Endoprothese für den teilweisen oder vollständigen Ersatz des Humerus
Die Erfindung bezieht sich auf eine Endoprothese für den teilweisen oder vollständigen Ersatz des Humerus nach dem Oberbegriff des Anspruchs 1.
Eine derartige Endoprothese ist aus dem DE-GM 88 14 434 bekanntgeworden. Sie weist einen Humeruskopfteil auf, an dessen distalem Ende ein Innenkonus gebildet ist. Bei dem vollständigen Ersatz ist mit dem Humeruskopfteil ein Schaft verbunden, der aus zwei in axialer Richtung verbindbaren, länglichen Schaftteilen besteht und der mit einem distalen Gelenkteil ebenfalls über eine Konusverbin-

dung verbunden ist. Bei dem teilweisen Ersatz ist das distale Ende des Schaftes mit einem nicht resezierten Humerusabschnitt verbunden. Unterhalb des sphärischen Kopfes des Kopfteils sind mehrere Löcher vorgesehen für die Verbindung mit Bändern bzw. die die Gelenkkapsel bildenden Gewebeabschnitte.
Der Erfindung liegt die Aufgabe zugrunde, eine Endoprothese für den teilweisen oder vollständigen Ersatz des Humerus zu schaffen, die eine besonders vorteilhafte Befestigung der Bänder des Schultergelenks an der Prothese ermöglicht.
Diese Aufgabe wird erfindungsgemäß gelöst durch die Merkmale des Anspruchs 1.
Bei der erfindungsgemäßen Endoprothese weist der Kopf distal unterhalb des größten Durchmessers eine zumindest teilweise umlaufende Nut auf zur Aufnahme eines den Kopf umschlingenden Bandes. Die Enden der Bänder bzw. des die Gelenkkapsel bildenden Gewebes können mit Hilfe eines Bandes aus körperverträglichem Material in der Nut wirksam festgelegt werden.
Nach einer Ausgestaltung der Erfindung liegt die Nut in
einer Meridianebene des Kopfes. Sie hat mithin eine Lage, die für eine Fixierung von Gewebe und Bändern optimal ist.
Eine andere Ausgestaltung der Erfindung sieht vor, daß die Durchbrüche in einer Meridianebene liegen, die der Nut benachbart sind, d.h. bezüglich der Nut proximal liegen. Durch die Durchbrüche können zusätzliche Befestigungsbänder gezogen werden.
Bei der bekannten Prothese ist der Kopf und der sich daran anschließende Abschnitt bis zum Konus massiv ausgeführt. Dadurch erhält dieses Prothesenteil ein erhebliches Gewicht. Es ist daher nach einer Ausgestaltung der Erfindung vorteilhaft, wenn der sich an den Kopf einteilig anschließende Schaftabschnitt kurz und hohl ist. Zur Verbindung mit dem sich anschließenden Schaftteil kann das Kopfprothesenteil entweder einen Innenkonus oder ein Innengewinde aufweisen, wobei dann das Schaftteil entsprechend einen Außenkonus bzw. ein Außengewinde hat.
Nach einer weiteren Ausgestaltung der Erfindung ist im Inneren des hohlen Kopfes eine Querwand mit einer Gewindebohrung vorgesehen zur Aufnahme eines Gewindestiftes, der Kopfteil und Schaftteil miteinander verbindet. Der Gewinde-
stift ist bei der Verbindung von Kopf- und Schaftteil distal durch den Schaftteil hindurchzuführen, damit er in das Kopfteil eingeschraubt werden kann.
Ausführungsbeispiele der Erfindung werden nachfolgend anhand von Zeichnungen näher erläutert.
Fig. 1 zeigt in Seitenansicht eine Endoprothese nach der Erfindung als teilweise Ersatz für einen Humerus.
Fig. 2 zeigt eine Ausführungsform eines Kopfteils einer Humerusendoprothese nach der Erfindung.
Fig. 3 zeigt einen Längsschnitt durch das Kopfteil nach Fig. 2.
Fig. 4 zeigt ebenfalls einen Längsschnitt durch ein Kopfteil, jedoch mit einer anderen konstruktiven Gestaltung im Inneren.
Das in Fig. 1 dargestellte Schultergelenk 10 wird von einem natürlichen Schulterblatt 12, aber z.B. mit künstlicher Gelenkkapsel und einem Kopfteil 14 einer Endoprothese 16 gebildet. An den als Rotationskopf ausgebildeten Kopfteil 14 schließt sich ein Schaftteil 18 an, der ein
abgestuftes Ende 20 mit einer Abflachung 22 aufweist, das zusammenwirkt mit einem Endabschnitt 24 eines distalen Schaftteils, das ebenfalls abgeflacht und mit der Abflachung 22 zusammenwirkt, wobei die Abschnitte über Schrauben miteinander verbindbar sind. Eine derartige Verbindung ist aus dem DE-GM 88 14 434 bekanntgeworden. Der distale Schaftteil weist eine seitliche Lasche 26 auf zur Befestigung am distalen Teil 28 eines natürlichen Humerus.
Das Kopfteil 14 ist in Fig. 4 näher dargestellt. Es weist einen distalen Schaftabschnitt 30 auf, an den sich proximal ein sphärischer Kopf 32 anschließt in einem Winkel, der der Anatomie des Humerus entspricht. Wie erkennbar, ist am Übergang zwischen Schaftabschnitt 30 und Kopf 32 eine Ringnut 34 geformt, die einen bogen- oder einen kreisförmigen Querschnitt aufweist. Die Nut 34 liegt in einer Meridianebene bezüglich des Kopfes 32, die distal unterhalb der maximalen Meridianebene liegt. In einer Ringanordnung sind Durchbrüche 36 im Kopf 32 geformt, die etwa im Bereich des maximalen Durchmessers bzw. der maximalen Meridianebene liegen.
Wie aus Fig. 1 hervorgeht, dient die Nut 34 dazu, ein körperverträgliches Band 38 aufzunehmen, das um Endabschnitte
von Bändern und Kapselgewebe herumgeschlungen ist und diese Teile am Kopfteil 14 festlegt. Bänder und Kapselgewebe sind bei 40 angedeutet. Die Durchbrüche 36 dienen für die Aufnahme weiterer Bänder oder dergleichen zu Befestigungszwecken .
Fig. 3 zeigt den Längsschnitt durch das Kopfteil 14 nach Fig. 2. Man erkennt, daß der distale Schaftabschnitt 30 einen Innenkonus 42 aufweist zur Aufnahme eines Außenkonus des Schaftteils 18 (Fig. 1). Die den Innenkonus 42 begrenzende Wand des Schaftabschnitts 30 erstreckt sich teilweise in den hohlen Kopf 32 hinein. Dadurch ist eine Querwand 44 gebildet, in der eine Gewindebohrung 46 geformt ist, die koaxial zum Schaftabschnitt 30 liegt. Die Gewindebohrung 46 dient zur Aufnahme eines nicht gezeigten Gewindestiftes, der von distal durch den Schaftteil 18 hindurchgesteckt und in der Gewindebohrung 46 verschraubt wird, um Kopfteil 14 und Schaftteil 16 gegeneinander festzulegen.
Die im Schnitt dargestellte Ausführungsform nach Fig. 4 unterscheidet sich von der nach Fig. 3 nur geringfügig. Es werden daher in Fig. 3 gleiche Teile mit gleichen Bezugszeichen versehen, denen ein a hinzugefügt ist.
Der distale Schaftabschnitt 30a weist eine Gewindebohrung 50 auf zur Aufnahme eines Außengewindeabschnitts eines Schaftteils, ähnlich dem Schaftteil 18 nach Fig. 1. Die Gewindebohrung 46a dient wiederum zur Aufnahme eines Gewindestiftes, um (nicht gezeigtes) Schaftteil mit dem Kopfteil 14a zu verbinden bzw. die beiden Teile gegeneinander zu sichern.

Claims (7)

Ansprüche ;
1. Endoprothese für den teilweisen oder vollständigen Ersatz des Humerus, mit einem Kopfteil, das mit einem länglichen Schaftteil verbindbar ist, wobei der sphärisch geformte Kopf Durchbrüche aufweist zur Festlegung von Bändern, dadurch gekennzeichnet, daß der Kopf (32) distal unterhalb seines größten Durchmessers eine zumindest teilweise umlaufende Nut (34) aufweist zur Aufnahme eines den Kopf 32 umschlingenden Bandes 38 aus körperverträglichem Material.
2. Endoprothese nach Anspruch 1, dadurch gekennzeichnet, daß die Nut (34) in einer Meridianebene des Kopfes (32) liegt.
3. Endoprothese nach Anspruch 2, dadurch gekennzeichnet, daß die Durchbrüche (36, 36a) in einer Meridianebene liegen, die der der Nut (34, 34a) benachbart ist.
4. Endoprothese nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß sich an den Kopf (32) einteilig ein hohler Schaftabschnitt (30, 30a) anschließt.
5. Endoprothese nach Anspruch 4, dadurch gekennzeichnet,
daß der Schaftabschnitt (30) einen Innenkonus (42) aufweist, der mit einem Außenkonus des Schaftteils (18) zusammenwirkt.
6. Endoprothese nach Anspruch 4, dadurch gekennzeichnet, daß der Schaftabschnitt (30a) ein Innengewinde (50) aufweist, der mit einem Außengewindeabschnitt des Schaftteils zusammenwirkt.
7. Endoprothese nach einem der Ansprüche 4 bis 6, dadurch gekennzeichnet, daß im Inneren des Kopfes (32, 32a) eine Querwand (44, 44a) mit einer Gewindebohrung (46, 46a) vorgesehen ist zur Aufnahme eines Gewindestiftes der Kopfteil (14) und Schaftteil (18) miteinander verbindet .
