Изобретение относится к медицине, реконструктивно-пластической и восстановительной хирургии, а именно к хирургическому устранению дефектов мягких тканей верхней конечности. Цель изобретения заключается в разработке нового способа выкраивания несвободного пахового лоскута с альтернативным источником кровоснабжения. Сущность изобретения: суть способа заключается в использовании в качестве питающего паховый лоскут сосуда поверхностной нижней эпигастральной артерии для укрытия дефекта кожи верхней конечности различной этиологии. Перед операцией при помощи портативного допплеровского датчика определяется ось прохождения ПНЭА, затем маркируются границы выкраивания пахового лоскута с планированием длины ножки для придания лоскуту достаточной мобильности и размерами, достаточными для укрытия дефекта верхней конечности. Разрез выполняется вдоль нижней границы лоскута, начиная с сосудистой ножки на уровне паховой складки, продолжаясь вверх, дойдя до самой верхней точки. В глубину разрез после рассечения поверхностной фасции (фасции Томпсона) проникает до апоневроза. Тупым путем производится отсепаровка лоскута от апоневроза наружной косой мышцы живота над всей площадью маркированного лоскута. Дополнительная диафаноскопия путем просматривания через все слои отсепарованного лоскута на фоне освещения операционной лампы позволяет визуально установить точную проекцию прохождения ПНЭА. The incision after dissection of the superficial fascia (Thompson fascia) penetrates to the depth of the aponeurosis. In a blunt way, the flap is separated from the aponeurosis of the external oblique abdominal muscle over the entire area of the marked flap. Additional diaphanoscopy by looking through all the layers of the separated flap against the background of the illumination of the operating lamp allows you to visually establish the exact projection of the passage of the PNEA. This will make it possible to cut out the rest of the flap without special risk by connecting the lower and upper points by cutting along the upper border of the flap.
