Description
- Die vorliegende Erfindung betrifft eine künstliche Bandscheibe, und insbesondere, aber nicht ausschließlich ein verschleißarmes, hartes, chrombeschichtetes, metallisches Kugelgelenklagersystem, das eine uneingeschränkte Bewegung ermöglicht und beim Ersatz von Wirbelscheibensegmenten verwendet wird. Es wird auf die US-A-5258031 Bezug genommen, die eine Scheibenvorrichtung entsprechend dem Oberbegriff des Anspruchs 1 unten zeigt.
- Es hat eine Reihe von verschiedenen Implantaten gegeben, die dazu bestimmt sind, beschädigte Wirbelscheibensegmente des menschlichen Körpers zu ersetzen, bei denen eine künstliche Bandscheibe verwendet wird, um eine deformierte, verletzte oder erkrankte natürliche Bandscheibe zu ersetzen. Solch eine künstliche Bandscheibe besteht im Allgemeinen aus einem festen Körper, der den zu der implantierten künstlichen Scheibe benachbarten Wirbel in seiner Fähigkeit einschränkt, relativ zu diesem sich zu bewegen. Einige dieser festen künstlichen Bandscheiben sind in den US-Patenten 4,349,921; 4,553,273 und 4,714,469 offenbart. Andere künstliche Scheiben sind mit einer Feder versehen, die die Bewegung in limitierten Richtungen des zu der implantierten, künstlichen Scheibe benachbarten Wirbels einschränken. Diese sind in den US-Patenten 4,309,777 und 4,759,769 beschrieben.
- Es ist ebenso vorgeschlagen worden, weiche, nachgiebige Polymere zu verwenden, um eine Trennung von beschädigten Scheibensegmenten sicherzustellen oder als Lageroberflächen zu verwenden. Polymere können jedoch mit der Zeit im menschlichen Körper abgebaut werden und neigen ebenso zu einer Abnutzungsbeschädigung durch die Knochenoberfläche. Polymerische Scheibenersatzteile neigen ferner dazu, sich langsam mit der Zeit zu deformieren (#creep#) und ihre Abmessungseigenschaften allmählich zu verändern. Polymere werden aber aufgrund ihrer geringen Steifheit und aufgrund der relativen Verbesserung bezüglich der Stoßabsorbiereigenschaften favorisiert. Künstliche Bandscheiben mit einem aus einem elastischen Polymer gebildeten Hauptkörper sind in den US-Patenten 3,867,729; 4,863,477 und 4,911,71 offenbart. Das US-Patent 5,071,437 offenbart eine künstliche Bandscheibe, die aus einem elastomerischen Kern, der zwischen zwei Metallplatten enthalten ist, zusammengesetzt ist. Die Scheibe ist ferner mit einer Mehrzahl von Spitzen zur Stabilisierung der zu der implantierten Scheibe benachbarten Wirbel versehen. Die Biokompatibilität dieser Scheibe ist jedoch ungewiss, da sie ein Polymer und ein Aushärtemittel enthält, das dazu verwendet wird, um die Metallplatten und den elastomerischen Kern zusammenzuhalten.
- Gelenkartige künstliche Wirbelscheiben sind vorgeschlagen worden, aber ohne Polymerkomponenten besitzen sie geringe Fähigkeiten, Stöße zu absorbieren. Ferner gibt es keinen klinischen Nachweis, dass die Stoßabsorption notwendig ist, da benachbarte Scheiben eine Kompensierung vorsehen können, um die Stoßabsorption in einer oder beiden Scheiben entlang der gesamten Wirbelsäule auf ein reduziertes Niveau zu bringen.
- Nicht-polymerische Scheibenkomponenten umfassen keramische und metallische Materialien, und entsprechende Designs umfassen scharnierartige, schiebeartige und kugelgelenkartige Lösungen. Das Scharnierdesign ist häufig einschränkend, und der Patient kann sich in gewissen Richtungen nicht bewegen. Gleitscheibenoberflächen besitzen im Allgemeinen nicht die Fähigkeit, Biege- oder Drehbewegungen unterzubringen, helfen aber bei natürlichen Translationsbewegungen innerhalb des Scheibenraums. Aufgrund des geringen Raumes zwischen benachbarten Wirbeln kann jedoch die Kontaktspannung mit lediglich dieser Art von einzigen (Translations-) Bewegungsfähigkeit zwischen den Gleitoberflächen zu einem übermäßigen Verschleiß führen und schließlich zu einer Fehlfunktion der künstlichen Bandscheibe.
- Die am logischsten erscheinendste Lösung für einen Wirbelscheibenersatz ist das Kugelgelenkdesign. Keramiken sind vorgeschlagen worden, sind aber extrem hart und neigen zu übermäßiger Kontaktspannung und Verschleiß, falls Drei- Körper-Partikel oder Ablagerungen zwischen die beiden Oberflächen gelangen. Ohne eine Filmschmierung, um die zwei keramischen Oberflächen zu trennen, können hohe Hertz'sche Kontaktspannungen die harte Oberfläche brechen, was zu einer fortwährenden und beschleunigten Oberflächenverschleißbeschädigung führt. Ablagerungen, wie z. B. Knochenchips, können auf einfache Weise diesen nachteiligen Drei-Körperverschleißprozess erzeugen. Aufgrund der relativ hohen Steifigheit von Keramiken muss das Implantatdesign und die Bearbeitung ferner derart sein, dass der Oberflächenkontakt optimiert ist. Solch eine präzise Bearbeitung von keramischen Oberflächen ist extrem schwierig.
- Harte metallische Oberflächen, welche mit relativ weichen, schmierigen Oxiden und Oxyhydroxiden (d. h. Cr-haltige Metalle) passiviert sind, sind bezüglich der gelegentlichen, hohen Kontaktspannungsverschleißbedingungen, die in der Wirbelsäule vorzufinden sind, tolerant. Diese Art von Kugelgelenksystem ist jedoch lediglich unter normalen, nicht extremen Bewegungen wirksam. Während einer extremen Bewegung und einem Biegen der Wirbelsäule kann die Kugel aus der Kugelaufnahme gelängen und auf die Vorrichtungskanten aufprallen, was hohe Spannungen und Verschleiß erzeugt. Metalle wären unter diesen extremen Bedingungen toleranter als harte Keramiken, aber metallische Komponenten können ebenso unter einigen Verschleißbedingungen leiden. Deshalb ist es ebenso erwünscht, in dem Implantatdesign die Kontaktspannungen in diesen extremen Bewegungsbedingungen zu minimieren.
- Andere Designs, die entweder Keramiken oder metallische Materialien verwenden, schränken die Bewegung relativ zu den benachbarten Wirbeln ein. Einige Fachmänner erkennen jedoch, dass aufgrund der Tatsache, dass natürliche, gesunde Scheiben eine Bewegungsgrenze von ungefähr 11º in der anteriorenposterioren Ebene (Beugung nach vorne/nach hinten) und eine Bewegungsgrenze von ungefähr 3-5º in der medialen-lateralen Ebene (Seiten-zu-Seitenbeugung) erfahren, der künstliche Scheibenersatz ebenso diese Grenze besitzen muss, was dazu führt, dass benachbarte Scheiben die Belastungen aufnehmen. Ferner behaupten einige Fachleute, dass es eine Einschränkung bezüglich der Drehung (1-2º) innerhalb der künstlichen Scheibe aus den gleichen Gründen geben muss. Zum Beispiel offenbart das US-Patent 4,759,769 ein Scharnier-/Federdesign mit einer Einschränkung von 20º bezüglich Krümmung/Verlängerung, die eine geringe Seite-zu-Seite- Bewegung ermöglicht. Eine Drehung ist mit dieser Vorrichtung eingeschränkt. Das US-Patent 5,071,437 beschreibt eine Implantatvorrichtung, das die Krümmung/Verlängerung und seitliches Beugen auf physiologische Winkel limitiert und eine Drehung von lediglich 2-3º ermöglicht. Diese Vorrichtung ermöglicht ebenso einen Übergang (#transition#) von 1-2 mm. Das US-Patent 4,759,766 beschreibt eine Vorrichtung, die zwei Kobalt-/Chromendplatten mit einer gewölbten Polyäthylenkernmitte umfasst, die eine Krümmung/Verlängerung von 10º, eine geringe seitliche Beugung ermöglicht und bezüglich der Drehung beschränkt ist. Der Zusatz dieser anatomischen Einschränkungen auf Scheibenersatzdesigns limitiert die Art und Wirksamkeit des Designs und der Materialien, die dafür eingesetzt werden können. Zum Beispiel, falls die Scheibenprothese ihre Beugungsbeschränkung von 11º überschreitet, wie dies oben erwähnt ist, kommen die Nicht-Lageroberflächen miteinander in Kontakt und verschieben die Prothese unangemessen oder erzeugen eine nachteilige Verschleißsituation.
- Man ist der Meinung, dass Beschränkungen bezüglich des Scheibenprothesendesigns nicht nur eine Performancezuverlässigkeit erzeugen, sondern ebenso nicht benötigt werden. Die umgebenden Bänder, Muskeln und anderes Gewebe sehen eine eingebaute Beschränkung bezüglich anteriorer/posteriorer, medialer/lateraler und Drehbewegungen der künstlichen Bandscheibe vor, zumindest bis zu dem Punkt, beidem benachbarte Scheiben nicht deutlich betroffen sind.
- Aus den oben genannten Gründen sieht die vorliegende Erfindung bei bevorzugten Ausführungsformen eine uneingeschränkte, kugelgelenkartige, künstliche Wirbelscheibe vor, die verschleißarme Lageroberflächen aus chromhaltigen Metallen aufweist, die präzisionsbearbeitet sind, um eine Kontaktspannung zu minimieren, und die mit benachbarten Wirbeln über eine Presspassung oder durch Zementier-Verfahren verankert sind, die auf dem Gebiet des Gelenkersatzes bekannt sind. Um extreme Bewegungen unterzubringen, sind die Umfangsbereiche der kugelgelenkartigen Bandscheibe derart ausgestaltet, um Spannung und Verschleiß zu minimieren. Dieses Design kann ebenso die Fähigkeit zu einer Translationsbewegung aufweisen, ähnlich des Designs für ein Miniskuslagerknie (#miniscal bearing knee#). Zusätzlich können die Lagerabschnitte der Scheibe der künstlichen Bandscheibe an einem unterschiedlichen Material angebracht sein, um die Scheibenabschnitte mit dem benachbarten Knochen zu verankern.
- Die vorliegende Erfindung ist in Anspruch 1 unten bestimmt und sieht eine künstliche Wirbelscheibe vor, die zwischen benachbarten Wirbeln chirurgisch eingesetzt wird. Die Vorrichtung der vorliegenden Erfindung sieht eine verschleißarme, künstliche Wirbelscheibe vor, die eine erste Komponente zum Anbringen an einen ersten Wirbel mit einer konkaven Aussparung umfasst, wobei die konkavförmige Oberfläche einen Umfang von 360º aufweist. Eine zweite Komponente zum Anbringen an einen zweiten Wirbel weist einen Vorsprung auf, der mit der konkaven Aussparung der ersten Komponente übereinstimmt, wobei die konvexförmige Oberfläche einen Umfang von 360º aufweist. Die entsprechenden umrissenen Oberflächen ermöglichen eine uneingeschränkte Drehbewegung und eine relativ uneingeschränkte Biege-/Verlängerungsbiegebewegung zwischen der ersten und zweiten Komponente relativ zur normalen Wirbelsäulenachse eines Patienten in einer Stehposition, in der sich die Wirbelsäule in einer aufrecht ausgerichteten Position befindet.
- In einer weiteren Ausführungsform der vorliegenden Erfindung ist eine künstliche Wirbelscheibe vorgesehen für den chirurgischen Einsatz zwischen einem ersten und zweiten benachbarten Wirbel der Wirbelsäule eines Patienten, umfassend:
- (a) eine erste Komponente zum Anbringen an den ersten Wirbel, wobei die erste Komponente einen Basisabschnitt und einen Lagerabschnitt umfasst, der Lagerabschnitt eine konkave Aussparung umfasst, und die konkavförmige Oberfläche einen Umfang von 360º besitzt;
- (b) eine zweite Komponente zum Anbringen an den zweiten Wirbel, wobei die zweite Komponente einen Basisabschnitt und einen Lagerabschnitt umfasst, der Lagerabschnitt einen Vorsprung umfasst, der in die konkave Aussparung der ersten Komponente passt, und die konvexförmige Oberfläche einen Umfang von 360º besitzt;
- (c) die entsprechenden Lagerabschnitte derart ausgestaltet, sind, so dass sie eine uneingeschränkte Dreh- und eine Biege-/Verlängerungsbiegebewegung zwischen der ersten und zweiten Komponente relativ zur normalen Wirbelsäulenachse eines Patienten in stehender Position erlaubt, in der die Wirbelsäule in aufrecht ausgerichteter Position sich befindet.
- In einer weiteren Ausführungsform der vorliegenden Erfindung ist eine künstliche Wirbelscheibe zum chirurgischen Einsatz zwischen einem ersten und zweiten benachbarten Wirbel einer Wirbelsäule eines Patienten vorgesehen, umfassend:
- (a) eine erste Komponente zum Anbringen an den ersten Wirbel, wobei die erste Komponente eine konkave Aussparung umfasst, und die konkavförmige Oberfläche einen Umfang von 360º besitzt;
- (b) eine zweite Komponente zum Anbringen an den zweiten Wirbel, wobei die zweite Komponente einen Vorsprung umfasst, der in die konkave Aussparung der ersten Komponente passt, und die konvexförmige Oberfläche einen Umfang von 360º besitzt;
- (c) die entsprechenden umrissenen Oberflächen derart ausgestaltet sind, dass sie eine uneingeschränkte Dreh- und eine Biege-/Verlängerungsbiegebewegung zwischen der ersten und zweiten Komponente relativ zur normalen Wirbelsäulenachse eines Patienten in stehender Position erlauben, in der die Wirbelsäule sich in aufrecht ausgerichteter Position befindet;
- (d) wobei die erste und zweite Komponente derart ausgestaltet sind, dass sie der künstlichen Wirbelscheibe ermöglichen, als eine Einheit implantiert zu werden, in der Öffnungen im Knochen des benachbarten Wirbels erzeugt werden, um zu ermöglichen, dass die künstliche Wirbelscheibe in die richtige Stellung gleiten kann, um so eine übermäßige Distraktion zu verhindern.
- Die Erfindung wird klarer, wenn die detaillierte Beschreibung der beispielhaften Ausführungsformen im Zusammenhang mit den beigefügten Zeichnungen betrachtet wird, von denen:

- Wie in den Fig. 1-4 gezeigt, umfasst eine verschleißarme, künstliche Wirbel- oder Bandscheibe 10 eine erste Komponente oder einen Zwischenwirbelstützblock 20 zum Anbringen der Endplatte eines ersten Wirbels, und eine zweite Komponente oder einen Zwischenwirbellagerblock 30 zum Anbringen der Endplatte eines zweiten angrenzenden Wirbels. Der Zwischenwirbelstützblock 20 besitzt eine Außenseite 22, die mit ein oder mehreren Vorsprüngen 24 versehen ist, wie z. B. Zapfen, Stäbchen oder Schrauben, und eine Innenseite 26, die eine konkave Aussparung besitzt, oder eine vertiefte Aufnahmebefestigung 28, die ein Segment bildet, das dem einer Fassung ähnelt, und das in der Nähe der Mitte des Stützblockes 20 (Fig. 10) sich befindet. Die konkavförmige Oberfläche der Aufnahmebefestigung 28 weist einen Umfang von 360º auf.
- Der Zwischenwirbellagerblock 30 besitzt eine Außenseite 32 mit ein oder mehreren Vorsprüngen 34, wie z. B. Zapfen, Stäbchen oder Schrauben, und eine Innenseite 36, die einen Vorsprung 38 mit einer konvexen Oberfläche besitzt, die ein kugelähnliches Segment (Fig. 11) bildet, wobei die konvexe Oberfläche einen Umfang von 360º besitzt. Die Stelle des Vorsprunges 38 entspricht der vertieften Aufnahmeöffnung 28 des Stützblockes 20 und ist derart ausgebildet, so dass er auf drehbare Weise in die vertiefte Aufnahmebefestigung 28 des Stützblockes 20, wie in den Fig. 10 bis 12 gezeigt ist, passt. Der Vorsprung 38 des Lagerblockes 30 und die vertiefte Aufnahmebefestigung 28 des Stützblockes 20 sind derart ausgebildet, dass sie eine kugelgelenkähnliche Vorrichtung bilden, die eine uneingeschränkte Drehbewegung (Fig. 13) und eine relativ uneingeschränkte Flexions-/Verlängerungskippbewegung (Fig. 12) zwischen dem Stützblock 20 und dem Lagerblock 30 relativ zu einer normalen Wirbelsäulenachse eines Patienten ermöglichen, wenn dieser sich in einer aufrechten Position befindet, in der die Wirbelsäule sich in aufrecht ausgerichteter Position befindet. Die Innenseite 26 des Stützblockes 20 und die Innenseite 36 des Lagerblockes 30 sind entsprechend mit einer Oberfläche versehen, die nach außen zu einem Umfang 21, 31 des Stützblockes 20 und des Lagerblockes 30, wie dies in den Fig. 2-4, 5 und 7 gezeigt ist, geringfügig gekrümmt oder geneigt ist. Auf diese Weise weist die Innenseite 26 des Stützblockes 20 geneigte Oberflächen zwischen dem Umfang 21 des Stützblockes 20 und der vertieften Aufnahmebefestigung 28 auf. Auf ähnliche Weise weist die Innenseite 36 des Lagerblockes 30 geneigte Oberflächen zwischen dem Umfang 31 des Lagerblockes 30 und dem Vorsprung 38, wie in den Fig. 5 und 12 gezeigt ist, auf.
- Folglich ermöglicht die Konfiguration des Stützblockes 20 und des Lagerblockes 30 eine Flexion/Verlängerung um einen beträchtlichen Winkel, der im Allgemeinen ungefähr 20-30º beträgt, um den beide sich drehen können, wie in Fig. 12 dargestellt ist, wodurch einem Patienten mit der implantierten, künstlichen Bandscheibe eine im Allgemeinen normale Bewegung und Drehung der Wirbelsäule ermöglicht wird.
- Die Außenseite 22 und 32 des Zwischenwirbelstützblockes 20 und des Zwischenwirbellagerblockes 30 können im Wesentlichen flach, geringfügig konkav gekrümmt sein oder verschiedene andere Formen aufweisen. Flache oder geringfügig konkave Außenoberflächen sind jedoch bevorzugt und können mit einer Beschichtung aus einem Material bedeckt sein, das dem Material entspricht, aus dem der Stützblock 20 und der Lagerblock 30 gebildet sind. Solch beschichtete Außenoberflächen sind bezüglich ihrer Struktur denjenigen ähnlich, die in dem US-Patent 5,071,437 offenbart sind.
- Mit Ausnahme der vertieften Aufnahmebefestigung 28 ist die Innenseite 26 des Stützblockes 20 im Wesentlichen flach oder geringfügig konvex. Eine geringfügig konvexe Innenseite 26 ist bevorzugt, da solch eine Oberfläche eine Rotation des Stützblockes 20 und des Lagerblockes 30 um 360º relativ zueinander ermöglicht. Mit Ausnahme des Vorsprunges 38 ist auf ähnliche Weise die Innenseite 36 des Lagerblockes 30 im Wesentlichen flach oder geringfügig konvex ausgebildet. Eine geringfügig konvexe Innenseite 36 ist aus dem gleichen Grund, wie oben beschrieben, bevorzugt.
- Die künstliche Wirbelscheibe 10 der vorliegenden Erfindung ist derart geformt, um eine minimale Einschränkung bezüglich des Kippens und keine Einschränkung bezüglich einer Drehbewegung zwischen benachbarten Wirbeln vorzusehen, von denen jede eine Beschränkung darstellt, die in anderen vorangegangenen Scheibendesigns vorgefunden werden. Bei einer bevorzugten Ausführungsform der vorliegenden Erfindung, wie in den Fig. 5 und 7 gezeigt, weichen die Kanten der vertieften Aufnahmebefestigung 28 oder des Fassungssegments rasch zurück, um den Kippabstand zu maximieren. Die Kanten der vertieften Aufnahmebefestigung 28 gehen allmählich in die Innenseite 26 mit einem konvexen Radius, der ähnlich dem des Vorsprungs 38 ist, über, um Kontaktspannungen und Verschleiß bei extremen Bewegungsbedingungen zu minimieren. Dieses Merkmal verbessert ebenso die Stabilität des Implantats unter extremen Bewegungsbedingungen. Der Vorsprung 38 oder das Kugelsegment tritt auf ähnliche Weise rasch von dem Lagerbereich zurück und geht allmählich in die Innenseite 36 über. Der Vorsprung 38 befindet sich bevorzugt ungefähr in der Mitte des Implantats 10, kann aber auch derart ausgestaltet sein, so dass der Chirurg eine Wahl bezüglich dieses Ortes, abhängig von den Bedürfnissen des Patienten, besitzt.
- Der Vorsprung 38 des Lagerblockes 30 besteht aus einem im Allgemeinen halbsphärischen Aufbau und entspricht bezüglich der Form der konkaven Oberfläche der Aufnahmebefestigung 28, so dass der Vorsprung 38 in der Lage ist, sich frei um 360º in der vertieften Aufnahmebefestigung 8 zu drehen. Die Form der vertieften Aufnahmebefestigung 28 ist im Wesentlichen ähnlich der Krümmung des Vorsprunges 38. Die konvexe Oberfläche des Vorsprunges 38 weist eine Tiefe oder Dicke auf, die der Tiefe der Konkavform der vertieften Aufnahmebefestigung 28 entspricht oder größer als diese ist. Um zu ermöglichen, dass der Stützblock 20 und der Lagerblock 30 sich frei zueinander drehen können, sollte die Tiefe oder die Dicke der konvexen Oberfläche des Vorsprunges 38 geringfügig größer als die Tiefe der Konkavform der vertieften Aufnahmebefestigung 28 sein, wobei angenommen wird, dass die vertiefte Aufnahmebefestigung 28 und der Vorsprung 38 entsprechend mit einer flachen Oberfläche versehen sind.
- Der Zwischenwirbelstützblock 20 und der Zwischenwirbellagerblock 30 weisen eine Gesamthöhe oder Dicke auf, die abhängig von der Höhe oder Dicke der zu ersetzenden, deformierten Zwischenwirbelscheibe bzw. Bandscheibe ist. Zusätzlich besitzen der Stützblock 20 und der Lagerblock 30 eine Form, die abhängig ist von der Form der Wirbel, die der zu ersetzenden Zwischenwirbelscheibe benachbart angeordnet sind. Bei einer bevorzugten Ausführungsform ist die Form der künstlichen Wirbelscheibe 10 im allgemeinen oval, wie dies in Fig. 6 und 8 gezeigt ist.
- Die vertiefte Aufnahmebefestigung 28 der künstlichen Wirbelscheibe 10 befindet sich in oder in der Nähe der Mitte des Stützblockes 20. Auf ähnliche Weise befindet sich der Vorsprung 38 der künstlichen Wirbelscheibe 10 in oder in der Nähe der Mitte des Zwischenwirbellagerblockes 30.
- Der Stützblock 20 und der Lagerblock 30 sind entsprechend mit ein oder mehreren Vorsprüngen 24, 34 versehen, die in Form von Zapfen, Stäbchen oder Schrauben vorgesehen sind und sich an einer beliebigen, vorher ausgewählten Position an den Außenoberflächen 22, 32 des Stützblockes 20 bzw. des Lagerblockes 30 befinden. Zusätzlich können flache Wulstoberflächen an den Außenoberflächen 22, 32 oder andere Sicherungsmittel verwendet werden, um die künstliche Wirbelscheibe 10 in dem benachbarten Wirbel zu halten.
- Eine Translation der primären Verschleißoberflächen ist nicht notwendig. Bei einer alternativen Ausführungsform des Wirbelimplantats 10A, wie in Fig. 9 gezeigt, weist jedoch ein Lagerblock 30A einen Vorsprung oder ein Kugelsegment 38A und eine Basisplatte 40 auf, dessen Aufbau dem eines Lagers des Miniskus im Knie ist, der eine gewisse Translation in Kombination mit der Rotation und dem Kippen ermöglicht, das durch die Kugelgelenkdesignmerkmale ermöglicht wird. Alternativ könnte der Stützblock 20 anstelle des Lagerblockes 30A eine vertiefte Aufnahmebefestigung und eine Basisplatte (nicht gezeigt), die identisch zu dem Lagerblock 30A ist, besitzen. Dieses Merkmal kann die Stabilität und den Widerstand bezüglich einer Dislokation des Implantats 10A verbessern. Bei dieser alternativen Ausführungsform kann eine keramische Oberfläche auf einer keramischen Oberfläche (zusätzlich zu Metall-Metall) für die beiden sich gegenüberliegenden, flachen Translationsoberflächen 42, 44 verwendet werden, da die Kontaktspannungen bedeutend durch solch einander entsprechenden Oberflächen reduziert sind. Um eine potentielle Dislokation des Kugelgelenks zu verhindern oder zu minimieren, können die Basisplatten 40, 42, die aus Titan oder einer Titanlegierung gebildet sein können, an dem Stützblock 20A oder dem Lagerblock 30A mechanisch oder über eine Schrumpfpassung oder Schweißverfahren angebracht sein.
- Der Zwischenwirbelstützblock 20 und der Zwischenwirbellagerblock 30 können aus Metall, wie z. B. rostfreier Stahl 316 LVM oder Titan-6-4 oder einer Kobalt/Chromlegierung, die allesamt für die orthopädische Chirurgie geeignet sind, hergestellt sein. Zusätzlich kann eine Keramik, wie z. B. Hydroxyapatit, oder ein biologisch kompatibles organisches Polymer, wie z. B. HDPE, verwendet werden, um die künstliche Wirbelscheibe 10 der vorliegenden Erfindung herzustellen.
- Bei einer bevorzugten Ausführungsform ist die vertiefte Aufnahmebefestigung 28 und der Vorsprung 38 der künstlichen Wirbelscheibe 10 aus einem harten chromhaltigen Metall gebildet, die an Basiskomponenten 23, 33 aus Titan, einer Titanlegierung, Kobalt/Chromlegierung, Zirkonium oder Zirkoniumlegierung angebracht sind, die entsprechend in Fig. 7 gezeigt sind. Das bevorzugte Material für die Basiskomponenten 23, 33 ist Titan oder eine Titanlegierung, um Schatteneffekte bei der MRI-Bildgebung zu minimieren. Obwohl Knochenzement verwendet werden kann, benutzt die bevorzugte Ausführungsform ferner eine makrotexturierte oder porös beschichtete Außenseite 22, 32, wobei die Verankerungszapfen oder Schrauben 24, 34 ebenso porös beschichtet oder makrotexturiert sind.
- Das bevorzugte Material für die vertiefte Aufnahmebefestigung 28 und den Vorsprung 38 ist ein chromhaltiges Eisen oder eine Kobältlegierung mit einem Chrom-Anteil von mehr als ungefähr 15 Gew.-%, und der Zusatz von ungefähr 2 bis 6 Gew.-% Molybdän, um die Korrosionsbeständigkeit in chloridhaltigen Umgebungen zu verbessern. Akzeptable Beispiele umfassen A.I.S.I. 316 L rostfreier Stahl oder ASTM F-75 Guss Co-Cr.Mo oder ASTM F-799 bearbeitete Co-Cr-Mo-Legierung, die allesamt gegenwärtig in medizinischen Implantatanwendungen verwendet werden. Aufgrund der relativ hohen Kontaktdrücke, die in Kugelgelenkverbindungen innerhalb des begrenzten Raumes zwischen benachbarten Wirbeln auftreten, ist es jedoch bevorzugt, ein oberflächengehärtetes chromhaltiges Metall zu verwenden, wie z. B. eines, das in dem Davidson US-Patent 5,415,704 beschrieben ist. Solche Legierungszusammensetzungen erzeugen minimalen Verschleiß, sind gegenüber einer Drei- Körper-Abrasion tolerant und erzeugen eine relativ schlüpfrige Chrom/Oxyhydroxid-Oberflächenschicht, die die Bewegung und die Verschleißbeständigkeit verbessert.
- Die vorliegende Erfindung sieht so eine uneingeschränkte kugelgelenkähnliche, künstliche Wirbelscheibe vor, die verschleißarme Lageroberflächen aus chromhaltigen Metallen enthält, die präzise bearbeitet sind, um Kontaktspannungen zu minimieren, und die mit dem benachbarten Wirbel durch Presspassung oder einem Zementierverfahren verankert wird, die auf dem Gebiet des Gelenkersatzes bekannt sind. Die künstliche Wirbelscheibe 10 und 10A kann implantiert werden, um eine degenerierte oder erkrankte Zwischenwirbelscheibe oder Bandscheibe durch herkömmliche, auf diesem Gebiet sehr gut bekannte, orthopädische Chirurgie zu ersetzen.
- Die künstliche Wirbelscheibe 10 und 10A ist derart konfiguriert, um so dem Implantat 10, 10A zu ermöglichen, als eine einteilige Einheit implantiert zu werden, im Gegensatz zum Einführen jeweils einer Komponente, was dazu führt, dass eine übermäßige Distraktion von benachbarten Wirbeln vermieden wird. Bei einem Knochenresektionsverfahren, welches die Implantierung des Implantats als eine einzige Einheit ermöglicht, werden Öffnungen oder Kanäle in dem Knochen des benachbarten Wirbels in einer Form gebildet, die den Vorsprüngen 24, 34 entspricht. Das Implantat 10, 10A wird als eine Einheit zwischen die Resektionslinien geschoben, was dazu führt, dass eine übermäßige Distraktion vermieden wird, die unnötige Spannungen auf die umgebenden Strukturen ausüben kann.
Claims (11)
1. Eine künstliche Wirbelscheibe (10, 10A) für den
chirurgischen Einsatz zwischen ersten und zweiten
benachbarten Wirbeln einer Wirbelsäule eines Patienten,
umfassend:
(a) eine erste Komponente (20) zum Anbringen an den
ersten Wirbel, wobei die erste Komponente (20)
einen Umfang (21), eine Innenseite (26) und in der
Mitte der Innenseite eine konkave Aussparung (28)
umfasst, und wobei die konkav umrissene Oberfläche
der Aussparung einen Umfang von 360º besitzt;
(b) eine zweite Komponente (30) zum Anbringen an den
zweiten Wirbel, wobei die zweite Komponente (30)
einen Umfang (31), eine Innenseite (36) und in der
Mitte der Innenseite einen Vorsprung (38) umfasst,
der in die konkave Aussparung (28) der ersten
Komponente (20) passt, und wobei der Vorsprung (38)
eine konvex umrissene Oberfläche mit einem Umfang
von 360º besitzt;
(c) die entsprechenden konkav und konvex umrissenen
Passoberflächen derart konfiguriert sind, dass sie
eine uneingeschränkte Dreh- und eine
Flexions-/Verlängerungskippbewegung zwischen der
ersten und zweiten Komponente (20, 30) relativ zur
normalen Wirbelsäulenachse des Patienten in einer
aufrechten Position, in der die Wirbelsäule in
einer aufrecht ausgerichteten Position sich
befindet, ermöglichen,
dadurch gekennzeichnet, dass
die Innenseite (26) des Stützblockes (20) und die
Innenseite (36) des Lagerblockes (30) gekrümmte
Oberflächen aufweisen, die allmählich in die
entsprechende konvex oder konkav umrissene Oberfläche
(38, 28) und den entsprechenden Umfang (21, 31)
übergehen.
2. Die künstliche Wirbelscheibe nach Anspruch 1, wobei
mindestens eine der Oberflächen aus einem chromhaltigen
Metall mit einem Chromanteil von mehr als 15 Gew.-%
gebildet ist.
3. Die künstliche Wirbelscheibe nach Anspruch 1, wobei
mindestens eine der umrissenen Oberflächen aus einer
Kobalt-Chrom-Molybdän-Legierung gebildet ist.
4. Die künstliche Wirbelscheibe nach Anspruch 1, wobei
mindestens eine der umrissenen Oberflächen mit einer
Kobalt-Chrom-Molybdän-Tantal-Legierung beschichtet ist,
in der Tantal Kobalt ersetzt und weniger als ungefähr
5 Gew.-% ausmacht.
5. Die künstliche Wirbelscheibe nach einem der
vorhergehenden Ansprüche, wobei die erste (20) und
zweite (30) Komponente Verankerungsmittel (24, 34) zum
Anbringen an die entsprechenden ersten und zweiten
Wirbel umfasst.
6. Die künstliche Wirbelscheibe nach Anspruch 5, wobei das
Verankerungsmittel (24, 34) eine porös beschichtete oder
makrotexturierte Oberfläche umfasst, um Knocheneinwuchs
zu erleichtern.
7. Eine künstliche Wirbelscheibe nach einem der
vorhergehenden Ansprüche, wobei die Komponenten im
Allgemeinen ovalförmig sind, und die umrissenen
Oberflächen sich ungefähr in der Mitte der Komponente
befinden.
8. Eine künstliche Wirbelscheibe nach einem der
vorhergehenden Ansprüche, wobei die erste und zweite
Komponente derart konfiguriert sind, um so der
künstlichen Wirbelscheibe zu ermöglichen, als eine
Einheit in einem Prozess, bei dem eine übermäßige
Distraktion eliminiert ist, implantiert zu werden.
9. Die künstliche Wirbelscheibe nach einem der
vorhergehenden Ansprüche, wobei mindestens eine der
Komponenten einen Basisabschnitt (40) und einen
Lagerabschnitt umfasst, und wobei mindestens einer der
Basisabschnitte einer Komponente ein Mittel für die
Translation zwischen den Lagerabschnitten umfasst.
10. Die künstliche Wirbelscheibe nach einem der
vorhergehenden Ansprüche, wobei mindestens eine der
Komponenten einen Basisabschnitt (40) und einen
Lagerabschnitt umfasst, und wobei mindestens einer der
Basisabschnitte aus einem Material gebildet ist, das
unterschiedlich zu dem Material des Lagerabschnittes
ist.
11. Eine künstliche Wirbelscheibe nach einem der
vorhergehenden Ansprüche, wobei die erste (20) und
zweite Komponente (30) derart konfiguriert sind, dass
sie der künstlichen Wirbelscheibe ermöglichen, als eine
Einheit implantiert zu werden in einem Prozess, bei dem
Öffnungen im Knochen des benachbarten Wirbel erzeugt
werden, wodurch die künstliche Wirbelscheibe (10, 10A)
an die richtige Stelle geschoben werden kann, um so eine
übermäßige Distraktion zu verhindern.
