Description
Die vorliegende Erfindung betrifft die Verwendung von substituierten
Phenylacetonitrilen zur Brechung von Resistenzen gegen Antibiotika.
Resistenzbildung gegen Chemotherapie, vor allem auch Mehrfachresistenz,
erweist sich immer wieder als ein großes medizinisches Problem. Bewährte
Arzneimittel können dadurch völlig nutzlos werden, und oft steht keine
befriedigende Alternative zur Verfügung. Die Wege der Resistenzbildung
sind noch weitgehend ungeklärt.
Die Beobachtung, daß Verapamil Resistenz in Krebszellen
(JP-US 83 624/1983) und Plasmodien (Proc. Nat. Acad. Sci. USA, 83, 7310
(1987), Science, 235, 899 (1987)) zu brechen vermag, hat daher in der
Onokologie rasch zu klinischen Anwendungen geführt. Hierbei erwies sich die
bekannte cardiovaskuläre Wirksamkeit des Verapamils als ein die praktische
Anwendbarkeit behindernder Nebeneffekt. Die erforderlichen hohen Dosen
setzen die Anwendung unter besonderen klinischen Kautelen (Intensivstation)
voraus bzw. verhindern sie überhaupt.
Es wurde nun gefunden, daß sich bestimmte substituierte Phenylacetonitrile
gut zur Resistenzbrechung von Antibiotika verwenden lassen.
Gegenstand der Erfindung ist die Verwendung von racemischen und optisch
aktiven substituierten Phenylacetonitrilen der Formel I
worin
R¹, R², R³ Wasserstoffatome, Halogenatome, C₁-C₄-Alkylgruppen, C₁-C₄-Alkoxygruppen, Trifluormethylgruppe oder zwei benachbarte Substituenten R¹ und R² oder R³ zusammen die Reste -CH₂-CH₂-CH₂-CH₂- oder -CH=CH-CH=CH- bedeuten,
R⁴ eine gesättigte oder ungesättigte Alkyl- oder Cycloalkylgruppe mit bis zu 15 C-Atomen oder eine Phenylgruppe darstellt,
R⁵ ein Wasserstoff oder eine C₁-C₄-Alkylgruppe ist, m und n jeweils die Zahlen 2, 3 oder 4 bedeuten und
R⁶ einen gesättigten oder ungesättigten Alkylrest mit bis zu 13 C-Atomen, einen Cyclohexylrest oder die Gruppe
R¹, R², R³ Wasserstoffatome, Halogenatome, C₁-C₄-Alkylgruppen, C₁-C₄-Alkoxygruppen, Trifluormethylgruppe oder zwei benachbarte Substituenten R¹ und R² oder R³ zusammen die Reste -CH₂-CH₂-CH₂-CH₂- oder -CH=CH-CH=CH- bedeuten,
R⁴ eine gesättigte oder ungesättigte Alkyl- oder Cycloalkylgruppe mit bis zu 15 C-Atomen oder eine Phenylgruppe darstellt,
R⁵ ein Wasserstoff oder eine C₁-C₄-Alkylgruppe ist, m und n jeweils die Zahlen 2, 3 oder 4 bedeuten und
R⁶ einen gesättigten oder ungesättigten Alkylrest mit bis zu 13 C-Atomen, einen Cyclohexylrest oder die Gruppe
bedeutet, worin R⁷, R⁸ und R⁹ gleich oder verschieden sind und Wasserstoffatome,
Halogenatome, C₁-C₄-Alkylgruppen, C₁-C₄-Alkoxygruppen,
Trifluormethylgruppen oder zwei benachbarte Substituenten R⁷ und R⁸ oder
R⁹ zusammen die Reste -CH₂-CH₂-CH₂-CH₂ oder -CH=CH-CH=CH- bedeuten sowie
deren Salzen mit physioligisch verträglichen Säuren zur Herstellung von
Arzneimitteln zur Brechung von Resistenzen gegen Antibiotika.
Die Verbindungen der Formel I eignen sich zur Brechung von Resistenzen von
allen Arten von Antibiotika, wie β-Lactam-Antibiotika - z. B. Penicilline
und Cephalosporine-, Tetracycline, Aminoglykoside, Chloramphenicol,
Thiamphenicol, Lincomycine und Makrolid-Antibiotika, Polypeptid-Antibiotika,
Chinolone, Nitroimidazole und Virustatika. Insbesondere sind
Tetrazykline zu nennen wie Tetrazyklin, Chlortetrazyklin, Demeclozyklin,
Doxyzyklin, Methazyklin, Minozyklin, Oxytetrazyklin und Rolitetrazyklin.
Der Ausdruck "Antibiotika" wie er hier verwendet wird, schließt Malariamittel
nicht ein.
Unter den Verbindungen der Formel I sind zur Brechung von Resistenzen gegen
Antibiotika folgende bevorzugt:
Noremopamil (=2-Isopropyl-5-phenethylamino-2-phenylvaleronitril) (A),
(+)-Devapamil (+)-5-(N-Methyl-N-3-methoxyphenylethyl)amino-2-(3,4-dimethoxyphenyl)--
2-isopropyl-valeronitril) (B),
Emopamil (=2-Isopropyl-5-(methylphenethylamino-(2-phenyl-valeronitril) (C) und seine Antipoden((+): D, (-): E), (+)-Gallopamil (F) und 5-[N-Methyl-N-(2-cyclohexylethyl)]amino-2-(3,4,5-trimethoxyphenyl)-2--isopropyl- valeronitril (G) und sein (+)-Antipode.
Emopamil (=2-Isopropyl-5-(methylphenethylamino-(2-phenyl-valeronitril) (C) und seine Antipoden((+): D, (-): E), (+)-Gallopamil (F) und 5-[N-Methyl-N-(2-cyclohexylethyl)]amino-2-(3,4,5-trimethoxyphenyl)-2--isopropyl- valeronitril (G) und sein (+)-Antipode.
Die oben genannten Substanzen können gewünschtenfalls in Form ihrer Salze
mit physiologischen Säuren bzw. Basen vorliegen. Als physiologisch
verträgliche Säuren kommen vorzugsweise in Betracht: Salzsäure,
Schwefelsäure, Phosphorsäure, Essigsäure, Zitronensäure, Malonsäure,
Salicylsäure, Maleinsäure, Fumarsäure, Bernsteinsäure, Ascorbinsäure,
Äpfelsäure, Methansulfonsäure, Milchsäure, Gluconsäure, Glucuronsäure,
Amidosulfonsäure, Benzoesäure, Weinsäure.
Als physiologisch verträgliche Basen eignen sich insbesondere Ammoniak,
Alkalimetallhydroxide - insbesondere des Natriums, Kaliums und Lithiums-,
Erdalkalimetallhydroxide - besonders des Calciums und Magnesiums-, sowie
organische Basen wie niedere Alkylamine - z. B. Methylamin oder Ethylamin-,
Cyclohexylamin, substituierte niedere Alkylamine - z. B. Diethanolamin,
Triethanolamin, Tris-(hydroxymethyl)-aminomethan-, Piperidin oder
Morpholin.
Die Verbindungen der Formel I können zusammen mit oder getrennt von den
Wirkstoffen verabfolgt werden. Bevorzugt ist jedoch die gleichzeitige
Applikation. Die Produkte werden in der Regel oral oder parenteral (z. B.
i. v., i. p.) gegeben.
Das Verhältnis von Verbindung I zu Wirkstoff hängt von der zu behandelnden
Krankheit, dem Krankheitszustand des Patienten und dem verwendeten Wirkstoff
ab. In der Regel beträgt das Verhältnis etwa 1 : 10 bis 10 : 1. Dabei
werden die Verbindungen I in der Regel in einer Menge von 50 bis 1000 mg
pro Patient und Tag bei oraler Gabe und 5 bis 100 mg bei intravenöser bzw.
10 bis 200 mg intraperitoneale Gabe pro Patient und Tag angewendet.
Die Wirkstoffe werden in der Menge appliziert, die auch für die alleinige
Gabe dieser Substanzen vorgesehen ist und die z. B. aus der "Rote
Liste 1988" bzw. den darin erwähnten wissenschaftlichen Prospekten
entnommen werden kann.
Die Substanzen können in Form von Tabletten, Kapseln und Dragees zur
oralen Applikation oder als Injektionslösung zur parenteralen (i. v., i. p.,
i. m.) Applikation vorliegen. Lösungen können auch infundiert werden. Die
Herstellung der Applikationsformen geschieht in bekannter Weise nach
üblichen Methoden.
Die resistenzbrechenden Eigenschaften der beanspruchten Verbindungen
gegenüber bakteriellen Erregern lassen sich wie folgt zeigen:
In einem standardisierten Nährmedium wird ein gegen Tetracyclin resistenter
Stamm von E-Coli mit Tetracyclin und mit einer Kombination von Tetracyclin
und Testverbindung behandelt. Die Zahl der Keime wird dabei während
der experimentellen Wachstumsphase bestimmt. Während Tetracyclin alleine
oder die Testverbindungen alleine keine Reduktion der Vermehrungsgeschwindigkeit
herbeiführen, gelingt dies mit der Kombination beider
Stoffe.
Ein anderes Maß für den Synergismus gegenüber resistenten E. Coli
Bakterien ist in den minimalen Hemmkonzentrationen zu sehen:
Es werden Kapseln mit folgenden Wirkstoffen hergestellt:
- A. 300 mg Tetrazyklin-Hydrochlorid
200 mg (+)-Gallopamil (F) - B. 250 mg Tetrazyklin-Hydrochlorid
100 mg G - C. 100 mg Doxyzyklin
10 mg (+)-Devapamil
Claims (1)
- Verwendung von racemischen und optisch aktiven substituierten Phenyl acetonitrilen der Formel I worin
R¹, R², R³ gleich oder verschieden sind und Wasserstoffatome, Halogenatome, C₁-C₄-Alkylgruppen, C₁-C₄-Alkoxygruppen, Trifluormethylgruppe oder zwei benachbarte Substituenten R¹ und R² oder R³ zusammen die Reste -CH₂-CH₂-CH₂-CH₂- oder -CH=CH-CH=CH- bedeuten,
R⁴ eine gesättigte oder ungesättigte Alkyl- oder Cycloalkylgruppe mit bis zu 15 C-Atomen oder eine Phenylgruppe darstellt,
R⁵ ein Wasserstoff oder eine C₁-C₄-Alkylgruppe ist,
m und n jewweils die Zahlen 2, 3 oder 4 bedeuten und
R⁶ einen gesättigten oder ungesättigten Alkylrest mit bis zu 13 C-Atomen, einen Cyclohexylrest oder die Gruppe bedeutet, worin R⁷, R⁸ und R⁹ gleich oder verschieden sind und Wasser stoffatome, Halogenatome, C₁-C₄-Alkylgruppen, C₁-C₄-Alkoxygruppen, Trifluormethylgruppen oder zwei benachbarte Substituenten R⁷ und R⁸ oder R⁹ zusammen die Reste -CH₂-CH₂-CH₂-CH₂ oder -CH=CH-CH= bedeuten, sowie von deren Salzen mit physiologisch verträglichen Säuren zur Herstellung von Arzneimitteln zur Brechung von Resistenzen gegen Antibiotika.
