技术领域

背景技术
尽管在肿瘤治疗领域已经有很多进展,但癌症仍然是一种发病率和死亡率很高的疾病。目前已经有了多种主动免疫治疗措施以诱导针对肿瘤抗原的免疫应答。但是,这些策略的临床应用仍然十分有限,这方面的挑战在于,例如,对肿瘤细胞上的自身抗原的耐受、出现免疫逃逸变体、以及需要鉴别有效的且广泛表达的抗原性靶位等。
针对这些挑战的一种免疫治疗措施是使用转染了肿瘤RNA的抗原呈递细胞。美国专利No.6,306,388和No.5,853,719以及相关专利和专利申请公开了采用携带RNA的抗原呈递细胞的癌症治疗方法。
另一种肿瘤治疗措施是抑制血管发生。血管发生是指通过毛细血管萌发而自已经存在的血管发生新的血管。内皮细胞通常是静止的,极少发生增殖。在特定的生理学过程中(如创伤愈合、毛发生长、排卵以及胚胎形成),以及一些病理过程中(如糖尿病视网膜病、银屑病、动脉硬化、类风湿性关节炎、肥胖以及癌症),内皮细胞增殖和新血管发生会明显增加。
除了体积最小的肿瘤,所有肿瘤均需要血供并依赖于肿瘤内的新血管发生。肿瘤的血供增加对于其持续的生长来说是必要的。对血管发生过程及其调控的分子机制的了解的最新进展导致产生了用于治疗癌症的抗血管发生疗法32,34。
制管张素(Angiostatin)和内皮抑素(Endostatin)是有效的特异性血管发生抑制剂的两个代表,其分别通过对其较大的前体,即纤溶酶原(Plasminogen)和胶原蛋白XVIII,进行翻译后裂解而产生。制管张素和内皮抑素的抗肿瘤活性已经在小鼠实验研究中得到证实。有关使用血管发生抑制剂治疗血管发生依赖性疾病(如癌症)的描述见于美国专利No.5,733,876;5,854,205;5,792,845;6,174,861;6,544,758以及相关专利。
基于单克隆抗体的被动疗法也已经被提出作为抑制肿瘤血管发生的一种方法。有关特异于各种血管发生相关抗原(如血管内皮细胞生长因子(VEGF)、如血管内皮细胞生长因子受体(VEGF-R)以及整联蛋白)的单克隆抗体及其作为血管发生抑制剂的用途的描述见于美国专利No.6,524,583;6,448,077;6,416,758;6,365,157和6,342,219。
另一个抗血管发生的积极研发领域涉及采用小分子抑制剂32,34。这些抑制剂被设计为干扰血管发生过程中的关键途径。
正在开发的最主要的一类抗血管发生剂针对的是基质金属蛋白酶(MMP)。一些MMP抑制剂具有一定的临床价值,也有不良反应。目前也在开发针对VEGF和VEGF-R的小分子抑制剂24。
血管内皮细胞生长因子(VEGF)及其受体(VEGFR)在血管发生过程中具有关键的作用,因此它们是治疗性干预的良好靶位。VEGFR-2仅表达于血管发生过程中的内皮细胞,且是内皮细胞中由VEGF介导的引起细胞增殖和迁移的信号的主要转导者。VEGFR-2信号转导对于肿瘤血管发生的重要性的证据表现在VEGFR-2的显性负突变体(Dominantnegative mutant)可阻止小鼠体内肿瘤的生长26。VEGFR-2在肿瘤相关性内皮细胞中被上调,但在肿瘤周围组织的血管中则未被上调27-30。鉴于VEGFR-2特异性表达于血管发生部位的增殖性内皮细胞,以及VEGFR-2信号转导在血管发生过程中具有的关键作用,因此干扰VEGFR-2信号转导代表了抗血管发生治疗的研发和临床测试的合理靶位31-34。
与VEGFR-2类似,Tie2是一种在增殖性内皮细胞上发生上调的受体酪氨酸激酶,并在与其配体促血管生成素-1(Angiopoietin-1)结合后传递促血管发生信号25,34。基因敲除和抑制研究显示,Tie2的功能对于胚胎形成35以及肿瘤的新血管发生36-38是必不可少的。
VEGF是VEGFR-2的配体,其是一种内皮细胞特异性生长因子,且对于血管发生来说是必需的24,31。灭活小鼠的VEGF基因导致血管发育异常以及胚胎死亡39,40。与VEGFR-2或Tie2不同,VEGF在血管发生过程中表达于基质细胞(Stromal cells)24,31。VEGF同样在肿瘤血管发生过程中具有不可或缺的作用,这一点表形在抑制VEGF的功能能够抑制小鼠体内的肿瘤生长41。作为对不断生长的肿瘤内部的渐进性缺氧条件42的反应,大多数人类和鼠科动物的肿瘤会诱导VEGF的表达24,31。实际上,在肿瘤血管发生过程中,肿瘤是VEGF的主要来源24,31。因此,作为一种抗原,VEGF具有靶向肿瘤和其血管的双重作用。
Id蛋白是4种相关的蛋白质的家族,可能参与了对分化和细胞周期进程的调控。在鼠科动物的神经发生和血管发生过程中,Id1和Id3在时间上和空间上均发生共表达,但不表达于来源于鼠科动物和人类的成熟正常组织中。Id1和Id3在生长的肿瘤的微血管系统中重新表达,同时,对基因敲除的小鼠的研究显示,Id1和Id3对于肿瘤异种移植物的血管发生和血管形成(Vascularization)是必需的。因此,这些分子代表了抗血管发生疗法的其他的潜在靶位。
尽管目前的针对癌症患者的抗血管发生疗法已经小时出一定的效果,但这种作用是细胞抑制性的而非细胞毒性的。抑制新血管发生可阻止大体积的肿瘤的生长并可缩小肿瘤的体积,但其无法清除微转移病灶。此外,使用多肽类抑制剂仍然存在制造、稳定性和成本方面的问题。因此,仍然需要寻找更为有效的包括抗血管发生成分的肿瘤治疗方法,其可以单独使用或与其他免疫治疗措施联合使用。本发明即是针对这一需求而提供了一种新的和有效的癌症治疗方法。

本发明题为血管免疫疗法,其提供了一种基于针对血管发生相关性抗原进行主动免疫的新的抗血管发生组合物和方法。术语“血管发生相关性抗原”在此是指那些在优先表达于肿瘤血管发生过程中的内皮细胞特异性产物或那些促成血管发生过程的因子。尽管已经存在被动施用特异性血管发生抑制剂的描述,但此前还没有有关针对血管发生相关性抗原进行主动免疫的报道。
本发明进一步提供了一种将抗血管发生治疗和主动免疫治疗相结合的新的治疗模式。这两种措施是具有相容性的治疗方法,可产生一种协同作用。
在另一方面,本发明提供了一种用于治疗或预防癌症的组合物。所述组合物包含多种抗原呈递细胞,所述细胞转染了编码至少一种血管发生相关性抗原的核酸。
所述抗原呈递细胞优选地是树突状细胞,且所述血管发生相关性抗原优选地选自Id1、VEGFR-2、Tie2和VEGF。
在一个具体优选实施方案中,树突状细胞进一步转染了编码至少一种肿瘤抗原的核酸。所述核酸可以是来自肿瘤细胞的总mRNA或合成的编码一种选定的肿瘤相关性抗原的mRNA。
在另一方面,本发明提供了一种用于预防或治疗癌症的方法。所述方法包含自需要治疗的患者获得抗原呈递细胞,在体外将编码一种血管发生相关性抗原的RNA导入这些抗原呈递细胞中,由此产生携带RNA的抗原呈递细胞,并将所述携带RNA的抗原呈递细胞施用于该患者。
在一个优选实施方案中,编码肿瘤抗原的RNA也被导入所述抗原呈递细胞中,由此产生的携带RNA的抗原呈递细胞能够呈递血管发生相关性抗原和肿瘤抗原。肿瘤RNA和血管发生相关性RNA可同时或序贯地导入。
在本发明的另一方面,可如上制备携带RNA的抗原呈递细胞。然后将携带RNA的抗原呈递细胞与T淋巴细胞接触,从而在体外产生免疫细胞。然后将体外产生的CTL施用于该患者。术语“免疫细胞”在此是指细胞毒性T细胞、辅助T细胞、B细胞、NK细胞和其他免疫调节细胞。

具体实施方式
本发明涉及一种治疗癌症的方法,其包含在需要这种治疗的患者中诱导主动免疫,所述主动免疫针对的是:i)一种优先表达于肿瘤血管发生过程中的内皮细胞特异性产物,和/或ii)一种促成血管发生过程的因子。在此将这些内皮细胞特异性产物和因子共同称为″血管发生相关性抗原″。本发明是基于如下认识:i)可刺激产生一种针对优先表达于(尽管不一定需要仅表达于)肿瘤微血管系统的正常基因产物的免疫应答,ii)这种免疫应答抑制肿瘤的生长,和iii)不产生明显的毒性(自身免疫)。本发明的治疗措施可联合其他一些癌症治疗方案,例如放疗、化疗和常规免疫治疗。
根据本发明,可采用各种方式诱导主动免疫。例如,可将血管发生相关性抗原作为包含单一抗原类型的组合物(疫苗组合物)或作为包含不同类型的血管发生相关性抗原混合物的组合物直接施用。使用的抗原可以是化学合成的或重组产生,也可以使用自天然来源分离的抗原。
根据本发明,也可通过施用编码一或多种血管发生相关性抗原的核酸(RNA或DNA)诱导主动免疫。可以将所述核酸掺入到一种载体中(如病毒载体,如腺病毒载体、腺相关病毒载体、或痘苗病毒载体)。或者,可将所述核酸与一种转染促进剂如脂质体相结合而施用。此外,也可将所述核酸(如质粒中的DNA)作为裸核酸而施用(见例如US专利5,589,466)或以基因枪进行施用(也就是说,将核酸包被于一种颗粒(如金珠)上)。
在一个优选实施方案中,通过给患者施用抗原呈递细胞(APC)而实现主动免疫,所述APC携带了血管发生相关性抗原或在体外转染了编码至少一种血管发生相关性抗原的核酸(DNA或RNA)。
可采用本领域人员熟知的常规技术实现核酸转染,例如通过脂质介导的转染、电穿孔和磷酸钙转染。可通过本领域普遍采用的方法实现将肽暴露于APC(Peptide pulsing ofAPCs)(见例如US专利5,853,719)。
优选地,所述APC是专职APC,例如树突状细胞或巨噬细胞。不过,也可使用任何APC(如内皮细胞或人工产生的APC)。尽管施用于患者的细胞优选地是来自于该患者的(自体)细胞,但APC也可来自于配型相合的供者或来自于体外的细胞培养物。进行单元型(halopytes)配型的方法是本领域已知的。
根据本发明,采用以RNA转染的APC的方法是特别优选的,例如优于使用暴露于蛋白/肽的APC(protein/peptide pulsed APC),原因在于其省却了产生抗原的步骤。有了一个序列,即可通过例如RT-PCR和反转录产生相应的mRNA,可选择将cDNA中间体克隆入细菌质粒,但这并不是必需的12,13。由此,可以省却制备蛋白质抗原或鉴别相应于特定MHC等位基因的I类和II类肽。
用于本发明的编码血管发生相关性抗原的核酸可分离自天然来源(如果需要可进行扩增)或采用常规方法进行化学合成或重组产生。
适合用于本发明的血管发生相关性抗原包括在肿瘤微血管系统中重新表达的胎儿或胚胎基因产物(如Id-1和Id-2)、在肿瘤微血管系统中被上调的VEGF受体(如VEGFR-2)、和内皮细胞特异性产物Tie-2。促血管生成素-1是另一种可用于本发明的抗原。
VEGF表达于肿瘤基质、肿瘤自身或两者均表达。因此,VEGF是一种原型抗原(Prototype antigen),其能够引发针对肿瘤血管系统和肿瘤或肿瘤基质的双重免疫应答。采用VEGF、Id-1和VEGF/Id-1等原型抗原(或其编码核酸)的免疫治疗可具有特别的优势。
根据本发明,可诱导单独针对血管发生相关性抗原或针对血管发生相关性抗原和肿瘤抗原的主动免疫(如TERT或总肿瘤衍生的抗原性混合物(total tumor derived antigenic mixture))(见US专利5,853,719)。
本发明可用于治疗患者(所述患者是指人或非人类动物)已有的肿瘤或用于预防患者发生肿瘤(如黑色素瘤、膀胱肿瘤、乳腺癌肿瘤、结肠癌肿瘤、前列腺癌肿瘤和卵巢癌肿瘤)。优选地在肿瘤形成之前或在肿瘤刚出现时开始治疗,并持续治疗直至癌症情况改善。不过,既便在肿瘤形成之后,本发明也同样适用。当根据本发明治疗患者时,最佳的剂量取决于患者的体重、癌症的严重程度和所针对的抗原的性质等因素。
如果采用APC,细胞剂量以体重为基础选择。通常,可给患者施用的剂量为105-108细胞/kg体重,优选地为106-107细胞/kg体重,以药物学可接受的赋形剂配制。可采用在癌症治疗中常规使用的输入技术施用这些细胞。本领域人员可通过监测患者的疾病的情况而容易地确定对于一个特定患者的最佳剂量和治疗方案,并相应地对治疗进行调整。治疗还可包括施用有丝分裂原(如植物血凝素(phyto-hemagglutinin))或淋巴因子(如IL-2或IL-4),以增强T细胞的增殖。
本发明证实,对癌症进行抗血管发生治疗和肿瘤免疫治疗的联合治疗具有协同作用。通过主动免疫治疗抑制血管发生以控制肿瘤的生长具有多种有吸引力的特征。首先,主动免疫治疗可降低血管发生的活性。其次,与其他抗血管发生策略相似,免疫治疗提供了多种共同靶位,即“通用”抗原,以抑制肿瘤血管发生。此外,由于内皮细胞和基质细胞具有遗传学稳定性和有限的增殖能力,因此,与肿瘤细胞相比,其极少出现抗原丢失或抗原加工丢失的变体(antigen-loss or antigen processing-lossvariants)。不仅如此,抗血管发生免疫治疗的一个特别有吸引力的特征在于其可以与肿瘤免疫治疗联合应用,这样可在一个共同的免疫过程中提供两种不同的并具有潜在的协同作用的治疗模式。
以mRNA转染的DC进行免疫正在成为一种有效的刺激细胞免疫的策略,本发明将这种策略的用途扩展至血管发生相关性靶位。采用由mRNA编码的抗原的一个特别有益的特征是mRNA易于分离和产生。可简单地通过RT-PCR技术自表达所需抗原的细胞中分离cDNA,并可使用无细胞酶反应产生纯化形式的大量mRNA。在下面的实施例中,采用了mRNA技术来研究三种血管发生靶位,即VEGFR-2、Tie2和VEGF。显然,通过基因组的革命性研究可提供更多的候选靶位,因此这些靶位的名单可容易地得到扩充。本发明的另一个优势在于产生mRNA编码的抗原是相对简便而廉价的,并且所需的调节也是简单明了的。
本发明的可行性已经得到以下实验研究的证实。尽管使用了特定抗原和方案,但本发明显然也函盖了其他的抗原和不同的分析方法。
如图1所示并如实施例4所进一步揭示的,与对照动物相比,以Id1 RNA转染的树突状细胞进行免疫使得肺转移癌明显减少。图2和实施例5显示,以Id1 RNA和B16(肿瘤)RNA转染的树突状细胞进行共免疫能够增强这种抗肿瘤作用。
如图3所示并如实施例7和8所进一步证实,诱导针对VEGF和VEGFR-2的CTL应答有可能突破针对血管发生相关性靶位的耐受性。这导致,如图4所示并在实施例9和10中所探讨的,经过免疫的动物具有降低的血管发生活性。这些结果说明,诱导针对VEGF和VEGFR-2的免疫应答具有有效的抗血管发生作用。
针对血管发生相关性产物VEGFR-2、Tie2或VEGF的免疫在B16/F10.9黑色素瘤转移癌和MBT-2膀胱癌模型中抑制了肿瘤的生长。如图5和6所示并如实施例11和12中所进一步探讨的,在进行肿瘤攻击之前对小鼠进行免疫,观察到肿瘤被抑制。如实施例13和14所探讨的并如图7所示,在已经存在肿瘤负荷的情况下,也观察到肿瘤被抑制。由于VEGFR-2或Tie2表达于增殖性内皮细胞中而不表达于MBT-2或B16/F10.9肿瘤细胞中,因此,所观察到的肿瘤的抑制是干扰肿瘤新血管形成过程的间接结果。与这一结论相一致的是,观察到针对VEGFR-2进行免疫后,经免疫的动物的血管发生水平出现下降。与VEGFR-2或Tie2不同,VEGF表达于基质细胞和肿瘤细胞,包括本研究所使用的B16/F10.9和MBT-2肿瘤细胞。因此,VEGF免疫可通过抑制血管发生或直接的抗肿瘤免疫而介导其抗肿瘤作用。
图6和7所示的实验确立了抗血管发生治疗和肿瘤免疫治疗的联合治疗的价值。以同基因型肿瘤RNA(B16/F10.9或MBT-2)进行免疫可刺激肿瘤特异性非交叉反应性保护性免疫,并由此直接靶向肿瘤,而以VEGFR-2或Tie2 mRNA进行免疫则靶向肿瘤血管形成过程。如图6所示,以同基因型肿瘤RNA和内皮细胞特异性mRNA(VEGFR-2或Tie2)两者进行免疫的小鼠,与单独使用两者之一的RNA进行免疫的小鼠相比,前者出现了更优越的抗肿瘤效应。此外,在预先已经存在疾病的情况下,针对肿瘤(TERT或TRP-2)和血管发生特异性(VEGFR-2)靶位进行共免疫,对肿瘤细胞的生长产生了显著的抑制作用(图5)。这些实验也说明,通过主动免疫治疗抑制血管发生具有另一个关键的特征,即在单独一个方案(免疫)中能够施用两种具有相容性和协同作用的癌症治疗模式。针对VEGF与TERT(图6B)、VEGFR-2(图7A)或TRP-2(图7B和7D)之一的联合免疫治疗也具有协同作用,提示针对两种确定的和广泛表达的(“通用”)抗原的策略具有应用价值。尽管在这种情况下还不清楚VEGF的作用究竟是抑制血管发生、直接的抗肿瘤免疫还是两者的结合。
针对血管发生相关性产物进行免疫的一个首要的问题是是否会干扰正常的血管发生,特别是这种干扰是否是持续性的。在本研究和已往的研究中,在取得明显的抗肿瘤效应的情况下,没有在针对血管发生相关性产物进行免疫的小鼠中观察到明显的不良反应。如图8所示,在接受免疫的动物中没有出现发病或死亡,只是在针对VEGFR-2(而非VEGF)进行免疫的小鼠中出现了生殖能力的一过性损害。与此一致的是,已往的研究发现,抗血管发生治疗对肿瘤生长和创伤愈合表现出不同的敏感性48,49,提示血管发生活性的部分和一过性降低可能足以影响肿瘤的生长,却不会引起严重的不良反应。不仅如此,由于保持功能性免疫记忆需要反复进行免疫50,51,因此可简单地通过终止免疫接种来对主动性抗血管发生免疫应答的持续时间进行控制。
上述结果说明,抗血管发生免疫治疗是一种有效的抗肿瘤模式。可通过与针对肿瘤抗原进行免疫相结合而增强针对血管发生相关性抗原进行主动免疫所产生的效应。尽管在此给出的是特定抗原和方案,但显然其他的血管发生相关性抗原和肿瘤抗原也可产生相似的效果。

实施例
尽管在实施例中采用了特定的术语,但这些术语均是描述性的而不是限制性的。其中所涉及的但在说明书和实施例中未具体描述的一些分子生物学、细胞生物学以及免疫学的方法均在科技文献中有记载,并且是本领域人员所熟知的。
实施例1.小鼠和鼠细胞系
小鼠。4-6周龄的C57BL/6小鼠(H-2b)和C3H/HeN小鼠(H-2k)来自Jackson Laboratory,Bar Harbor,ME。在进行本文的研究过程中,研究者遵守了由国家研究理事会生命科学实验室动物资源委员会(LaboratoryAnimal Resources Commission on Life Sciences,National Research Council)所提议的“实验室动物的保护和使用规范(Guide for the Care and Use ofLaboratory Animals)”。Duke大学动物饲养场得到了美国实验室动物保护资格鉴定协会(American Association for Accreditation of LaboratoryAnimal Care)充分认可。
细胞系。C57BL/6来源的B16黑色素瘤F10.9克隆是一种具有高度的转移性、低免疫原性和低I类分子表达的细胞系16。EL4是一种胸腺瘤细胞系(C57BL/6,H-2b)。鼠MBT-2细胞系来自于在C3H小鼠中以致癌物诱导的膀胱肿瘤17,由Dr.T.Ratliff(Washington University,St.Louis,MO)提供。SV40转化的B6成纤维细胞细胞系,BLK.SV(TIB-88),来自ATCC。细胞维持在DMEM中,添加了10%FCS,25mM HEPES,2mM L-谷氨酰胺和1mM丙酮酸钠。在存在收集自以GM-CSF cDNA转染的F10.9细胞的GM-CSF上清液的情况下,产生了来自鼠前体细胞的DC。将生长活跃的F10.9/GM-CSF细胞培养在RPMI 1640中,添加了5%FCS,1mM丙酮酸钠,0.1mM非必需氨基酸,100IU/ml青霉素,100μg/ml链霉素和5×10-5Mβ-巯基乙醇和10mM HEPES(完全RPMI),37℃和5%CO2。经毛细管培养24小时后收集含有GM-CSF的上清液。最终稀释至0.1%的该GM-CSF上清液用于产生鼠DC。所用的GM-CSF的浓度由ELISA测定。
实施例2.制备RNA转染的树突状细胞
按照先前的描述18自骨髓前体细胞产生BMDC(骨髓前体细胞来源的树突状细胞)。简言之,收集来自C57BL/6小鼠胫骨和股骨的骨髓,然后以氯化铵Tris缓冲液在37℃处理前体细胞3分钟,以去除红细胞。将前体细胞接种于RPMI-5%FCS,添加了GM-CSF(15ng/ml)和IL-4(10ng/ml,Peprotech(Rocky Hill,NJ))。细胞以106/ml接种并培养于37℃和5%CO2条件下。3天后,去除漂浮的细胞(主要是粒细胞),并在粘附的细胞中补充新鲜的含有GM-CSF和IL-4的培养基。4天后,收集非粘附的细胞(第7天的未成熟的DC),洗涤并以106/ml接种于含有GM-CSF和IL-4的培养基中。1天后,收集非粘附细胞,洗涤,并以RNA进行电穿孔。
使用RNeasy试剂盒(Qiagen),按照生产商的方案,自活跃生长的肿瘤细胞系中分离总RNA。
按照先前描述的用于人DC的方法进行电穿孔19,20,进行了小的改动。简言之,在第8天收集DC,洗涤,并以2.5×107/ml轻柔地重悬于Opti-MEM(GIBCO,Grand Island,NY)。保留使用过的DC培养基作为条件培养基以备后用。在2mm小管中进行细胞的电穿孔(200μl的DC(5×106细胞),300V,500μs,使用Electro Square Porator ECM 830,BTX,San Diego,CA)。每106DC使用IVT RNA 2μg和总肿瘤RNA 10μg。将细胞立即转移至60mm组织培养用皮氏培养皿中,其中有按1∶1组合的DC生长条件培养基和新鲜的RPMI-5%FCS,并添加了GM-CSF和IL-4。将转染的细胞培养在37℃,5%CO2中过夜,PBS洗涤2次,然后给小鼠注射。
实施例3.肿瘤攻击模型
B16/F10.9黑色素瘤模型:DC以各种RNA制剂进行转染,给原初(naive)同基因型小鼠进行静脉免疫,每只小鼠5×105前体细胞来源的DC(200μl PBS中),共3次,每次相隔7天。末次免疫后8-10天,以静脉注射5×104 F10.9细胞对小鼠进行攻击。当对照组因转移癌死亡时处死小鼠。通过对肺进行称重测定转移癌的负荷。
MBT-2鼠膀胱肿瘤模型:DC以各种RNA制剂进行转染,给原初同基因型小鼠进行静脉免疫,每只小鼠5×105前体细胞来源的DC(200μlPBS中),共3次,每次相隔7天。末次免疫后8-10天,以(侧腹部)皮下注射2-5×105MBT-2细胞对小鼠进行攻击。自第6天开始,隔日一次测定肿瘤的生长。待肿瘤体积达到20mm时处死小鼠。
为进行测试不同抗原之间的协同作用的实验,以每只小鼠各3×105DC(100μl)的每种抗原,即总共6×105DC(200μl),免疫小鼠,共2次。
实施例4.针对Id-1的免疫
在B16黑色素瘤实验性转移系统中测试了针对Id1的免疫所诱导的保护性抗肿瘤免疫。如上所述,首先给小鼠机械能免疫,然后静脉注射B16黑色素瘤细胞(使用了高度转移性克隆F10.9)进行能攻击。28天后,处死小鼠,并通过对肺进行称重来确定肺部的转移癌负荷。如图1所示,以B16肿瘤RNA转染的DC进行免疫,在该模型中明显减少了肺部转移。以Id1 RNA转染的DC进行免疫也降低了转移负荷。
实施例5.以Id1和B16 RNA进行联合治疗
为了确定抗Id1和抗肿瘤免疫治疗是否具有协同作用,采用了与上述相同的实验方案,不同之处在于将免疫的强度自3个循环减少到2个循环,以便更好地观察肿瘤RNA免疫接种与Id1+肿瘤RNA免疫接种之间的差别。
如图2所示,以Id1转染的DC或B16 RNA转染的DC进行免疫明显抑制了肺转移癌,这证实了图1的结果。以Id1+B16 RNA进行联合免疫接种更加有效,尽管在统计学上没有显著意义。这可能是因为,在这个特定的实验中,两种包括肿瘤RNA的免疫均非常明显地减少了肿瘤的转移,这可能在很大程度上掩盖了包括Id1的共免疫的潜在附加作用。
实施例6.制备VEGF、VEGFR-2、Tie2、TRP-2、端粒酶和肌动蛋白RNA
构建pSP73-Sph/A64。将含有64个A-T碱基对且后面接一个Spe I限制性位点的寡核苷酸置于pGEM4Z(Promega)的EcoR I和Nar I位点之间,构建了质粒pGEM4Z/A64。将pGEM4Z/A64的Hind III-Nde I片段克隆入以Hind III和Nde I消化的pSP73(Promega)中,以构建pSP73/A64。通过以Sph I消化pSP73/A64构建了质粒pSP73-Sph,以T4DNA聚合酶添补末端并重新连接。pSP73-Sph/A64/Not含有一个与Spe I邻近的Not I限制性位点。含有小鼠VEGF、VEGFR-2和Tie2的质粒由C.Kontos(Duke University Medical Center,Durham,NC)惠赠。以Advantage DNA聚合酶(Clontech)扩增得到用于克隆入pSP73-Sph的cDNA。
克隆SP73-Sph/VEGF/A64。使用正向引物5’-TATATATCTAGAGCCACCATGGCACCCACGACAGAAGGAGAGCAGAAG-3’(SEQ ID NO:1)和反向引物5’-TATATAGAATTCTCACCGCCTTGGCTTGTCACATC-3’(SEQ ID NO:2)自所述质粒中扩增截断形式的VEGF编码区域(不包括信号序列),并克隆入pSP73-Sph/A64的Xba I-EcoR I位点之间。

克隆pSP73/Tie2/A64/Not。使用了正向引物5’-TATATATCTAGAGCCACCATGGACTCTTTAGCCGGCTTAGTTC-3’(SEQ ID NO:9)和反向引物5’-TATATAGAATTCCTAGGCTGCTTCTTCCGCAGAGCAG-3’(SEQ IDNO:10)自质粒DNA中扩增Tie2编码序列。将这些片段克隆入pSP73/A64/Not中的Xba I-EcoR I位点。
克隆pSP73-Sph/TRP-2/A64。自活跃生长的B16/F10.9细胞中分离总RNA。以一种锚定寡dT引物(anchored oligo dT primer)引发反转录,使用正向引物5’-GATGGATCCAAGCTTGCCACCATGGGCCTTGTGGGATGG-3’(SEQ ID NO:11)和反向引物5’-GTTAGATCTGCGGCCGCTAGGCTTCCTCCGTGTATC-3’(SEQ ID NO:12)自第一链扩增TRP-2 cDNA。以Bgl II和BamH I消化所得的产物并克隆入pSP73-Sph/A64的BamH I位点。
克隆pGEM4Z/murineTERT/A64。将pGRN188(Geron Corp.,MenloPark,CA)的EcoR I片段克隆入pGEM4Z/A64/Not的EcoR I位点。以Not I进行线性化,随后进行体外转录(Ambion mMessage mMachine kit,Austin,TX),得到一种转录产物,其含有61nt的来自pGEM4Z的多接头(polylinker),随后是34nt的mTERT的5′UTR,3366nt的mTERTORF,36nt的mTERT的3′UTR,64A残基,一个Spe I位点和一个Not I半位点。
克隆pGEM4Z/murine actin/A64。以寡dT引发来自F10.9细胞的总RNA的反转录,并使用PowerScript反转录酶(Clontech)进行反转录。使用正向引物5’-TATATAAGCTTCTTTGCAGCTCCTTCGTTG-3’(SEQID NO:13)和反向引物5’-TTTATGGATCCAAGCAATGCTGTCACCTTCCC-3’(SEQ ID NO:14)自第一链cDNA扩增肌动蛋白的编码序列。将PCR片段克隆入pGEM4Z/A64的Hind III-BamH I位点。
实施例7.体内诱导CTL
产生CTL。如上所述产生来源于骨髓前体细胞的DC并以RNA进行转染。以静脉注射的形式免疫原初(
)同基因型的小鼠,给每只小鼠注射5×105来源于前体细胞的DC(200μl PBS中),共3次,每次相隔7天。末次免疫后8-10天收集脾细胞,并以氯化铵Tris缓冲液去除红细胞。将107脾细胞与2×105刺激细胞(stimulator cells)(以RNA进行电穿孔的DC)培养于6孔培养板中的各个孔中,每孔5ml IMDM,添加了10%FCS,1mM丙酮酸钠,100IU/ml青霉素,100μg/ml链霉素和5×10-5M β-巯基乙醇。以用于免疫的相同抗原刺激应答细胞。将细胞在37℃和5%CO2条件下培养5天。在第5天以Histopaque 1083梯度收集效应细胞,然后用于CTL分析。
体外细胞毒性分析。在4℃以铕标记5-10×106靶细胞20分钟。将104铕标记的靶细胞和系列稀释的效应细胞以不同的E∶T比接种于200μl的完全RPMI 1640中。将培养板500g离心3分钟,并在37℃孵育4小时。收集50μl的上清液,通过由时间分辩荧光(time resolvedfluorescence)来测量铕的释放21。使用下式确定特异性的细胞毒活性:%特异性释放={(实验性释放-自发释放)/(总释放-自发释放)}×100。在所有的分析中,靶细胞的自发释放低于总释放(以去垢剂引起)的25%。一式三份的培养物的平均值的标准误低于5%。
实施例8.应答于以血管发生相关性抗原进行免疫的CTL活性
为了确定免疫是否能够克服对血管发生相关性产物的免疫耐受,以转染了VEGFR-2或VEGF mRNA的同基因型DC对C57BL/6小鼠进行免疫,并如上所述进行体外刺激后,测量脾细胞群的CTL应答。用于测定CTL的靶位是转染了肌动蛋白mRNA、VEGF mRNA或VEGFR-2mRNA的同基因型的BLK.SV肿瘤细胞(H-2b)。通过RT-PCR测定发现,BLK.SV细胞与大多数肿瘤细胞相似,表达VEGF(结果未显示)。如图3所示,以转染了VEGF mRNA的DC免疫小鼠刺激了CTL,后者识别所有的BLK.SV靶细胞。图3证实,来自针对VEGFR-2进行免疫的小鼠的CTL仅识别以VEGFR-2 mRNA转染的靶细胞。与此一致的是,BLK.SV肿瘤细胞不表达VEGFR-2(结果未显示)。与此相反,转染了肌动蛋白mRNA或其他mRNA的BLK.SV细胞不能被来自针对肌动蛋白进行免疫的小鼠的CTL识别。这说明,尽管VEGF或VEGFR-2是正常的基因产物,但针对VEGF或VEGFR-2的免疫仍有可能克服免疫耐受,而针对肌动蛋白则不能。大概这是因为,VEGF和VEGFR-2,以及很多其他血管发生相关性产物,具有受限的组织特异性表达模式。实施例9.背部皮褶Window Chamber分析(dorsal skin-fold windowchamber assay)
用于背部皮褶Window Chamber分析的详细的设计和手术技术另有描述22,23。简言之,以转染了VEGF或VEGFR-2的DC或以PBS免疫小鼠,将小鼠随机分为3组。由一位不了解实验详情的研究者进行所有剩余的过程和测量。手术设置window chamber后5天,给小鼠植入肿瘤细胞(表达GFP的B16/F10.9细胞)。这种方法能够确保不会因手术引起的血管改变而对结果分析产生干扰。肿瘤植入后第4天,评价免疫对小鼠的肿瘤生长和血管形成的影响。以整体肿瘤的低倍放大图像对肿瘤区域进行测量。使用高倍镜(物镜,×20)随机选取4该肿瘤区域来评价肿瘤的血管系统。使用图像分析软件来测量每幅图像中所有清晰的血管的累积长度。将画面中的总的血管长度密度除以画面的面积,由此计算出血管长度密度。所有图像均经过同样放大倍数的测微计图像进行校正。
实施例10.以皮瓣window chamber模型(skin flap window chamber model)测定新血管发生
为了确定针对VEGFR-2或VEGF之一对小鼠进行免疫是否会抑制血管发生,采用了如上所述的皮瓣window chamber模型来实时监测小肿瘤植入体的新血管系统的发展。给小鼠注射PBS或以VEGFR-2或VEGF mRNA转染的DC进行免疫,每个一周一次,共3次。末次免疫后4周,通过手术设置window chamber。5天后,在window chamber中植入表达绿色荧光蛋白(GFP)(以便于随后进行分析)的B16/F10.9黑色素瘤细胞。如以往所述23,通过图像分析来每天监测肿瘤区域的血管侵入并对其进行定量。图4显示了血管侵入表达GFP的植入的瘤块内(绿色-图4的第二和第四栏)。注射PBS的小鼠显示出微血管侵入植入的肿瘤内部的典型图形,说明存在正常的血管发生。与此相反,在针对VEGFR-2或VEGF之一进行免疫的小鼠这观察到,植入的肿瘤中明显缺少微血管系统。这说明,针对这些抗原进行免疫与血管发生的部分抑制相关联。采用图像分析来测定微血管侵入的时间以及微血管系统的密度证实了注射PBS的对照小鼠和针对血管发生产物进行免疫的小鼠之间的差别(结果未显示)。图4所示的结果是各组中随时间观察到的结果的代表性结果。
实施例11.针对内皮细胞产物和肿瘤抗原的免疫具有协同作用为了确定在针对血管发生相关性产物进行免疫的小鼠中观察到的血管发生率的降低是否影响了肿瘤的进展,在B 16/F10.9黑色素瘤实验性转移癌模型16和皮下植入的MBT-2膀胱肿瘤模型11,17中测试了以VEGFR-2、Tie2或VEGF进行免疫的小鼠中的肿瘤生长抑制作用。RT-PCR分析证实,B16/F10.9和MBT-2肿瘤细胞均表达VEGF,而两种肿瘤细胞均不表达VEGFR-2或Tie2(结果未显示)。在图5A所示的实验中,使用了B16/F10.9实验性转移癌模型来测定免疫对肺转移癌的影响。将相应于VEGF、VEGFR-2或Tie2的mRNA转染入同基因型的骨髓来源的DC,并用于免疫C57BL/6小鼠,隔周一次,共3次。末次免疫后8天,以B16/F10.9肿瘤细胞静脉注射攻击小鼠,并在35天后测定肺部的转移癌。以注射PBS的小鼠或以转染了鼠肌动蛋白mRNA的DC免疫的小鼠作为对照。正如先前在该实验系统中所观察到的,使用以B16/F10.9肿瘤RNA转染的DC进行免疫抑制了肺转移癌的发生(图5A)。以VEGFR-2 mRNA转染的DC进行免疫产生了与之相当的抗转癌移效应。另一方面,以Tie2或VEGF mRNA转染的DC进行免疫产生了更加显著的作用。相似的肿瘤抑制作用也见于MBT-2膀胱肿瘤模型(图5B)。由于VEGFR-2或Tie2表达于增殖性内皮细胞但不表达于MBT-2或B16/F10.9肿瘤细胞,而针对以上各种产物的免疫又抑制了小鼠的肿瘤生长,因此,所观察到的抑制作用必然是由对肿瘤血管发生的抑制作用所介导的。对这一结论构成支持的是,针对VEGFR-2进行免疫在接受免疫的小鼠中降低了血管发生的水平。
实施例12.抗血管发生和免疫治疗相联合
为了确定靶向肿瘤的免疫学损伤并同时预防肿瘤血管形成是否能够产生协同作用,使用了以B16/F10.9和MBT-2肿瘤RNA转染的DC来刺激针对肿瘤细胞表达的抗原的免疫应答。肿瘤RNA的来源是去除了正常细胞(如内皮细胞)的培养的肿瘤细胞系。还应当注意的是,在以肿瘤RNA转染的DC进行免疫的小鼠中引发的免疫应答针对的是独特的、且非共有的肿瘤抗原,这可由这些肿瘤之间不存在交叉反应性这一点而断定11。图6A显示,在B16/F10.9肿瘤模型中,以B16/F10.9肿瘤RNA和Tie2 mRNA进行共免疫的效果优于以两者中的任何一种单独进行免疫的效果。类似地,图6B和6C显示,在MBT-2模型中,以MBT-2 RNA和VEGFR-2 mRNA转染的DC进行共免疫的效果优于以两种抗原中的任何一种单独进行免疫的效果,其显著延迟了肿瘤的出现。这些实验证实了针对肿瘤和其血管系统进行联合免疫的价值。端粒酶在正常组织中是沉默的,但在超过85%的癌症中发生重新激活43,端粒酶的蛋白(TERT)组分可作为癌症免疫接种的广谱抗原11,44,45。以往证实,针对TERT的免疫可激发出抗多种无关来源的肿瘤的CTL和保护性肿瘤免疫11。图6B进一步证实,针对VEGF和TERT两者对小鼠进行免疫优于单独针对VEGF或TERT的免疫,提示靶向两种广泛表达的原型“通用”肿瘤抗原能够增强抗肿瘤免疫接种的效力。不过,如上所述,由于VEGF也表达于肿瘤细胞,包括本研究所用的B16/F10.9和MBT-2肿瘤细胞,因此还不清楚抗VEGF免疫的抗肿瘤效应反映的是其对肿瘤的直接作用,或是其对肿瘤血管系统的作用,还是两者均有。
实施例13.对已经存在的疾病进行免疫治疗
B16/F10.9黑色素瘤模型:以1×104 F10.9细胞(侧腹部)皮下注射攻击小鼠。植入肿瘤后3天,给每只小鼠静脉注射5×105前体细胞来源的DC(200μl PBS中)来免疫小鼠,每个7天一次,共3次。自第10天起隔日评价肿瘤的生长。待肿瘤大小达到20mm时处死小鼠。
为了测试不同抗原之间的协同作用,以3×105DC(100μl)的各种抗原免疫小鼠,每只小鼠6×105DC(200μl)。
实施例14.抗血管发生和抗肿瘤治疗对已经存在的疾病的影响
为了确定抗肿瘤和抗血管发生免疫治疗对已经存在的疾病的影响,首先给小鼠植入B16/F10.9肿瘤细胞,随后,按如上所述在植入肿瘤3天后开始进行免疫方案。图7A显示,在这种情况下,抗VEGF的免疫治疗与针对TERT或VEGFR-2的免疫治疗相比具有更显著的效果。小鼠针对TERT和VEGFR-2或针对VEGF和VEGFR-2的共免疫具有协同作用,产生了增强的抗肿瘤效应。图7B和7C进一步说明,针对另一种肿瘤表达的抗原TRP-2(B16黑色素瘤的一种显性抗原46)与VEGF或VEGFR-2的共免疫具有协同作用,显著延迟了肿瘤的生长。
实施例15.抗血管发生治疗对生育能力的影响
在先前的两项研究中,以载有VEGFR-2蛋白的DC进行免疫后10天,接受了免疫的小鼠无法受孕9,而以一种编码VEGFR-2 cDNA的减毒沙门氏菌载体进行免疫的小鼠产生了轻度的伤口愈合延迟,但对生育能力没有影响10。
为了确定本发明的抗肿瘤免疫治疗是否对生育能力产生影响,使用以VEGF、VEGFR-2或肌动蛋白RNA电穿孔的DC对小鼠进行免疫,隔周一次,共3次。末次免疫后1周和8周,将小鼠与未接受免疫的雄性小鼠进行交配。按照3只小鼠一组(每笼2雌1雄)进行实验。记录产下的幼鼠数量,检查幼鼠有无疾病表现和畸形,并记录其断奶后的体重。
在本研究中,尽管在针对VEGFR-2或VEGF进行免疫的小鼠中观察到血管发生率降低,但经过为期6个月以上的观察,没有发现疾病或死亡的现象。不过,在针对VEGFR-2(而非VEGF)进行免疫接种的小鼠中观察到了小鼠的生殖能力受到显著的、尽管是一过性的影响。如图8所示,针对VEGFR-2进行免疫并在其后1周进行交配的小鼠未能受孕,但如果将交配推迟8周进行,则以VEGFR-2免疫的小鼠能够繁殖,其产下的幼鼠的大小和体重均与未经过免疫的小鼠的后代相当。这些结果提示,针对血管发生相关性产物进行免疫接种可产生一过性的不良反应,这可能是主动抗血管免疫应答能够持续一段有限的时期的反映。尽管抗VEGF和VEGFR-2免疫对血管发生产生了相似程度的抑制作用(图4),但两者对生育能力具有不同的作用(图8A),其中的原因尚不清楚,并需要进一步的研究。这提示,在免疫治疗方面,血管发生相关性产物/抗原可能产生不同的毒性表现,同时有可能鉴别出那些具有显著的抗肿瘤活性而又毒性较低的血管发生靶位。
实施例16.统计学分析
本研究中采用Kruskal-Wallis检验对不同的实验组进行比较。采用Mann-Whitney U检验来确定两组之间的肺重量的显著性差异。以小于0.05(P<.05)的可能性代表具有统计学显著性。为了确定肿瘤抗原和血管发生相关性抗原之间的联合治疗的显著性,我们测定了各组中出现肿瘤的时间(出现可触知的肿瘤)。两组间比较采用了对数级检验(Mantel-Haenszel检验)。通过测定各组的肿瘤出现时间来进行两组间的额外比较。

6、权利要求4的方法,其中所述抗原呈递细胞转染了编码至少一种血管发生相关性抗原的mRNA。
7、权利要求4的方法,其中所述抗原呈递细胞是树突状细胞。
8、权利要求1的方法,其中所述血管发生相关性抗原选自Id1、Id3、VEGF、VEGFR-2、促血管生成素和Tie-2。
9、权利要求6的方法,其中所述抗原呈递细胞进一步转染了编码至少一种肿瘤抗原的mRNA。
10、一种用于治疗或预防癌症的组合物,其包含呈递至少一种血管发生相关性抗原的抗原呈递细胞。
11、权利要求10的组合物,其中所述血管发生相关性抗原选自Id1、Id3、VEGF、VEGFR-2、促血管生成素和Tie-2。
12、权利要求10的组合物,其中所述抗原呈递细胞是树突状细胞。
13、权利要求10的组合物,其中所述抗原呈递细胞转染了编码至少一种血管发生相关性抗原的mRNA。
14、权利要求10的组合物,其中所述抗原呈递细胞还呈递至少一种肿瘤抗原。
15、权利要求14的组合物,其中所述抗原呈递细胞转染了编码至少一种肿瘤抗原的mRNA。
16、一种治疗癌症的方法,包括如下步骤:
i)自癌症患者获得抗原呈递细胞;
ii)在体外将编码一种血管发生相关性抗原的mRNA和编码一种肿瘤抗原的mRNA导入这些细胞,由此产生经过转染的抗原呈递细胞;和
iii)将所述经过转染的抗原呈递细胞施用于该患者。
17、一种治疗癌症的方法,包括如下步骤:
i)自癌症患者获得抗原呈递细胞;
ii)在体外以编码一种血管发生相关性抗原的mRNA和编码一种肿瘤抗原的mRNA转染所述抗原呈递细胞;
iii)将步骤ii)的经过转染的抗原呈递细胞与T淋巴细胞相接触以产生免疫细胞;和
iv)将所述免疫细胞施用于该患者。
18、权利要求2的方法,其中所述免疫原性组合物进一步包含一种肿瘤抗原。
