Abstract
本发明涉及一种含有固态碳酸氢钠的药筒(10)。该药筒(10)可用在血液透析机中连续产生缓冲溶液,它与另外的溶液混合形成透析溶液。该药筒(10)还含有固态酸或酸酐,或二氧化碳气体。已经发现,这可以克服透析操作早期阶段透析溶液pH值暂时增大带来的问题。
Description
本发明涉及含有固体碳酸氢钠的透析药筒。
发明背景
使用含有固态药物或其它物质的药筒,使水或溶液流过药筒以连续溶解所述固态物质,例如,用于连续地给病人用药,这早已是公知的了。
这方面的例子可参见WO-A-86/03417和US-A-4432756。
如EP-A-0278100所描述的,把固态碳酸氢钠用作血液透析的缓冲剂也是公知的。碳酸氢钠与其它含有钙、镁离子的透析溶液分开存放,以阻止碳酸钙和碳酸镁沉淀。装碳酸氢钠粉的药筒插在血液透析机上,使水流过药筒。粉末逐渐溶解,从而连续产生碳酸氢钠溶液。该溶液连续流过透析机,与其它的来自透析机管线上游的透析溶液混合。因此,它们混合后在机器中仅有短时间的共存,从而避免了碳酸钙和碳酸镁沉淀的问题。
但是,这种药筒仍存在问题。混合透析溶液的pH值在透析机上游监测。如果pH值超出给定的范围,则引发警报。已经发现,当机器开始工作时,在第一个二十分钟的流动期间经常发生这种情况。这段时间过去后,不再出现问题。这对于操作透析机的人员非常不方便,因为每次警报出现时都不得不调查问题和重新设定机器。
本发明者发现,此问题可能是因为碳酸氢钠粉被少量碳酸钠污染了引起的。由于碳酸氢盐的溶解度小于碳酸盐的溶解度,因此在早期阶段碳酸盐溶解造成了高的pH值。一旦碳酸盐溶解完了,问题就消失了。但是,生产不被碳酸钠污染的碳酸氢钠粉是困难和昂贵的。
一旦认识到了问题的原因是碳酸钠的沉淀,一个可能解决该问题的方法是在管线上游增设一个pH值监测仪器,在使用药筒的第一个二十分钟内把稀释的酸溶液加入到透析溶液中。这可以在药筒的上游或下游进行。但是,这造成了透析溶液的改变,使用了额外的溶液和额外的操作控制。
本发明者发现,通过改变药筒中的物质能相当简单地解决此问题。
发明概述
本发明涉及药筒,它具有可开启的密闭的入口和可开启的密闭的出口,与血液透析机的管线连接,用于使水或溶液流过药筒,药筒中含有固态碳酸氢钠。
根据本发明,药筒还含有固态的酸或酸酐,或者含有二氧化碳气体。
当药筒装在血液透析机上并且水流过药筒时,所述酸或酸酐(包括二氧化碳)逐渐溶解,降低了所得溶液的pH值,从而抵消了碳酸钠污染引起的pH值的任何暂时增大。
所提供的酸或酸酐的量优选地是预定的,这样在最初10到30分钟内,即,在碳酸钠也类似地从药筒中浸出的一段时间内,酸或酸酐从药筒中浸出。
在制造过程中,二氧化碳也可以在密封药筒之前以固态,即干冰,加入药筒中。
可以固态形式使用的酸可以是有机酸,例如柠檬酸或酒石酸,考虑医疗上的可接受性,柠檬酸是优选的。
酸或酸酐在药筒中的含量为每1000g碳酸氢钠至少0.2g,优选的是每1000g至少0.5g,最优选的是每1000g至少1g。在优选的实施方案中,药筒中含有每1000g碳酸氢钠2.7g酸或酸酐。附图

优选实施方案
下面描述本发明的优选实施方案。血液透析机的结构是公知的,血液透析机上使用的碳酸氢钠的结构同样也是公知的。因此,不再详细描述血液透析机和药筒。药筒的类型可以是意大利Bieffe MeditalS.p.A销售的EASYCART牌的。
药筒10包括主体14,它由盖15密闭,形成室11。主体和盖由聚丙烯注塑成形。室11含有颗粒状的晶体碳酸氢钠,但也可以使用其它固态形式。盖15通过超声焊接密封在主体14上。盖15上有入口12,主体14上有出口13,二者分别通过与其连成整体的膜17、18保持成形状态时的密封状态。
药筒10通过第一线20连接在管线中,在21处接收去离子水,在23处把碳酸氢钠溶液供到主线22中。在把药筒夹在机器上时,用机器上提供的穿刺装置在膜17、18上穿孔。主线在24处也接收去离子水。容器30中装有透析溶液的其它成分的溶液,通过第二线25在26处连接到主线22。在点26处形成最终的透析溶液,主线22把最终透析溶液输送到透析机40。pH值检测仪50连接在主线22的点26的下游和透析机的上游之间。检测仪与控制系统相连(未示出),如果pH值超出预定的范围,则产生警报。这个pH值范围通常是6.8~7.9,也可以用其它装置监测,例如通过电导率测量。
容器30中的溶液可以含有任何通常用于透析溶液的成分,例如氯化钙、氯化镁、氯化钠和渗透剂,如葡萄糖。
按照本发明,药筒中除了碳酸氢钠外,还含有固态的酸或酸酐,或者二氧化碳气体,用于避免任何碳酸钠污染引起的透析溶液pH值暂时增大到足以超过预定阈值并引发警报。
药筒的优选实施例含有750g碳酸氢钠和2g柠檬酸,二者都是颗粒状的晶体。碳酸氢钠的量不同时使用相似的重量比。
柠檬酸的替代品优选地以相同的重量比使用。
进行测试时使用的药筒含有750g碳酸氢钠,并分别加入柠檬酸、酒石酸或二氧化碳(以干冰的形式加入)。这些药筒与不加入酸或二氧化碳的相似药筒进行对比。
使用不同的药筒,在Integra(商标)血液透析机上进行了测试。对由于混合透析溶液的pH值超出预定值范围而发出警报信号的药筒进行记录。将每种溶液的实际最大pH值也记录下来。“酸”溶液,即含有透析溶液中其它成分的溶液,是Gambro生产的标准溶液。在每种情况下,所供给的水的pH值为6.1。结果列在下面的表中。参照药筒出现了很多错误警报,但本发明的药筒没出现错误警报。

使用不同的“酸”溶液和不同pH值的水进行了其它测试。在每种情况下,本发明的药筒不会因pH值高或低产生任何警报。
Claims (9)
1.一种药筒,具有可开启的密闭的入口和可开启的密闭的出口,用于与血液透析机的管线连接,使水或溶液流过药筒,药筒含有固态的碳酸氢钠,
其特征在于该药筒还含有固态的酸或酸酐或二氧化碳气体。
2.如权利要求1所述的药筒,其特征在于酸是粉末形态的。
3.如权利要求1或2所述的药筒,其特征在于酸是柠檬酸。
4.如权利要求1或2所述的药筒,其特征在于酸是酒石酸或其它有机酸。
5.如上述权利要求所述的药筒,其含有每1000g碳酸氢钠至少0.2g酸、酸酐或二氧化碳。
6.如权利要求5所述的药筒,其特征在于该药筒含有每1000g碳酸氢钠至少0.5g酸、酸酐或二氧化碳。
7.一种阻止透析溶液的pH值暂时增大的方法,其中所述透析溶液在血液透析机中从不同的成分源连续地产生,所述成分源包括含有固态碳酸氢钠的药筒,所述方法包括使药筒中含有固态的酸或酸酐或二氧化碳气体。
8.如权利要求7所述的方法,其特征在于药筒和其中的物质与权利要求1到6中任一项所述相一致。
9.一种把二氧化碳气体加入到如权利要求1所述的药筒中的方法,其特征在于二氧化碳以干冰的形式加入。
