Description
Die Erfindung betrifft ein Knochenimplantat mit stift
artigen Befestigungsmitteln zur Verankerung der Tibia-
und Femur-Komponenten bei Kniegelenkendoprothesen
gemäß dem Oberbegriff des Anspruchs 1.
Derartige Knochenimplantate sind mit Bohrungen bekannt, durch
welche von der Implantseite her Schrauben in das Knochengewebe des
Patienten zur Verankerung des Implantates geschraubt werden können.
Dabei ist nachteilig, daß die als Schrauben ausgebildeten Befestigungs
mittel direkte Verbindung zur Kunststoffschale behalten und die abge
riebenen Kunststoffpartikel entlang der Schraube in den Knochen
eindringen können und Entzündungen und vorzeitige Lockerung
hervorrufen, so daß eine erneute Operation zu einem unerwünscht
frühen Zeitpunkt erforderlich wird.
Bei einer Revisionsoperation mit Knochendefekt, die eine Knochen
transplantation erforderlich macht, ist das Einsetzen von Schrauben
von Nachteil, da dadurch die Inkorporation des Knochentransplantates
negativ beeinflußt wird.
Die Anordnung fest an das Implantat angeformter Stifte entspricht
nicht den individuell vorliegenden Gegebenheiten des einzelnen
Patienten, so daß die Stifte nicht individuell dort geordnet werden
können, wo die Knochensubstanz bei, Patienten zur Verankerung einer
Prothese am geeignetsten ist. Derartige Anordnungen sind
bekannt aus: US 4 778 473, US 4 659 331, DE 693 05 434 T2 (EP 0 577 529 B1), EP 0 013 864 A1, FR 2 691 355 A1.
Da aber der Knochen einen individuellen Aufbau aufweist, ist es notwendig, daß
an verschiedenen Stellen verschieden lange Stifte in nur während der
Operation ermittelte Punkte eingesetzt werden.
Der Erfindung liegt die Aufgabe zugrunde, ein gattungsgemäßes
Knochenimplantat dahingehend zu verbessern, daß es während der
Operation an den jeweiligen Patienten individuell angepaßt und
befestigbar ist und während der Benutzung durch den Patienten keinen,
oder einen erheblich reduzierten Verschleiß aufweist.
Dieser der Erfindung zugrundeliegenden Aufgabe wird durch die
Ausgestaltung eines Knochenimplantates gemäß den Merkmalen des
Anspruches 1 gelöst.
Die Erfindung schlägt mit anderen Worten vor, keine fest an das
Implantat angeordneten Stifte vorzusehen, sondern von außen in das
Implantat einsetzbare Stifte, die individuell so positioniert werden
können, wie es bei der jeweils vorliegenden Knochenstruktur am ge
eignetsten ist. Weiterhin wird ein Eindringen der Stifte in den
Gelenkraum durch die Anschläge oder durch Vermeidung von
Verbindung zur Kunsstoffschale ausgeschlossen, so daß kein Abrieb der
innen angeordneten Bauteile durch zu weit nach innen ragende Stifte
verursacht werden kann und kein Abwandern der Abriebpartikel
entlang der Stifte möglich ist.
Vorteilhaft können die Ausnehmungen als achsparalelle Bohrungen
ausgebildet sein. Hierdurch wird eine preiswerte Herstellung der Im
plantate ermöglicht. Zudem wird hierdurch die Handhabung
vereinfacht gegenüber einer denkbaren Lösung, bei der die
Ausnehmungen selbst nicht achsparallel sind und bei denen jeweils
speziell geformte Stifte verwendet werden müssen, um insgesamt eine
achsparalelle Anordnung der in dem Implantat eingesetzten Stifte zu
erzielen.
Ein einfache Montage der Stifte kann dadurch erfolgen, daß die
Bohrungen als zum Inneren des Implantates konisch zulaufende
Bohrungen ausgebildet sind, in welche entsprechend konisch und
gewindelos ausgeformte Abschnitte der Stifte, beim Bedarf in
verschiedener Länge, eingepreßt werden.
Der Konussitz bewirkt einerseits eine sichere Festlegung der Stifte
beim Einsetzen, wobei die die Verbindung zwischen Stift und
Implantat durch den beim Einsetzen des Implantates auftretenden
Gegendruck verbessert wird. Ein derartiger Gegendruck wird in der
Praxis beispielsweise dadurch erzielt, daß die Bohrlöcher für die Stifte
so bemessen sind, daß die Stifte nur unter Aufbringung eines gewissen
Druckes in die Bohrlöcher eingeschoben werden können. Da das
Implantat auf Druck aufgebaut ist, wird die Fixierung der Stifte in
Konusform nach bekannten Stand der Technik eine sichere Fixierung
zwischen Knochen, Stifte und Implantat erlauben.
Eine Konusverbindung zwischen Stift und dem Implantat stellt zudem
gegenüber einem Gewinde sicher, daß keine reibende oder schleifende
Bewegung zwischen Stift und dem Implantat bei der Operationsvorbe
reitung nötig ist und daß hierdurch die Möglichkeit eines Metallab
riebes ausgeschlossen werden kann.
Vorteilhaft könnte sein, wenn die Stifte unterschiedliche Längen
aufweisen. Auf diese Weise ist eine individuellere Ausgestaltung des
Implantates möglich, bei der die Stifte nach ihrer Länge ausgewählt und
dort eingesetzt werden, wo sie die optimale Fixation im Knochen der
Patienten finden.
Der Vorteil dieser variablen Technik liegt besonders bei der Revision-
Arthroplastik des Kniegelenkes, wobei häufig wegen vorhandenen
Knochendefekten eine aus der Knochenbank eingesetzter homologer
Knochenblock als Ersatz für die verlorengegangene Knochenstruktur
Verwendung findet.
Dabei ist es besonders wichtig, die Lage und Dicke des
Knochenimplantats bei der Auswahl der Stiftlänge und Lage zu
berücksichtigen. Da dies aber nicht vorhersehbar ist, und
festangebrachte Stifte bzw. Befestigungsmittel eine fallbedingte und
individuelle Notwendigkeit nicht berücksichtigen können, wird dieses
variable System eine fallgerechte und optimale Fixierung ermöglichen.
Vorteilhaft kann die Anzahl der Ausnehmungen des Implantates größer
sein als die Anzahl der Stifte die an einem Implantat verwendet
werden. Implantate mit sehr vielen Stiften auszustatten oder - dem
derzeitigen Stand der Technik - mit einer großen Anzahl von Schrauben
ist nachteilig, da hierdurch das Implantat tragende Knochengewebe
geschwächt werden kann. Dennoch ist es vorteilhaft, die Anzahl der
Ausnehmungen größer vorzusehen als die Anzahl der
erwartungsgemäß zu verwendenden Stifte, da hierdurch die
Anpaßbarkeit und Individualisierbarkeit des Knochenimplantates
vergrößert wird, in dem die Befestigungsmittel dort angeordnet werden
können, wo sie in jedem Einzelfall die optimale Halterung des
Knochenimplantates ermöglichen.
Schließlich ist erfindungsgemäß vorteilhaft, daß die Stifte in der
Richtung angeordnet sind, die der Einsetzrichtung des
Knochenimplantates entspricht. Auf diese Weise ist es möglich, in der
beschriebenen Weise zunächst das Implantat mit den Stiften zu
bestücken und dann in den Knochen der Patienten zu implantieren.
Die Lage der Stifte wird mit eine Schablone und anhand von Röntgen
aufnahmen ermittelt. Die Schablone wird mit zwei Drähten im Knochen
fixiert. Nach Anbohren an gewünschten Stellen und Ermitteln der
gewünschten Stiftslänge wird die Schablone entfernt, wobei die
Fixationsdrähte im Knochen verbleiben. Das so mit Stiften bestückte
Implantat wird über den im Knochen liegenden Draht exakt zu
vorbereiteten Bohrkanälen geführt.
Würde die Längsrichtung der Stifte von der Einsetzrichtung des
Implantates abweichen, müßte zunächst das Implantat eingesetzt
werden, und anschließend von Innen die Stifte durch das Implantat
eingeführt werden, wobei beispielsweise federelastische Rastmittel
erforderlich wären, um die Anschläge zu bilden, die eine ins Innere des
Implantates gerichtete Bewegung der Stifte abstützen und auffangen
würden.
Ausführungsbeispiele der Erfindung werden anhand der Zeichnungen
im folgenden näher erläutert. Dabei zeigt

In Fig. 1 ist ein Tibiaimplantat gezeichnet, das im Kniegelenk eines
Patienten eingesetzt werden kann. Das Implantat weist mehrere
achsparalelle und konisch verlaufende Bohrungen auf, in welche Be
festigungsmittel eingesetzt werden können. Wie aus der Abbildung
ersichtlich ist, sind die Befestigungsmittel als Stifte ausgebildet.
Die Stifte sind insgesamt gewindelos ausgebildet und weisen einen
konischen unteren Bereich auf, mit dem sie in die Bohrungen des
Implantates eingesetzt werden können.
In Anpassung an die jeweils bei jedem einzelnen Patienten
vorliegenden Verhältnisse sind verschieden lange Stifte ausgewählt, so
daß die Bestückung der Bohrungen mit den verschiedensten Stiften in
Abhängigkeit von den jeweils beim Patienten vorherrschenden
Verhältnissen erfolgen können. Die Tiefe einer jeweils eingebrachte
Bohrung kann mit Hilfe aus dem Stand der Technik bekannter
Instrumente überprüft und sichergestellt werden.
Insgesamt ist die Dauer, die zur Vorbereitung des Einsetzens und zum
Einsetzen des Implantates in das Kniegelenk der Patienten erforderlich
ist, relativ kurz und für den Patienten dementsprechend schonend.
Bei eingesetztem Implantat im Knochen des Patienten ist ebenfalls eine
Schonung für den Patienten dadurch erzielt, daß die konische Ausge
staltung der Bohrungen verhindert, daß die Stifte ins Innere des
Implantates gelangen können, auch wenn sich die Stifte im Knochen
des Patienten lockeren sollten. In jedem Fall ist ausgeschlossen, daß
die Stifte ins Innere des Gelenkes gelangen können oder dorthin zu
einem Abrieb an der Kunststoff- oder Metallschale führen können die
als Gleitschale im Gelenkinneren vorgesehen sein kann. Da auf diese
Weise vermehrte Abriebpartikel verhindert werden die zu lokalen Ent
zündungen führen können, wobei durch die Entzündungen das
Knochengewebe angegriffen werden kann, wird durch die
erfindungsgemäße Versorgung nicht nur beim Einsetzen selbst,
sondern auch beim späteren Tragen eine Schonung für den Patienten
ermöglicht.
Bei der Entnahme des Knochenimplantates werden entweder die Stifte
zusammen mit dem Implantat aus dem Knochen herausgezogen wenn
aufgrund des Konussitzes eine feste Verbindung zwischen den Stiften
und dem Implantat besteht, oder die Stifte verbleiben in der Knochen
substanz, wenn dort die Haltekräfte größer sind als im Bereich der
Bohrungen. In diesem Fall können die frei in den Knochenraum
ragenden konischen Bereiche der Stift mit einer geeigneten Zange
erfaßt und entfernt werden.
Claims (6)
1. Knochenimplantat mit stiftartigen Befestigungsmitteln zur
Verankerung der Tibia- und Femur-Komponente bei Kniegelenkendo
prothesen, wobei sowohl Tibia- als auch Femur-Komponente Aus
nehmungen zur Aufnahme der Befestigungsmittel aufweisen, und
wobei die Ausnehmungen für eine achsparalelle Anordnung der
Befestigungsmittel ausgebildet sind, dadurch gekennzeichnet, daß die
Ausnehmungen Anschläge ihr eine zum Inneren des Implantats
gerichtete Bewegung der Befestigungsmittel aufweisen und daß die
Befestigungsmittel in dem Bereich gewindelos ausgebildet sind,
welcher in den Knochen einführbar ist.
2. Knochenimplantat nach Anspruch 1, dadurch gekennzeichnet, daß die
Ausnehmungen als achsparallele Bohrungen ausgebildet sind.
3. Knochenimplantat nach Anspruch 2, dadurch gekennzeichnet, daß die
Bohrungen zum Inneren des Implantates konisch zulaufen und daß
die Stifte in dem Bereich konisch ausgebildet sind, der in das
Implantat einführbar ist.
4. Knochenimplantat nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet, daß die Stifte unterschiedliche Längen
aufweisen.
5. Knochenimplantat nach einem der vorhergehende Ansprüche, dadurch
gekennzeichnet, daß die Anzahl der Ausnehmungen größer ist als die
Anzahl der am Implantat befestigten Stifte.
6. Knochenimplantat nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet, daß die Längsachsen der Ausnehmungen und
der Stift achsparallel zur Einsetzrichtung in dem Knochen verlaufen.
