Abstract

发明领域
本发明涉及用于定量测定样品中游离PAPP-A的生物亲和性分析,游离PAPP-A定义为未与主要碱性蛋白的原形(proform)(proMBP)复合的妊娠相关的血浆蛋白A(PAPP-A)。本发明进一步涉及以游离PAPP-A作为标志物来诊断人体中急性冠状动脉综合征的方法。
发明背景
这里使用的旨于解释发明背景的出版物及其它材料,特别是一些给出额外实施细节的例子,在此引入作为参考。
七十年代初首次鉴定妊娠相关的血浆蛋白A(PAPP-A)是在人晚期妊娠血清中发现的一种高分子量成分(1)。血清中的浓度随著妊娠而增加,直至分娩(2)。PAPP-A最初被表征为一个同四聚体(Homotetramer)(1,3),但后来证实妊娠期间循环的PAPP-A是与嗜酸性的主要碱性蛋白的原形(proMBP)以2∶2复合的通过二硫键结合的500kDa异四聚体(Heterotetramer),称作PAPP-A/proMBP(4)。不过,据报道妊娠血清或血浆中也含有痕量(<1%)未复合的PAPP-A(5)。
妊娠中PAPP-A和proMBP二者都产生于胎盘,但主要由不同类型的细胞产生。通过原位杂交分析,发现大部分的PAPP-A是在合胞滋养层中合成,而所有的proMBP是在绒毛外细胞滋养层中合成(6)。通过对克隆的cDNA的分析显示PAPP-A亚基是一个由1547个残基组成的多肽(7)。它含有一个延长的锌结合基序,三个Lin-Notch重复序列和5个短共有重复序列(8)。
proMBP是糖基化的蛋白聚糖,它包含90个残基的强酸性原片段和117个残基的高度碱性的MBP成熟形式(9,10)。后者是一种细胞毒性蛋白,存在于嗜酸性白细胞的颗粒中(11)。通过脱颗粒将其从嗜酸性白细胞中释放,在这些细胞的效应物功能中发挥多种作用(12)。尽管在嗜酸性细胞中成熟MBP来源于proMBP的蛋白水解加工,但没有证据显示MBP能产生于PAPP-A/proMBP复合物中的proMBP。就proMBP在PAPP-A/proMBP复合物中的作用而言,有研究显示proMBP在体外起PAPP-A的蛋白酶抑制剂的作用(5,13)。除PAPP-A外,proMBP还与血管紧张素原或补体C3dg形成共价复合物(14)。但是proMBP在其它复合物中的作用仍然不清楚。
近来,PAPP-A已被发现在体外是胰岛素样生长因子(IGF)结合蛋白(IGFBP)-4和IGFBP-5的特异性蛋白酶(15,16)。值得一提的是,IGFBP-4的切割是以IGF依赖性方式,而IGFBP-5的切割是以IGF非依赖性方式。不过,PAPP-A在体内的生理功能仍不清楚。胰岛素样生长因子I和II(IGF-I和IGF-II)主要通过I型IGF受体的介导,在促进众多生物系统的细胞分化和增殖过程中发挥作重要的作用。IGF-I和IGF-II的生物学活性由6种同源的高亲和性IGF结合蛋白调控,这些蛋白与IGF结合,然后阻断IGF与其受体的结合(17)。PAPP-A切割IGFBP-4和IGFBP-5,导致释放结合的IGF,这样就增加了用于与IGF膜受体相互作用的可生物利用的IGF。
在临床上,降低的PAPP-A血清水平与唐氏综合征(DS)妊娠有关联(18)。作为一个标志物,PAPP-A目前已被广泛地用来在第一个三月期筛查DS(19)。仅仅在最近,PAPP-A被显示在不稳定动脉粥样硬化斑块(冠状动脉和颈动脉)中存在(20,21),其循环水平在急性冠状动脉综合征(ACS)患者中升高(20,22)。此外,循环中PAPP-A的存在对于冠状动脉病患者来说还是一个独立的预后分层指标(23)。到目前为止,仍不清楚PAPP-A在斑块中的作用。不过,已经提出,在ACS中观察到的通过IGFBP-4蛋白水解而增加的IGF的生物利用度在冠状动脉粥样硬化的进展及再狭窄中发挥作关键的作用(20,24)。
在技术上,循环中的PAPP-A的可测性与PAPP-A的分子结构紧密相关。存在于孕妇血中的PAPP-A的分子结构是否与存在于ACS患者血中的PAPP-A的结构相同是一个非常重要的问题。到目前为止,尚无解决此重要问题的报道。目前用于在上述两种情况下进行PAPP-A测量的所有分析都是基于针对PAPP-A/proMBP复合物的PAPP-A亚基的特异性抗体(20,25,26,27)。从方法学观点上看,该事实使得不能将妊娠中的循环PAPP-A与ACS中的循环PAPP-A区分开来。
这里,我们首次提供数据显示妊娠期中循环PAPP-A分子不同于ACS中循环PAPP-A分子。这些发现对于更早期和更特异性地检测循环中的动脉粥样硬化相关的PAPP-A具有重要的临床意义。
发明目的以及发明概述
本发明的目的是提供更灵敏和更特异的用于早期诊断具有患急性冠状动脉综合征危险的个体的方法。特别是,目的是获得一种诊断方法,其优于通常使用的基于心脏肌钙蛋白I的分析并且优于建议的基于以总PAPP-A作为标志物的分析。
因此,一方面,本发明涉及一种用于定量测定样品中的游离PAPP-A的生物亲和性分析,游离PAPP-A定义为未与主要碱性蛋白的原形(proMBP)结合的妊娠相关的血浆蛋白A(PAPP-A)。根据本发明,游离PAPP-A:
i)确定为计算出的测量的总PAPP-A和测量的与proMBP复合的PAPP-A的差,或
ii)通过仅仅测量游离PAPP-A的直接生物亲和性分析而确定。
另一方面,本发明涉及用游离PAPP-A或者一种比值作为标志物来诊断人体中的急性冠状动脉综合征的方法,所述比值是
-游离PAPP-A/总PAPP-A,
-游离PAPP-A/与proMBP复合的PAPP-A,或
-与proMBP复合的PAPP-A/总PAPP-A。
第三方法,本发明涉及一种结合剂,它与游离PAPP-A结合,但不结合于与proMBP复合的PAPP-A。

图4显示ACS患者PAPP-A的血清动力学。PAPP-A由分析T和分析C来进行检测。

发明详述
术语″PAPP-A″应当被解释为包括未与主要碱性蛋白的原形(proMBP)结合的任何PAPP-A。因此,″游离PAPP-A″包括绝对游离的PAPP-A还有与除proMBP外的任何物质结合的PAPP-A。
术语″结合剂(binder)″应当被解释为特别地包括抗体和抗体片段(任选经过基因工程化的)、适体和蛋白支架衍生的结合物比如Affibodies和Flurobodies。不过,术语″结合剂″并不局限于上面所提到的例子,任何可用于生物亲和性分析的结合剂都应当被理解为被此定义所包涵。
根据一种优选的实施方案,游离PAPP-A确定为计算出的测量的总PAPP-A和测量的与proMBP复合的PAPP-A的差。
这一选项例如可以使用两次独立的分析来实现,其中将样品的一份等分样品暴露于与总PAPP-A结合的结合剂,并且检测该结合剂以得到总PAPP-A。将样品的另一份等分样品暴露于仅仅结合于与proMBP复合的PAPP-A的结合剂检测该结合剂,以得到与proMBP复合的PAPP-A。最后,游离PAPP-A的量计算为确定的总PAPP-A和与proMBP复合的PAPP-A的差。两次分析可以是竞争性分析,或更优选是非竞争性夹心分析,其中特异性结合剂是捕获结合剂或检测性(标记的)结合剂。
或者,可以在一个单次双分析物分析中测量游离PAPP-A和与proMBP复合的PAPP-A。可以将样品暴露于与总PAPP-A结合的捕获结合剂和用不同标记物进行标记的两种检测性结合剂,使得用第一种标记物标记的第一种检测性结合剂针对任何PAPP-A分子中存在的表位,其中第一种标记物的信号用于得到总PAPP-A。用第二种标记物标记的第二种检测性结合剂针对分子的proMBP亚基中的表位,其中第二种标记物的信号被用于专门得到与proMBP复合的PAPP-A。
措辞″分子的proMBP亚基中的表位″应当被理解为包括仅仅位于所述proMBP亚基中的表位,和一部分位于proMBP亚基中,另一部分位于PAPP-A分子的另一部分中的表位。因此,也可以通过仅仅与一部分位于proMBP亚基中,另一部分位于PAPP-A分子的另一部分中的表位反应的结合剂,特异性测量与proMBP复合的PAPP-A。
根据另一种优选实施方案,游离PAPP-A由一种直接的仅检测游离PAPP-A的生物亲和性分析来测量。根据一种选项,可以通过将样品暴露于结合游离PAPP-A但不结合于与proMBP复合的PAPP-A的抗体(包括抗体片段如Fab和单链可变区(scFv)片段)或其它结合剂而进行,并且检测抗体或其它结合剂以得到游离PAPP-A。这样的抗体或其它的结合剂可以例如针对PAPP-A的表位而产生,所述表位仅仅在未与proMBP结合的分子中,如氨基酸381-652的区域中可获得。对游离PAPP-A具有特异性的多克隆抗体可以通过用游离PAPP-A和免疫佐剂免疫宿主动物如兔和羊来制备。而对游离PAPP-A具有特异性的单克隆抗体可以通过杂交瘤技术和同样的免疫原(用来免疫的宿主动物常常是小鼠)来制备。此外,对游离PAPP-A具有特异性的抗体或其片段如Fab和单链可变片段(scFv)还可以通过从合成或天然抗体文库中使用噬菌体展示来制备。游离PAPP-A可以从ACS斑块或者从未与proMBP复合的妊娠PAPP-A或者从编码PAPP-A的DNA序列的重组表达中得到。
或者,可以进行仅仅测量游离PAPP-A的生物亲和性分析,这是通过使与proMBP复合的PAPP-A不能参与生物亲和性反应,所述反应中样品暴露于结合于总PAPP-A的抗体或其它结合剂。有两种方法可以达到这种目的。一种涉及到使用如图8B所显示的吸附,与proMBP复合的PAPP-A在前面的步骤中通过抗体A11吸附被去除,从而能够测量游离PAPP-A。另一种方法涉及到应用阻断策略,其中某种PAPP-A亚基特异性抗体或者其它结合剂与其表位的接近被阻断,这是由于proMBP反应性抗体/其它用特定基团或不用特定基团衍生化的结合剂的结合。阻断可以在分析前的步骤中进行或者与分析同时进行。这样,游离PAPP-A也能被有效地测量出来。

材料和方法
试剂
ITC-TEKES Eu3+荧光螯合剂(4-[2-(4-异硫氰酰苯基)乙炔基]-2,6,-二{[N,N-二(羧甲基)-氨基]甲基}吡啶)和生物素异硫氰酸酯(BITC)获自Innotrac Diagnostics Oy。DELFIA分析缓冲液和洗涤溶液根据以前的描述(28)制备。补0.01%变性的小鼠IgG和0.02%天然小鼠IgG的分析缓冲液称作修饰的分析缓冲液系。低荧光12孔Maxisorp微量滴定条(紫外光淬灭的)购自NUNC。链亲合素包被的单孔和条获自Innotrac Diagnostics Oy。牛血清白蛋白(BSA)购自Intergen。NAP-5TM和NAP-10TM柱购自Pharmacia Biotech。所有使用的其它化学试剂是分析级。
6种称作mabB1、2、3、4、5和6的PAPP-A/proMBP复合物的特异性单克隆抗体由丹麦国家血清研究所的Michael Christiansen博士馈赠。其他的11种称作mabA1、2、3、4、5、6、7、8、9、10和11的PAPP-A/proMBP复合物的特异性单克隆抗体由芬兰HyTest Oy的Maria Severina博士馈赠。
校准物是通过以下方法制备的:在含有60g/L牛血清白蛋白、50mmol/L的Tris-HCl(pH7.75)、15mmol/L NaCl和0.5g/L NaN3的缓冲液中稀释10份第3个三月期的妊娠血清的过滤后(通过0.22微米孔径的过滤器)的合并物,针对第3个三月期妊娠合并血清来源的WHO IRP 78/610进行妊娠相关蛋白(WHO生物标准国际实验室,国家血清研究所,丹麦哥本哈根)的校准。PAPP-A和proMBP的水平以微单位/升表示。校准物储存在-20℃直到使用。
血清样品
8例ACS患者(4位男性,平均年龄57±5岁和4位女性,平均年龄81±3岁)有长期胸痛并伴有ST段升高和异常增加的CKMB与cTnI水平。在进入Turku大学中心医院心脏科时以及此后1、2、4、6、24、48和72小时采集这些患者的血清样品。此外,两份第一个三月期血清样品(孕龄9和11周)也包括在本研究中。所有的血清样品都是在知情同意下获得的。遵循的程序是按照1996年修订过的1975年版赫尔辛基宣言。所有的样品在测量前都储存在-20℃(妊娠样品)或-70℃(ACS样品)。
用镧系金属螯合物和生物素标记抗体
用本身具有荧光的铕螯合物标记抗体(29)。标记反应的操作如同以前报道(25)。简单地说,用溶于50mmol/L碳酸钠缓冲液(pH9.6)的100倍摩尔过量的螯合物在室温下标记抗体过夜(16-20h)。在用25ml/h的50mmol/L Tris-HCl(pH7.75)、15mmol/L NaCl、0.5g/L NaN3操作的Superdex 200HR10/30凝胶过滤柱(Pharmacia Biotech,瑞典)上将标记的抗体从过量的游离螯合物和聚集的蛋白分离出来。收集0.45ml的级分。合并含有标记的抗体的级分,用铕校准溶液确定标记的程度。抗体的标记程度为5-15Eu3+螯合物/每分子IgG。
抗体的生物素标记反应用溶于50mmol/L碳酸钠缓冲液(pH9.6)的50倍摩尔过量的生物素-异硫氰酸酯室温下进行3小时。
通过将反应混合液经过以50mmol/L Tris-HCl(pH7.75)、15mmol/L NaCl、0.5g/L NaN3为洗脱液的NAP-5TM和NAP-10TM柱(Amersham Bioscience AB),实现生物素化的抗体与游离生物素化试剂的分离。加入终浓度为1g/L的BSA,溶液通过0.22μm孔径的过滤器进行过滤,然后储存于4℃。
表位作图
所有针对PAPP-A/proMBP复合物的抗体被成对地测试,每种抗体或用作捕获抗体或用作检测抗体。采用一步夹心分析方式,并且采用100mIU/L PAPP-A校准物及空白溶液。程序与以前描述的类似(30)。简单地说,在已被直接包被0.4μg抗体的孔中以三份重复加入10μl的PAPP-A校准物或空白溶液以及溶解于20μl分析缓冲液中的100ng的Eu3+标记的抗体。37℃下振荡下温育10分钟和60分钟(900rpm,iEMS温育器/振荡器,Labsystems Oy,芬兰)。此后,洗孔6次并用热的干燥气流吹干5分钟,然后用VictorTM1420多标记计数器(Perkin/Elmer Life Sciences,Wallac Oy,芬兰)测量荧光。

两种免疫分析被应用于此项研究。一种分析称作分析T,由生物素化的mabA1和铕标记的mabB4构建;另一种分析称作分析C,由生物素化的mabA1和铕标记的mabA11构建。两种免疫分析均以带有iEMS温育器/振荡器的常规微量滴定板分析形式进行。对于这两种分析而言,首先通过室温下缓慢振荡的条件下将每孔300ng的生物素化mab A1在50μl DELFIA分析缓冲液中温育60分钟,将生物素化的mabA1固定在包被有链亲合素的微量滴定板孔上。然后,对于分析T,将10μl的校准物或样品以及溶于20μl的修饰的分析缓冲液中的200ng Eu3+标记的mabB4加入每个孔中。37℃下缓慢振荡的条件下将孔温育30分钟,随后洗孔6次。此后,使孔干燥5分钟,并且直接从带有VictorTM1420多标记计数器的干燥表面测量时间分辨的铕荧光。通过对对数转化的数据使用spline算法的MultiCalc免疫分析程序(Perkin-Elmer Life Sciences,Wallac Oy,芬兰),针对来自校准孔的校准曲线校准荧光信号,获得未知样品的浓度。对于分析C,每孔加入10μl校准物或样品和20μl修饰的分析缓冲液。37℃下缓慢振荡的条件下将孔温育30分钟,洗孔两次。此后,每孔加入溶于30μl修饰的分析缓冲液的300ng Eu3+标记的mabA11,37℃下缓慢振荡的条件下将孔温育30分钟,随后洗孔6次。后面的步骤与分析T相同。
凝胶过滤层析
这是在Superose TM6(3.2×300mm)精密柱PC3.2/30(PharmaciaBiotech,瑞典)上操作的,该柱以含有0.15mol/L NaCl和0.02%NaN3的50mmol/L磷酸钠缓冲液,pH7.0平衡并洗脱,流速为0.04ml/min。上样量为50μl(血清已用洗脱缓冲液两倍稀释并用0.22μm孔径的过滤器过滤)。在280nm处监测柱洗脱液,在最初洗脱0.6ml之后,收集100μl级分。全部过程操作时间为75分钟。10℃下在PharmaciaSmart系统(Pharmacia Biotech,瑞典)上操作层析柱,且用下面的蛋白校准:甲状腺球蛋白(669kDa),脱铁铁蛋白(481kDa),免疫球蛋白G(160kDa),牛血清白蛋白(67kDa),凝乳酶原A(25kDa),和核酸酶A(13.7kDa)。第一个三月期妊娠血清样品和ACS血清样品通过凝胶过滤层析分级分离。
从正常血清样品吸附PAPP-A
此实验使用链亲和素包被的微量滴定板孔和生物素化的mab A1或生物素化的A11。将30μl血清样品加入表面已固定了300ng生物素化的单克隆抗体A1或400ng生物素化的单克隆抗体A11的每个孔中。对生物素化的单克隆抗体A1来说,室温缓慢振荡下温育1小时,然后,从每个孔中取出10μl处理过的血清,应用到上面的用于测量PAPP-A的免疫分析中。对生物素化的单克隆抗体A11来说,室温缓慢振荡下温育1小时,血清样品然后被转移到另一个包被过的孔中,然后温育1小时。此后,重复进行转移步骤和温育直到进行第三次温育。最后,10μl处理过的血清被应用到上面的用于测量PAPP-A的免疫分析中。
统计学分析
统计学分析是使用StatView(SAS Institute,Cary,美国)进行的。
结果
由17种抗体确定的妊娠PAPP-A的表位图
图1显示的表位示意图是根据获自每一种可能的抗体双位点组合的数据制作的。每种抗体的定位关系是在一种抗体的结合是否可能允许或干扰另一种抗体的独立结合的基础上决定的。在17种单克隆抗体中,B1、B2、B3和B4此前已被显示为特异性地结合PAPP-A/proMBP复合物的PAPP-A亚基,而B5和B6与PAPP-A/proMBP复合物的proMBP亚基发生反应(5,31)。MabA11能与除mabB5和B6外的所有其它的单克隆抗体形成夹心,说明它应该与PAPP-A/proMBP复合物的proMBP亚基发生反应。在其它与PAPP-A/proMBP复合物的PAPP-A亚基反应的14种单克隆抗体中,两种抗体(A10和B4)并不与其它的抗体有共同的表位。校准曲线
显示在图2中的两种分析的校准曲线是用来自第3个3月期妊娠血清合并物的标准材料获得的。两条曲线在1.0mIU/L-300mIU/L的浓度范围中都是线性的。对于分析T来说,分析的不精确度是低的,表现为分析内浓度CV在1.0mIU/L-300mIU/L的浓度范围内低于10。对于分析C来说,分析的不精确度为:在1mIU/L时高于20%,在3.0mIU/L-300mIU/L低于15。更为重要的是,两个校准曲线彼此平行,表示标准材料中的PAPP-A是同等地被这两种分析方法检测出来。
妊娠PAPP-A的分子谱和免疫反应性
一份第一个三月期血清样品通过大小排阻层析进行分级分离,通过两种免疫分析来分析级分。这两种分析显示的妊娠PAPP-A在与甲状腺球蛋白(669kDa)相同的位置以单峰洗脱(见图3)。此外,通过两种分析获得的两个峰完全彼此重叠。
由分析T测出的PAPP-A浓度略微高于分析C测出的PAPP-A浓度。
ACS患者中的PAPP-A
通过两种分析测量4名ACS患者的系列血清样品中的PAPP-A水平。使用分析T,在胸痛发生后的不同时间观察到所有4例患者中PAPP-A水平高于5.68mIU/L(22)的参照水平(示于图4中)。尽管PAPP-A水平最大增加的程度不一样,PAPP-A水平的显著增加早期发生在所有的4例ACS患者胸痛发生后的2小时内。使用分析C,在这4例患者中没有发现PAPP-A水平的显著增加,其中PAPP-A浓度在所有血清样品中均低于4mIU/L。这样的结果显示存在于循环中的ACS特异性的PAPP-A不能被proMBP反应性抗体检测出来。
ACS PAPP-A的分子谱和免疫反应性
对来自于另外4例ACS患者的4份含有水平显著增加的PAPP-A的血清样品通过大小排阻层析进行分级分离。由分析T对级分进行分析。结果发现PAPP-A的免疫反应性洗脱为单峰,在相当于脱铁铁蛋白(481kDa)的位置洗脱下来(见图5)。不考虑PAPP-A的浓度,从所有4份ACS血清样品的洗脱模式相同,并且,明显地与来自两份妊娠样品(669kDa)的曲线分开。这种在妊娠PAPP-A和ACSPAPP-A间的分子大小的差别被清楚地展示在图5中,并且,当用更小体积级分(25μL)进行分级分离时,这种差别变得更显著。
正常受试者中的PAPP-A
在正常受试者(n=130,年龄为50到69岁)中,血清PAPP-A的水平小于7.6mIU/L,中值为3.0mIU/L。存在于正常受试者中的这样的循环PAPP-A水平不仅可以被分析T而且可以被分析C检测出来。
图7A显示正常受试者中低水平的循环PAPP-A可被分析T和分析C检测。此外,通过PAPP-A亚基特异性抗体A1(图7A)或proMBP特异性抗体A11(图8A),吸附处理可以除去正常血清中的PAPP-A。
ACS患者中的PAPP-A
在ACS患者中的PAPP-A可以被分成两类,即与proMBP复合的形式和未与proMBP复合的形式,二者一起构成分析T检测到的总PAPP-A。与proMBP复合的形式构成基础水平的PAPP-A,可被分析C特异地检测出来。未与proMBP复合的形式和ACS相关,能被Δ值特异地确定,Δ值是用上面提到的两种分析或者双标记分析或者阻断分析或者特别是游离PAPP-A特异性分析来获得。
图7B显示ACS患者中的PAPP-A(与proMBP复合的形式和未与proMBP复合的形式)能被分析T测量,而与proMBP复合的形式(ACS无关的)能被分析C测量。用mabA1吸附有效地去除与proMBP复合的形式和未与proMBP复合的形式(图7B)。不过,用mabA11吸附仅仅去除与proMBP复合的形式(图8B),因此,使得未与proMBP复合的形式,即游离PAPP-A被检测出来。
对来自ACS血清样品和来自妊娠血清样品的级分的分析显示主要由分析C检测到的信号实际与妊娠PAPP-A一致,已知妊娠PAPP-A是与proMBP复合的(参见图9)。
基础PAPP-A水平在正常受试者中的分布
在130例正常老年男性中发现通过分析C测定的PAPP-A浓度与仅仅用PAPP-A亚基特异性抗体通过分析C测定的浓度相关(Y=0.6131+0.59669X,R=0.63,P<0.001)。图10显示的是分别由两种分析测量的血清PAPP-A浓度和相对于正常老年人群的Δ值的数据分布特性的框图。此外,直方图表明Δ值的分布是最少偏斜的,峰度为0.1,而PAPP-A浓度的分布是正偏斜,且具有大得多的峰度(分别为3.47和1.58)。由于与高斯分布相容,适合的Δ值的判断限是可以被推断出来的。
Δ值的临床用途
原则上讲,Δ值仅仅反映ACS相关的PAPP-A。所以,它不受基础PAPP-A水平的影响。基础PAPP-A水平在患者中彼此不同,这可能是干扰仅仅使用PAPP-A亚基特异性抗体的分析得到的测量结果(假阴性和假阳性)的一种原因。
为了解决可能的假阴性问题,我们用两种分析对来自29例ACS患者的亚组的系列血清样品(所有患者的cTnI都升高)进行了分析。当判断限是来源于通过仅仅使用PAPP-A亚基特异性抗体的分析得到的正常受试者PAPP-A浓度的97.5%参考上限(5.68mIU/L)时,所有的患者都是PAPP-A阴性的。不过,当判断限基于定义为平均值+3SD的Δ值时,29例患者中的20例转为PAPP-A阳性。图11显示使用这两种判断限进行比较的一些实例的结果。
用于测量总PAPP-A、或与proMBP复合的PAPP-A或游离的PAPP-A的抗体组合
目前可行的所有可能的双位点抗体组合的特征都在于源于妊娠的PAPP-A和获自动脉粥样硬化斑块的ACS相关的PAPP-A(又称作动脉粥样硬化或斑块来源的PAPP-A)。如表1所示,发现有三种分析或抗体组合。第一种,用于总PAPP-A的抗体组合对源于妊娠的PAPP-A和ACS-相关的PAPP-A均产生高的特异性信号,表明这些分析同样地适合源于妊娠的PAPP-A以及ACS相关的PAPP-A的测量。第二种,仅仅识别PAPP-A/proMBP的抗体组合仅对源于妊娠的PAPP-A而不对ACS相关的PAPP-A产生高的特异性信号,表明这些分析仅适用于妊娠相关的PAPP-A的测量。第三种类型的抗体组合(比如3C8/7A6)对源于妊娠的PAPP-A产生低的特异性信号,但是对优先由游离或未复合形式的PAPP-A组成的ACS相关的PAPP-A产生高的特异性信号。由于PAPP-A在非妊娠非ACS个体中的分子形式与妊娠中发现的形式(即PAPP-A/proMBP)是相似的,像这种形式的分析优选适合于对ACS或动脉粥样硬化相关的PAPP-A的特异测量。
总之,以PAPP-A/proMBP复合物形式存在的基础水平的PAPP-A在样品间经常波动。ACS患者中的病变相关的PAPP-A仅被总分析检测到,而不是被复合物分析检测到。另一方面,基础水平的PAPP-A可被应用这两种分析检测出来。这样的事实为使用去除个体间基础PAPP-A水平波动的影响的游离PAPP-A或Δ值(间接反映游离PAPP-A)的使用奠定了基础。因此,基于游离PAPP-A或Δ值确立的判断限比简单地基于总PAPP-A浓度的判断限更加合理,并且,允许对循环中ACS PAPP-A的精确测量。
讨论
我们已经证明了妊娠中的循环PAPP-A与ACS中的循环PAPP-A是不同的。我们首先证实妊娠PAPP-A是与proMBP复合的复合物,因为它能同等地用抗PAPP-A亚基或者proMBP亚基的抗体作为示踪迹而确定。第二,我们显示了证据,表明ACS相关的PAPP-A不与proMBP复合,因为它仅能用抗PAPP-A亚基的抗体确定。这样的结果被采用另外两种proMBP亚基反应性抗体即mabB5和B6(数据未示出)的分析所证实。第三,我们的结果显示妊娠相关的PAPP-A的分子大小比ACS相关的PAPP-A要大,进一步明确地表明ACS相关的PAPP-A不同于妊娠PAPP-A。
有两种原因能使被单克隆抗体A11识别的表位检测不到。第一,表位消失,这可由于携带表位的亚基缺失或者由于修饰导致分子构象发生改变所致。第二,表位与抗体的结合被表位与另一大分子物质的共价连接或者仅仅存在于ACS血清中的干扰因子所阻断。因为通过掺入妊娠PAPP-A到ACS血清样品中获得了正常的回收结果(数据未示出),干扰因子在ACS血清中存在的可能性可以被排除。并且,由于ACS PAPP-A分子明显地小于妊娠PAPP-A,包括与另一物质的桥接的PAPP-A/proMBP复合物的修饰似乎非常地不可能。因此,循环中的ACS PAPP-A显然不与proMBP复合。
循环中存在的ACS PAPP-A平均大约20mIU/L(22,23),相当于大约6μg/L(26)。这样低的浓度使得难以直接用SDS聚丙烯酰胺凝胶电泳(SDS-PAGE)和Western印迹分析从血样直接检测ACSPAPP-A。所以,需要其它的高灵敏度和高特异性的检测技术用于相关的研究中。镧系螯合物的时间分辨的荧光术是目前存在的最灵敏的检测技术之一(27)。应用这种技术和特异性的抗体,建立了针对总PAPP-A和与proMBP复合的PAPP-A复合物的两种免疫分析。正如所预测的一样,这些分析的优越的灵敏度使得直接从血样检测总PAPP-A和与proMBP复合的PAPP-A成为可能。
妊娠中,循环中超过99%的PAPP-A是以二硫键结合的500kDa的以2∶2与proMBP复合的复合物形式存在,小于1%的PAPP-A是以二聚体的形式存在(5)。在PAPP-A/proMBP复合物中,通过一个单一的二硫键将PAPP-A二聚化,而proMBP是通过两个二硫键二聚化的;每个PAPP-A亚基通过两个二硫键与一个proMBP亚基相连接(32)。最近的研究显示,共价结合对于proMBP抑制PAPP-A的催化活性是必要的,但是,具体的有关复合物形成的机制仍然在很大程度上不为所知。
到目前为止,在循环中尚未发现游离形式的proMBP,尽管有报道proMBP的血清水平在分子基础上超过PAPP-A水平4-10倍(14)。妊娠中其它循环proMBP以两种复合物的形式存在,即一种是proMBP与血管紧张素原的2∶2二硫键结合的复合物,另一种是proMBP、血管紧张素原和补体C3dg的2∶2∶2复合物(14)。在本研究中,两步法被用于以proMBP特异性抗体作为示踪迹的分析中。这种形式的分析有效地防止存在于妊娠血清中的其它proMBP复合物与铕标记的proMBP特异性抗体发生反应,因此,能够特异性地测定PAPP-A/proMBP复合物。
除妊娠外,PAPP-A也被发现可从多种培养的人细胞,比如成纤维细胞(15),成骨细胞(15),卵巢颗粒细胞(33),子宫内膜基质细胞(13)和冠状动脉血管平滑肌细胞(34)分泌。并且,使用免疫组化和原位杂交分析,在卵巢(35),血管斑块(20)和愈合中的人皮肤(26)中体内鉴定出PAPP-A的表达。不过,到目前为止,仅仅证明了从人成纤维细胞条件培养基分离了PAPP-A,和重组表达为400kDa的二聚体(5,15)。
已经证明了由条件培养基中的上述细胞中分泌的PAPP-A具有蛋白水解活性。从所有这些体外来源产生的PAPP-A似乎以二聚体的形式存在。然而,将体外观察结果外推到体内情况是没有证明的。仅仅具有蛋白酶活性并不能用作PAPP-A是以游离形式存在的证据。在妊娠期间,尽管超过99%的PAPP-A是以复合物的形式存在,妊娠血清和纯化的妊娠PAPP-A被证明具有蛋白酶活性(5,15,37)。妊娠血清或妊娠PAPP-A具有可测的蛋白酶活性的原因被归因于少于1%的不受抑制的PAPP-A二聚体的存在或者是来自不能被完全抑制的2∶1PAPP-A/proMBP复合物(5)。
妊娠期间,proMBP在胎盘X细胞中合成,而PAPP-A主要在合胞滋养层中合成(6)。所以,共价的PAPP-A/proMBP复合物必须在分泌后在细胞外腔室内形成。PAPP-A蛋白酶活性可能受到局部环境中的旁分泌作用的调节(13)。在非妊娠的情况下,proMBP已被证实仅仅存在于发育中的嗜酸性细胞中,而不是存在于成熟的嗜酸性细胞中(38)。但是最近,proMBP的免疫反应性也被在体外受精患者的雌激素占优势的卵泡液中检测到(33),并且,proMBP的来源可能与多种卵巢细胞相关(39)。此外,有报道使用β-佛波醇12,13二癸酸酯(β-PDD)进行处理,能诱导培养的产生PAPP-A的人成纤维细胞表达proMBP,从而导致细胞条件培养液中的PAPP-A蛋白酶活性受到抑制(40),表明PAPP-A蛋白酶活性在体内也可能受到自分泌作用的调节。但是,目前尚不清楚为什么proMBP的抑制作用需要复合结构。
以前,动脉粥样硬化被视为“管道”问题。但是近来的研究显示,炎症在动脉病变的发展中起作关键性的作用(41)。和稳定斑块相比,破裂的斑块含有许多炎症细胞(42)。PAPP-A被显示为存在于动脉粥样硬化的不稳定斑块中,但是不存在于稳定的斑块(20)。细胞因子,如来自炎症细胞的TNFα能显著地增加PAPP-A的表达(43)。PAPP-A可能在动脉粥样硬化斑块的炎症反应中发挥重要的作用,从而有助于动脉粥样硬化的进展。最近我们报道了主要的心脏终点的累积危险性与循环PAPP-A水平成正相关(23)。该结果至少间接地支持这样的观点,即PAPP-A(独自或通过IGF)是促进动脉粥样硬化的不良事件的介质。
我们的发现,即循环中源于ACS的PAPP-A不与proMBP复合,可能具有重要的临床意义。在非ACS个体中不同的PAPP-A水平可被测量(22,26,44)。这种免疫反应性的来源尚不清楚,但它们或许源自已被报道存在PAPP-A表达的精液、卵泡液、黄体、睾丸以及其它的器官/组织中(45)。从80例非ACS的年龄为50-69岁的男性受试者中测到的PAPP-A浓度为1.51到7.59mIU/L,其97.5%参考上限为5.68mIU/L(22)。在ACS中PAPP-A浓度增加的程度广泛变化,但通常低于30mIU/L(22,23)。使用参考上限作为判断限,表明显著比例的ACS患者会被漏诊。我们以前已经报道过(22),14例心梗患者中的5例患者的PAPP-A值低于参考上限,但随时间显示明显的动力学改变。最理想的是找出不受个体基础PAPP-A浓度差异影响的判断限。这里显示的我们的初步观察结果揭示在没有ACS的情况下,基础PAPP-A值大多数情况下被用于总PAPP-A测量的分析T和用于PAPP-A/proMBP复合物测量的分析C同等地检测出来,提示PAPP-A的基础免疫反应性由PAPP-A/proMBP复合物所致。低水平(<4mIU/L)的这种复合形式的PAPP-A也在心梗患者中测到,但在急性状况中没有显示或仅显示少许的动力学变化。很明显,与ACS相关的PAPP-A并不与proMBP形成复合物。然而,到目前为止用于ACS的PAPP-A的分析都是测量总的PAPP-A,不论PAPP-A是与还是不与proMBP复合。这种状况对于评价ACS中的PAPP-A构成主要的限制。正如已经显示的,一种用于测定ACS相关的PAPP-A的方法是计算测量到的总PAPP-A和测量到的与proMBP复合的PAPP-A的差(Δ值)。另一种可行的选择是应用正如表1所示的优选用于游离PAPP-A的分析。我们预测,被设计出来用于测量特异性地从不稳定动脉粥样硬化斑块释放的循环形式的PAPP-A的免疫分析很可能会改进PAPP-A作为心脏危险性标志物的临床特异性和灵敏度。
总之,我们的结果提供了第一手证据,即循环的ACS相关PAPP-A不同于循环的妊娠相关PAPP-A,因为它不与proMBP复合。由于早期测量循环的PAPP-A可能对于疑为ACS的患者具有诊断和预后价值,上面的发现因此对于精确测量循环中动脉粥样硬化相关血浆蛋白(AAPP-A)的分析的设计具有重要的临床意义。
可以理解,本发明的方法可以整合为多种实施方案的形式,在这里仅仅公开了其中的一些。本领域技术人员可以明白,存在其它不偏离本发明精神的实施方案。因此,此处描述的实施方案是说明性的,不应该解释为限制性的。

Claims (16)
1.用于定量测定样品中的游离PAPP-A的生物亲和性分析,所述游离PAPP-A定义为未与主要碱性蛋白的原形(proMBP)结合的妊娠相关血浆蛋白A(PAPP-A),其中游离PAPP-A:

2.权利要求1的分析,其中游离PAPP-A是根据选项i)测定的,并且进行了两次分析,其中样品的一份等分样品暴露于与总PAPP-A结合的结合剂,并且检测该结合剂,所述样品的另一份等分样品暴露于仅仅结合于与proMBP复合的PAPP-A的结合剂,并且检测该结合剂,游离PAPP-A的量计算为测定的总PAPP-A和与proMBP复合的PAPP-A的差。
3.权利要求2的分析,其中所述分析是非竞争性夹心分析。
4.权利要求3的分析,其中所述结合剂是捕获结合剂。
5.权利要求3的分析,其中所述结合剂是标记的结合剂。
6.权利要求1的分析,其中游离PAPP-A是根据作为单次双分析物分析的选项i)测定的,其中将样品暴露于与总PAPP-A结合的捕获结合剂和用不同标记物进行标记的两种检测性结合剂,使得用第一种标记物标记的第一种检测性结合剂针对任何PAPP-A分子中存在的表位,其中第一种标记物的信号用于得到总PAPP-A,而用第二种标记物标记的第二种检测性结合剂针对分子的proMBP亚基中的表位,其中第二种标记物的信号被用于得到与proMBP复合的PAPP-A。
7.权利要求1的分析,其中游离PAPP-A是根据选项ii)测定的,是通过将样品暴露于与游离PAPP-A结合但不结合于与proMBP复合的PAPP-A的结合剂,并且检测所述结合剂结合的游离PAPP-A。
8.权利要求1的分析,其中游离PAPP-A是根据选项ii)测定的,是通过使PAPP-A与不能参与生物亲和性反应的proMBP复合,所述反应中将样品暴露于与总PAPP-A结合的结合剂。
9.权利要求8的分析,其中与proMBP复合的PAPP-A被阻断或预先吸附。
10.权利要求2-9中任何一项的分析,其中所述结合剂是抗体、抗体片段或适体。
11.用游离PAPP-A或一种比值作为标志物来诊断急性冠状动脉综合征的方法,所述比值是:
游离PAPP-A/总PAPP-A,
游离PAPP-A/与proMBP复合的PAPP-A,或
与proMBP复合的PAPP-A/总PAPP-A。
12.权利要求11的方法,其中游离PAPP-A是根据权利要求1-10中的任何一项测定的。
13.权利要求11或12的方法,其中总PAPP-A和与proMBP复合的PAPP-A是通过权利要求2-5的两次不同分析或权利要求6的一个单次分析而测定的。
14.权利要求11或12的方法,其中游离PAPP-A是根据权利要求7-10中的任何一项测定的。
15.与游离PAPP-A结合但不结合于与proMBP复合的PAPP-A的结合剂。
16.权利要求15的结合剂,其是抗体、抗体片段或适体。
