[0014] 本实用新型气管插管导引槽在麻醉诱导时作为气管插管的定位与导引装置,在气管导管拔出后可以作为口咽部通气道,进行口咽部通气。由于气管插管导引槽具有良好的定位导引作用,因此,可以完全替代喉镜插管,可以说本实用新型提供了一种完全有别于借助喉镜、光棒、纤支镜的新型气管插管技术;另外,由于气管插管导引槽还可以作为口咽部通气道,因而在患者的自主呼吸不理想或没有自主呼吸的情况下就可以早早拔下气管导管,保证了患者的生命安全。
[0015] 口咽通气道气囊在充满气之后可以填充满整个口腔,起到固定气管插管导引槽的作用,还可以封堵咽喉部,防止在通气时漏气;另外,口咽通气道气囊分布于气管插管导引槽槽体的两侧,在充满气之后在槽体上方形成夹层空间,从而可以在槽体的导引基础上扩大气管插管的导引空间。食管导管伸入于食道内,可以置入胃管进行胃肠减压,食管导管气囊主要用于封堵食道,防止呕吐,返流,误吸的发生。
[0016] 综上,本实用新型的有益效果在于:操作简便,由于省去了喉镜、光棒、纤支镜等,使得气管插管手术简便易行,手术过程大大简化;可有效保证患者安全,避免了气管插管无法有效固定或插入拔出无防护造成患者口腔黏膜损伤、出血,牙齿松动、脱落的问题,而且可提早拔除气管导管,防止气管导管对患者气管的刺激;手术同时可进行胃肠减压并可防止呕吐、返流、误吸的发生。

[0020] 如图1和2所示,一种可实现食道封堵和气管插管导引的口咽通气道,包括气管插管导引槽1,气管插管导引槽I由槽体101和槽壁102构成;其中,槽壁102的截面呈椭圆形,槽体101的截面呈U形,气管插管导引槽I的近端103设有向外凸出的挡板8,槽体101的远端具有一向上翘起的弧形导引部1011,槽壁102上穿设有食管导管2,食管导管2的远端202伸出于槽壁102外,食管导管2的近端201与气管插管导引槽I的近端103相平齐(也可稍伸出于槽壁102外);气管插管导引槽I的远端104设有口咽通气道气囊3,食管导管2的远端202设有食管导管气囊4,口咽通气道气囊6对气管插管导引槽I的槽壁102形成包覆并密闭设置,食管导管气囊4对食管导管2形成包覆并密闭设置,气管插管导引槽I的槽壁102上还穿设有口咽通气道气囊注气管5和食管导管气囊注气管6,口咽通气道气囊注气管5的远端502与口咽通气道气囊3相连通、近端501安装有单向活瓣7,食管导管气囊注气管6的远端602与食管导管气囊4相连通、近端601也安装有单向活瓣7 ;气管插管导引槽I弯曲设置,且其弯曲角度大于90°并小于160°。
[0021] 在紧急气道、短小手术等需要建立人工气道的场合时,将本实用新型装置置入患者口腔,并在挡板8作用下,气管插管导引槽I的近端103露出于患者齿外,调整使食管导管2伸入于食道内,使气管插管导引槽I的远端104位于声门位置,气管插管导引槽槽体101的近端对准下颌中切牙,患者可通过牙齿将气管插管导引槽I进行咬合固定。然后,通过口咽通气道气囊注气管5和食管导管气囊注气管6向口咽通气道气囊3和食管导管气囊4内注入空气直至饱满。此时,口咽通气道气囊3充满整个口腔,对气管插管导引槽I起到固定作用,食管导管气囊4则对食道进行封堵,防止呕吐、返流、误吸的发生。然后,在患者口部连接面罩或者氧气导管就可以进行口咽通气,这时,气管插管导引槽I的槽体101即作为口咽部通气道;必要的话还可以在食管导管2中置入胃管进行胃肠减压。
[0022] 在需要气管插管时,准备步骤如前所述,在口咽通气道气囊3和食管导管气囊4注满空气后,将经过润滑的气管导管顺着气管插管导引槽I的槽体101置入口腔,这时,气管插管导引槽I的槽体101作为气管导管的导引装置,在气管导管伸入约14cm后,向气管导管气囊内注入空气约20ml,继续置入直至有阻力后按压胸廓有气雾形成,确定位于声门,然后抽除气管导管气囊内的空气,无阻力继续置入,听诊确定气管导管位于气管后,连接呼吸机,进行机械通气。在此过程中,气管导管在气管插管导引槽I的导引作用下出入顺畅、准确,不容易对患者造成损伤。特别地,当气管导管沿着槽体101行至气管插管导引槽远端104后,在槽体101弧形导引部1011的引导作用下,气管导管被挡住直行去向并向上翘起,从而避免进入食道内,而且,如此还有利于气管导管进入上方声门,使得本实用新型装置更加符合人体咽喉部生理结构。

1.一种可实现食道封堵和气管插管导引的口咽通气道,其特征在于,包括气管插管导引槽,所述气管插管导引槽由槽体和槽壁构成,所述槽壁上穿设有食管导管,且所述食管导管的远端伸出于槽壁外;所述气管插管导引槽的远端设有口咽通气道气囊,所述食管导管的远端设有食管导管气囊,所述口咽通气道气囊对气管插管导引槽的槽壁形成包覆并密闭设置,所述食管导管气囊对食管导管形成包覆并密闭设置,所述气管插管导引槽的槽壁上还穿设有口咽通气道气囊注气管和食管导管气囊注气管,所述口咽通气道气囊注气管的远端与口咽通气道气囊相连通,所述食管导管气囊注气管的远端与食管导管气囊相连通。
2.根据权利要求1所述的一种可实现食道封堵和气管插管导引的口咽通气道,其特征在于,所述气管插管导引槽的槽壁呈椭圆形。
3.根据权利要求1所述的一种可实现食道封堵和气管插管导引的口咽通气道,其特征在于,气管插管导引槽的近端设有向外凸出的挡板。
4.根据权利要求1所述的一种可实现食道封堵和气管插管导引的口咽通气道,其特征在于,所述槽体的远端具有一向上翘起的弧形导引部。
5.根据权利要求1所述的一种可实现食道封堵和气管插管导引的口咽通气道,其特征在于,所述口咽通气道气囊注气管和食管导管气囊注气管的近端分别安装有单向活瓣。
6.根据权利要求1所述的一种可实现食道封堵和气管插管导引的口咽通气道,其特征在于,所述气管插管导引槽弯曲设置,且其弯曲角度大于90°并小于160°。
