Abstract
Die Erfindung betrifft eine künstliche Hüftgelenkspfanne, die aus einer Pfanne (1c), die im Beckenknochen verankert wird und einem Einsatz (3), der im montierten Zustand mit dem Gelenkkopf der Femurprothese zusammenwirkt, besteht. Die Verbindung zwischen der Pfanne (1c) und dem Einsatz (3) umfasst eine neuartige Verdrehsicherung, die durch Anbringen von wenigsten zwei Rändelzähnen (2d, 2e) an der Innenseite der Pfanne (1c) oder an der Aussenseite des Einsatzes (3) gebildet wird.
Description
Künstliche Hüftgelenkspfanne
Die Erfindung betrifft eine künstliche Hüftgelenkspfanne aus wenigstens zwei miteinander verbindbaren Teilen.
Herkömmliche Hüftgelenkspfannen bestehen aus einer Pfanne, die im Beckenknochen verankert wird, und einem Einsatz, der, im montierten Zustand, mit dem Gelenkkopf der Femurprothese zusammenwirkt. Häufig nimmt der Einsatz eine weitere Einlage auf, wobei eines der beiden Teile das unmittelbare harte Lager für den Kopf der Femurprothese bildet und das jeweils andere Teil ein in der Regel eher elastisches Bindeglied zwischen der harten inneren Schale und der zumeist harten Pfanne darstellt.
In der vorliegenden Anmeldung geht es um die Verbindung zwischen der Pfanne und dem Einsatz, unabhängig davon, ob der Einsatz eine weitere Einlage aufnimmt oder nur einstückig ausgebildet ist.
Bei herkömmlichen Hüftgelenksprothesen wird die Verbindung zwischen Pfanne und Einsatz z. B. durch Umspritzgiessen des Einsatzes mit dem Material der Pfanne (vgl. z.B.: EP-A2-726066 und EP-B1-544214), durch Schnappverbinden des Einsatzes mit der Pfanne (vgl. z.B.: EP-A1-242633) oder durch Verschrauben des Einsatzes mit der Pfanne oder dergleichen hergestellt.
Vor allem bei den Aufbauten mit Schnappverbindung gibt es das Bedürfnis der drehsicheren Verankerung der beiden Teile relativ zueinander. Bei den umspritzten Versionen ist die Drehsicherung automatisch gegeben. Bei den verschraubten Versionen gibt es zwei Varianten: jene mit Verriegelungsschrauben und jene, bei denen zwischen Pfanne und Einsatz ein Gewinde ausgebildet ist (vgl. z.B.: EP-A1- 242633). Letztere ist dann verdrehgesichert, wenn das auftretende Drehmoment unter Belastung durch den Patienten in Einschraubrichtung wirkt. Wirkt das Moment jedoch in Ausschraub- oder Lösrichtung, könnte das zu einem Lockern der Drehsicherung führen.
Die Drehsicherung zwischen den beiden Teilen ist aus verschiedenen Gründen wünschenswert:
1. Unkontrollierte Reibung durch Relativbewegungen zwischen der Pfanne und dem Einsatz kann zu unkontrolliertem Abrieb führen, der in jedem Fall unerwünscht ist. Abriebpartikel können durch Gewebebeeinflussungen z.B. eine Lockerung des Prothesensitzes bewirken.
2. Bei bestimmten Formen von Einsätzen - z. B. jene die zur Luxationsbekämpfung eingesetzt werden - ist ferner die relative Drehlage des Einsatzes zur Pfanne kritisch. Diese Relativlage wird bei der Operation eingestellt und soll für die Lebensdauer der Prothese beibehalten bleiben.
Bisher wurde die Verdrehsicherung durch eine der folgenden Massnahmen realisiert:
a) Im Inneren der Pfanne sind vier symmetrisch verteilte Vorsprünge angebracht, die mit gegengleichen Nuten korrespondieren.
b) Es gibt auch Systeme mit Spitzkegeln, die sich axial in das relativ weiche Material des Einsatzes eindrücken (vgl.z.B.: US-A-5092897.
c) Am stirnseitigen Rand der Pfanne werden rippenförmige Zähne ausgebildet, die mit gegengleichen Zähnen an einem den Rand überragenden Kragen des Einsatzes, ebenso in axialer Richtung kooperieren.
d) Im Randbereich der Pfanne werden Nuten ausgebildet, die mit gegengleichen Rippen des Einsatzes kooperieren.
e) Durch Oberflächenbehandlung, z. B. Sandstrahlen, wird die Innenfläche der Pfanne aufgerauht. Die relativ rauhe Innenfläche der Pfanne genügt zur reibungsbedingten Drehsicherung zwischen Pfanne und Einsatz, was allerdings nur bei konischen Pfannen genügend Wirkung erzielt.
Alle diese bekannten unterschiedlichen Arten von Verdrehsicherungen zwischen den beiden Teilen erfüllen ihre Verdrehsicherungsaufgabe mehr oder weniger gut.

Jede der erwähnten Arten hat jedoch auch Nachteile:
a) Die im Inneren der Pfanne angebrachten Vorsprünge und die gegengleichen Nuten am Einsatz müssen in aufwändigen Fräsverfahren hergestellt werden. Ausserdem können die Drehstellungen nicht stufenlos gewählt werden.
b) Die eingedrückten Spitzkegel ergeben eine zu geringe Verdrehsicherungswirkung.
c) Die am stirnseitigen Rand der Pfanne ausgebildeten rippenförmigen Zähne, die mit gegengleichen Zähnen an einem den Rand überragenden Kragen des Einsatzes kooperieren, sind relativ aufwändig herstellbar.
d) Die im Randbereich der Pfanne ausgebildeten Nuten, die mit gegengleichen Rippen des Einsatzes kooperieren, sind ebenfalls relativ aufwendig herstellbar. Sowohl die Lösung c) als auch d) sind nicht stufenlos verstellbar.
e) Sandstrahlen bietet bei sphärischen Verbindungen zu wenig Wirkung.
Der Erfindung liegt somit die Aufgabe zugrunde, ein neues Drehverriegelungssystem zu finden, das die erwähnten Nachteile vermeidet, einfach herstellbar ist und über eine ausreichende Drehsicherung verfügt.
Gelöst wird diese Aufgabe durch das Anbringen einer Rändelung aus wenigstens zwei Zähnen an der Innenseite der Pfanne oder an der Aussenseite des Einsatzes.
Diese Rändelung wird insbesondere an dem härteren der beiden Teile ausgebildet, so dass sich die Rändelzähne bei der Montage in die Oberfläche des jeweils anderen weicheren Teils eingraben. Die Erstreckung der Rändelzähne ist bevorzugt etwa axial im Hinblick auf die Einsetzrichtung, so dass die Rändelung durch axiales Stossen mit einem Stosswerkzeug hergestellt werden kann.
Die Höhe der Rändelzähne über dem Grund des Innendurchmessers der Pfanne entspricht etwa dem Doppelten bis Dreifachen des Spiels zwischen der Pfanne und dem Einsatz.
Bevorzugt sind mehrere Gruppen von Rändelzähnen über den Umfang der Aussenseite des Einsatzes bzw. über den Umfang der Innenseite der Pfanne verteilt. Insbesondere bevorzugt sind es drei Gruppen, um je 120° zueinander versetzt. Für besonders drehmomentstarke Verdrehsicherung kann die Rändelung auch durchgehend über den gesamten Umfang ausgebildet sein.
Die Rändelung ist bevorzugt an einem ringförmigen Vorsprung ausgebildet, der gegebenenfalls in den Bereichen ohne Rändelung unterbrochen ist.
Gemäss einer Weiterbildung der Erfindung kann die Rändelung auch durch Querrippen unterteilt sein, die zusätzlich zur Rotationssicherung auch eine Rastsicherung in Axialrichtung ermöglichen.
Erfindungsgemäss ist die Höhe der Rändelzähne auf ein bestimmtes Mass beschränkt, so dass ein Eindringen der Zähne in die Oberflächenstruktur des jeweils weicheren Teils ermöglicht wird, ohne einen scherenden Materialabtrag zu bewirken oder die erforderliche Eintreibkraft des Einsatzes unnötig zu erhöhen. Dieser Effekt wird dadurch noch begünstigt, indem die Stirnseiten der Rändelzähne angeflacht oder abgerundet sind.
Hinsichtlich der Anzahl der Rändelzähne pro Rändelzähnegruppe bieten sich dem Fachmann nach Kenntnis der Lehre der vorliegenden Anmeldung und in Abhängigkeit der Materialwahl verschiedene Möglichkeiten an, wie schon erwähnt, zwischen wenigstens zwei über wenigstens fünf, wenigstens zehn, wenigstens zwanzig bzw. wenigstens vierzig Zähnen. Die Form der Zähne bzw. der Täler zwischen den Zähnen kann im Rahmen der Erfindung gewählt werden, wobei eine im Querschnitt etwa dreieckförmige Zahnform bevorzugt ist.
Figurenbeschreibung
Die Figuren werden zusammenhängend und übergreifend beschrieben. Gleiche Bezugszeichen bedeuten gleiche Bauteile; Bezugszeichen mit unterschiedlichen Indizes geben funktionengleiche Bauteile an.
Es zeigen:
Figur 1 einen schematischen Schnitt durch eine neuartige Pfanne mit Rändelzähnen;
Figur 2 ein vergrössertes Detail einer Ausführungsvariante gemäss Figur 1 ;
Figur 3 eine Unteransicht von Figur 1 ;
Figur 4 eine Unteransicht einer Variante zu Figur 3;
Figur 5 einen schematischen Schnitt durch ein System mit Pfanne, Einsatz und Einlage; und
Figur 6 einen schematischen Schnitt einer Variante mit Rändelzähnen und
Quernuten.
Figur 1 zeigt schematisch den Schnitt durch eine neuartige Pfanne 1a mit Gruppen von Rändelzähnen 2a und 2b. Bevorzugt sind drei Gruppen von Rändelzähnen vorgesehen, wie in der Sicht von unten auf Figur 1 gemäss Figur 3 zu sehen ist. In Figur 3 ist die Pfanne 1a sowie die drei Gruppen von Rändelzähnen 2a, 2b und 2c zu sehen. Eine Variante zu Figur 3 ist in Figur 4 dargestellt, bei der die Rändelzähne 2f über den gesamten Umfang der Pfanne 1d angeordnet sind.
Figur 2 zeigt ein vergrössertes Detail einer Ausführungsvariante gemäss Figur 1, bei der die Rändelzähne 2b an einem Steg 9 ausgebildet sind, der seitlich durch zwei Einstiche 6, 7 begrenzt ist. Der Einstich 7 ist als Schnappnute ausgebildet, welche die axiale Sicherung des Einsatzes in der Pfanne 1 b bewirkt.
In Figur 5 sieht man einen Schnitt durch ein System mit Pfanne 1c, Gruppen von Rändelzähnen 2d, 2e, Einsatz 3 und Einlage 4. Die Einlage 4 hält im Einsatz 3 beispielsweise gemäss der Patentanmeldung der Anmelderin WO-A 1-01/32108. Ein Vierkant 5 verbessert dabei die Haltbarkeit, wie der erwähnten WO-A1 entnommen werden kann.
In Figur 6 ist eine Variante dargestellt, bei der zu den Rändelzähnen 2g an der Pfanne 1e Quemuten 8a, 8b an der Pfanne 1e angeordnet sind. Diese Quernuten bewirken ein geringfügiges Eindringen des relativ weichen Materials des Einsatzes in diese Nuten, so das sich bei diesem Aufbau auch eine axiale Bewegungssicherung ergibt.

Claims
1. Künstliche Hüftgelenkspfanne, mit einer Pfanne, die im Beckenknochen verankert wird und einem Einsatz, der im montierten Zustand mit dem Gelenkkopf der Femurprothese zusammenwirkt, mit einer Verdrehsicherung zwischen der Pfanne und dem Einsatz, dadurch gekennzeichnet, dass die Verdrehsicherung durch Anbringen von wenigsten zwei Rändelzähnen (2) an der Innenseite der Pfanne (1) oder an der Aussenseite des Einsatzes (3) ausgebildet wird.
2. Künstliche Hüftgelenkspfanne gemäss Anspruch 1 , dadurch gekennzeichnet, dass die Erstreckung der Rändelzähne (2) etwa axial ist.
3. Künstliche Hüftgelenkspfanne gemäss Anspruch 1 oder 2, dadurch gekennzeichnet, dass mindestens zwei Gruppen von wenigstens zwei
Rändelzähnen (2) über den Umfang des Einsatzes (3) bzw. über den Umfang der Innenseite der Pfanne (1) verteilt sind.
4. Künstliche Hüftgelenkspfanne gemäss Anspruch 3, dadurch gekennzeichnet, dass drei Gruppen von wenigstens zwei Rändelzähnen (2a, b, c), um je 120° zueinander versetzt, über den Umfang verteilt sind.
5. Künstliche Hüftgelenkspfanne gemäss einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass jede Gruppe wenigstens fünf Rändelzähne (2) aufweist.
6. Künstliche Hüftgelenkspfanne gemäss einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass jede Gruppe wenigstens zehn Rändelzähne (2) aufweist.
7. Künstliche Hüftgelenkspfanne gemäss einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass jede Gruppe wenigstens zwanzig Rändelzähne (2) aufweist.
8. Künstliche Hüftgelenkspfanne gemäss einem der vorhergehenden Ansprüche dadurch gekennzeichnet, dass jede Gruppe wenigstens vierzig Rändelzähne (2) aufweist.
9. Künstliche Hüftgelenkspfanne gemäss Anspruch 4, dadurch gekennzeichnet, dass die Rändelzähne (2f) durchgehend über den gesamten Umfang angeordnet sind.
