvre de l'invention, l'adénovirus recombinant est un adénovirus humain du groupe C. De manière plus préférentielle, il s'agit d'un adénovirus Ad2 ou Ad5.
Avantageusement, l'adénovirus recombinant utilisé dans le cadre de l'invention comprend une délétion dans la région El de son génome. Encore plus particulièrement, il comprend une délétion des régions Ela et Elb. A titre d'exemple, on peut citer des délétions affectant les nucléotides 454-3328; 386-3446 ou 357-4020 (par référence au génome de l'Ad5).
Selon une autre variante, l'adénovirus recombinant utilisé dans le cadre -de l'invention comprend en outre une délétion dans la région E4 de son génome. Plus particulièrement, la délétion dans la région E4 affecte l'ensemble des phases ouvertes. On peut citer à titre d'exemple précis les délétions 33466-35535 ou 33093-35535. D'autres types de délétions dans la région E4 sont décrites dans les demandes WO95/02697 et WO96/22378, incoφorées à la présente par référence.
La cassette d'expression contenant l'acide nucléique codant pour un facteur angiogénique peut être insérée en différents sites du génome recombinant. Elle peut être insérée au niveau de la région El, E3 ou E4, en remplacement des séquences délétées ou en suφlus. Elle peut également être insérée en tout autre site, en dehors des séquences nécessaires en cis à la production des virus (séquences ITR et séquence d'encapsidation).
Au sens de la présente invention on entend par "cassette d'expression" d'un acide nucléique, un fragment d'ADN qui peut être inséré dans un vecteur à des sites de restrictions spécifiques ; le fragment d'ADN comprend, outre la séquence nucléotidique codant pour un ARN ou un polypeptide d'intérêt, les séquences nécessaires à l'expression (enhanceur(s) , promoteur(s), séquence de polyadénylation ...) de ladite séquence d'intérêt. Le fragment d'ADN et les sites de restriction sont conçus pour assurer une insertion dudit fragment dans un cadre de lecture approprié pour la transcription et/ou la traduction.
Les adénovirus recombinants sont produits dans une lignée d'encapsidation, c'est-à-dire une lignée de cellules capables de complémenter en trans une ou plusieurs des fonctions déficientes dans le génome adénoviral recombinant. Parmi les lignées d'encapsidation connues par l'homme du métier, on peut citer par exemple la lignée 293 dans laquelle une partie du génome de l'adénovirus a été intégrée. Plus précisément, la lignée 293 est une lignée de cellules embryonnaires humaines de rein contenant l'extrémité gauche (environ 1 1-12 %) du génome de l'adénovirus sérotype 5 (Ad5), comprenant l'ITR gauche, la région d'encapsidation, la région El, incluant
Ela et Elb, la région codant pour la protéine pIX et une partie de la région codant pour la protéine pIVa2. Cette lignée est capable de trans-complémenter des adénovirus recombinants défectifs pour la région El, c'est-à-dire dépourvus de tout ou partie de la région El, et de produire des stocks viraux ayant des titres élevés. Cette lignée est également capable de produire, à température permissive (32°C), des stocks de virus comportant en outre la mutation E2 thermosensible. D'autres lignées cellulaires capables de complémenter la région El ont été décrites, basées notamment sur des cellules de carcinome de poumon humain A549 (WO94/28152) ou sur des rétinoblastes humains (Hum. Gen. Ther. (1996) 215). Par ailleurs, des lignées capables de trans-complémenter plusieurs fonctions de l'adénovirus ont également été décrites. En particulier, on peut citer des lignées complémentant les régions El et E4 (Yeh et al., J. Virol. Vol. 70 (1996) pp 559-565; Cancer Gen. Ther. 2 (1995) 322 ; Krougliak et al., Hum. Gen. Ther. 6 (1995) 1575) et des lignées complémentant les régions El et E2 (WO94/28152, WO95/02697, WO95/27071).
Les adénovirus recombinants sont habituellement produits par introduction de l'ADN viral dans la lignée d'encapsidation, suivie d'une lyse des cellules après environ 2 ou 3 jours (la cinétique du cycle adénoviral étant de 24 à 36 heures). Pour la mise en oeuvre du procédé, l'ADN viral introduit peut être le génome viral recombinant complet, éventuellement construit dans une bactérie (WO96/25506) ou dans une levure (WO95/03400), transfecté dans les cellules. Il peut également s'agir d'un virus recombinant utilisé pour infecter la lignée d'encapsidation. L'ADN viral peut aussi être introduit sous forme de fragments portant chacun une partie du génome viral recombinant et une zone d'homologie permettant, après introduction dans la cellule d'encapsidation, de reconstituer le génome viral recombinant par recombinaison homologue entre les différents fragments.
Après la lyse des cellules, les particules virales recombinantes sont isolées par centrifugation en gradient de chlorure de césium. Une méthode alternative a été décrite dans la demande WO98/00528 incoφorée à la présente par référence.
A titre d'exemple de vecteur convenant pour la mise en oeuvre de la présente invention on peut notamment citer : l'adénovirus recombinant comprenant le gène codant pour le FGF- 1 humain ou le VEGF-B humain tel que décrits dans la présente invention , ou l'adénovirus recombinant comprenant le gène codant pour l'isoforme 165 du VEGF humain tel que décrit dans Mϋlhauser J et al (VEGF 165 expressed by a replication-deficient recombinant adénovirus vector induces angiogenesis in vivo. Cire Res. 195;77:1077-1086) incoφoré par référence à la présente.
L'invention concerne également une composition pharmaceutique comprenant un vecteur tel que décrit ci-avant et un véhicule physiologiquement acceptable. Les compositions pharmaceutiques de l'invention peuvent être formulées en vue d'une administration par voie orale, parentérale, intranasale, intraartérielle, intraveineuse, etc.
Préférentiellement, la composition pharmaceutique contient des véhicules pharmaceutiquement acceptables pour une formulation destinée à être administrée par voie intratrachéale, en particulier par instillation, ou par voie intraveineuse. Il peut s'agir en particulier de solutions salines (phosphate monosodique, disodique, chlorure de sodium, potassium, calcium ou magnésium, etc, ou des mélanges de tels sels), stériles, isotoniques, ou de compositions sèches, notamment lyophilisées, qui, par addition selon le cas d'eau stérilisée ou de sérum physiologique, permettent la constitution de solutés destinés à une instillation intratrachéale.
Les doses utilisées pour l'instillation ou les injections peuvent être adaptées en fonction de différents paramètres, et notamment en fonction du mode d'administration utilisé, du gène à exprimer, ou encore de la durée de l 'expression recherchée. D'une manière générale, les virus recombinants selon l'invention sont formulés et administrés sous forme de doses comprises entre 10^ et 10^ pfu, et de préférence 10" à 101" pfu. Le terme pfu ("plaque forming unit") correspond au pouvoir infectieux d'une solution virale, et est déterminé par infection d'une culture cellulaire appropriée, et mesure du nombre de plages de cellules infectées. Les
techniques de détermination du titre pfu d'une solution virale sont bien documentées dans la littérature.
En outre, les composition selon l'invention peuvent également comprendre un agent de transfert chimique ou biochimique. On entend par le terme "agent de transfert chimique ou biochimique " tout composé (i.e., autre qu'un virus recombinant) facilitant la pénétration d'un acide nucléique dans une cellule. Il peut s'agir d'agents non viraux cationiques comme des lipides cationiques, des peptides, des polymères (Polyéthylène Imine, Polylysine), des nanoparticules ; ou d'agents non viraux non cationiques comme des liposomes non cationiques, des polymères ou des nanoparticules non cationiques.
Selon un mode préféré, les compositions selon l'invention comprennent un vecteur recombinant défectif comprenant un gène codant pour un facteur de croissance des cellules endothéliales et sont formulées pour une administration intratrachéale. Avantageusement, les compositions de l'invention comprennent de 104 à 1014 pfu, et de préférence de 106 à 1010 pfu.
L'invention a également pour objet un procédé de préparation d'un médicament utile pour la prévention, l'amélioration et/ou le traitement de l'hypertension artérielle pulmonaire caractérisé en ce que l'on mélange un vecteur recombinant comprenant un acide nucléique codant pour un facteur de croissance avec un ou plusieurs adjuvants compatibles et pharmaceutiquement acceptables.
L'invention concerne également une méthode de traitement d'un mammifère, et notamment de l'homme, présentant une hypertension artérielle pulmonaire comprenant l'administration d'une quantité efficace d'un vecteur recombinant comprenant un acide nucléique codant pour un facteur de croissance des cellules endothéliales.
La présente invention sera décrite plus en détail à l'aide des exemples qui suivent qui doivent être considérés comme illustratifs et non limitatifs.

MATERIEL ET METHODES
Techniques générales de biologie moléculaire
Les méthodes classiquement utilisées en biologie moléculaire telles que les extractions préparatives d'ADN plasmidique, la centrifugation d'ADN plasmidique en gradient de chlorure de césium, l'électrophorèse sur gels d'agarose ou d'acrylamide, la purification de fragments d'ADN par électroélution, les extraction de protéines au phénol ou au phénol-chloroforme, la précipitation d'ADN en milieu salin par de l'éthanol ou de l'isopropanol, la transformation dans Escherichia coli, etc ... sont bien connues de l'homme de métier et sont abondamment décrites dans la littérature
[Maniatis T. et al., "Molecular Cloning, a Laboratory Manual", Cold Spring Harbor Laboratory, Cold Spring Harbor, N.Y., 1982; Ausubel F.M. et al. (eds), "Current Protocols in Molecular Biology", John Wiley & Sons, New York, 1987.
Pour les ligatures, les fragments d'ADN peuvent être séparés selon leur taille par électrophorèse en gels d'agarose ou d'acrylamide, extraits au phénol ou par un mélange phénol/chloroforme, précipités à l'éthanol puis incubés en présence de l'ADN ligase du phage T4 (Biolabs) selon les recommandations du fournisseur.
Le remplissage des extrémités 5' proéminentes peut être effectué par le fragment de Klenow de l'ADN Polymérase I d'E. coli (Biolabs) selon les spécifications du fournisseur. La destruction des extrémités 3' proéminentes est effectuée en présence de l'ADN Polymérase du phage T4 (Biolabs) utilisée selon les recommandations du fabricant. La destruction des extrémités 5' proéminentes est effectuée par un traitement ménagé par la nucléase S 1.
La mutagénèse dirigée in vitro par oligodéoxynucléotides synthétiques peut être effectuée selon la méthode développée par Taylor et al. [Nucleic Acids Res. L3_ (1985) 8749-8764] en utilisant le kit distribué par Amersham.
L'amplification enzymatique de fragments d'ADN par la technique dite de
PCR [Polymérase-catalyzed Chain Reaction, Saiki R.K. et al., Science 230 (1985)
1350-1354; Mullis K.B. et Faloona F.A., Meth. Enzym. 155 (1987) 335-350] peut être effectuée en utilisant un "DNA thermal cycler" (Perkin Elmer Cetus) selon les spécifications du fabricant.
La vérification des séquences nucléotidiques peut être effectuée par la méthode développée par Sanger et al. [Proc. Natl. Acad. Sci. USA, 74 (1977) 5463- 5467] en utilisant le kit distribué par Amersham.
Exemple 1 : Construction d'adénovirus recombinants exprimant- la protéine FGF-1.
Cet exemple décrit la construction d'un vecteur adénoviral portant le gène codant pour la protéine FGF- 1 opérationnellement lié au promoteur CMN.
L'ADΝc humain codant pour le FGF-1 humain comporte 490 paires de bases et code pour un polypeptide de 154 acides aminés nommé également ECGFβ pour betα-endothelial cell growth factor . Il existe deux polypeptides naturels dérivés de la forme ECGFβ par un mécanisme de maturation post-traductionnel, il s'agit du FGF acide (acidic FGF : aa 15 à 154) et de l'ECGFα (alpha-endothelial cell growth factor; aa 21 à l54) ou FGF-l.
La forme de FGF-1 (sp-FGF-1) présente dans l'adénovirus décrit ci-après est en fait une protéine de fusion entre le FGF-1 (aa 21-154) et un peptide signal de l'interféron betα humain décrite par Jouanneau et al (P.Ν.A.S. (1991) 88 : 2893 - 2897).
L'expression du sp-FGF-1 est sous le contrôle de l'enhancer/promoteur du cytomégalovirus (CMV, nucléotides -522 à +72) (Boshart et al. 1985, Cell, 41 : 521 - 530). Le site de polyadénylation du virus SV40 (nucléotides 2538 à 2759 d'après génome de SV40, Genbank locus SV4CG) est inséré dans le sens tardif en 3' de l'ADΝc du sp-FGF-1. L'ensemble formé par (i) l'enhancer/promoteur du cytomégalovirus , (ii) l'ADΝc du sp-FGF-1 et (iii), le site de polyadénylation du virus SV40 est appelé ci-après la cassette d'expression de FGF- 1.
L'adénovirus a été construit selon la méthode décrite par Crouzet et al (PΝAS vol 94 p 1414, 1997) par recombinaison homologue entre le plasmide pXL3208 et le plasmide pXL3215. Le plasmide pXL3215 contient le génome d'un adénovirus contenant une cassette RSV-LacZ insérée dans sa région El. Le principe de la construction est décrit dans la figure 1. Le plasmide pXL3264 généré par cette double recombinaison contient le génome d'un adénovirus de type 5 délété pour la région E 1
et E3 et contenant la cassette d'expression CMV-spFGFl-SV40. Cette construction est vérifiée par séquençage de la cassette d'expression de FGF-1.
L'adénovirus AV1.0 CMV-FGF1 est généré par transfection de l'ADN de pXL3264 digéré par Pacl dans la lignée 293 (ATCC CRL-1573). Les particules virales obtenues sont ensuite amplifiées dans cette même lignée et des stocks de virus produits par double gradient de CsCl.
Les particules virales sont ensuite utilisées pour étudier l'expression du gène spFGFl humain dans des cellules C2C12, des cellules myoblastiques de souris (ATCC CRL-1772) ou les cellules W162 (Weinberg D.H. and Ketner G.A. 1983, PNAS 80 : 5383 - 5386).
Un Northern-blot est pratiqué sur les cellules W162 après infection à des
MOI croissantes de 100 à 3000 particules virales (pv)/cellule ou transfection des cellules en présence de lipofectamine (Gibco-BRL) avec le plasmide pXL3179 qui contient la même cassette d'expression de FGF-1 comme contrôle. Le plasmide pXL3179 est représenté sur la figure 2.
Un Western-blot a été pratiqué sur des cellules C2C12 après infection à des MOI de 30 à 3000 et récolte des surnageant après 48 h. Le FGFl est révélé par un anticoφs anti-FGFl polyclonal de lapin purifié suivi par un anticoφs anti lapin de chèvre conjugé à la peroxidase. L'activité peroxydase est ensuite révélé par chemiluminescence (ECL, Amersham) et détectée sur un Lumi-Imager (Roche diagnostics).
Le surnageant de culture des cellules C2C12 est utilisé pour vérifier que la forme exprimée du FGF est biologiquement active. Des dilutions sérielles (1/200 et
1/50) de ce surnageant ont ensuite été ajoutées à des cultures de cellules NIH 3T3. L'effet trophique sur ces cultures a été observé par incoφoration de thymidine radiomarquée."
L'ensemble des résultats obtenus confirme que l'adénovirus AV1.0 CMV- FGF1 exprime une forme biologiquement active de FGF-1.
Exemple 2 : Construction d'adénovirus recombinants exprimant la protéine VEGF-B
Cet exemple décrit la construction d'un vecteur adénoviral portant le gène codant pour la protéine VEGF-B opérationnellement lié au promoteur CMV.
Il existe deux formes du VEGF-B humain : VEGF-B167 et VEFG-B186 ,dont les séquences nucléotidiques correspondantes sont accessibles sous les références HSU48801 (VEGF-BI67) et HSU43368 (VEFG-B186) dans GenBank.
Les adénovirus AVI .0-CMV-VEGF-B167 et AVI .0-CMV- VEFG-B186 ont été construits de manière similaire au vecteur AVI.0-CMV-spFGF-l à partir des vecteurs pXL3636 et pXL3637 (figure 3). L'expression du VEGF-B167 ou du VEFG-BI86 est placée sous le contrôle de l'enhancer/promoteur du cytomégalovirus (CMV, nucléotides -522 à +72) (Boshart et al. 1985, Cell, 41 : 521 - 530). Le site de polyadénylation du virus SV40 (nucléotides 2538 à 2759 d'après génome de SV40, Genbank locus SV4CG) est inséré dans le sens tardif en 3' de l'ADNc du VEGF-B167 ou du VEFG-Bl86
L'adénovirus a été construit selon la méthode décrite par Crouzet et al (PNAS vol 94 pl414, 1997) par recombinaison homologue entre le plasmide pXL3636 (VEGF-B167) et le plasmide pXL3215 ou le plasmide pXL3637 (VEFG-B,86) et le plasmide pXL3215. Le plasmide pXL3215 contient le génome d'un adénovirus contenant une cassette RSV-LacZ insérée dans sa région El. Le principe de la construction est décrit dans la figure 1.
Le plasmide généré par cette double recombinaison contient le génome d'un adénovirus de type 5 délété pour la région El et E3 et contenant soit la cassette
d'expression CMV- VEGF-B l67- S V40, soit la cassette d'expression CMV- VEGF- B186-SV40.
Les adénovirus AV1.0-CMN-NEGF-B167 et AV1.0-CMV- VEFG-BI86 sont générés par transfection des plasmides générés par la double recombinaison digérés par Pacl dans la lignée 293 (ATCC CRL-1573). Les particules virales obtenues sont ensuite amplifiées dans cette même lignée et des stocks de virus produits par double gradient de CsCl.
Exemple 3 : transfert intrapulmonaire de AV 1.0 CMV-FGF1 chez le rat.
Cet exemple décrit le transfert du gène codant pour le FGF- 1 humain chez le rat avec le vecteur AV1.0 CMV-FGF1 décrit ci-avant. Un adénovirus recombinant identique mais ne contenant pas la cassette d'expression du FGF-1 (AV 1.0 CMV.Νull) a été utilisé chez les animaux contrôles.
Des rats Wistars mâles d'un mois d'âge (200-250 gr) sont répartis de façon aléatoire en deux groupes. Après anesthésie par injection intrapéritonéale d'un mélange de Kétamine (100 mg/kg) et Xylasine (2 mg/kg), ils reçoivent soit le vecteur AV1.0 CMV-FGF1 soit le vecteur contrôle AV1.0 CMV.Νull en instillation intratrachéale à la dose de 108 pfu (dilués dans du PBS pour un volume final de 150 μl). Cette dose a été choisie après une étude dose-réponse comprenant un dosage ELISA de la protéine FGF- 1 dans le liquide de lavage bronchoalvéolaire et dans le sérum des animaux. 48 heures après l'administration du virus, les animaux sont exposés à un mélange gazeux hypoxique (Fio2 10%) dans des conditions normobariques (armoire Flufrance, Cachan, France) pour une durée de 15 jours.
Exemple 4 : transfert intrapulmonaire de AV1.0-CMV-VEGF-B167 et AV1.0-CMN- NEFG-B186 chez le rat.
Le transfert intrapulmonaire de AN1.0-CMN-NEGF-B167 et AV1.0-CMV- NEFG-B,86 chez le rat a été réalisé dans des conditions identiques à celles décrites â l'exemple 3. Un adénovirus recombinant ne contenant pas la cassette d'expression du NEGF-B (AV 1.0 CMV.Νull) a été utilisé chez les animaux contrôles.
Exemple 5 : évaluation de l'efficacité du transfert intrapulmonaire du gène codant pour le VEGF-B humain chez le rat.
L'évaluation a lieu après 15 jours d'exposition à l'hypoxie selon les critères énumérés ci-dessous.
a - Evaluation de l'efficacité de transduction par le dosage (ELISA) de la protéine FGF- 1 dans le liquide de lavage bronchoalvéolaire.
b - Evaluation de l'HTAP par étude hémodynamique par cathétérisme cardiaque sous anesthésie, avec mesure de la pression artérielle pulmonaire, pression artérielle systémique et fréquence cardiaque.
c - Evaluation de l'hypertrophie ventriculaire droite par le calcul de l'index de Fulton : poids du ventricule droit / poids du ventricule gauche + septum
d - Etude histologique et histomoφhométrique des poumons avec évaluation du degré de la muscularisation des artérioles pulmonaires au niveau alvéolaire et canal alvéolaire (diamètre < 200μm).
2 a - Efficacité de la transduction
Le dosage ELISA de la protéine VEGF-B humaine est réalisé dans le sérum et le liquide broncoalvéolaire (LBA) des animaux après 15 jours d'exposition à l'hypoxie. Le facteur VEGF-B n'est retrouvé dans le sérum d'aucun des animaux contrôle quelque soit les conditions appliquées ( e. avec ou sans hypoxie). De même, la concentration de VEGF-B est nulle dans le LBA des animaux traités par les
vecteurs adénovirus recombinants défectifs codant pour VEGF-B (forme 167 ou forme 186).
2b - Evaluation de l'HTAP
L'évaluation de l'HTAP est réalisée par étude hémodynamique par cathétérisme cardiaque sous anesthésie, avec mesure de la pression artérielle pulmonaire, pression artérielle systémique et fréquence cardiaque.
Le poids coφorel est identique dans les deux groupes de rats 15 jours après le début d'exposition à l'hypoxie. Les rats traités par AV1.0 CMVNEGF-B167 ou AV1.0 CMV-VEGF,86 ont une pression artérielle pulmonaire significativement moins élevée que les rats ayant reçu l'AdCMV.Νull, alors que la pression artérielle systémique et la fréquence cardiaque ne sont pas significativement différentes entre les deux groupes (traités et contrôle).
Ces résultats montrent que le transfert de gène codant pour le VEGF-B permet de prévenir de manière très efficace l'augmentation de la pression artérielle liée à l'hypoxie.
2 c - Evaluation de l'hypertrophie ventriculaire droite
L'hypertrophie ventriculaire droite est évaluée par le rapport du poids du ventricule droit sur le poids du ventricule gauche + septum.
Les résultats obtenus montrent que l'hypertrophie ventriculaire droite est significativement plus importante chez les rats ayant reçu AV 1.0 CMV.Νull que chez ceux traités par AV1.0 CMVNEGF-B,67 ou AV1.0 CMV-VEGF186.
2 d - Etude histologique des poumons
L'étude histologique des poumons montre qu'à la dose de 108 pfu, les lésions inflammatoires sont très limitées : absence d'oedème et d'hémorragie, absence de lésions des épithéliums bronchiques ou alvéolaires. Il est souvent observé un
granulome interstitiel à dominante macrophagique et ceci, quelque soit le traitement administré.
L'étude histomoφhométrique des poumons est réalisée selon deux critères (i) l'étude de l'épaisseur de la paroi artérielle (artérioles de diamètre < 200μm) normalisée à la taille de l'artère et (ii), l'étude de pourcentage d'artères non muscularisées, partiellement muscularisées ou complètement muscularisées au niveau alvéolaire et canal alvéolaire.
L'épaisseur de la paroi artérielle est déterminée et les résultats obtenus montrent que l'épaisseur de la paroi artérielle normalisée à la taille de l'artère est significativement plus faible chez les rats ayant reçu le vecteur AV 1.0 CMVNEGF- B que chez ceux traités par AV1.0 CMV.Νull.
Le pourcentage d'artères non muscularisées, partiellement muscularisées ou complètement muscularisées au niveau alvéolaire et canal alvéolaire a été déterminé et les résultats obtenus montrent que le pourcentage d'artères non muscularisées aussi bien au niveau alvéolaire que au niveau du canal alvéolaire est significativement plus importante chez les rats traités par les vecteurs adénovirus recombinants défectifs codant pour VEGF-B.
Ces résultats démontrent clairement que la surexpression dans les poumons d'un facteur de croissance des cellules endothéliales tel que le VEGF-B a un effet protecteur contre le développement de l'HTAP hypoxique.
Exemple 6 : évaluation de l'efficacité du transfert intrapulmonaire du gène de FGF-1 humain chez le rat.
L'évaluation a lieu après 15 jours d'exposition à l'hypoxie selon les critères énumérés ci-dessous.
a - Evaluation de l'efficacité de transduction par le dosage (ELISA) de la protéine FGF-1 dans le liquide de lavage bronchoalvéolaire.
b - Evaluation de l'HTAP par étude hémodynamique par cathétérisme cardiaque sous anesthésie, avec mesure de la pression artérielle pulmonaire, pression artérielle systémique et fréquence cardiaque.
c - Evaluation de l'hypertrophie ventriculaire droite par le calcul de l'index de Fulton : poids du ventricule droit / poids du ventricule gauche + septum
d - Etude histologique et histomoφhométrique des poumons avec évaluation du degré de la muscularisation des artérioles pulmonaires au niveau alvéolaire et canal alvéolaire (diamètre < 200μm).
Les résultats obtenus montrent que la surexpression dans les poumons d'un facteur de croissance des cellules endothéliales tel que le FGF- 1 a un effet protecteur contre le développement de l'HTAP hypoxique.
Claims
1. Utilisation d'un vecteur comprenant un acide nucléique codant pour un facteur angiogénique pour la préparation d'une composition pharmaceutique destinée à la prévention, à l'amélioration et/ou au traitement de l'hypertension artérielle pulmonaire.
2. Utilisation selon la revendication 1, caractérisée en ce que le facteur angiogénique est un facteur de croissance des cellules endothéliales choisi parmi la famille du FGF ou du VEGF.
3. Utilisation selon la revendication 2, caractérisée en ce que le facteur de croissance des cellules endothéliales est choisi parmi le FGF-1, le FGF-2, le FGF-4, le FGF-5, ou leurs variants.
4. Utilisation selon la revendication 2, caractérisée en ce que le facteur de croissance des cellules endothéliales est choisi parmi le VEGF, le VEGF-B ou le VEGF-C ou leurs variants.
5. Utilisation selon l'une des revendications 1 à 4, caractérisée en ce que le vecteur est un plasmide, un cosmide ou tout ADN non encapside par un virus.
6. Utilisation selon l'une des revendications 1 à 4 caractérisée en ce que le vecteur est un virus recombinant, de préférence dérivé d'un adénovirus, d'un rétrovirus, d'un virus de l'heφès ou d'un virus adéno-associé.
7. Utilisation selon la revendication 6, caractérisée en ce que le virus recombinant est un adénovirus recombinant défectif.
8. Utilisation selon la revendication 6 pour la préparation d'une composition pharmaceutique destinée à une administration par voie intratrachéale comprenant de 104 à 10*4 pfu, et de préférence de 10^ à 10*0 pfu.
9. Procédé de préparation d'un médicament utile pour la prévention, l'amélioration et/ou le traitement de l'hypertension artérielle pulmonaire caractérisé en ce que l'on mélange un vecteur recombinant comprenant un acide nucléique codant pour un facteur angiogénique avec un ou plusieurs adjuvants compatibles et pharmaceutiquement acceptables.
10. Composition pharmaceutique comprenant un vecteur recombinant défectif comprenant un acide nucléique codant pour un facteur angiogénique caractérisée en ce qu'elle est formulée pour une administration intratrachéale.
11. Composition pharmaceutique selon la revendication 10, caractérisée en ce qu'elle comprend de 104 à 10 '4 pfu, et de préférence de 10 > à l010 pfu.
