Abstract
Description
The invention relates to a medicament containing benfotiamine, as well as its use.
Benfotiamine (S-benzoylthiamine-O-monophosphate) is a lipid-soluble analogue of vitamin B 1 . The symptoms of vitamin B 1 deficiency include loss of appetite, vomiting, absorption disorders, fatigue, paralysis and psychological changes such as loss of memory, confusion, depression. These symptoms of deficiency are known as the so-called Beri-Beri disease. Vitamin B 1 , as a water-soluble vitamin, is therefore not so well suited as a medicament for the treatment of these vitamin B 1 deficiency states, since it has only limited bioavailability (only approx. 5% of an oral dose is absorbed) and has retention. Therefore, vitamin B 1 may have to be added in very high doses.
It is already known to administer the lipid-soluble analogue of vitamin B 1 , namely benfotiamine in the form of dragées, in order to achieve an improved absorption in the intestinal tract compared to the water-soluble vitamin B 1 . After being absorbed into the cell system, benfotiamine is split and the actual active ingredient, namely thiamine, is released.
A disadvantage of the known mode of administration is that localized delivery is not possible by oral administration. It is therefore necessary to administer such a dose that there is sufficient benfotiamine at the point where, for example, a vitamin B 1 deficiency is to be remedied or a therapeutically necessary concentration is to be present, in order to convert thiamin (vitamin B 1 ) to be split. If, for example, a painful inflammation of an ankle caused by vitamin B 1 deficiency is to be treated, sufficient benfotiamine must be administered to ensure that after absorption in the intestinal tract there is sufficient benfotiamine at the site to be treated, which is then broken down into thiamine and as Active ingredient is available. Naturally, the absorbed benfotiamine is not only transported to this point by the organism, but also distributed over the entire organism. As a result, benfotiamine is broken down with the release of thiamine even in cells in which there is no vitamin B 1 deficiency per se, although this is not necessary in the corresponding cells. An increased vitamin B 1 intake in the body manifests itself through an extremely unpleasant body odor.
The object of the present invention is to overcome these disadvantages overcome and a drug of the type mentioned create that can be used locally.
According to the invention, the object is achieved by a medicament, in which the benfotiamine is distributed in a carrier and is applied externally to the skin.
The inclusion of benfotiamine in a carrier can localized ointment or cream, for example to be applied to those skin areas, among which the to treating body parts are arranged. The carrier holds the benfotiamine in place and opens up the possibility that the benfotiamine penetrates through the skin and into the one below located cell system penetrates. One finds in the cell system Dephosphorylation by cell-specific phosphatases to S- Benzoylthiamine (SBT) instead. Then SBT is replaced by enzymatic Debenzoylation to thiamine, which in turn is caused by thiamine kinase is converted into the metabolically active cocarboxylase.
It was found that the lack of sufficient cocarboxy lase amounts in diseased areas to an enrichment intermediate breakdown products such as pyruvate, lactate and ketoglutarate leads. This causes inflammation, with pain are connected. Benfotiamine inhibits after skin penetration and conversion to cocarboxylase, the accumulation of these toxic substances. Studies have shown that a antinociceptive effect is achieved, d. H. through the thiamine pain receptors are established and thereby mediation hindered the sensation of pain.
The introduction of the benfotiamine into the carrier now creates the possibility of delivering benfotiamine locally via the skin to the parts of the body to be treated, whereupon this is then converted into the active ingredient thiamine or cocarboxylase. The advantages are not only that a localized application is possible, but also that, owing to the special penetration mechanism and subsequent release of the thiamine, it is possible to deliver the active ingredient to the site to be treated much more quickly. Furthermore, it is also possible, compared to the application form of coated tablets, to use a lot less benfotiamine overall, since only a relatively small proportion of the benfotiamine is converted to thiamine on the relatively short transport route between the skin and the parts of the body to be treated actually reached cell areas to be treated. The mechanism of conversion from benfotiamine to thiamine takes place in the cells regardless of whether a clinical picture prevails or not, ie such a conversion takes place with oral administration immediately after absorption into the cell system of the gastrointestinal tract. It must then be ensured that sufficient benfotiamine is then still available at the actual treatment site in order to be converted on the spot into the active substances thiamine or cocarboxylase. These disadvantages are overcome by external use and the side effects of unpleasant body odor with high vitamin B 1 administrations are also eliminated. The topically analgesic and anti-inflammatory effect which is achieved with the external administration according to the invention is locally effective with a rapid effect, with a long-lasting effect and can be achieved with substantially lower amounts of benfotiamine in comparison with the oral administration.
In a further embodiment of the invention, the carrier is an ointment-shaped lipophilic carrier.
This measure has the advantage that the drug is in the form an ointment or cream that is easy to handle and can be applied or adhered well to human skin this can be massaged. The benfotiamine can for example as a finely divided solid in the lipophilic carrier matrix be distributed. Massage the ointment or cream like carrier shortens the penetration phase.
In a special embodiment of the invention, the Carrier liposomes, in the aqueous interior of which benfotiamine is included.
B. Lecithin) in wäßrigen Medien herstellen. Liposomes are spherical structures from one or several concentric lipid bilayers with aqueous Inner space. Such bubbles can be mechanically Fine distribution of phospholipids (e.g. lecithin) in aqueous Create media. The measure now proposed, namely the benfotiamine in the aqueous phase in the interior of the liposomes It has the considerable advantage that the benfotiamine in itself relatively safely protected from external influences the lipophilic carrier is included. It also has this measure the considerable advantage that the liposomes pass through very quickly penetrate the skin areas and only in the body's own The cells beneath the skin release the benfotiamine. Consequently is a rapid targeted feeding of benfotiamine into the to treat areas of the body. After release The benfotiamine can then be described in the cells of the previously described Conversion mechanism to thiamine and cocarboxylase take place. This measure now has the further considerable advantage that the benfotiamine almost completely, undecomposed and in the Liposomes encapsulated the areas of the body to be treated can be supplied.
This is another extremely quick and targeted treatment possible with relatively small amounts of active ingredient.
In a further embodiment of the invention, the carrier is a lipophobic carrier containing detergents that have a Distribution of benfotiamine in the lipophobic phase digits.
This measure has the advantage that known per se Detergents the benfotiamine in a lipophobic carrier is acceptable, so that the drug is roughly the consistency a liquid that is very quick and can be evenly distributed over large areas. Such lipophobic carriers can then be provided which, for the most part, ver soon after application to the skin vaporize so that a film-like layer is formed on the skin becomes. For example, this is desirable if the skin areas immediately after application of the drug, to be covered again by clothing without these then stick to the skin areas and thereby uncomfortable prepare opportunities.
In a further embodiment of the invention, the drug medium applied in the form of an aerosol.
This measure has the advantage that the drug is quick and can be easily applied to the skin. It can the benfotiamine as a suspension in a liquefied under pressure ten gas as a propellant and released of the blowing agent on the body as a finely divided solid be applied. It is also possible to take the benfotiamine in incorporate lipophilic or lipophobic phase in the propellant, so that after striking the aerosol, the carrier including benfotiamine as a finely divided film-like conviction on the skin brought.
Untersuchungen haben ergeben, daß ein solches Arzneimittel zur Behandlung folgender Krankheiten herangezogen werden kann, nämlich rheumatische Beschwerden, allgemeine Gelenk- und Muskelbeschwerden, Wurzelreizsyndromen der Wirbelsäule, HWS- Syndrom, Schulter-Arm-Syndrom, Symptomen bei Polyarthritikern, Tennisarm, steifer Hals, Hexenschuß, Ischias, Bandscheiben beschwerden, Arthrosen, Polyneuropathien, Neuritiden, Migräne, Neuralgien, Gürtelrose, Facialislähmung.Research has shown that such a drug can be used to treat the following diseases, namely rheumatic complaints, general joint and Muscle discomfort, root irritation syndromes of the spine, cervical spine Syndrome, shoulder-arm syndrome, symptoms of polyarthritics, Tennis elbow, stiff neck, lumbago, sciatica, intervertebral discs complaints, arthrosis, polyneuropathies, neuritis, migraines, Neuralgia, shingles, facial paralysis.
With all of these uses, this becomes in the ointment-shaped carrier ingested benfotiamine on the corresponding skin areas applied in the area of the diseased parts of the body and can its effect very quickly and specifically, usually after just a few Unfold minutes.
It is understood that the above and the following pending features to be explained not only in the given combination, but also in other combinations or can be used alone without the scope of the to leave the present invention.
The invention is selected below on the basis of a few Exemplary embodiments described and explained in more detail:
