Künstliche Gelenkpfanne
Die Erfindung betrifft eine künstliche Gelenkpfanne einer Totalendoprothese
für ein Hüftgelenk.
Ein bei Gelenkendoprothesen bisher im allgemeinen noch nicht gelöstes
Problem ist der Verschleiss an den aufeinander gleitenden Oberflächen der Prothese; vor allem bei dem hochbelasteten Gleiten
des kugelförmigen Gelenkkopfes in einer Gelenkpfanne einer Totalprothese für ein Hüftgelenk versucht man daher, mit Hilfe von
Körperflüssigkeit eine Flüssigkeitsschmierung zwischen den aufeinander
gleitenden Flächen der genannten Elemente zu erreichen.
In diesem Zusammenhang liegt der Erfindung die Aufgabe zugrunde, diese Flüssigkeitsschmierung, die wegen der hohen Flächenpressungen
und der engen zwischen der genannten Flächen verbleibenden Spalte bisher sehr oft ungenügend ist, zu verbessern.
Diese Aufgabe wird erfindungsgemäss dadurch gelöst, dass die
konkave, den kugelförmigen Gelenkkopf aufnehmende Schalenfläche mindestens eine vertiefte Nut aufweist. Die erfindungsgemässe Nut
ermöglicht- vor allem wenn in bekannter Weise der Durchmesser der Pfanne geringfügig grosser ist als derjenige des Gelenkkopfes -,
dass die bei einer Belastung des Gelenks auf der einen Seite der Gelenkpfanne verdrängte Flüssigkeit sich in der Nut sammelt, so

dass bei Nachlassen des Druckes zwischen den aufeinander gepressten
Flächenteilen die gesammelte Flüssigkeit eine gute Benetzung dieser Flächen unter Ausbildung eines ausreichenden
Schmierfilmes bewirkt, wobei aus dem bei Entlastung kleiner
werdenden Volumen auf der nicht belasteten Seite des Gelenkes zusätzlich Flüssigkeit durch die Nut zwischen die zuvor aufeinander
gepressten Gleitflächenteile verdrängt wird. Die erfindungsgemässe
Nut kann dabei beliebig - beispielsweise als Ring- oder Schraubennut - ausgebildet sein, sofern darauf geachtet
wird, dass ihre freien Enden innerhalb der durch den Gelenkkopf bei Belastungen abgedichteten hohlkugelartigen
Schalenflächen liegen.
Die erfindungsgemässe Schmierwirkung kann verbessert werden, wenn die konkave Schalenfläche eine Rotationsfläche ist, deren
erzeugendes Kreisbogenstück einen Radius hat, der demjenigen des zugeordneten Gelenkkopfes entspricht, dessen Mittelpunkt
jedoch gegenüber der Rotationsachse einen Abstand aufweist, wobei der Abstand zweckmässlgerweise 0,1 bis 0,5 mm betragen
kann. Eine weitere Verbesserung der Schmierung ergibt sich zusätzlich, wenn die Schale im Bereich ihres Scheitelpunktes eine
Tasche aufweist.
Im folgenden wird die Erfindung an Hand eines Ausfuhrungsbeispiels
im Zusammenhang mit der Zeichnung näher erläutert, deren einzige Figur eine Gelenkpfanne, in die der zugehörige Gelenkkopf unter
einer angenommenen Belastung aus der Richtung des Pfeiles von links oben eingezeichnet ist, im Schnitt darstellt.

D^e vergrössert dargestellte Gelenkpfanne 1, die vorteilhafterweise
aus hochmolekularem Hartpolyäthylen hergestellt ist, hat auf ihrer konkaven Aussenseite ringartige Vorsprünge 2 und Vertiefungen
3, mit denen ihre Verankerung im Beckenknochen verbessert werden soll; diese Verankerung erfolgt vorzugsweise mit
Hilfe eines bekannten Knochenzements auf Methylmethacrylat-Basis.
Am Aequator endet die halbkugelartige Schale 1 in einen Flansch 4,
der gegen die konkave, innere Schalenfläche 5 abgeschrägt ist.
Die Schale 5 dient zur Aufnahme eines kugelartigen Gelenkkopfes einer nicht weiter dargestellten, im Femurknochen verankerbaren
HUftgelenkprothese; diese ist vorzugsweise aus einem der in der
Implantattechnik verwendeten Metalle oder einer Metallegierung gefertigt, wobei der Kopf jedoch auch aus sogenannter Biokeramik
- beispielsweise aus sehr reinem Aluminiumoxid (Al9O.,) - bestehen
kann. In der Zeichnung ist die relative Lage der Pfanne 1 zum Gelenkkopf 6 so dargestellt, wie sie sich unter der Wirkung einer
in Richtung des Pfeiles 8 wirkenden Belastung einstellt.
Erfindungsgemäss ist in der konkaven Schalenfläche 5 eine Nut 7
vorgesehen, die in dem gezeigten Beispiel als Ringnut ausgebildet ist.
Nach einem weiteren Gedanken der Erfindung ist die Schalenfläche nicht eine Hohlkugelfläche, sondern stellt eine Rotationsfläche
dar, die durch Rotation eines Viertelkreisbogens um die Rotationsachse R entsteht, wobei der Kreisbogen den gleichen Radius wie
der zugehörige Gelenkkopf 6 hat, sein Mittelpunkt M jedoch um einen

geringen Abstand, der etwa 0,1 bis 0,5 mm beträgt, gegen die
Rotationsachse R versetzt ist. Auf diese Weise erhält der Gelenkkopf 6 bei Be- und Entlastungen in der Pfanne mehr Spielraum, ohne
dass dabei die bei gleichen Durchmessern von Kopf und Pfanne relativ grossen - relativ zu. denjenigen bei den bekannten Pfannen,·
die einen grösseren Durchmesser als der Gelenköpf haben - tragenden
Teilflächen verkleinert werden. Selbstverständlich ist jedoch eine erfindungsgemasse Nut 7 auch bei den bekannten, eine Hohlkugelfläche
darstellenden Pfannen mit gegenüber dem Gelenkkopf 6 vergrössertem Durchmesser anwendbar.
Eine weitere Teilmenge von Flüssigkeit kann bei Belastung des Gelenkes in eine Tasche 9, die im Scheitelbereich der Schale 5
angeordnet ist, verdrängt und dort gespeichert werden und bei Entlastung zusätzlich als Schmierfilm auf die aufeinander gleitenden
Oberflächen ausfliessen.
Die Herstellung der neuartigen Schalenfläche erfolgt beispielsweise
durch spanabhebende Bearbeitung des rotierenden Pfannenkörpers 1 mit einem auf einer entsprechenden Kurve bewegten
Werkzeug oder mit einem solchen, das auf einem Kreisbogen geführt und gegenüber der Rotationsachse des Körpers 1 versetzt
angeordnet ist.
Wie schon kurz angedeutet, dient die erfindungsgemasse Nut 7 beim
Belasten des Gelenks als Auffang- und Sammelgefäss für verdrängte Körperflüssigkeit, die dann beim Entlasten diese Flüssigkeit wieder
zwischen die Gleitflächen abgibt; die gleichzeitige Verbindung mit

einem sich beim Entlasten verkleinernden Volumen auf der zuvor entlasteten Seite des Gelenks bewirkt darüberhinaus, dass durch
die Nut zusätzlich Flüssigkeit in den Bereich zwischen die beim Belasten aufeinander gleitenden Flächen gelangt.

Claims (4)
1.^Künstliche Gelenkpfanne einer Totalendoprothese für ein
Hüftgelenk, dadurch gekennzeichnet, dass die konkave, den kugelförmigen Gelenkkopf (6) aufnehmende Schalenfläche (5)
mindestens eine vertiefte Nut (7) aufweist.
2. Gelenkpfanne nach Anspruch 1, dadurch gekennzeichnet, dass die konkave Schalenfläche (5) eine Rotationsfläche ist, deren
erzeugendes Kreisbogenstück einen Radius hat, der demjenigen
des zugeordneten Gelenkkopfes (6) entspricht, dessen Mittelpunkt (M) jedoch gegenüber der Rotationsachse (R) einen Abstand aufweist.
3. Gelenkpfanne nach Anspruch 2, dadurch gekennzeichnet, dass der Abstand 0,1 - 0,5 mm beträgt.
4. Gelenkpfanne nach Anspruch 1, dadurch gekennzeichnet, dass die Schale (5) im Bereich ihres Scheitelpunktes eine Tasche (9)
aufweist.
