2.背景技术
胃与食管的基底打摺是一种治疗胃与食管倒流病(GERD)的手术,这种疾病的症状是胃酸逆流到食道中,引发食道炎、难以控制的呕吐、哮喘和吸入性肺炎中的一种或几种。基底打摺方法包括将胃的基底折叠环绕在食道的下端并将其固定在适当的位置。传统上,这种方法要经过切开式外科手术,用缝合术将已打摺的胃的基底围绕食道固定,而不穿刺(切开)胃。虽然传统的基底打摺包括将基底和食道打摺,但是在此处,使用该词语包括在食管附近将基底自身打摺。

现在参照附图1,本发明所述的夹子安装器10包括一个柔性卷绕外蛇管12,它具有一个近端14和一个远端16。一个端部操纵器组件18联接到蛇管12的远端16上,一个致动器组件20联接到蛇管12的近端14上。数根推拉线58、60(参照附图2-4在下面进行显示和描述)延伸穿过蛇管12并将端部操纵器组件18联接到致动器组件20上。该夹子安装器10与2001年12月12日提交的在先共有申请序号10/010,096中详细描述的夹子安装器类似。然而,在本申请中,端部操纵组件18专门设计用于基底打摺,所用的夹子明显大于前述共同申请的夹子安装器中的夹子。
附图2-4对本发明第一实施例所述的端部操纵组件18的细节作了说明。端部操纵组件18包括一对夹钳22、24,它们与U形夹26可旋转联接。尤其是,该U形夹26具有一个由U形夹臂30、32限定的中央通道28(在附图4中可以看得最清晰)。虽然因为在内窥器械领域中的通用性而使用词语“U形夹”,但是最好将该“U形夹“26的顶部和底部覆盖,从而使通道28的唯一出口位于远端。夹钳22通过轴34与U形夹臂30可旋转联接,夹钳24通过轴36与U形夹臂32可旋转联接。轴34、36的尺寸被加工成不会明显遮盖通道28。
夹钳22、24基本是相同的。每个夹钳22、24都包括一个近端柄脚38、40,一个安装孔42、44,一个远端钩形砧座46、48和数个中间齿50、52。如附图4中所示,将中间齿50、52排列在夹钳的一侧上,矮壁51、53排列在夹钳的相对侧上,以限定出一个凹槽(或导引通道)54、56。凹槽54、56与每个都具有螺旋表面的砧座46、48相会合。如下面参照附图19-24描述的,该砧座的内部(近端)螺旋表面使夹子固定器弯曲。
每个夹钳的近端柄脚38、40分别经两对连杆62、64和66、68联接到相应推拉线58、60上。连杆62、66基本为L形,并在靠近弯头处通过轴70、72可旋转地联接到U形夹臂30、32上,该轴不会明显遮盖U形夹臂之间的通道28。连杆62、66的一端联接到推拉线58、60上,连杆62、66的另一端与连杆64、68的一端可旋转地联接。连杆64、68的另一端可旋转地联接到柄脚38、40上。每个夹钳22、24与每根推拉线58、60之间的组合联接构成了一个杠杆机构,该杠杆机构增大了从推拉线传递到夹钳的力。尤其是,当夹钳闭合时,提高了机械效益。
如2001年12月12日提交的,名称为“柔性外科夹子安装器”的在先共有申请序号10/010,096中所述,推拉线58、60的近端与致动器组件(在附图1中为20)相联接。
如2001年12月12日提交的,名称为“柔性外科夹子安装器”的在先共有申请序号10/010,096中所述,设置在蛇管内部的夹子推进器(未示出)联接到与致动器组件相联接的推动线(未示出)上。与在先共有申请不同,和在先共有申请中所示的5-7毫米的夹子相比,本夹子安装器的夹钳明显较长,被设计成与大约17-20毫米长的夹子一起使用。
现在参见附图5-8,其中说明了夹钳22′、24′的第二实施例。夹钳22′、24′基本上彼此相同,都被设计成可与附图19-24中说明的任何一个夹子一起使用。每个夹钳22′、24′包括一个近端柄脚38′、40′,一个安装孔42′、44′,一个远端钩型砧座46′、48′和数个中间齿50′、52′。中间齿50′、52′排列在夹钳的一侧,矮壁51′、53′排列在夹钳的相对侧,以限定一个凹槽(或导引通道)54′、56′。凹槽54′、56′与绕单一轴弯曲的砧座46′、48′的内表面相会合。如参照附图19-24在下面进行的描述和附图5中的夹子310所示,砧座的内表面使得夹子固定器弯曲。根据该实施例,如附图6-8中所示,导引通道54′、56′和砧座46′、48′与夹钳22′、24′的垂直轴成一定角度。当夹子被推动穿过夹钳时,该角度使得夹子扭曲,从而夹子的端部发生偏移,例如,如附图5中所示。根据当前优选实施例,导引通道54′、56′和砧座46′、48′与夹钳22′、24′的垂直轴成大约为22度的角。根据本发明所述的方法,与该实施例的夹钳一起使用的夹子,在桥形部预先进行弯曲,以方便穿过成角的通道的移动。
现在,参照附图9,对夹钳22″、24″的第三实施例进行说明。夹钳22″、24″彼此之间是不同的,它们被设计成与附图17-18中说明的那种类型的夹子一起使用。每个夹钳22″、24″包括一个近端柄脚38″、40″和一个安装孔42″、44″。一个夹钳22″以两个间隔开的分离式远端钩子46″、47″为终端,并且具有两排中间齿50″。另一个夹钳24″以一个单一远端钩形砧座48″为终端,并具有两排中间齿52″。中间齿50″、52″排列在夹钳的两侧,一个凹槽(或导引通道)54″、56″位于两排齿之间。如在共有序号10/010,096中所述,凹槽54″以一个底切井(未示出)为终端,而凹槽56″与砧座48″的内部相连续,该砧座的内部为绕单一轴弯曲的表面。本领域普通技术人员可以理解,当夹钳闭合时,砧座48″将停留在钩子46″、47″之间,齿50″将与齿52″交叉在一起。如下面参照附图17-18中所描述的以及在先共有申请序号09/891,775和序号10/010,096中所描述的那样,砧座48″的内表面将夹子固定器弯曲。
现在,参照附图10-14,结合一个现有的内窥镜100对使用本发明的夹子安装器的方法进行说明,该现有内窥镜具有一个单一的空腔,一个小型抓取器102从该空腔中穿出,该现有内窥镜还具有一个附着在内窥镜100上的外部工作通道104,夹子安装器通过该通道传送。该外部工作通道104最好是在2001年8月16日提交的,名称为“当内窥镜在本体空腔中时,在其上方传送医疗器械的方法和装置”的在先共有申请序号09/931,528描述的类型中的一种。
根据本发明的方法,在内窥镜装置从口腔传送到处理部位之后,如附图10中所示,抓取器抓住基底并将其拉入到夹子安装器的已打开夹钳之间。然后,如附图11所示,夹子安装器的夹钳闭合到进入其中的基底上。如附图11和12所示,随着夹钳的闭合,夹钳的中间齿穿透进入其中的基底。如附图12所示和2001年12月12日提交的共有序号10/010,096的描述,当夹钳完全闭合(或尽可能大的闭合)时,最好将它们锁定,然后有选择地释放抓取器,并致动夹子推进器而向前推进夹子106。
在夹子106安装之后,如附图13所示,夹子安装器的夹钳打开,并且夹子106停留在位置上将基底摺叠。根据夹子的位置和病人状况的性质,一个夹子可能是够用的。如果操作者认为还需要其他夹子,那么将夹子安装器移开并重新装入另一个夹子。在重新传送夹子安装器之后,如附图14所示,该过程可以在另一位置重复进行。根据设定的本发明夹子的尺寸,一般会使用1-4个夹子。
根据本发明的一方面,夹子安装器的夹钳上的中间齿应该足够长和足够尖,以充分损伤基底,从而当发生基底愈合粘结时,将已折叠的基底绑定到不再需要夹子的程度。这样,最好是,齿有足够的长度能够穿透基底的所有层。
从前面的描述中,本领域普通技术人员将会意识到,本发明的方法可以用不同类型的抓取器实行。尤其是,如“螺丝锥”之类的可选抓取装置也能与本发明的夹子安装器共同使用来实现本发明的方法。
还可以理解,如在2001年8月16日提交的,名称为“当内窥镜在本体空腔中时,在其上方传送医疗器械的方法和装置”的在先共有申请序号09/931,528中所述,本发明的夹子安装器也可以用其他方法附着到内窥镜上。
如上所述,本发明的夹子安装器的外径大约为6毫米。如附图10-14所示,该夹子安装器与外径大约为12毫米的内窥镜共同使用。如在先共有申请序号09/931,528和60/292,419中所述,为了容纳该夹子安装器,外径大约为7毫米的外部工作通道可选地联接到内窥镜上。
附图15是用水平阴影线示出的12毫米内窥镜100的横截面积的比例显示,该内窥镜带有附着的外部7毫米工作通道104。外径大约为24毫米的现有技术装置108的横截面用斜线阴影线示出。从附图15中可以理解,本发明的方法和装置优先考虑较小的装置,它们更容易从口腔传送,也更容易操纵。与现有技术装置的314平方毫米相比,本发明装置的总横截面积大约为152平方毫米。
如上所述,本发明的夹子安装器也可以与双重空腔内窥镜共同使用。附图16是一个双重空腔内窥镜110的比例显示,该内窥镜具有一个光学空腔112和两个6毫米的工作空腔114、116。与附图15中的装置108相比,内窥镜110具有比现有技术装置更小的横截面积。
本发明的夹子安装器所使用的夹子充分地长于在先共有申请序号09/891,775中和同时提交的申请中所述的,在一般外科手术应用中足够用的7毫米长的夹子,为了确保能够穿透基底的所有层,本发明的夹子的固定器部分充分地更长。
现在参照附图17-18,本发明所述的外科手术夹子210的第一实施例分别包括第一和第二臂212、214,以及它们之间的一个桥形部216,从而两个臂和桥形部总体上呈大致U形结构。第一臂12具有一个卡钩220,第二臂214扩展为(或过渡为)一个可变形固定器222,该变形固定器222具有一个组织刺穿尖端224和数个卡钩结合头,例如226、228。这两个臂在它们之间限定出一个开放空间230。尽管可以使用其它适合的医用材料,但是该夹子210最好由整体的片状钛、钛合金、不锈钢、钽、铂、其他高阻抗(基本不透射线的)材料、镍钛合金、马氏体合金、或者塑料制成。该第一和第二臂212、214以及桥形部216相对坚硬,并且在使用过程中在施加到壁上的力的极限范围内不会发生塑性变形,而固定器222相对容易被夹子安装器塑性变形。
现在参照附图2-4和17-18,当夹子安装器的夹钳22、24处于闭合状态下,在夹子安装器中向前推动夹子210时,如附图18中所示,固定器222弯曲以过位于第一和第二臂212、214之间的开口230并与第一臂212的端部卡钩220相结合。由位于夹子安装器夹钳的钩子46、48的内表面上的凹槽形成的砧座导引固定器222的弯曲,使它刺穿基底并与端部卡钩220联接。
附图17和18中所示的夹子210具有一个可任意弯曲的倒刺232,它提供了一个辅助稳定固定点,借助于防止夹子转动。如附图18中所示,当夹子从基底上方向前推动时,组织抓住倒刺232并将它弯曲。
夹子210在桥形件形物区域216还具有一个耳状物233。当夹子装载到夹子安装器中时,该耳状物由推动机构(未示出)使用来抓住夹子末端。
附图19和20示出了本发明所述的夹子310的第二实施例。该夹子310具有两个臂312、314,它们通过桥形件316相连。这两个臂都终止于分别具有尖端321、323的固定器320、322中。夹子310在桥形部316上还具有一对耳状物333、335。当夹子装载到夹子安装器中时,该耳状物由推动机构使用来抓住夹子末端。本实施例倾向于和带有钩子的夹子安装器共同使用,所述的钩子具有分叉的或者处于平行平面中的内部凹槽。参照附图2-4和15-16,当向前推动夹子310时,固定器320被钩子46内部的凹槽弯曲,固定器322被钩子48内部的凹槽弯曲成附图20中所示的结构。从附图20中可以理解,每个固定器穿透基底两次,基本形成环状的紧固件。从而,也可以理解,固定器320、322明显比附图17和18中所示的固定器222长,并且其长度最好至少是两臂312、314之间距离的π倍。如果达到固定器320、322中的每一个都形成完整紧固件的程度,那么臂312、314和桥形件316的作用将是多余的。
附图21-23说明了本发明所述的夹子410的第三实施例。除了从臂412、414上去除了固定器420、422之外,夹子410与夹子310类似(对于类似部件用类似附图标记加100来表示)。臂412、414终止于阴性联接器413、415中,这些阴性联接器以略微干涉配合接纳固定器420、422的端部。夹子410在桥形部416还具有一对耳状物433、435。当夹子装载到夹子安装器中时,该耳状物由推动机构(未示出)使用来抓住夹子末端。该耳状物433、435也可以用作这样一个结构,通过该结构,使已安装到组织上的夹子例如与圈套器相配合,从组织上将夹子拉动并移除。夹子410用基本与上述参照架子310描述的方法相同的方法安装到基底上。然而,在固定器420、422被砧座弯曲且夹钳打开之后,夹子410没有从夹子安装器上释放,固定器与臂412、414分离。附图23示出了由固定器420、422所形成的紧固件。它们实际上是两个基本平行的“b”形紧固件。这样,只需要安装例如由附图24所示的一个单一固定器就可以了。
附图25、26说明了夹子510的第四实施例。夹子510与夹子310类似(对于类似部件用类似附图标记加200来表示),增加了在两臂512、514之间延伸的中央叉540,从而使得两臂和该叉共同构成“E”形的结构。中央叉540最好包括一系列组织不规则突起(或者不规律性)542,以及一个带倒刺的尖端544。可选的是,该中央叉可以具有一个辐射式的或平的尖端,但其尺寸应该适合于穿刺组织。另外,该中央叉不需要包括不规则突起。然而,当具有该组织不规则突起时,它刮擦和扰乱浆膜引起预期粘结。当夹子被应用到该基底上,中央叉540刺穿基底的组织时,该中央叉可以防止夹子510在垂直于中央叉轴线的方向作不期望的移动。可以用与上述夹子310基本相同的方式,将该夹子510应用到基底上。
附图27至29说明了夹子610的第五实施例。夹子610与夹子510类似(对于类似部件用类似附图标记加100来表示),不同之处在于,臂612、614的固定器622、624是分叉的。该分叉固定器622的第一部份652被夹子安装器的夹钳装置的砧座弯曲,而较短的第二部分654最好基本保持为直的。同样,该分叉固定器624的第一部份656被夹子安装器的夹钳装置的砧座弯曲,而较短的第二部分658最好基本保持为直的。当将夹子应用到基底上并且该直的部分654、658沿着基本平行于中央叉640的方向穿刺到组织中时,该直的部分654、658防止夹子610绕延伸穿过夹子的中央叉的轴作不期望的转动。可以用基本与上述夹子310基本相同的方式,将夹子610应用到基底上。
附图30和31说明了夹子710的第六实施例。夹子710与夹子510类似(对于类似部件用类似附图标记加200来表示),不同之处在于,每个臂712、714都包括一个与固定器722、724相邻的向内突起760、762。该突起760、762对夹子起到防滑功能。同样地,该突起760、762防止夹子710绕延伸穿过夹子中央叉的轴线作不期望的转动,并且辅助防止夹子710在垂直于中央叉轴线的方向移动。可以用于夹子310基本相同的方式,将夹子710应用到基底上。
附图32和33说明了夹子810的第七实施例。夹子810与夹子710类似,只是突起860、862的方向向外。
附图34和35说明了夹子910的第八实施例。夹子910与夹子210类似(对于类似部件用类似附图标记加100来表示)。夹子910的每个臂912、914都具有一个与其末端相邻的卡钩920、921,以及一个最好只有其一半宽度的变形固定器922、924;也就是,该固定器最好为臂912、914宽度的大约一半。固定器922在其末端具有一个穿刺尖端924,以及数个卡钩结合头或倒刺926、927,并且固定器924在其末端同样具有穿刺尖端925,以及数个卡钩结合头或倒刺928、929。该宽度只有一半的固定器922、924彼此之间横向偏置。同样,参照附图36和37,当夹子安装器的夹钳使固定器922、924变形时,每个固定器最好都弯曲180度,并且固定器922上的其中一个卡钩结合头,即卡钩结合头926,最好转回来与卡钩921相配合,而固定器924上的一个卡钩结合头,即卡钩结合头928,最好转回来与卡钩920相配合。另外,夹子910最好具有两个倒刺931、932,分别位于每个臂912、914的内侧,它们在插入组织之后可以弯曲,并起到防止夹子910旋转的辅助稳定固定点的作用。此外,夹子910最好还具有耳状物933、935,它们允许夹子结合用于通过夹子安装器的推进,以及夹子的移除。
相对于上述实施例,可以理解,不需要每个固定器都只有臂的一半宽度,但是固定器的组合宽度最好小于臂的宽度;也就是一个固定器的宽度可以只占臂的三分之一宽度,而其他固定器的宽度可以占臂的三分之二宽度。
在所有的夹子实施例中,夹子的最佳尺寸范围如下。夹子的臂的长度最好大约为15至40毫米,而额外的固定器长度最好大约为2至10毫米。夹子的宽度最好大约为2至6毫米。如果具有中央叉的话,它的长度最好大约为2至20毫米。然而,可以理解,可以将夹子设置为其他相关尺寸。
在此,已经描述和说明了用于胃食管倒流病基底打摺的方法和装置的若干实施例。尽管已经描述了本发明的特别实施例,但并不是希望将本发明限制在该范围内,而是希望本发明的范围宽到现有技术允许的程度,对申请文件的阅读也是这样。另外,尽管已经描述了胃附着到食道上这一种基底打摺方式,但是应该认识到,胃的一部分也能附着到胃的其他部分上,以减小食管下部括约肌的柔顺性,并且上述夹子也能用于这种方法。并且,也应该理解,几个夹子的不同特征能够与其他夹子的特征进行组合,而且不是相对于每个夹子所示出的所有特征都是必需的。例如,在夹子上不具有中央叉的位置处可以设置固定器上的突起。因此,本领域普通技术人员可以理解,在不背离本发明所声明的精神和范围的前提下,可以对已公开的本发明作出另外的改进。
Claims (8)
1.一种外科手术夹子安装器,包括:
a)具有近端和远端的蛇管;
b)联接到所述蛇管的所述远端上的U形夹;
c)可转动地联接到所述U形夹上的第一夹爪;
d)与所述第一夹爪成相对关系可转动地联接到所述U形夹上的第二夹爪,所述第一和第二夹爪具有相应纵向长度,所述第一和第二夹爪中的至少一个限定基本沿着所述纵向长度延伸的通道,且形状加工成对外科手术夹子进行导向;
e)至少一个拉/推线联接到所述第一和第二夹爪上,并穿过所述蛇管延伸至所述蛇管的所述近端;及
f)致动器组件,联接到所述蛇管的所述近端上,所述第一推/拉线的所述近端用于将所述第一推/拉线移动穿过所述蛇管,以使得所述第一和第二夹爪绕着所述U形夹从打开向关闭位置转动,其特征在于,
所述夹爪中的至少一个设置有数个齿,这些齿设置用于刺穿并损伤靠近外科手术夹子的组织。
2.根据权利要求1所述的外科手术夹子安装器,其特征在于,两个所述夹爪都设置有数个齿,这些齿设置用于刺穿并损伤靠近外科手术夹子两侧的组织。
3.根据权利要求1所述的外科手术夹子安装器,其特征在于,所述夹爪中的至少一个具有夹子导向通道,并在所述通道末端具有钩形砧座。
4.根据权利要求3所述的外科手术夹子安装器,其特征在于,每个所述夹爪具有夹子导向通道,并在所述通道末端具有钩形砧座。
5.根据权利要求4所述的外科手术夹子安装器,其特征在于,每个所述砧座都具有螺旋面。
6.根据权利要求4所述的外科手术夹子安装器,其特征在于,每个所述砧座的内部都为绕单一轴弯曲的曲面。
7.根据权利要求4所述的外科手术夹子安装器,其特征在于,每个所述夹爪都具有纵轴和垂直于纵轴的垂直轴,并且
每个所述通道都相对于所述垂直轴呈一角度设置。
8.根据权利要求7所述的外科手术夹子安装器,其特征在于,所述角度大约为22度。
