Description

  • Beschreibung:
  • Hüftpfannen- Implantate mit Gewinde zur zementlosen Verankerung, auch mit selbstschneidendem Gewinde, haben bislang häufig den Nachteil, daß der Operateur bei solchen Hüftpfannen mit grobem Gewinde eine zu große Kraft aufwenden muß, sodaß auch Frakturen im Becken auftreten können. Bei kleineren Gewindeprofilen, insbesondere bei solchen mit Dreiecks-oder Sägezahnform besteht die Gefahr, daß eine ungenügende Verankerung durch die relativ glatt bearbeiteten Gewindeflächen verursacht wird.
  • Die Lockerung des Implantates ist die Folge.
  • Der Erfindung liegt daher die Aufgabe zugrunde, bei der Verankerung einer zementlosen Hüftgelenkpfanne mit Gewinde, diese Nachteile zu vermeiden. Zu diesem Zweck wird zwar ein Gewinde mit dreieckigem Profil vorgesehen, das aber einen breiten Gewindegrund hat, ähnlich, wie bei einer Spongiosaschraube. Der Gewindegrund wird durch Sandstrahlen, Plasmastrahlen o.ä. mit einer Oberflächenstruktur versehen. Beim Einschrauben setzt sich Blut in die Oberflächenstruktur und wirkt so als Gleitmittel, das die Kraft zum Einschrauben wesentlich reduziert. Nach einiger Zeit wächst dann Knochen in die Oberflächenstruktur und bildet ein solides Implantatlager.Ebenfalls zur Begrenzung der aufzubringenden Kraft beim Einschrauben wird die Anzahl der Gewindegänge so beschränkt, daß diese nur etwa bis zur Hälfte der Höhe des Implantates reichen. Desweiteren ist die äussere Form des Gewindes konisch, sodaß der Einschraubvorgang erleichtert wird. Das schalenartige Hüftgelenkpfannen- Implantat ist zur Aufnahme eines Lagereinsatzes für den mech. Hüftkopf vorgesehen. Der Lagereinsatz wird in bekannter Weise mittels Federring und/ oder Preßsitz im vorgenannten Hüftpfannen- Implantat befestigt, Wenn nach längerem Gebrauch infolge der Abnutzung ein neuer Lagereinsatz erforderlich ist, braucht nur dieser ausgetauscht zu werden, während das Implantatlager erhalten bleibt.
  • Die Erfindung wird in folgendem anhand von Ausführungsbeispielen näher erläutert, aus denen sich weitere wichtige Merkmale ergeben.
  • Es zeigt: Fig.1 Schematisch den Querschnitt des Hüftgelenkpfannen-Implantates (vergrößert) ohne Lagereinsatz für den mechanischen Hüftkopf; Fig.2 die äussere Ansicht des Pfannen- Implantates mit den für das selbstschneidende Gewinde erforderlichen Aussparungen; Fig.3 vergrößert den Gewinde- Querschnitt von Fig.1; Fig.4 schematisch teilweise den Querschnitt des Hüftpfannen- Implantates mit Lagereinsatz und dessen Befestigung.
  • #ig.1und 3 stellen die spiralförmig um die Achse (1) der Hüftpfanne angeordneten Gewindegänge (2) mit Gewindegrund (3) dar, dessen Oberfläche zur Verbesserung des Implantatlagers mit einer Struktur versehen ist. Letztere kann nachträglich oder während des Verformungsvorganges aufgebracht werden. Die Öffnung mit dem Durchmesser D (4) dient dazu, daß der Operateur nach dem Einsetzen der Hüftpfanne in das Becken die M#öglichkeit hat, zwischen der kugelartigen Oberfläche mit dem Radius R1 und dem Knochen Hohlräume zu erkennen und diese mit Knochenspänen auszufüllen.
  • Die äussere Form des Gewindes ist zur Erleichterung des Einschraubvorganges konisch (5) ausgebildet. Zum Einschrauben des Implantates in den Knochen dienen mehrere konzentrisch angeordnete Nuten(6), in die ein Instrument mit entsprechenden Nocken eingreift. Zur Sicherung des Lagiereinsatzes gegen axiale Verschiebung ist eine Ringnut (7) vorgesehen, in die ein federnder Ring einrastet und so den Lagereinsatz axial fixiert, Fig.2 zeigt die Aussparungen (8) für das selbstschneidende Gewinde, in die Knochen einwächst und so zur Verbesserung des Implantatlagers beiträgt.
  • Fig.4 zeigt im Detail den Lagereinsatz (9) an dessen zylindrischem Teil (10) eine Ringnut (7) für den Federring (ii) angeordnet ist.
  • Zum Becken ist der Einsatz kugelförmig mit Radius R2. Der Lagereinsatz ist mit einem Kragen (12) versehen, der eine Beschädigung der Oberfläche des mechanischen Hüftkopfes (13) an den Gewinde -spitzen beim Reponieren verhindert.

Claims (3)

  1. Patentansprüche 1.) In den Knochen einschraubbare Schalen für eine Hüftgelenkpfanne, dadurch gekennzeichnet, daß das spiralartig um die Längsachse angeordnete Gewinde eine konische äussere Form hat und einen breitflächigen fiewindegrund, der mit einer Oberflächenstruktur versehen ist.
  2. 2.) Schalen nach Anspruch 1, dadurch gekennzeichnet,daß deren äussere Gewindebegrenzung aus verschieden geneigten Konen besteht.
  3. 3.) Schalen nach Anspruch 1 und 2, dadurch gekennzeichnet, daß deren Lagereinsätze mit einem Ring versehen sind, dessen Aussendurch -messer zur Vermeidung von Beschädigungen der Lauffläche des Kopfes durch die Gewindespitzen über den Aussendurchmesser des Gewindes hinausragt.
