Abstract

Modulare Hüftgelenkprothese mit keilförmigem, im Markkanal fixiertem Prothesenschaft und dreidimensional verstellbarem Gelenkkopf (1) mit Zugankerfixierung am Prothesenschaft (2). Zur Befestigung des Gelenkkopfs (1) dient eine im Schafthalszapfen (3) gelenkseitig eingesetzte Spannschraube (5) mit konischem Kopf, welche an ihrem gegenüberliegenden Ende ein Gewinde aufweist, in welches eine Ankermutter (7) so eingreift, dass durch ein von außen eingeführtes Schraubwerkzeug eine Zugankerwirkung auf die Spannschraube (5) erzeugt werden kann, wodurch sich ein spannzangenartiger Klemmsitz zwischen Gelenkkopf (1) und Schafthalszapfen (3) erreichen lässt.

Description

  • Die Erfindung betrifft eine Vorrichtung entsprechend dem Oberbegriff des Anspruchs 1.
  • Die erfindungsgemäße Konstruktion zeigt eine modulare Hüftgelenkprothese mit verstellbarem Gelenkkopf.
  • Modulare Hüftgelenkprothesen mit vordefinierter, starrer Gelenkkopfstellung oder verstellbarem Gelenkkopf sind in unterschiedlicher Ausführung bekannt und dienen im Falle defekter natürlicher Hüftgelenke als Ersatzgelenk zum orthopädischen Einsatz bei Menschen und Tieren.
  • Dabei wird der Oberschenkelknochen unterhalb des Gelenkkopfs durchtrennt und der Prothesenschaft in den geöffneten Markkanal des Oberschenkelknochens eingesetzt. Da der Prothesenschaft je nach Zustand der Knochensubstanz mehr oder weniger tief in den Markkanal eindringt, um eine ausreichende Verbindung mit dem Femur zu erreichen, ist es sinnvoll den Gelenkkopf, welcher auf einen seitwärts auskragenden Zapfen des Prothesenschafts aufmontiert wird, durch entsprechende Verstellbarkeit in Relation zum Prothesenschaft in die orthopädisch günstigste Position zur Gelenkpfanne des Beckens bringen zu können.
  • Aufgabe der Erfindung ist es, eine modulare Prothese bestehend aus Prothesenschaft (2), Gelenkkopf (1), Spannschraube (5) und Ankermutter (3) zur Verfügung zu stellen, welche es ermöglicht, den Gelenkkopf (1) sowohl axial als auch radial zu verstellen und in der geeigneten Position durch einfachen Schraubvorgang sicher zu fixieren, d.h. es geht grundsätzlich um die künstliche Wiederherstellung des Gelenkrotationszentrums anhand einer Prothesenkonstruktion, die es erlaubt, den geometrischen Gelenkdrehpunkt wieder möglichst genau den natürlichen orthopädischen Vorgaben des jeweiligen Patienten anzupassen.
  • Die Aufgabe wird durch eine modulare Hüftgelenkprothese mit den Merkmalen des Anspruchs 1 gelöst.
  • Die erfindungsgemäße modulare Hüftgelenkprothese besteht im Wesentlichen aus dem Prothesenschaft (2), dem Gelenkkopf (1) mit der Option dreidimensionaler Verstellbarkeit sowie einer Spannschraube (5) und einer Ankermutter (7) mit welcher Prothesenschaft (2) und Gelenkkopf (1) optimal positioniert, durch eine Zugankermechanik zu einer starren Einheit möglichst einfach verbindbar sind.
  • Als wesentliche Besonderheit der erfindungsgemäßen Konstruktion wird ein Prothesenschaft (2) vorgeschlagen, welcher über einen zur Hüftgelenkpfanne auskragenden Schafthalszapfen (3) über eine Durchgangsbohrung verfügt, welche sich durch den Prothesenschaft (2) axial fortsetzt und somit einen diagonalen Durchgang durch den Prothesenschaft (2) bildet. Eine Spannschraube (5) mit konischem Kopf ist gelenkseitig in den offenen Schafthalszapfen (3) eingeführt und aus entgegengesetzter Richtung zunächst lose mit einer Ankermutter (7) verschraubt. Der Gelenkkopf (1) wird auf den zum Hüftgelenk auskragenden Schafthalszapfen (3) des Prothesenschafts (2) aufgeschraubt, wobei sich einerseits aufgrund der gewählten Einschraubtiefe, andererseits bedingt durch die exzentrisch angeordnete Gewindebohrung (8) im Gelenkkopf (1) die Möglichkeit ergibt, das Rotationszentrum in einem vorgegebenen Bereich sowohl axial als auch radial zu verstellen und die gewählte Position des Rotationszentrums durch einen Schraubvorgang zu fixieren. Dazu wird die Ankermutter (7) durch eine Öffnung in der lateralen Knochenwand anhand eines Werkzeugs soweit angezogen, bis sich die der konische Kopf der Spannschraube (5) spannzangenartig in der dort sich verjüngenden Wandung des Schafthalszapfens (3), derart mit dem Gelenkkopf (1) verkeilt, dass aufgrund des erzeugten Klemmsitzes eine starre und stabile Verbindung zwischen Gelenkkopf (1) und Prothesenschaft (2) zustande kommt. Dabei wird die Ankermutter (7) gegen einen Anschlag am Prothesenschaft (2) gepresst, sodass sie als Zuganker zur Spannschraube wirkt.
  • Der aufgebohrte Schafthalszapfen (3) verfügt an seinem äusseren Ende über eine Wandung mit Längsschlitzen (4), sodaß der konische geformte Kopf der Spannschraube (5) beim Einschrauben einen spannzangenartigen Klemmsitz zwischen Gelenkkopf (1) und Schafthalszapfen (3), bzw. Prothesenschaft (2) erzeugt.
  • Als zusätzliche Sicherung des Gelenkkopfs (1) auf dem Schafthalszapfen (3) wird vorgeschlagen, eine oder mehrere Sicherungsschrauben (6) aus Querrichtung durch den Gelenkkopf (1) vorzugsweise in die Längsschlitze (4) des Schafthalszapfens (3) einzuschrauben. Damit ist ein selbsttätiges Lösen der Spannverschraubung praktisch ausgeschlossen, sodaß die Prothesenkonstruktion ihren homogenen Aufbau behält und nur durch einen absichtlichen Eingriff lösbar oder ggf. neu einstellbar ist. Die beschriebene spannzangenartige Befestigung gewährleistet eine stabile und spielfreie Fixierung der beiden Prothesenmodule, nämlich Gelenkkopf (1) und Prothesenschaft (2).
  • Die beschriebene radiale Verstellbarkeit wird, wie beschrieben, dadurch ermöglicht, daß die Gewindebohrung (8) im Gelenkkopf (1) exzentrisch angeordnet ist, sodaß der Gelenkkopfmittelpunkt bedingt durch den Schraubvorgang wendelartig um die Achse des Schafthalszapfens (3) kreist und dadurch in einer Vielzahl von Positionen justierbar ist. Somit bietet sich dem Anwender ein relativ großer Verstellbereich innerhalb eines Volumens, welches einerseits durch die axiale Verstellhöhe und andererseits durch den radialen Abstand von der Achse des Schafthalszapfens (3) zum Gelenkkopfmittelpunkt definiert wird. Dem Anwender bietet sich dadurch eine dreidimensionale Verstellbarkeit innerhalb des vorgegebenen Volumens.
  • Das System erlaubt es im Falle eines nachträglichen Eingriffs ohne Ausbau des Prothesenschafts eine aktualisierte Einstellung des Rotationszentrums vorzunehmen bzw. das Gelenk erneut zu justieren. Ebenso bietet sich die Möglichkeit systemartig verschiedene Gelenkkopfgrößen oder Prothesenschaftformen gegeneinander auszutauschen und zum Einsatz zu bringen.
  • Die erfindungsgemäße Konstruktion erlaubt es auch grundsätzlich, die Krafteinleitung vom Beckenknochen zum Oberschenkelknochen sowohl auf den, wie beschrieben, im Markkanal verankerten, keilförmigen Prothesenschaft, als auch über eine auf der Resektionsschnittfläche des Femurs aufliegende Druckscheibe (9) auf den Oberschen kelknochen überzuleiten. Ziel ist dabei, die durch die Prothese aufgenommenene Krafteinleitung auf eine möglichst große Fläche des Oberschenkelknochens gleichmäßig zu verteilen, wie dies durch die Patentanmeldung mit Aktenzeichen 10 2006 035 506.7 bekannt ist.
  • Der erfindungsgemässe Vorschlag eines modularen Hüftgelenkprothesenschafts mit verstellbarem Gelenkkopf bietet sowohl den Vorteil eines minimalen Resektionsanteils, als auch die Fixierung des Gelenkkopfs ohne auf die laterale Oberfläche des Knochens aufgesetzte Platten zur Stabilisierung der Prothese. Die erforderliche seitliche Bohrung durch die Knochenwand dient lediglich als Zugang für ein Schraubwerkzeug und kann nach erfolgter Applikation mit Knochenzement oberflächenbündig geschlossen werden.
  • Die Konstruktion erlaubt einen sicheren Applikationsvorgang, da die offene Durchgangsbohrung als Bohrlehre für die Richtbohrung durch die laterale Knochenwand dient, sodass danach die eigentliche Bohrung für den Werkzeugzugang passgenau ausführbar ist.

Modulare Hüftgelenkprothese bestehend aus Prothesenschaft (2) und verstellbarem Gelenkkopf (1), dadurch gekennzeichnet, daß eine axial im Schafthalszapfen (3) des Prothesenschafts (2) gelenkkopfseitig eingesetzte Spannschraube (5) mit konischem Kopf am gegenüberliegenden Ende ein Gewinde aufweist, in welches eine Ankermutter (7) so eingreift, dass durch ein von aussen in die Ankermutter (7) eingeführtes Schraubwerkzeug eine Zugankerwirkung zwischen Ankermutter (7) und Spannschraube (5) zustande kommt und sich ein spannzangenartiger Klemmsitz zwischen dem in den Gelenkkopf (1) eingeschraubten Schafthalszapfen (3) und dem Gelenkkopf (1) erzeugen lässt.
  2. Modulare Hüftgelenkprothese bestehend aus Prothesenschaft (2) und verstellbarem Gelenkkopf (3), dadurch gekennzeichnet, daß nach Anspruch 1 bei einer modularen Hüftgelenkprothese nach Anspruch 1 der Gelenkkopf (1) eine exzentrische Gewindebohrung aufweist, sodass der in diese eingreifende, auf dem schräg seitwärts auskragenden Schafthalszapfen (3) befindliche Gelenkkopf (1) sowohl axial als auch radial zum Prothesenschaft (2) verstellbar und durch eine Zugankermechanik in der gewählten Position fixierbar ist.
  3. Modulare Hüftgelenkprothese bestehend aus Prothesenschaft (2) und verstellbarem Gelenkkopf (1), dadurch gekennzeichnet, daß nach Anspruch 1 und 2 der Schafthalszapfen (3) eine axiale Durchgangsbohrung aufweist, welche sich auch axial durch den Prothesenschaft (2) fortsetzt und dort proximalseitig einen Anschlag für die Ankermutter (7) aufweist.
  4. Modulare Hüftgelenkprothese bestehend aus Prothesenschaft (2) und verstellbarem Gelenkkopf (1), dadurch gekennzeichnet, daß nach Anspruch 1 bis 3 die Ankermutter (7) neben dem Innengewinde eine Ausformung aufweist, welche es dem dort axial eingesetzten Schraubwerkzeug erlaubt, in die Ankermutter (7) einzugreifen und diese in radiale Bewegung zu bringen.
  5. Modulare Hüftgelenkprothese bestehend aus Prothesenschaft (2) und verstellbarem Gelenkkopf (1), dadurch gekennzeichnet, daß nach Anspruch 3 die Wandung des Schafthalszapfens (3) distalseitig Längsschlitze (4) aufweist und die Wandstärke formbündig passend zum konischen Kopf der dort eingesetzten Spannschraube (5) verjüngt ist.
  6. Modulare Hüftgelenkprothese bestehend aus Prothesenschaft (2) und verstellbarem Gelenkkopf (1), dadurch gekennzeichnet, daß nach Anspruch 1 bis 5 quer zur Gewindebohrung (8) des Gelenkkopfs (1) in diesen Sicherungsschrauben (6) einsetzbar sind, welche in die Längsschlitze (4) des Schafthalszapfens (3) eingreifen und geeignet sind, diesen gegen axiale Verstellung zusätzlich zu sichern.
