Description

Die Erfindung betrifft eine in einem Beckenknochen fixierbare Gelenkpfanne für ein künstliches Hüftgelenk mit einer Fassung, die auf ihrer Außenseite ein selbstschneidendes Gewinde aufweist.
Eine derartige Gelenkpfanne ist beispielsweise bekannt aus der DE 83 35 354 U1. Die Gelenkpfanne wird in diesem Falle quasi wie eine Fassung in den natürlichen Beckenknochen geschraubt. Probleme hierbei bereitet im wesentlichen die Langzeitfixation. Durch Mikrobewegungen lockert sich der Sitz der Gelenkpfanne in ihrem knöchernen Bett.
Aus der DE 36 02 081 A1 ist ein Hüftgelenksimplantat bekannt geworden, welches mit einem selbstschneidenden Gewinde auf seiner Außenseite versehen ist. Gegenüber der Gelenkpfanne gemäß der vorerwähnten Druckschrift ist hier darüber hinaus vorgesehen, am Gewindegrunde eine sogenannte Oberflächenstruktur vorzusehen, in die zur Langzeitfixation Knochenmaterial einwachsen soll. Wenn dies gegenüber einer einfachen Gelenkpfanne mit Außengewinde auch schon ein Fortschritt sein mag, werden hier Probleme mit der ausreichenden Versorgung des Knochenmaterials, welches in die Oberflächenstruktur einwachsen soll, auftreten, da die Knochentrapekel nicht von allen Seiten mit Nährstoffen versorgt werden können. Dies resultiert in einer nicht hinreichenden Langzeitfixation.
Vor diesem Hintergrund ist es die Aufgabe der vorliegenden Erfindung, die eingangs erwähnte Gelenkpfanne so weiterzubilden, daß sie eine hinreichende Langzeitfixation erreichen kann.
Dieses Ziel wird erreicht dadurch, daß die Gewindespitzen aus aneinandergereihten Partikeln gebildet sind, die jeweils aus einem Grundkörper und wenigstens vier davon abragenden Zapfen bestehen. Die einzelnen Partikel liegen mit wenigstens drei ihrer Zapfen auf dem abgeflachten, massiven Gewinde auf. Der Umriß des Gewindes mit Partikeln kommt dabei dem Umriß eines idealisierten Gewindes nahe. Die erfindungsgemäße Gelenkpfanne kann wie eine herkömmliche Gelenkpfanne mit Außengewinde in den natürlichen Beckenknochen eingeschraubt werden. Für die Langzeitfixation sorgen die auf dem abgeflachten Gewinde sitzenden Partikel, die einige offene Maschen begrenzen, in die Knochenmaterial einwachsen kann.
Die Gelenkpfanne und die Partikel sind bevorzugt in einem feingießtechnischen Verfahren unter Anwendung eines verlorenen Positivmodells hergestellt, bestehen also aus einem Guß. Die Partikel liegen aus Stabilitätsgründen bereits beim Positivmodell mit wenigstens drei ihrer wenigstens vier Zapfen auf dem abgeflachten Gewinde auf. Dadurch, daß die Außenkonturen des abgeflachten Gewindes mit den darauf befindlichen Partikeln in etwa dem Umriß eines idealisierten Gewindes entspricht, ist beim Einschrauben der erfindungsgemäßen Gelenkpfanne in den natürlichen Beckenknochen nicht mit größerem Widerstand beim Eindrehen zu rechnen.
In einer bevorzugten Ausführungsform weist das Gewinde mit Partikeln den Umriß eines Spitzgewindes auf.
Alternativ hierzu kann das Gewinde den Umriß eines Trapezgewindes aufweisen.
Die Wahl des einen oder anderen Umrisses hängt unter anderem vom Zustand des Beckenknochens ab und kann vom Operateur in Abhängigkeit davon ausgewählt werden.
Zum erleichterten Einschrauben ist vorteilhaft vorgesehen, daß die Fassung der Gelenkpfanne im wesentlichen eine trapezförmige Kontur aufweist, wobei die breitere Basis die Basiskante der Gelenkpfanne bildet. Unter Basiskante wird hierbei jene Kante verstanden, auf deren Seite die Gelenkkugel eines künstlichen Gelenkes in die Gelenkpfanne eingesetzt wird.
Zur unterstützenden Fixation und zur Rotationsarretierung ist bevorzugt vorgesehen, daß zumindest eine in etwa radial vom Grundkörper der Gelenkpfanne abgehende Knochenschraube die Kalotte in einer Durchbohrung durchsetzt und in den Beckenknochen eingeschraubt werden kann.
Eine Ausführungsform der erfindungsgemäßen Gelenkpfanne wird anhand eines Ausführungsbeispiels gemäß den Zeichnungen näher erläutert. Hierbei zeigt
Fig. 1 die Schnittansicht der Gelenkpfanne und
Fig. 2 die vergrößerte Ansicht der Einzelheit Z aus Fig. 1.
Wie aus Fig. 1 deutlich wird, weist die Gelenkpfanne 1 eine Fassung 2 auf, welche auf ihrer Außenseite mit einem selbstschneidenden Gewinde 3, hier als Trapezgewinde ausgebildet, versehen ist. Im Inneren der Fassung 2 ist ein Inlay 9 eingesetzt. Dieses besteht beispielsweise aus hochverdichtetem Polyethylen, welches als Gleitpartner für eine in die Ausnehmung 10 gesetzte künstliche Gelenkkugel (nicht dargestellt) dient.
Wie deutlich erkennbar, ist die Fassung 2 vorliegend im wesentlichen von trapezförmigem Querschnitt, wobei die breitere Basis 8 die Basiskante der Gelenkpfanne 1 bildet. Diese Formgebung erleichtert das Einschrauben der Fassung 2 in den Beckenknochen. Das Gewinde 3 ist abgeflacht. Auf seinen abgeflachten Enden sitzen aneinandergereiht Partikel 5. Einzelheiten sind in Fig. 2 dargestellt, welche eine vergrößerte Ansicht der Einzelheit Z in Fig. 1 zeigt. Die Partikel 5 bestehen aus einem Grundkörper 6, von denen vorliegend sechs Zapfen 7 abragen. Mit drei Zapfen 7 liegt jedes Partikel 5 auf dem abgeflachten Gewinde 3 auf.
Die Primärstabilität der Gelenkpfanne wird durch das Einschrauben der Fassung in dem Beckenknochen erzielt. Die Sekundärstabilität wird erzielt durch das Einorganisieren von Knochenmaterial in die durch die Partikel 5 ausgebildeten Maschen, die begrenzt sind durch die wenigstens drei auf dem abgeflachten Gewinde 3 aufliegenden Zapfen 7. Die frei von dem Grundkörper 6 abragenden Zapfen 7 sorgen im übrigen für eine Reizung des nach der Implantation dann anliegenden Beckenknochens, was einer Stimulation des Knochenwachstums gleichkommt.
Eine Knochenschraube 12 (Fig. 1) durchsetzt die Fassungswandung durch eine Durchbohrung 11. Sie wird in den Beckenknochen geschraubt und dient so der Rotationsarretierung.

Claims (6)

1. In einem Beckenknochen fixierbare Gelenkpfanne (1) für ein künstliches Hüftgelenk mit einer Fassung (2), die auf ihrer Außenseite ein selbstschneidendes Gewinde (3) aufweist, dadurch gekennzeichnet, daß die Gewindespitzen gebildet sind aus aneinandergereihten Partikeln (5), die jeweils aus einem Grundkörper (6) und wenigstens vier davon abragenden Zapfen (7) bestehen, derart, daß die einzelnen Partikeln (5) mit wenigstens drei ihrer Zapfen (7) auf dem abgeflachten, massiven Gewinde (3) aufliegen, wobei der Umriß des Gewindes (3) mit Partikeln dem Umriß eines idealisierten Gewindes nahekommt.
2. Gelenkpfanne nach Anspruch 1, dadurch gekennzeichnet, daß die Partikel (5) integraler Bestandteil der Fassung (2) sind.
3. Gelenkpfanne nch Anspruch 1 oder 2, dadurch gekennzeichnet, daß das Gewinde (3) den Umriß eines Spitzgewindes aufweist.
4. Gelenkpfanne nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß das Gewinde (3) den Umriß eines Trapezgewindes aufweist.
5. Gelenkpfanne nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß ihre Fassung (2) im wesentlichen eine trapezförmige Kontur aufweist, wobei die breitere Basis (8) die Basiskante der Gelenkpfanne (1) bildet.
6. Gelenkpfanne nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß wenigstens eine Knochenschraube (12) radial von der Fassung nach außen abragt.
