Abstract

Description
技术领域
本发明涉及放射治疗设备及操作和控制该设备的方法。
现有的放射治疗设备设法引导辐射波束到达病人体内肿瘤,以便杀死癌细胞。就其本性而言,辐射是有害的,因而正在努力限制辐射照到病人的健康区。通常,这样将涉及使用波束准直器及定向技术,在定向技术中,从各个方向把波束引向肿瘤,从而使肿瘤部位的辐射剂量最大而在邻近区最小。
这种技术依赖于了解肿瘤位置的操作者。这些位置知识常常是从以前的探查及在医疗之后进行脱机分析获得的,供改善随后医疗使用。但是,即使在短的时间内,软组织往往随机移动,限制了病史信息的使用。
这个问题在前列腺癌的医疗中特别严重。前列腺紧靠其他器官,如直肠和膀胱,这两种器官对治疗剂量的辐射作用是敏感的。如果不小心,容易对这些部分产生不希望出现的副作用。但是,这些器官和前列腺的移动是常有的,能够达到3到8mm。这个距离明显大于这些器官之间的间隔,因而给医疗带来困难。
发明内容
已经建议把放射治疗加速器与计算机化的断层摄影(CT)设备结合,以便对前列腺当前位置定位并据此引导辐射波束。把CT扫描的三维结果呈现给操作者,可鉴别前列腺并确认建议需要辐照的体积。这种配置只要能够控制,将提供大的多的医疗精度。
一种困难是,不能用三维(全息)显示,因此必需把数据集投影到两维屏幕上。然后,必须在需要辐照的三维器官的两维屏幕上跟踪。适用的软件能够分析数据集并提供辐照图,但提供的辐照图仍须操作者能看懂,才能使辐照图发生效力。
因此,本发明提出一种适合提供治疗辐射和成像辐射的放射治疗设备,且包括供该成像辐射使用的两维成像器,一计算装置从成像器输出准备断层摄影数据,该治疗源能根据断层摄影数据的反馈进行控制,其中,用该计算装置从成像器输出准备多个相交的切面视图。
这些切面像与操作者熟识的形式类似,操作者习惯以“门”像(portalimage)工作,“门”像是在医疗过程中从通过病人的辐射导出的。但是,该切面像将给出更高反差并显示切面的细节,而不仅仅是投影。解释和看见一系列切面视图比解释和看见三维视图更容易。
这些切面视图为便于观察,最好相互垂直地排列。至少使用三个切面视图,因为每一切面提供两个自由度,从而三个视图将提供六个自由度。这些自由度能交叉校验,确保数据集是可靠的和操作者没有犯严重错误。
这些视图理想的是基本在治疗源的等量点上相交,因为治疗区很可能就在该点,且治疗加速器最佳精度也在该邻域。
每一切面视图是包括许多像素的数字像。一种建立像素的简单途径,是使像素与断层摄影数据集中体像素(voxel)的2D平面对应。但是,这样得出的像很可能包含大量噪声。这些噪声一般通过对邻近像素求平均消除,但代价是降低分辨率。在典型的CT集中,分辨率约每体像素1mm3,因而分辨率的降低可能产生问题。
在本发明中,提供的像的切面性质,能使像素成为横切切面排列的多个体像素的平均结果,通常是相互垂直并理想的是成线性关系。这样能够保持CT集的分辨率,同时通过平均,降低像中的噪声。与3d数据集的显示比较,这样存在以相同分辨率提供改进质量的像的前景,该像还更容易解释。通常用5到20个体像素就能获得好的结果。约10个体像素,即在7到15之间,是适当的。
理想的是,有显示装置向操作者显示这些切面视图。然后根据操作者通过输入装置发出的指令,控制治疗源,输入装置最好是与显示器关联的例如鼠标或其他指针。这种关联体现在显示器上叠加的像,该像能响应输入装置的操作而运动。叠加的像能够从以前对病人的探查和以前对病人的医疗之一导出,所以它与正被查找的区域或像对应。该叠加的像最好是轮廓线,以便清楚看见下面的像。

具体实施方式
现在参考图1,病人10躺在床12或其他支承上。在床的脚(或头)部,是可旋转的载物台14,其上安装有:产生治疗辐射的医疗源16、产生成像辐射的诊断源18、和(例如)非晶硅平板成像器20。通过把成像器与CCD或光电二极管阵列结合,该成像器能把入射成像辐射转换为可见的像输出或(在本例中)数字的像输出。
载物台14可绕水平轴旋转,水平轴与等量点22重合。该等量点是需要医疗的体积的标称中心,并因此在病人工作台之上一段短的距离上。把源16、18对准,以便把辐射引向等量点。通过把诊断源18绕工作台的轴旋转,使之偏离医疗源16,以便两个源都清楚地看见病人。据此把成像器20在载物台14上对准,以便对着诊断源18。
源16、18之间的旋转偏离要充分,以便把成像器20置于医疗源16的辐射场24之外。这样可以保护成像器20,因为成像器20如果与暴露在诊断源的KeV能量辐射相反,暴露在医疗源16的MeV能量辐射中,成像器20将被毁坏。
能产生MeV和KeV两种辐射的源,可以代替医疗源16和诊断源18两者。在这种情形下,必须在医疗时能使成像器移出辐射场。该类型设备在GB-A-2353458中说明。
因此,随着载物台14的旋转,两个源16、18及成像器20一起绕病人旋转,并保持对准。这样可使诊断源18和成像器20建立完整的锥形波束CT数据集。这样还可使治疗源16从许多对准中,把辐射引向病人的病变区域,从而限制把辐射剂量照到需要医疗区周围的非病变区。
图2画出数据处理设备。诊断源18和载物台14受重构服务器26控制,重构服务器26还从成像器20收集数据,并把收集的数据重构成三维CT数据集。这样可把数据提供给医疗控制的计算机28使用,计算机28有显示器30和输入装置32,以便与操作者交互作用。一旦作出医疗决定,使用这些装置控制医疗源16和载物台14。
图3按2D投影画出CT数据集34。数据集34内的器官,包括放大的前列腺36和直肠38。在该视图中,很难在三维像的不同部分认出相对位置。通过软件分析数据集并识别各器官是可能的,但软件的性质及可能配置的装置数量,由操作者确定是明智的。如果这样,则要求操作者对数据集有理解能力。
据此,从数据集34提取三个相互垂直的切片40、42、44。这些切片在数据集34中部的等量点相交。因为设备通常瞄准等量点,这些切片通常包括要治疗的区。然后,如图4所示,在显示器30上显示该三个切片。根据这些像,前列腺36和直肠38(在本例中)之间的区别更为明显。本设备把需要医疗的前列腺像46叠加在其上,该像46是从病人更早诊断中探查拍摄的,或以前医疗中收集的数据得到的。该像46在图4中以虚线表示,虽然该像实际上可取的是用反色显示。
从图4可见,自从收集该像以来,前列腺已经移动;这是常见的,因为病人消化的食物和饮料,使直肠和膀胱对前列腺产生压力所致。直肠和膀胱也移动,这表明,没有同期定位数据,则必须确定辐射场,以避开这些器官先前的定位和环绕它们的安全边界。这表明,整个前列腺被辐照,从而危及医疗是不大可能的。
通过使用同期CT数据,能够查明靶的实际定位和非靶器官,并对剂量作相应调整。操作者能够操纵像46,把它叠在前列腺36上,同时,控制的计算机使用该信息引导医疗源16。该种软件是熟知的,例如,本申请人的iViewTM软件可供使用。
该软件显示的切片类似于“门”像,即用通过病人并被衰减的治疗辐射拍摄的像。操作者习惯于解释门像,因而能容易解释本发明的像。但是,以这种方式使用门像是不适当的,因为目的是要避免对病人施加治疗级的辐射,直到建议的医疗已经被批准为止。
图5画出在上述安排中如何改进像的质量。如在上面指出,从数据集抽取单一切片48。三维数据集包括体像素的3D阵列,各有相应的强度,等等。该片为5到20个体像素厚,通常是10个厚。软件在沿切片48厚度伸延的体像素50中选择一条直线。然后,对这些体像素的强度求平均,把平均值用作对应像54中像素52的强度值。
结果是,像内局部的瞬变值被平滑化,并使像失去许多颗粒,器官变得更容易认出。但是,在平均时通常会丢失的像分辨率没有损失。
显而易见,可以对上述的实施例作许多改变而不偏离本发明的范围。
Claims (18)
1.适合提供治疗辐射和成像辐射的放射治疗设备,且包括供该成像辐射使用的两维成像器,一计算装置从成像器输出准备断层摄影数据,该治疗源能根据断层摄影数据的反馈进行控制,
其中,用该计算装置从成像器输出准备多个相交的切面视图。
2.按照权利要求1或权利要求2的放射治疗设备,其中的切面视图是相互垂直排列的。
3.按照权利要求1的放射治疗设备,其中至少准备三个切面视图。
4.按照权利要求3的放射治疗设备,其中的视图基本上相交于治疗源的等量点。
5.按照前面权利要求任一项的放射治疗设备,其中每一切面视图,是包括如下像素的像,这些像素的值,是从断层摄影数据集中多个体像素导出的。
6.按照权利要求5的放射治疗设备,其中的多个体像素横切关心的切面排列。
7.按照权利要求6的放射治疗设备,其中的多个体像素与关心的切面相互垂直排列。
8.按照权利要求6或权利要求7的放射治疗设备,其中的多个体像素线性地排列。
9.按照权利要求5到8任一项的放射治疗设备,其中采用5到20个体像素之间。
10.按照权利要求5到9任一项的放射治疗设备,其中采用7到15个体像素之间。
11.按照权利要求5到10任一项的放射治疗设备,其中采用约10个体像素。
12.按照前面权利要求任一项的放射治疗设备,其中有显示装置,用于显示切面视图。
13.按照权利要求12的放射治疗设备,其中的治疗源是根据操作者通过输入装置给出的指令控制的。
14.按照权利要求13的放射治疗设备,其中的输入装置是与显示关联的。
15.按照权利要求14的放射治疗设备,其中的关联体现在显示器上叠加的像,该像能响应输入装置的操作而运动。
16.按照权利要求15的放射治疗设备,其中叠加的像是从以前对病人的探查和以前对病人的医疗之一导出的。
