Abstract

一种活动式螺纹状椎间融合器,由弹性钛合金箱笼和中间聚乙烯髓核组成,弹性钛合金箱笼为圆柱状空心结构,在管状外壁有两侧间断的螺纹,间断处管壁的中央设有窗口,窗口两端的管壁上各有数条相互交错的切口,切口使该管壁形成“Z”形弹簧结构,中间聚乙烯髓核可楔入弹性钛合金箱笼内。由于本实用新型中弹性钛合金箱笼两侧壁具有切口弹簧,使之具有前屈、后伸、左右侧弯、纵向弹性及微量水平移位运动功能;内腔中间的髓核能够起到支撑和调整运动轴心的作用。

Description

活动式螺纹状椎间融合器
本实用新型涉及一种融合器,特别是一种活动式螺纹状椎间融合器。
对椎间盘退变等脊柱不稳症行脊柱融合术后,由于存在植骨块的吸收、内固定物折断、脱落等不良现象,常代之以界面固定器,目前国内外使用的界面固定器(如[中华创伤杂志.1998.14(1):28;Spine.1997;22(6):667;Spine.1994;19(13):1436;Spine.1998;23(11):1267]),虽然可支撑固定并使相邻椎体融合,但弹性极小而且不能活动,术后脊柱强直,患者活动受限制,更为严重的是作为一种长期植入物,以往的界面固定器不具备活动性和弹性功能,不能与人体脊柱的力学环境相适应,在椎体融合后,本节段活动丧失,相邻节段产生代偿性脊柱不稳,引起或加速邻近椎间盘的退行性变。
本实用新型的目的在于提供一种既能对脊柱起到融合固定作用,又具有与椎间盘近似的活动功能,且不扰动人体脊柱力学环境的活动式螺纹状椎间融合器。
本实用新型的目的可以通过以下措施来达到:一种活动式螺纹状椎间融合器,其特征在于:由弹性钛合金箱笼和中间聚乙烯髓核组成,弹性钛合金箱笼为圆柱状空心结构,在管状外壁有两侧间断的螺纹,间断处管壁的中央设有窗口,窗口两端的管壁上各有数条相互交错的切口,切口使该管壁形成“Z”形弹簧结构,中间聚乙烯髓核可楔入弹性钛合金箱笼内。
本实用新型的目的还可以通过以下措施来达到;弹性钛合金箱笼上下两侧壁的螺纹上有若干小孔。中间聚乙烯髓核呈梭形。

本实用新型下面将结合附图(实施例)作进一步详述:
参照图1~图7,本实用新型由弹性钛合金箱笼1和中间聚乙烯髓核2组成,弹性钛合金箱笼1为圆柱状空心结构,在管状外壁有两侧间断的螺纹,螺纹上有若干个小孔3,间断管壁的中央有矩形窗口4,窗口4两端的管壁上各有数条相互交错的切口5,切口5使该处管壁形成“Z”形弹簧结构,这样实际上箱笼就成了由四个切口弹簧支撑的上下两个半环形螺钉样结构。半环形螺钉样结构的内面为椭圆形光滑凹槽,中央髓核2为椭圆形实心结构,其横断面左右径大于上下径,略小于箱笼内腔。手术时安TFC安装程序,先将箱笼旋入椎间隙内,再将髓核楔入内腔,所用器械除装入器改成从两侧无螺纹处固定箱笼外,其余器械与TFC器械相同。植入的融合器要处于水平位,上下两面的螺纹要埋入椎体骨质,螺纹表面骨相容性处理和螺纹上的小孔能共同增强箱笼与椎体的融合强度。
本实用新型的实施方式,适合以精密铸造工艺实现。先以软金属在机床上加工模型,再经反制模铸造完成。模型可采用如下方式制成。取直径20mm的锡合金棒材作为加工椎间融合器模型的材料,分两步加工:1、锡合金箱笼。取26mm长的棒材,外车螺纹至直径16mm,内部车空成近似梭形,最小壁厚为6mm的管;在上下侧壁的螺纹各开4排直径为1mm的小孔。在弹性锡合金箱笼的两外侧壁中部用电切割加工出一6mm×8mm矩形切口,在头尾两端每侧各加工四个6mm×2mm的切口。2、中间聚乙烯髓核。用聚乙烯材料加工一近似梭形核,两端断面为圆形,中部为椭圆形,最宽处为10mm,并与锡合金箱笼的内腔相适应。
本实用新型相比现有技术具有如下优点:
把融合器植入椎体间之后,上下两面的螺纹与上下椎体骨质融合,融合器本身通过切口弹簧的拉伸和压缩实现椎间的活动,其活动度可满足脊柱伸、屈、侧弯、环转及微量水平移位运动,与正常脊柱的运动方式相似;内腔的聚乙烯髓核可起到支撑和调整运动轴心的作用,弹性钛合金箱笼的内腔光滑,不会产生对人体有害的粉末,从而具有较好的人体相容性。

1、一种活动式螺纹状椎间融合器,其特征在于:由弹性钛合金箱笼(1)和中间聚乙烯髓核(2)组成,弹性钛合金箱笼(1)为圆柱状空心结构,在管状外壁有两侧间断的螺纹,间断处管壁的中央设有窗口(4),窗口(4)两端的管壁上各有数条相互交错的切口(5),切口使该管壁形成“Z”形弹簧结构,中间聚乙烯髓核(2)可楔入弹性钛合金箱笼(1)内。
2、根据权利要求1所述的融合器,其特征在于:弹性钛合金箱笼(1)上下两侧壁的螺纹上有若干小孔(3)。
3、根据权利要求1所述的融合器,其特征在于:中间聚乙烯髓核(2)呈梭形。
