敷料和用于所述敷料的治疗溶液
本发明涉及权利要求1的前序部分的敷料以及权利要求16前序部分的治疗溶液。
基本上已知可以通过提供湿环境、尤其是通过使用有助于伤口愈合的介质或至少包括含有药物的介质、例如施用溶液或凝胶来改善伤口的愈合。目前实际应用的方法是将敷料或由吸附剂材料构成的一层的这种材料浸入相应的溶液然后将该敷料施用在伤口上。使用该步骤操作方式较为复杂且尤其是当护理较大或较深的伤口时这种情况更为明显,需要频繁地更换敷料,尤其需要受过教育或受过训练的人员
本发明的一个目的是提供导致愈合过程得到改善并简化应用的敷料。为了解决这一目的问题,制成了权利要求1的敷料。制备了权利要求16的可以与该敷料共同使用的治疗介质。
本发明的敷料具有包括应用方式简单在内的使得通过使用一种介质、尤其是改善伤口愈合的溶液在湿环境中护理伤口的优点,且除此之外更换该敷料所用的时间期限也明显增加。例如,当使用本发明的敷料时,能够仅在第一次处理伤口后3天更换敷料,这意味着在一段时间内也将深度伤口封闭至感染的危险明显降低的这类程度。由于时间期限较长,所以可以将该敷料维持在应用的伤口上,且这一结果使得保持伤口处于静止状态,这对快速愈合、尤其是在随后治疗伤口的前几天内是必不可少的。这是可能的,因为治疗介质所用的室中配有至少一个从敷料中延伸出来以用于再注满和/或更新所述治疗介质的连接管。
由于使用了特征在于极为有效和低成本结构的本发明敷料的优选实施方案,所述室由分成几部分的和/或通过治疗介质表面的缠绕物的至少一个通道形成。该室优选至少是软管的一部分。

在附图中,优选适合于护理慢性伤口、但也用于护理开放性、深度伤口的敷料1基本上如多层平面材料所示、例如成截成方形或截成长方形的形状。
在敷料1所示的实施方案中,包括形成施用在伤口上的涂敷层的第一层无菌外层和诸如带孔薄膜或由合适的塑料、例如PVC制成的栅板这样的带孔平面材料。一片外层2下面是由几片无菌吸附剂气体和透湿性材料、诸如粗织物或无纺材料形成的层3。就该附图的实施方案而言,层3包括几片无菌纱布、例如由4片无菌纱布构成。敷料1的另一面或外侧面在实际使用时是面对伤口的敷料的外表面,它由层4构成,层4由平面材料或薄膜、优选塑料薄膜组成的所示实施方案中的一片制成。层4具有半透性,这意味着它对空气和气体是透性的,而不能透过液体。在面对层3的层4内侧上固定有腔或软管部分5,它以蜿蜒曲折型延伸接近通过层4的全部表面且由此通过敷料1的全部表面而形成从敷料1侧面延伸的两端或连接管6,按照这类方式使用表示的实施方案,使得这些端6在相应的敷料1”的两个对侧面1’上彼此延伸。

在应用中,将带有层2的敷料1施用在所护理的伤口上然后使其沿边缘经粘性绷带7或以任何其它方式固定。为了在湿环境中使伤口愈合得到改善,通过端6、例如通过附图2的上端提供适合于治疗伤口的溶液,所用的时间期限为将该溶液排入另一端或连接管6的时间期限;且由此由软管部分5形成的完整室中充满了所述的治疗溶液。随后例如通过合适的塞子封闭其两端6。离开软管部分5中的治疗溶液经敷料的完整表面且由此也通过完整的伤口区域被精细分散。每天将软管部分5内的溶液更新并替换几次,例如每天更新和替换两次。
通过本发明的敷料使伤口保持封闭状态且以最佳方式保持湿润,使得获得了基本上得到改善的愈合效果。经观察,本发明敷料的具体优点在于仅需在几天后、例如仅每隔3天更换所述敷料,也使得一般在一段时间期限后才需要首次更换所述的敷料,在所述的时间期限内甚至深度开放性伤口也已经改善到基本上可以排除更换敷料导致其它感染危险的程度。鉴于施用敷料与首次更换所述敷料之间的间隔时间较长,所以尤其是对护理伤口前几天内开始愈合过程所需的伤口静止也需要得到保证。本发明的敷料还合并了在湿环境中使伤口愈合的优点与长静止期伤口愈合过程的优点,这明显有助于愈合过程、尤其是在前几天内。
作为本发明另一实施方案的附图3表示了也由多层构成且还用于在湿环境中改善伤口愈合的敷料1a的示意图和截面。敷料1a包括将该敷料施用在伤口上的下层10。在伤口上形成无菌涂敷层的该层10由无菌带孔平面材料、诸如适宜塑料、例如PVC或硅的带孔或栅板样薄膜构成。
在层10上配有层11,它用作治疗液体的分布层且由无菌的柔软和吸附剂材料、诸如纱布或无纺材料(vleece)制成。在层10表面上配有面对层11的另一层12,它包括胶囊包裹形式的液体治疗介质,使得通过打开并破碎层12中存在的相应囊而使治疗介质在层11内分布至分布器材料的下侧且由此排入伤口区域。在层12上配有由半透性平面材料制成的覆盖层13,这意味着平面材料不能透过或基本上不能透过液体治疗介质而能够透过空气和气体。

作为治疗介质,例如使用包括锌和铁以及蒸馏水和硫酸的水溶液,即每升溶液中包括:10-100mg Zn和6.5-65mg Fe;例如15-45mgZn和10-30mg Fe;其中可以使用共享的硫酸,条件是获得的pH值为2.5-3.5,优选2.8。在一个优选的实施方案中使用95-97%的硫酸。所述的蒸馏水是DAB 10的双蒸水。作为例如ZnCl2和FeSO4这样的原料,每升溶液中含有约20-209mg ZnCl2和32-325mg FeSO4是合适的。
下面给出了所述治疗溶液的组合物样品:
一升溶液中包括:
双蒸水DAB 10            980g
FeSO4                  0.097g
ZnCl2                  0.063g
剩余的为直到获得pH值为2.8的共享的95-97%硫酸。
就治疗介质的低pH值而言,所述的介质不仅产生酸而且由此使伤口的环境达到最佳。基于这种酸性环境,改善愈合过程的代谢也特别受到刺激。此外,通过所述酸与铁组分组合的治疗介质也以制微生物方式起作用,这意味着产生了预防病菌和细菌生长的环境且该环境起排斥它们的作用,使得可以有效预防进入伤口并在其中发育的病菌导致的炎症。
上述治疗介质(溶液)尤其适合于与敷料1相应的1a联用、例如与用于护理或覆盖移植部位、而且用于治疗已经除去所植的皮肤的供体(例如基层)的皮肤移植。已经通过使用上述治疗介质对猪进行了实验,所用的治疗介质中包括含有30mg锌和20mg铁/升溶液(下文中的0.003%溶液)的组分的治疗介质和仅包括10mg锌和约7mg铁/升溶液(下文中的0.01%溶液)的对比溶液以及包括100mg锌和66mg铁/升溶液(下文中的0.001%溶液)的对比溶液。已经通过进一步的一系列对比实验、使用已知的护理伤口的方法测试了愈合过程。
一般认为,对烧伤的所述实验结果也与护理慢性伤口的功效相关,所述的慢性伤口诸如褥疮、合并有糖尿病的溃疡性跛行伤口愈合缺损等。
附图4、5、6和7的示意图中表示了结果,即附图4中是使用0.001%溶液的结果,附图5中是使用0.01%溶液的结果,附图6中是使用0.003%溶液的结果,且附图7中是暴露在空气中的伤口的结果。这些附图表明使用0.003%溶液获得了最佳愈合效果。
在附图2中,参考号16记述了指示剂的特征,它使用所示的实施方案包括配有指示物的平面材料基质或条状物,在敷料变干的情况中,应用中的所述敷料1的湿度降至预定值以下时,指示剂的颜色改变。将指示剂16整入敷料1,按照这类方式,如果施用敷料1,那么它是可见的。使用所示的实施方案,在面对伤口侧上的敷料层下配有指示剂16,这意味着层4与层5之间带有敷料1且在层13下带有敷料1a。
用于随湿度改变颜色的指示剂的适宜材料对本领域技术人员而言是已知的。指示剂材料基本上适合于本发明,这取决于改变颜色的pH值,因为如果敷料变干,那么pH值会改变。
可以将另外或非表示敷料是否变干的指示剂16的指示剂整入敷料1或1a,该指示剂基于可能增加的敷料微生物污染,使得可以及时更换该敷料。此外,形成指示剂16的这种指示剂且优选配有该指示剂,使得可以从所施用于患者伤口上的所述敷料上看见;且构成了载体材料,它配置了因微生物荷载或污染增加而改变颜色、例如也通过改变pH值的物质。
也基本上存在使相应指示剂16定位于面向伤口的敷料侧面上的可能性,该指示剂指示敷料的含湿量和/或该敷料的微生物条件。
上述已经结合实施方案描述了本发明。然而,可以做大量改变和修订而不会脱离本发明基于的理念。

Claims (21)

1.使用上述权利要求中任意一项的治疗溶液护理伤口、尤其是用于慢性或术后伤口治疗的敷料,它包括彼此邻接排列且相互结合的多层,即至少一层形成覆盖伤口的带孔第一层(2,10)、优选多层化柔软且同时是吸附剂的第二层(3,11)和用于治疗伤口时形成敷料外表面的第三层(4,13),其特征在于在所述层形成的多层排列顺序内配有至少一个将液体治疗介质排入所述的第二层(3,11)中的室(5,12)。
2.权利要求1的敷料,其特征在于至少一个室(5)包括至少一个用于在所述敷料(1)的外表面上提供治疗介质的连接管(6)且该室排列在以精细分散方式将所述治疗介质排入第二层(3)的壁上。
3.权利要求1或2的敷料,其特征在于在至少一个室(5)的壁上带有细小的孔。
4.上述权利要求中任意一项的敷料,其特征在于至少一个室(5)形成为一个通道且带有延伸通过敷料(1)表面的几个通道部分和/或缠绕物。
5.权利要求4的敷料,其特征在于至少一个室(5)在其两端上各形成一个连接管(6)。
6.上述权利要求中任意一项的敷料,其特征在于至少一个室由至少一个软管部分(5)形成。
7.上述权利要求中任意一项的敷料,其特征在于至少一个室(5,12)排列在形成敷料外表面的第三层(4,13)与例如所述第二层(3,11)这样的另一层之间。
8.上述权利要求中任意一项的敷料,其特征在于在面对第一层及其邻接层的第二层的侧面上形成有至少一个室(5,12)。
9.上述权利要求中任意一项的敷料,其特征在于在优选形成为多层的第二层(3,11)中形成有至少一个室(5,12)。
10.上述权利要求中任意一项的敷料,其特征在于所述的第三层(4,13)是半透性层以便使气体和空气进入而对液体形成屏障。
11.上述权利要求中任意一项的敷料,其特征在于至少一个连接管(6)从敷料(1)的一侧延伸。
12.上述权利要求中任意一项的敷料,其特征在于至少一个软管部分(5)固定在所述的第三层(4)上。
13.上述权利要求中任意一项的敷料,其特征在于至少一个室由用于接收治疗介质的至少一个囊形成、优选由多个囊组成的层(12)形成。
14.上述权利要求中任意一项的敷料,其特征在于至少一个室(5,12)用于接收治疗溶液,每升该溶液中除含有水和酸、优选硫酸之外还包括:
10-100mg锌(Zn);
6.5-65mg铁(Fe);
例如
15-45mg锌;
10-30mg铁;
其中可以使用其它酸,条件是获得的pH值约为2.5-3.5,优选2.8。
15.上述权利要求中任意一项的敷料,其特征在于至少一种指示剂(16)用于可选地指示所述敷料的含湿量和/或微生物荷载条件。
16.用于护理伤口、尤其是用于治疗慢性或术后伤口的治疗溶液,其特征在于含有水和酸、优选硫酸的水溶液的每升溶液中还包括:
10-100mg锌(Zn);
6.5-65mg铁(Fe);
例如
15-45mg锌;
10-13mg铁;
其中可以使用其它酸,条件是获得的pH值约为2.5-3.5,优选2.8。
17.权利要求16的治疗溶液,其特征在于它使用了95-97%的硫酸。
18.权利要求16或17的治疗溶液,其特征在于它将ZnCl2用作锌组分,优选其用量为10-100mg/升溶液、例如46-80mg/升溶液。
19.上述权利要求中任意一项的治疗溶液,其特征在于它将FeSO4用作铁组分,优选其用量为32-325mg/升溶液、例如80-114mg/升溶液。
20.上述权利要求中任意一项的治疗溶液,其特征在于每升该溶液包括优选通过使用约63mg ZnCl2和约97mg FeSO4制备的约30mg锌和20mg铁。
21.上述权利要求中任意一项的治疗溶液,其特征在于该水溶液除含有锌和铁外还包括蒸馏水和酸。
