Les matelas ou sur-matelas sont en général fabriqués suivant deux modèles principaux :
- 1) par soudage de deux feuilles plastiques formant au gonflage des éléments gonflables parallèles, longitudinaux, transversaux ou en diagonale, reliés entre-eux par séries paires et impaires, gonflées alternativement. On peut utiliser également des formes en points ou en boules.
Ce système présente plusieurs défauts :- a) au gonflage, l'ensemble se rétracte, et en raison des alternances, il se produit un mouvement d'auto-reptation qui oblige à une remise en place fréquente du matelas.
- b) de par sa conception, il n'autorise pas des éléments gonflables ou des points très épais et nécessite une pression relativement forte, jusqu'à 150 hPa, pour être efficace ce qui le rend inconfortable pour le malade.
- c) en raison de sa forme et de la largueur des soudures, la surface en contact avec le corps représente parfois seulement à chaque alternance, le tiers de la surface générale.
- 2) Aussi, a-t-on été amené à faire des matelas à gros éléments gonflables, généralement interchangeables et, soit les maintenir par des rubans plastiques, soudés sur une base lisse, soit les introduire dans des housses alvéolées difficiles à fabriquer et à désinfecter.
