Description

Die Erfindung betrifft eine Hüftgelenkkopf-Prothese, bestehend aus einem vorzugsweise mit einem Halsansatz versehenen Kugeikopfteil mit einer Bohrung und einem Schenkelschaftteil mil eil-ίπ der Bohrung des Kugelkopfteils angepaßten Zapfen, auf den der Kugelkopfieil aufsteckbar ist, und wobei die Achse der Bohrung im Kugelkopfteil von der Durchmesserlinie abweicht. .JO
Eine derartige Hüftgelenkkopf-Prothese ist bekannt aus der CH-PS 5 07 704. bei welcher der Zapfen, auf welchen der Kugelkopfteil aufsteckbar ist. radial vorstehende Rippen aufweist, welche den aufgesteckten Kugelkopfteil gegen Verdrehung sichern. Im Kugel· kopfteil bzw. in einem eventuell vorhandenen, besonderen Halsteil sind dabei radiale Nuten einzuarbeiten, in welche die Rippen beim Aufsetzen des Kugelkopfteils «intreten können, was zu einer umständlichen Bearbeitung führt. Aber auch die Herstellung und insbesondere genaue Bearbeitung des Zapfens ist durch die an ihm vorstehenden radialen Rippen erheblich erschwert. Das ist ganz besonders der Fall, wenn am Zapfen mehrere Rippen und im Kugelkopfteil mehrere Nuten vorgesehen sein sollten, um eine Verstellung in mehrere Lagen vornehmen zu können. Bei der bekannten Hüftgelenkkopf-Prothese sind daher nur zwei diametral liegende Rippen bzw. Nuten vorgesehen, was nur gestattet, den Kugelkopfteil in zwei um 180' versetzte Lagen aufzusetzen.
Es ist das ZieJ der Erfindung, die Herstellung der Prothese erheblich zu vereinfachen und damit auch die Möglichkeit für weitgehendere Einstellmöglichkeiten zu schaffen. Die Hüftgelenkkopf-Prothese ist erfindungsgemäß dadurch gekennzeichnet, daß die zur Boh- f>.s rung senkrechte Auflagefläche des Kugelkopftcils Nokken aufweist und der Bund des Schaftteile entsprechende Vertiefungen. Damit fallen Rippen am Zapfen bzw. Nuten im Bereiche der Bohrung des Kugelkapfe«, weg, so daß die genaue Bearbeitung dieser vorzugsweise konischen Flachen keinerlei Probleme bietet. Die Orientierungsmittel sind ausschließlich an die Stirnseiten des Kugelkopfteils bzw. des Bundes des Schaftteils verlegt, wo ihre Bearbeitung einfach ist. Die Nocken und Vertiefuneen können z. B. kreuzweise angeordnet sein, deraa daß der Kugelkopfteil um je 90 versetzt auf den Bund des Schaftteils aufgesetzt werden kann, was vielseitige Einstellmöglichkeiten mit sich bringt.
Die Zeichnung zeigt ein Ausführungsbeispiel der erfindungsgemäßen Hüftgelenkkopf- Prothe-e.

Fig.4a, 4b, 5a und 5b zeigen Kugelkopfrichtungen lateral und medial in Seitenansicht, sowie anterior und posterior in Aufsicht.
In F i g. I erkennt man das Becken 13 und einen Oberschenkelknochen 14, in dessen Medullarhöhle 56 ein Prothesenschaft 16 eingesetzt ist. Wie ersichtlich, liegt er mit seiner Auflagefläche 18 des Hundes 20 auf dem Femurhalsrest 22 auf. Die Zeichnung zeigt das rechte Hüftgelenk, bei dem der Kugelkopfteil 37 direkt in die natürliche Gelenkpfanne 25 hineinragt und mit dieser zusamme·!.arbeitet.
Aus F i g. 2 und 3 ersieht man, wie ein erfindungsgemäßer Schenkelhalsschaft 16 im Detail aufgebaut sein soll. Es ist dabei zu vermerken, daß die Lange des Schaftes 16 den Skelettverhältnissen sowie den Kräfteverhältnissen entsprechend angepaßt werden kann. Um den Schaft 16 vor ungewolltem Absinken in den Oberschenkelknochen 14 (F i g. 1) zu schützen, ist die Auflagefläche 18 an der medialen Seite sehr groß ausgebildet. Durch die seitliche Verstärkung 27 am oberen Teil wird ein Verdrehen im Oberschenkelknochen 14 (F i g. I) verhindert. Der Bund 20 weist in seinem hinteren Teil eine Bohrung 28 am', die dazu dient, den eventuell abgetrennten Trochonter major mittels Cerclagedraht wieder zu fixieren. Ebenso kann bei einer eventuellen Infektion das Ausziehgerät in dieser Bohrung eingehängt werden. Der Bund 20 weist auf seiner dem Schaft 16 und der Auflagefläche 18 gegenüberliegenden Seite vier Vertiefungen 29 auf. die in F i g. 2 teils in Seitenansicht und in F i g. 3 in Aufsicht gezeigt sind. Um den Kugelkopfteil 38 aufnehmen zu können, weist der Schenkelschaft 16 im Schnittpunkt der kreuzweise angeordneten Vertiefungen 29 einen konischen Zapfen 30 auf, dessen Achse 58 mit der Stielachse im Normalfall einen Winkel von 126' bildet. Die Oberfläche des Schaftes 16 kann glatt sein, wie in den F i g. 1, 3. 4a, :5a und 5b gezeigt, oder sie kann gemäß F i g. 2 mit Zähnen 39 versehen sein.
Der Kugeikopfteil 37 sveist einen asymmetrischen Halsansaiz 66 und eine dezentrierte Bohrung -57 auf. deren Achse 68 mit der Durchmesserlinic 69 des Kugelkopfteil* Yl einen Winkel bildet, der zwischen 5 und 15" betragen kann. An der dazu senkrechten Auflagefläche des Halsansatzes 66 sind kreuzweise angeordnete Nocken 38 vorgesehen, die bei aufgesetztem Kugeikopfteil in die Vertiefungen 29 eingreifen.
Aus F i g. 4a, 4b, 5a und 5b sind verschiedene Grundrichtungen der Hüftgelenkachsen ersichtlich, die allesamt mit der dargestellten Prothese berücksichtigt werden können, ohne daß auch nur ein einzementierter

Prothesenieil ausgewechselt werden müßte. Die Riehlung lateral, Fig. 4u, wird erreicht, wenn der Kugelkopfteil 37 so auf den Zapfen 30 aufgesteckt wird, daß der Kugelmittelpunkt 61 oberhalb der Zapfenachse 68 Regt. Die Richtung medial wird demgegenüber erzielt, wenn derselbe Kugelkopfteil 37 so auf den Zapfen 30 aufgesteckt wird, daß der Kugelmittelpunkt 61 unterhalb der Zapfenachse 68 liegt (F i g. 5a). Die Richtungen anterior und posterior werden wiederum mit demselben Kugelkopfteil 37 erreicht, wenn dieser, wie aus F i g. 4b und 5b hervorgeht, so auf den konischen Zapfen 30 aufgesteckt wird, daß dar Kugelmittelpunkt 61 hinter bzw. vor der Zapfenachse 58 liegt. Diese vier Grundrichtungen werden durch die Lage der Nocken 38 des Kugelkopfteils 37 sowie die Lage der entsprechenden Vertiefungen 29 des Bundes 20 genau festgelegt.

Claims (4)

Patentansprüche:
1. Hüftgelenkkopf-Prothese, bestehend aus einem vorzugsweise mit einem Halsansatz versehenen Kugelkopfteil mit einer Bohrung und einem Schenkelschaftteil mit einem der Bohrung des Kugelkopfteils angepaßten Zapfen, auf den der Kugelkopfteil aufsteckbar ist, wobei die Achse der Bohrung im Kugelkopfteil von der Durchmesserlinie abweicht, dadurch gekennzeichnet, daß die zur Bohrung (67) senkrechte Auflagefläche des Kugelkopfteils (37) Nocken (38) aufweist und der Bund (20) des Schaftteils (16) entsprechende Vertiefungen (29).
2. Prothese nach Anspruch 1, dadurch gekennzeichnet, daß im Kugelkopf (37) der Bohrung (67) die Achse (68) um 5 bis 15° von der Durchmesserlinie (69) abweicht.
3. Prothese nach Anspruch 1, dadurch gekennzeichnet, daß die Auflagefläche des Kugelkopfes
(37) vier gleichmäßig verteilt angeordnete Nocken
(38) und der Bund (20) des Schaftteils (40) vier entsprechend angeordnete Vertiefungen (29) aufweist.
4. Prothese nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß der Halsansatz (66) des Kugelkopfes (37) gegenüber der Kugelform asymmetrisch ist.
