背景技术
图像引导下经皮肺穿刺活检(“PTNB”)是具有疑似病理过程(例如支气管癌)的患者的既定手术。该手术的目的是获得用于细胞学或组织学检查的组织。该手术通常由放射科医生在图像引导下执行。所利用的成像方式包括荧光检查、计算机断层摄影(CT)和超声。
PTNB根据针的类型进行分类。细针抽吸活检是为了提供细胞学标本,而较大直径的切割针则产生组织学标本。历史上,切割针的并发症发生率相对较高,但随着自动切割针的引入,已经证明细针抽吸和切割针的并发症发生率相当。
在PTNB期间,在图像引导下放置抽吸针(18-22号)或切割针(14-20号),用于样本回收。同轴技术可用于允许在肺道内多次通过,并减少胸膜穿刺的次数。在该技术中,首先插入薄壁引导针(13-19号),定位于病灶处,随后插入抽吸针或切割针。
尽管该手术被认为是安全有效的,但气胸的发生率仍然很高,从12%到61%不等,其中2%到15%需要胸腔引流。如果病灶不靠近胸膜,则气胸的风险显著增加。大多数并发症在活检后立即发生或发生在活检后第一个小时内。因此,在手术之后,患者被置于穿刺部位向下的位置并且在监督下保持至少一小时。
作为PTNB的替代方法,经支气管针吸(“TBNA”)是一种允许对纵隔淋巴结进行采样的微创技术。当与支气管内超声检查(“EBUS”)结合时,能够精确定义纵隔结构。现代装置将超声支气管镜集成到针中,从而允许对感兴趣区域进行实时可视化。据报道,肺癌筛查中EBUS-TBNA的诊断率的灵敏度高达95.7%。因此,EBUS-TBNA正被广泛采用为纵隔淋巴结采样的护理标准。
EBUS-TBNA装置包括超声线性处理阵列和可回缩针。EBUS-TBNA最初用专用的22号抽吸针进行;然而,最近引入了较大的21号针。EBUS-TBNA在9级支气管的近侧管腔中进行,因为它们受到支气管镜外径(6.9mm)的限制。
尽管EBUS-TBNA的并发症非常低,但气胸的发生率仍然很高。EBUS-TBNA术后,气胸发生率估计为0.53%至16.7%。此外,PTNB和EBUS-TBNA手术会造成直径约28mm、长度约5cm的相对较大的圆柱形损伤。
观察到气胸扩大的患者必须放置胸管进行治疗。然而,没有普遍接受的方法来降低气胸率。多种解决方案已被采用,包括快速翻身、深呼气和屏息技术,但这些技术仅显示出轻度/中度效果,风险降低0.1%至15.7%。
其他人已经研究了将各种密封剂材料灌注到损伤中,包括自体血凝块、纤维蛋白胶和凝胶泡沫,但都没有在日常实践中得到广泛应用。这些方法也受到了可变结果的影响,可能是操作者依赖性、制造差异性和实践中的变化的结果。自体血凝块已显示出中等疗效,但存在手术室准备时间长的问题。尽管纤维蛋白胶和明胶技术已经在已发表数据中证明了一些有前景的结果,但尚未对其进行广泛研究。
最近,一种合成的聚乙二醇塞子已经作为BioSentry道密封剂系统(AngioDynamics,Inc.)的一部分被商业化。在随机多中心临床试验中,BioSentry系统导致85%的患者不存在气胸,这在统计学上大于对照组(69%)。然而,如Yousem,S.A.等人,Pulmonary pathologic alterations associated with biopsy inserted hydrogelplugs.Hum Pathol,2019.89:第40至43页所报道,BIOSENTRY塞子的固体性质在21天内诱发异物巨细胞反应和水凝胶的包封。因此,更多孔的塞子将导致降低的异物反应和更快速的愈合。
WO 2008/016983涉及伤口密封组合物,其包含第一可交联组分和第二可交联组分以及至少一种水凝胶形成组分。该组合物还可以包括快速作用材料,例如组织封闭剂,并且该组合物表现出最小的溶胀特性。第一可交联组分和第二可交联组分可各自例如为聚乙二醇,并且水凝胶形成组分可例如为明胶,其可包含完全水合时尺寸在约0.01mm至约5mm范围内的亚单元并且具有约400%至约5000%范围内的平衡溶胀。第一组分和第二组分在体内条件下反应以形成交联的基质,而水凝胶形成组分快速吸收通过组织裂口流出的生物流体,以及增强第一组分和第二组分交联时形成的所得物理密封剂基质屏障。
可吸收明胶海绵(可从Ethicon Inc.,Somerville,NJ商购获得)是交联的、基于明胶的止血剂,为干燥、固体、海绵形式。/>可吸收海绵是无菌的、来自猪的、可吸收的明胶海绵,当施用于出血粘膜区域时,其能够在2至5天内液化,并且在4至6周内被完全吸收。/>海绵可以两种形状获得,立方体或扁平。尽管已知明胶在血液中吸收其重量的40倍,并在体内溶胀至其初始体积的200%,但这种溶胀相对缓慢并导致低溶胀压力。因此,/>标签建议“当放入腔体或封闭的组织空间中时,建议进行最小程度的初步压缩,并应小心避免过度填充(海绵在吸收液体后膨胀)。海绵在吸收流体时可溶胀至其原始尺寸,从而可能造成神经损伤”。
粉末是通过研磨SURGIFOAM海绵并使其通过小于3mm的开口进行筛分而制备的干粉。
也可从Ethicon Inc.商购获得的止血基质是用于生产止血明胶糊剂的试剂盒,其通过将明胶与盐水混合并且随后将明胶基质-盐水溶液混合物在两个连接的注射器之间来回转移数次来制备。/>中的明胶颗粒比/>粉末具有更高的交联度。
可从Baxter商购获得的止血基质同样是用于生产止血明胶糊剂的试剂盒。一旦获得基本上均匀的糊剂组合物,可通过从注射器挤出糊剂将止血糊剂施用于出血部位以促进止血。
申请人现已发现,可通过组合以下各组分来制备改进的组合物,其即使在较大的道(诸如由PTNB或EBUS-TBNA手术造成的那些道)中,也能控制止气(pneumostasis)和止血:(a)第一组分,该第一组分包含:(i)纤维蛋白原溶液,和(ii)水溶液中的预润湿明胶颗粒;以及(b)第二组分,该第二组分包含:(i)凝血酶溶液,和(ii)干燥明胶颗粒。第一组分和第二组分分开储存,例如储存在试剂盒中,并且被配置成用于在使用时混合在一起以形成例如在37℃下可流动且易于交联的组合物。可将该组合物插入到组织道中,以有效和快速地封闭组织道。
发明内容

如本文所用,“泡沫”是指具有通向材料表面的孔的固体多孔材料。
如本文所用,“生物相容性”是指与活组织或活系统相容,对其无毒、无害或无生理反应性,并且不会因此引起免疫排斥。
如本文所用,“生物可吸收的”或“可再吸收的”是指能够在体内降解成较小的分子,这些较小分子的尺寸允许它们被输送到血流中。此类降解和输送逐渐将所述材料从施用部位移除。例如,明胶可被组织蛋白水解酶降解为可吸收的较小分子,由此当施用于组织时,明胶通常在约4至6周内被吸收,而当施用于出血表面或粘膜时,明胶通常在2至5天内液化。
如本文所用,“止血”是指出血减少或停止的过程。在止血期间,三个步骤以快速顺序发生。血管痉挛是第一个反应,因为血管收缩以减少血液流失。在第二步,血小板栓形成,血小板粘在一起形成临时密封,以覆盖血管壁中的破损处。第三步也是最后一步称为凝血或血液凝固。凝血过程中,纤维蛋白丝起到“分子胶”的作用,从而加固了血小板栓。因此,止血材料或化合物能够刺激止血。
如本文所用,“止气”意指将材料递送至肺组织以封闭或密封一个或多个漏气处。
除非另有说明,百分比和量是指按重量计的百分比或量,比率是重量比。除非另外阐明,否则所有范围均包括端值,例如,“4至9”包括端值4和9。
明胶颗粒
预润湿明胶颗粒和干燥明胶颗粒均由符合美国药典(例如,USP 29)规定的规格的可吸收明胶海绵制成。海绵的多孔结构和交联度按照USP方法通过吸水率和消化率来测量。对于每种类型的颗粒,海绵应当吸收不少于其重量35倍的水,并且胃蛋白酶的平均消化时间不超过75分钟。

通过研磨明胶海绵来制备干燥明胶颗粒。它们可具有小于2000微米的粒度D90。即,90%的干燥明胶颗粒可具有小于2000微米的直径。干燥明胶颗粒中的交联度使得它们具有通过USP消化测试测量的低于30分钟的消化时间。当与水溶液混合时,干燥明胶颗粒比预润湿明胶颗粒具有更高的溶胀度(吸收为其自身干重至少35倍的液体,如USP 34专论中所述)。
第一组分
本发明的试剂盒包含第一组分,第一组分包含纤维蛋白原溶液和预润湿明胶颗粒,该预润湿明胶颗粒为湿糊剂形式,还包含水溶液(约150mg/mL明胶)。
纤维蛋白原溶液
本发明的制剂、试剂盒和方法中的纤维蛋白原浓度可在15mg/ml与150mg/ml之间、40mg/ml与100mg/ml之间或55mg/ml与85mg/ml之间的范围内。
在一个试剂盒中,组分由纤维蛋白原和助稳定剂组成,助稳定剂诸如精氨酸、赖氨酸或4-(氨基甲基)-环己烷羧酸(氨甲环酸)、ε-氨基己酸(EACA)以及它们的组合。
根据本发明,纤维蛋白粘合剂的组分可由初始血液组合物制备。血液组合物可以是全血或血液成分,即全血产品诸如血浆。纤维蛋白原组分、蛋白水解酶和催化剂可以是自体的、人的(包括组合的血浆)或非人来源的。
在本发明的一个实施方案中,纤维蛋白原组分由生物活性组分(BAC)组成,该生物活性组分是来源于血浆的蛋白质溶液,其可另外包含氨甲环酸和精氨酸或赖氨酸或精氨酸和赖氨酸的混合物,或其药学上可接受的盐。BAC可来源于冷沉淀,特别是来源于浓缩的冷沉淀。术语“冷沉淀”是指从由全血制备的冷冻血浆获得的血液组分。当通常在0℃至4℃的温度下将冷冻血浆解冻时可获得冷沉淀,这导致形成含有纤维蛋白原和因子XIII的沉淀蛋白。例如,可通过离心来收集沉淀。BAC溶液还包含因子VIII、纤连蛋白、von Willebrand因子(vWF)、玻连蛋白等,例如,如US-B-6,121,232和WO9833533中所述。优选地,BAC组合物可包含稳定剂,诸如凝血酸和盐酸精氨酸。通常,BAC中纤维蛋白原的量在约55mg/ml至约85mg/ml的范围内。BAC溶液优选地不含氨甲环酸,或其可含有约0mg/ml至约80mg/ml至约110mg/ml的氨甲环酸。盐酸精氨酸的量可为约15mg/ml至约25mg/ml。
任选地,将溶液缓冲到生理相容的pH值。缓冲液可由甘氨酸、柠檬酸钠、氯化钠、氯化钙和作为载体的注射用水组成。甘氨酸可以约6mg/ml至约10mg/ml的量存在于组合物中,柠檬酸钠可在约1mg/ml至约5mg/ml的范围内,氯化钠可在约5mg/ml至约9mg/ml的范围内,并且氯化钙的浓度可为约0.1mg/ml至0.2mg/ml。
在另一个实施方案中,使用如美国文献-B-7,125,569和WO02095019中所述的程序,将BAC组合物中纤溶酶原和纤溶酶的浓度降至等于或小于15μg/ml,诸如5μg/ml或更少的纤溶酶原。
纤维蛋白粘合剂试剂盒还可以包含刺激凝块形成的组分,诸如Ca2+、因子VIII、纤连蛋白、玻连蛋白、von Willebrand因子(vWF),这些组分可作为单独的组分提供或与纤维蛋白粘合剂的组分一起配制。
纤维蛋白粘合剂的蛋白质组分可通过重组程序制备。还可以通过重组程序制备纤维蛋白粘合剂的部分蛋白质组分或全部蛋白质组分。
来源于血液组合物的纤维蛋白粘合剂组分通常经历病毒灭活过程,以去除或最小化感染性颗粒。病毒灭活过程可通过纳米过滤、溶剂/去污剂处理、热处理(诸如但不限于巴氏灭菌、γ或UVC辐射(<280nm))或通过本领域已知的任何其他方法进行。术语“感染性颗粒”是指可在生物有机体的细胞中感染或传播的微观颗粒,诸如微生物或朊病毒。感染性颗粒可以是病毒颗粒。
病毒灭活程序可通过在纯化程序之前和/或期间向血液的组合物或成分中添加分子来进行。添加的分子和它们的产物可通过重力、柱色谱法或本领域已知的任何其他方法除去。
感染性颗粒的去除可通过纳米过滤或通过选择性吸收程序(诸如疏水、亲和和离子交换色谱法)进行。病毒灭活程序可分若干阶段进行。例如,可对组合物进行溶剂/去污剂处理、热处理、选择性色谱法和纳米过滤。
预润湿明胶颗粒
预润湿明胶颗粒具有更高的交联度,因此比干燥明胶颗粒溶胀程度小。这些颗粒的预润湿赋予所得密封剂最佳的流动性。此外,预润湿明胶颗粒增加了粘度并物理阻断了反应组分,延长了反应过程并为最终用户提供了更长的工作时间。
预润湿明胶颗粒通常源自猪源,但也可源自其他动物源,诸如源自牛源或鱼源。明胶可以合成方式制备,即通过重组方法制备。明胶可以是交联的。可使用本领域技术人员已知的任何合适的交联方法,包括化学和物理交联方法。
优选地,将预润湿明胶润湿到超过环境吸收的水分,诸如1g干燥明胶粉末含有1g至10g水、盐水或其他水溶液,更优选3g至10g水/溶液。
用于或用作预润湿明胶颗粒的合适明胶颗粒是可从Ethicon Inc.商购获得的止血基质的明胶颗粒。可用于或用作可溶胀明胶颗粒的其他可商购获得的明胶材料包括Gelfoam(Pfizer)、Curaspon(Cura Medical)、Gelitaspon(Gelita Medical)和Gelaspon(KDM)。
第二组分
试剂盒还包含第二组分,该第二组分包含:(i)凝血酶溶液,和(ii)干燥明胶颗粒。
凝血酶溶液
凝血酶溶液含有由凝血酶组成的蛋白水解酶。凝血酶溶液通常包含凝血酶和氯化钙。在添加显像剂之前凝血酶的初始活性可在约2IU/ml至约4,000IU/ml范围内,或在约400IU/ml至约1,200IU/ml范围内。氯化钙在溶液中的浓度可在约2mg/ml至约6.2mg/ml范围内,或在约5.6mg/ml至约6.2mg/ml范围内,诸如浓度为5.88mg/ml。凝血酶溶液还可包含赋形剂。如本文所用,术语“赋形剂”是指添加到药物组合物中的惰性物质。赋形剂的示例包括但不限于人白蛋白、甘露醇、乙酸钠和注射用水。溶液中的人白蛋白可在约2mg/ml至约8mg/ml范围内。甘露醇的浓度可在约15mg/ml至约25mg/ml范围内。也可将约2mg/ml至约3mg/ml范围内的乙酸钠添加到溶液中。
干燥明胶颗粒
相对于第一组分的预润湿明胶颗粒,第二组分中的干燥明胶颗粒具有较低的交联度,因此为所得密封剂组合物提供较大的溶胀度。然而,干燥明胶颗粒不具有第一组分内的预润湿明胶颗粒的所需流动性。
干燥明胶颗粒通常也源自猪源,但也可源自其他动物源,诸如源自牛源或鱼源。明胶可以合成方式制备,即通过重组方法制备。明胶可以是交联的。可使用本领域技术人员已知的任何合适的交联方法,包括化学和物理交联方法。
用于或用作干燥明胶颗粒的合适明胶粉末是可从Ethicon Inc商购获得的可吸收明胶粉末。可用于或用作可溶胀明胶粉末的其他可商购获得的明胶材料包括粉末或泡沫形式的GELFOAM(Pfizer)、CURASPON(Cura Medical)、GELITASPON(Gelita Medical)和GELASPON(KDM)。
制备和使用组合物
组合物可与一种或多种其他生物相容性药剂(诸如能够刺激止血、伤口愈合或组织愈合的那些)同时施用或其自身包含所述一种或多种其他生物相容性药剂。本发明描述了一种原位形成的明胶、纤维蛋白密封剂水凝胶/水胶体复合密封剂,其在经皮或支气管内活检手术后施用于肺道中以防止气胸。肺道基于手术可具有不同的直径,从直径在0.41mm至1.8mm范围内的穿刺活检手术到切除肿瘤结节的新型取芯手术(直径<20mm)。密封剂可在30秒内聚合,产生生化密封,并且复合密封剂可在水合时溶胀,导致形成附加的机械密封。另外,密封剂可用于密封任何实体器官道,包括但不限于肝、肾和脾。
其他药剂的示例包括生物活性和非生物活性药剂,包括但不限于造影剂(诸如碘海醇)、抗感染剂(诸如抗生素)和抗病毒剂;止痛药和止痛药组合;抗蠕虫剂;抗关节炎药;抗痉挛药;抗抑郁药;抗组胺药;抗炎剂;抗偏头痛制剂;抗肿瘤药;抗帕金森病药物;抗精神病药;退热剂、解痉药;抗胆碱能剂;拟交感神经药;黄嘌呤衍生物;心血管制剂,包括钙通道阻滞剂和β阻滞剂,诸如吲哚洛尔(pindolol)和抗心律失常药;抗高血压药;利尿剂;血管扩张剂,包括一般冠状动脉、外周和大脑;中枢神经系统兴奋剂;激素,诸如雌二醇和其他类固醇,包括皮质类固醇;免疫抑制剂;肌肉松弛剂;副交感神经阻断药;精神振奋药物;天然来源或遗传工程蛋白、多糖、糖蛋白或脂蛋白;寡核苷酸、抗体、抗原、胆碱能药、化疗药、放射性药剂、不透射线药剂、骨诱导剂、膀胱抑制剂、肝素中和剂。
组合物可作为糊剂或作为干燥的栓插入到肺道内。例如,可将组合物浇铸到直径例如小于20mm的圆柱形模具中。然后将其冷冻、冷冻干燥并从模具中取出。根据本发明,所得的塞子坚硬而有韧性,并且可用于密封肺穿刺活检道。
任选地,模具可在其内表面上包括一些特征,使得所得的塞子包括例如线、肋、倒钩、凹槽、凹口、狭缝、通道、尖状物、台阶以及它们的组合。或者此类特征可通过刻划、切割或其他机械手段添加到成品塞子上。
可使用本领域已知的施用器施用组合物。
任选地,用圆柱形网保持组合物,将其移动到适当位置,并且通过使网扩张来部署塞子。
以下非限制性实施例将进一步说明本发明。
在实施例中,“明胶I”是指在水溶液中的预润湿明胶颗粒(例如),其满足美国药典(例如USP 29)所规定的规格,“明胶II”是指干燥明胶颗粒粉末(例如粉末),其满足美国药典(例如USP 29)所规定的规格。
实施例1
使用图1中所示的方法如下制备比较试剂盒。该装置是明胶I-明胶II制剂,其形成由内源纤维蛋白进一步稳定的纤维蛋白互连网络。该实施方案含有一(1)单位的明胶I和一(1)单位的明胶II、来自EvicelTM止血试剂盒的五(5)mL的纤维蛋白原和来自EvicelTM止血试剂盒的五(5)mL的凝血酶。所得的塞子是稠糊剂,在使用填塞物时可很好地贴合缺损处,并且交联后具有韧性、弹性和可压缩性。
制备方法
为了制备原位纤维蛋白、明胶复合密封剂,可首先将纤维蛋白原添加到明胶II中,并通过剧烈摇动粉末容器将其掺入到粉末中。应提供时间以允许纤维蛋白原与明胶发生物理相互作用。明胶II为纤维蛋白原的形成提供了支架,使密封剂能够溶胀,并增加了粘度。纤维蛋白原将形成包围明胶颗粒的单层。
如目前在临床中所进行的,应将凝血酶添加到明胶I中。这可以使用双注射器交换方法进行至少六次来产生。
在使用时,可将纤维蛋白原浸泡的粉末滚成球并手动转移到20mL注射器中。然后可将明胶I-凝血酶分散体与纤维蛋白原浸泡的粉末通过双注射器交换方法混合10次。密封剂具有大约30秒的工作时间(注:通常,纤维蛋白密封剂具有小于5秒的工作时间)。
如图1中所示,通过在容器1中剧烈摇动,将来自Evicel止血试剂盒的五(5)mL纤维蛋白原与一(1)单位的明胶II合并。纤维蛋白原/明胶II混合物形成材料球。从注射器1中取出柱塞,将材料球转移到注射器1中,并将柱塞装回。注射器2填充有来自Evicel止血试剂盒的五(5)mL凝血酶。注射器3填充有一(1)单位的明胶I。注射器2和3通过双注射器交换方法混合6次。注射器4填充有凝血酶和明胶I的混合物。通过双注射器方法(8次)将注射器1和4混合在一起以活化密封剂。密封剂具有30秒的工作时间。
使用图2中所示的方法如下制备根据本发明的试剂盒。该装置是需要低挤出力的明胶I-明胶II制剂。该实施方案含有一(1)单位的明胶I、一(1)单位的明胶II、来自Evicel止血试剂盒的五(5)mL的纤维蛋白原和来自Evicel止血试剂盒的五(5)mL的凝血酶。所得的塞子是稠糊剂,在使用填塞物时可很好地贴合缺损处,并且交联后具有韧性、弹性和可压缩性。

密封方法
密封剂可使用典型的明胶I施用器尖端来施加并在30秒内交联。密封剂的粘度和密度可防止密封剂在通气时受到肺正压的干扰。
与第二实施方案相比,该实施方案产生的密封剂使用双注射器方法混合所需的挤出力显著减小。该密封剂具有类似的流动性、稠度和工作时间。
实施例2
开发用于肺道密封的明胶/生物制剂原位密封剂
本研究的目的是评估明胶(即明胶I或明胶II)组分与纤维蛋白密封剂(即Evicel)的生物组分的组合的不同实施方案。
材料:
明胶I
明胶II
Evicel纤维蛋白密封剂(或止血)试剂盒,其由一小瓶冻干的纤维蛋白原(BAC2)和一小瓶凝血酶冷冻干燥粉末以及重组溶液和施用装置组成。

使用ThermoHaake Mars IQ Air通过连续旋转流变测定法测量粘度。使用在24℃下保持恒定的35mm直径平行板几何形状。在10个线性步骤中测试100/s至1000/s应变速率。注:明胶II和纤维蛋白原在应变速率>200/s时是不稳定的,因此从10/s至200/s进行测试。在200/s下比较制剂之间的粘度。
结果在图3和图4中示出。纤维蛋白原和凝血酶表现出牛顿流体特性。基于明胶的制剂表现出剪切稀化特性。存在由生物制剂(p=0.003,双因素ANOVA,附录)和明胶(p=0.002,双因素ANOVA,附录)引起的显著效果。
加入明胶使粘度显著增加了一个数量级以上。对于明胶I制剂,凝血酶的加入使粘度几乎加倍,从405mPas增至710mPas。纤维蛋白原的加入使粘度增至1950mPas,几乎是原来的5倍。对于明胶II,凝血酶进一步加剧了这种趋势,导致粘度从1498mPas略增至1642mPas。纤维蛋白原的掺入导致增加了一个数量级,达到15073mPas。具有纤维蛋白原的明胶II不能在300/s以上测量,因为它超出其线性粘弹性区域,因此是不稳定的。
讨论:
与单独的Evicel纤维蛋白密封剂相比,明胶I和明胶II与盐水的组合使粘度增加了1个数量级以上。明胶I与盐水、凝血酶和纤维蛋白原的组合以及明胶II与盐水和凝血酶的组合获得相当的粘度。然而,当纤维蛋白原与明胶II组合时,粘度比其他明胶制剂增加了一个数量级。
实施例3
离体结果
评估原型在离体猪大肺道模型中实现止气的能力。在该模型中,在测试当天采集新鲜的肺标本,并在测试之前保持湿润。在测试之前,将肺放置在呼吸机上以补充塌陷的肺泡(目的是打开塌陷的无气肺泡)。在测试时,将肺连接到Respironics呼吸机以精确控制通气循环期间的压力。将压力设定为25cm水的吸气压力和5cm水的呼气压力(Δ20cm水)。
用直径为18mm的取芯装置产生肺缺损,所得的穿刺尺寸为直径约20mm、深3cm。通过气泡测试将缺损处的漏气评估为严重。当应用原型时,将压力降低至10cm水的吸气压力和10cm水的呼气压力(无变化)以保持肺扩张。
在原型应用后,典型地对原型进行局部压缩1分钟,同时肺仍然扩张并处于正压下。为了测试性能,在低压下开始对肺通气,并增加到25cm水的吸气压力和5cm水的呼气压力(Δ20cm水)。通过使盐水通过穿刺部位并记录漏气的存在和严重程度来进行气泡测试。对于另一挑战,将通气压力增加至40cm水的吸气压力和5cm水的呼气压力(Δ35cm水)。在压力测试后,从穿刺部位取出原型并对周围组织的粘附性进行定性评估。所测试的特定原型列在每个图像的标题中。
结果示于表3中,其中L1-1和L3-1是指优选的实施方案,其中通过混合明胶I与纤维蛋白原以及单独混合明胶II与凝血酶来制备密封剂的组分。在施用时,将两种组分混合并施用于靶组织。L2-6代表单独使用纤维蛋白密封剂的对照,其是无效的。

将组分的组合混合成糊剂并直接注射到使用取芯装置产生的缺损中。对糊剂施加局部压缩3分钟,同时使肺扩张并处于正压下。在20cm水压下没有观察到渗漏。密封剂良好地粘附到周围组织上。在相同的测试期间,在不同的肺(L3-1)上测试该相同的原型,也成功地密封了用20mm活检穿孔器产生的缺损。
Claims (7)
1.一种用于制备用于密封组织道的组合物的试剂盒,包含:
a.第一组分,所述第一组分包含:(i)纤维蛋白原溶液,和(ii)水溶液中的预润湿明胶颗粒;以及
b.第二组分,所述第二组分包含:(i)凝血酶溶液,和(ii)干燥明胶粉末;
其中所述第一组分和所述第二组分分开储存,并且被配置成用于混合在一起以形成可流动且可交联的组合物。
2.根据权利要求1所述的试剂盒,其中90%的所述预润湿明胶颗粒具有小于1000微米的直径。
3.根据权利要求1所述的试剂盒,其中所述预润湿明胶在过量环境吸收的水分中润湿。
4.根据权利要求1所述的试剂盒,其中所述干燥明胶粉末为颗粒形式,其中90%的所述颗粒具有小于2000微米的直径。
5.根据权利要求1所述的试剂盒,其中所述水溶液是盐水溶液。
6.一种密封组织道的方法,包括:
a)混合
(1)第一组分,所述第一组分包含:
(i)纤维蛋白原溶液,和
(ii)水溶液中的预润湿明胶颗粒;以及
(2)第二组分,所述第二组分包含:
(i)凝血酶溶液,和
(ii)干燥明胶粉末;
以在37℃下形成可流动的、可快速交联的组合物;以及
b)将所述组合物注射到组织道中。
7.根据权利要求6所述的方法,其中所述组织选自由肺组织、肾组织、脾组织、肝组织和骨组织组成的组。
