Description

  • Die vorliegende Erfindung hat eine Gelenkpfanne für eine Hüftprothese zum Ziel.
  • Herkömmliche Hüftprothesen umfassen ein Oberschenkelelement sowie ein als Gelenkpfanne bezeichnetes Element; die Gelenkpfanne hat im wesentlichen eine halbkugelförmige Ausnehmung, welche mit dem kugelförmigen Kopf des Oberschenkelelementes zusammenwirkt, um das natürliche Hüftgelenk zu ersetzen. Gegebenenfalls ist zwischen dem Oberschenkelelement und der Gelenkpfanne, die häufig aus Metallmaterial sind, ein Zwischenteil aus einem Kunststoffmaterial eingesetzt, um die Reibungen zu vermindern und das Gleiten zu erleichtern.
  • Die herkömmlichen Gelenkpfannen müssen in der Vertiefung des Hüftgelenks des Patienten mit mechanischen Befestigungsmitteln in Position gehalten werden, welche eine Traumatisierung des Knochens verursachen können, aufgrund beispielsweise von Rissen, welche durch Befestigungsschrauben provoziert sind, wobei ein Bohren der Wände der Gelenkpfanne erforderlich ist, welche Reibungen und Blockaden durch Erzeugung metallischen Abriebes verursachen, gegebenenfalls beides zur gleichen Zeit.
  • Die DE-A-36 02 081 beschreibt eine Gelenkpfanne, deren Befestigung mit Hilfe eines an der Außenseite der Gelenkpfanne ausgebildeten Gewindes erfolgt.
  • Die FR-A-24 13 078 beschreibt eine Hüftprothese für eine chirurgische Behandlung von krankhaften Fehlfunktionen der Hüfte, bei der so wenig Druck wie möglich zwischen der Prothese und dem Hüftgelenk übertragen wird und bei der die Reibungen zwischen der Prothese und dem Hüftgelenk maximal vermindert sind.
  • Die vorliegende Erfindung hat zum Ziel, diese Nachteile zu überwinden.
  • Sie besteht aus einer Gelenkpfanne für eine Hüftprothese, in Form einer sphärischen Kalotte, wobei die Außenwand der Gelenkpfanne sphärisch ausgebildet und der Mittelpunkt der die Außenwand definierenden Kugel auf der Schwenkachse der Gelenkpfanne in der durch das offene Ende der Gelenkpfanne bezüglich des Poles der Kalotte definierten Ebene angeordnet ist, wobei die Gelenkpfanne zum Einsetzen unter Krafteinwirkung in eine Vertiefung am Knochen ausgelegt ist und eine Außenbeschichtung aus Hydroxyapatit aufweist.
  • Bei einer bevorzugten Ausführungsform sind die Innen- und Außenwände der Gelenkpfanne sphärisch ausgebildet, wobei die die Außenwand definierende Kugel einen Radius hat, der größer ist als der der die Innenwand definierenden Kugel, und der Mittelpunkt der letztgenannten Kugel dem Mittelpunkt des runden, offenen Endes der Gelenkpfanne entspricht.
  • Einer derartige Gelenkpfanne bietet den Vorteil, dass, um sie unter Kraft in die Knochenausnehmung des Hüftgelenks einzusetzen, der Durchmesser der Höhe der Gelenkpfanne entspricht; wobei die Überdicke am offenen äquatorialen Ende der Gelenkpfannenblocks vorgesehen ist, wobei letzterer durch Druck auf den Knochen auf dem gesamten Umfang der Vertiefung des Hüftgelenks in Position gehalten wird. Die Außenbeschichtung aus Hydroxyapatit oder Periapatit (Handelsname = pèri-apatite) hat die Wirkung einer Verankerungsvorrichtung.
  • Um eine ungewollte Drehung der Gelenkpfanne in der Vertiefung zu vermeiden, kann man auf ihrer Außenwand Elemente zur Drehsicherung, wie Umfangsflügel, vorsehen.
  • Die Verankerung des Gelenkpfanne in der Vertiefung kann in gleicher Weise durch das Vorhandensein einer Gruppe von Ringnuten auf der Außenwand der Gelenkpfanne erleichtert werden, welche in zu deren offenen Ende parallelen Ebenen, zwischen dem Pol und dem offenen Ende angeordnet sind.
  • Die Gruppe von Nuten in Ebenen, die den Breiten entsprechen, können durch Nuten ergänzt sein, welche sich vom Pol zum offenen Ende hin in Ebenen erstrecken, die mit den Längen übereinstimmen.
  • Die Gelenkpfanne, welche aus einer Titanlegierung oder anderen Materialien besteht, kann an ihrem Innenumfangsabschnitt nahe dem offenen Ende mit einem verzahnten Kranz und/oder einem Blockierungssystem versehen sein, welches die Aufnahme und das Halten eines Zwischeneinsatzes aus einem Kunststoffmaterial mit geringem Reibungskoeffizienten, wie Polyolefin, oder einem Keramikmaterial ermöglicht.
  • Die Erfindung wird nun mit Hilfe der beigefügten Zeichnungen näher beschrieben, in denen:
  • 1 eine Schnittansicht entlang der Linie II-II in 3 eines erfindungsgemäßen Ausführungsbeispieles zeigt;
  • 2 eine Vorderansicht der in 1 gezeigten Gelenkpfanne zeigt,
  • 3 eine Draufsicht und 4 eine Unteransicht der in den zuvor genannten Fig. gezeigten Gelenkpfanne zeigen.
  • Die erfindungsgemäße Gelenkpfanne ist in den Fig. mit dem Bezugszeichen (1) bezeichnet. Sie hat die Form einer sphärischen Kalotte mit einer sphärischen Außenwand (2) und einer sphärischen Innenwand (3) und hat ein rundes offenes Ende (4).
  • Bei dem in den Fig. dargestellten Ausführungsbeispiel ist der Mittelpunkt der die Innenwand (3) definierenden Kugel im Mittelpunkt des runden offenen Endes (4) angeordnet.
  • Im Gegensatz hierzu ist der Mittelpunkt der die Außenwand (2) definierenden Kugel auf der Schwenkachse der Gelenkpfanne über die das Ende definierten Ebene hinaus angeordnet, also über dieser Ebene, wie in der Ansicht in 1 gezeigt ist.
  • Wie man dieser Fig. eindeutig entnehmen kann, ergibt sich eine Gelenkpfannenwandstärke, die an den Polen am geringsten und an ihrem Umfang, welcher dem offenen Ende (4) entspricht, am größten ist.
  • Die Außenwand (2) der Gelenkpfanne ist an ihrem Innenumfangsabschnitt nahe des Endes (4) mit Umfangsflügeln (5) versehen, welche dazu ausgelegt sind, in der Hüftknochenvertiefung zu verkrusten und eine ungewollte Drehung des Gelenkpfanne (vgl. 2) zu verhindern.
  • Eine Gruppe von Ringnuten (6) in parallel zur Ebene des offenen Endes (4) verlaufenden Ebenen ist in regelmäßigen Abständen zwischen dem Pol und dem äquatorialen Ende der Gelenkpfanne auf der Außenwand (2) dieser Darstellung ausgenommen.
  • In gleicher Weise sind an der Außenwand (2) Längsnuten (7) ausgenommen, die sich vom Pol in Richtung des Äquators in den Ebenen erstrecken, die mit den Längen übereinstimmen (siehe die 2 und 3).
  • Schließlich ist am offenen Ende (4) der Gelenkpfanne an dessen Innenumfang ein verzahnter Kranz (8) sowie Rastvertiefungen (9) für die Aufnahme und Blockierung eines Einsatzes aus einem Kunststoff- oder Keramikmaterial vorgesehen, welcher zwischen der metallischen Gelenkpfanne und dem sphärischen Kopf des Oberschenkelelementes angeordnet sein soll (siehe 4).
  • Eine derartige Gelenkpfanne kann mit Kraft in eine Vertiefung eingebracht werden, deren Durchmesser der Gesamthöhe der Gelenkpfanne entspricht, gemessen ausgehend vom Pol der Außenwand in Richtung der Schwenkachse bis zum Mittelpunkt ihres innen offenen Endes.
  • Sie wird durch das Oberschenkelelement der Prothese unter Druck gehalten; sie kann sich Dank der Elemente zur Drehsicherung (5) nicht drehen und ihr Verankerungssystem aus Nuten (6) und (7), das gegebenenfalls durch eine Außenbeschichtung aus Hydroxyapatit ergänzt ist, stellt ihre Kompatibilität mit der Hüftgelenksvertiefung des Hüftknochens des Patienten sicher.

Claims (6)

  1. Gelenkpfanne für eine Hüftprothese, in Form einer sphärischen Kalotte, wobei die Außenwand (2) der Gelenkpfanne sphärisch ausgebildet und der Mittelpunkt der die Außenwand definierenden Kugel auf der Schwenkachse der Gelenkpfanne in der durch das offene Ende (4) der Gelenkpfanne bezüglich des Poles der Kalotte definierten Ebene angeordnet ist, und wobei die Gelenkpfanne zum Einsetzen unter Krafteinwirkung in eine Vertiefung am Knochen ausgelegt ist und eine Außenbeschichtung aus Hydroxyapatit aufweist.
  2. Gelenkpfanne nach Anspruch 1, dadurch gekennzeichnet, dass die Innen- (3) und Außenwände (2) der Gelenkpfanne sphärisch ausgebildet sind, wobei die die Außenwand (2) definierende Kugel einen Radius hat, der größer ist als der der die Innenwand (3) definierenden Kugel, und der Mittelpunkt der letztgenannten Kugel dem Mittelpunkt des runden, offenen Endes (4) der Gelenkpfanne entspricht.
  3. Gelenkpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass sie auf ihrer Außenwand (2) Elemente zur Drehsicherung, insbesondere Umfangsflügel (5), aufweist.
  4. Gelenkpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass sie auf ihrer Außenwand (2) eine Gruppe von Ringnuten (6) aufweist, welche in zu ihrem offenen Ende (4) parallelen Ebenen, zwischen dem Pol und dem offenen Ende (4) angeordnet sind.
  5. Gelenkpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass sie auf ihrer Außenwand (2) eine Gruppe von Längsnuten (7) aufweist, welche sich vom Pol zum offenen Ende (4) hin in Ebenen erstrecken, die mit den Längen übereinstimmen.
  6. Gelenkpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass sie an ihrem Innenumfangsabschnitt nahe dem offenen Ende (4) einen verzahnten Kranz (8) sowie gegebenenfalls Rastvertiefungen (9) für die Aufnahme und Blockierung eines Einsatzes aus einem Kunststoff- oder Keramikmaterial aufweist.
