Abstract
Description
OČní aplikátor k injekčnímu v právo vání substance do tkáně okaAn ophthalmic applicator for injecting in the right of introducing a substance into the tissue of the eye
Oblast technikyTechnical field
Vynález se tyká očního aplikátoru k injekčnímu vpravování substance, zejména pak léčebné nebo diagnostické tekutiny nebo tekutiny ke změně refrakce, do tkáně oka, především do tkáně rohovky. The invention relates to an ophthalmic applicator for injecting a substance, in particular a therapeutic, diagnostic or refractive fluid, into an eye tissue, in particular into a cornea tissue.
Dosavadní stav technikyBACKGROUND OF THE INVENTION
At present, a conventional syringe is often used in surgery to deliver therapeutic or diagnostic fluid substances to eye tissues, particularly corneal tissue. The difficulty in using it, however, is the complicated injection procedure, which in this case is dependent on the experience and skill of the surgery by the performing physician.
Ve snaze alespoň částečně eliminovat výše uvedené negativní vlivy lidského faktoru a usnadnit podání injekce tak, aby mohlo být prováděno za pokud možno stejných, reprodukovatelných podmínek i delší dobu léčení byly vytvořeny dále uvedené známé přípravky (šablony) k řízení vpichu chirurgické jehly do oka.In order to at least partially eliminate the above-mentioned negative effects of the human factor and to facilitate the injection so that under the same, reproducible conditions and for longer periods of treatment, the following known formulations (stencils) can be created.
The subject of the international patent application WO 2008/097072 is a preparation for facilitating intraocular administration of a substance, for example medicament, into the human or animal eye by means of a syringe needle, which consists of an annular support element with an angled holder and an opening. This support element is placed on the eye and oriented so that through the opening it is possible to guide the needle through the syringe through the necessary points of the eye.
Another similar patent application WO 01/49226 is also the subject-matter of principle and functionally similar. In this case, it is a platelet fixture with a central domed-dome-shaped part with a holder in the axis of this central part and with holes for guiding the needle of the syringe in the flat lateral, adjacent central parts.
Obviously, both of the above solutions relate to jigs of the type template, which, while facilitating and to some extent refining the injection site. However, since the needle of the separate syringe is only inserted into their orifices, these instruments are unable to reliably guarantee an accurate and reproducible injection line during treatment. Even an experienced and skilled doctor cannot completely exclude a possible complication caused by even partial or momentary inaccuracy.
Certain efforts to overcome this problem are to be solved by another international patent application WO 2008/084063. In this solution, the domed needle positioning device is attached directly to the eye of the special syringe by application to the eye, the injection needle extending through the central part thereof. The advantage of this embodiment is the higher puncture accuracy, but at the same time there is a clear limitation due to the design of the jig. The fact that the injection needle has to pass through the central part of the domed positioning device, due to the principle of solution, significantly limits the application possibilities of this solution. In addition, an instrument that makes an injection into the central part of the cornea eye is not suitable for ophthalmic surgery at all. Generally, there is an effort to avoid punctures in the optical axis.
A common drawback of all the above-mentioned known solutions is also that more separate punctures are required to deliver substances to a larger area, which extends and complicates the entire procedure.

Podstata vynálezuSUMMARY OF THE INVENTION
K odstranění těchto problémů přispívá do značné míry oční aplikátor k injekčnímu vpravování substance do tkáně oka podle vynálezu. To a large extent, the ophthalmic applicator contributes to the injection of the substance into the tissue of the eye according to the invention.
The principle of the invention consists in that the ophthalmic applicator is constituted by a base body in which one or more application injectors, in particular one or more application needles, are mounted directly or through a segment.
The base body may preferably have the shape of a conical or cylindrical ring with at least one side cut-out. In the cavity of the applicator base body there is then rotatably mounted a segment designed as a hollow body with a sheath corresponding to this cavity and with at least one face base with an opening. Inside the segment, at least two exchangeable yoke needle holders are mounted slidingly, which carry on the curved inner faces of the injection needle and are provided on the sides with thumbs engaging in arcuate grooves on the inside of the base of the segment.
The applicator base may also have a disc shape that is directly provided with needles for receiving needles.
The tubes of the injection needles retained in the base body or the yoke holders are the channels within the base body or the yoke holders connected to the source of the therapeutic or diagnostic fluid substance. This interconnection can be realized such that the base body or bracket holders are provided with openings for connecting an external source of therapeutic or diagnostic fluid substance. Another possible solution consists in that the base body or the bracket holders have spaces for the internal reservoirs of the therapeutic or diagnostic fluid substance directly formed therein.
Preferably, the ophthalmic applicator body may be provided with ribbed supports on the surface and may be made of a transparent material.
The main benefit of the ophthalmic applicator of the invention is that it significantly reduces the risk of complications due to possible inaccuracy of puncture in the delivery of therapeutic or diagnostic fluid substances to eye tissues, particularly corneal tissue.
In this case, the applicator is not based on a needle injection positioning device which is an integral part of the syringe as in WO 2008/084063 and the injection needle does not pass through its central part and therefore does not guide the optical axis of the eye.
Navíc oční aplikátor podle PV 2009-66 může obsahovat více aplikačních injektorů a řeší tedy i společný nedostatek všech výše uvedených známých řešení tj., že pro vpravování substancí do větší oblasti je třeba provést více samostatných vpichů, což prodlužuje a komplikuje celý zákrok. In addition, the ophthalmic applicator according to PV 2009-66 may contain multiple injectors and thus solves the common drawback of all the above known solutions, i.e., that more separate injections are required to inject substances into a larger area, which extends and complicates the procedure.

Základní těleso 1 aplikátoru je na povrchu opatřeno žebrovitými držáky 4.The applicator base body 1 is provided with ribbed supports 4 on the surface.
Aplikátor je při použití přiložen na oko a otáčením segmentu 3 nastane dostředivý pohyb třmenových držáků 2 jehel, kterým dojde ke vpichu jehel do rohovky. Poté je tlakově aplikována tekutá substance. Následným pootočením segmentu 3 zpět nastává odstředivý pohyb třmenových držáků 2 jehel, kterým jsou jehly vysunuty z rohovky. Nakonec je aplikátor sejmut z oka.In use, the applicator is placed on the eye and by rotating the segment 3 the centripetal movement of the needle holders 2 occurs and the needles are injected into the cornea. The liquid substance is then applied by pressure. Subsequent rotation of the segment 3 reverses the centrifugal movement of the yoke holders 2 by which the needles are withdrawn from the cornea. Finally, the applicator is removed from the eye.
