背景技术
糖尿病肾病(Diabetic nephropathy,DN)是世界范围内终末期肾病(ESRD)的主要病因。DN已成为中国住院的慢性肾病(CKD)的首位病因。目前,DN的主要干预方法仍然是对症治疗,如控制血压、控制血糖、减少蛋白尿、改善肾脏高灌注和高滤过。临床医生仍然缺乏适当的早期诊断、早期干预和分子靶向药物来延迟或终止DN患者的发生和发展。流行病学调查数据显示,在过去20年中,糖尿病引起的其他重要并发症,如心血管事件和中风,得到了有效控制。糖尿病引起的ESRD患病率没有显著变化。由此可见,进一步阐明DN的新发病机制,发现新的分子诊断标志物,揭示新的分子干预靶点,进而建立更准确的DN诊断和治疗模型,已成为遏制这一影响人类健康的重要慢性疾病的紧迫任务和关键。
开展分子标志物研究对DN早期诊断、疗效监测及预后评估有重要意义,同时也为发病机制的研究和治疗靶点的探寻提供新的线索,更有助于靶向个体化精准医疗的实现。但是,DN发病机制非常复杂,不同因素之间还存在相互调控。因此,不能简单的将其拆分后进行独立研究,而更应运用“系统生物学”思想,多层面、多角度进行研究。近年来,组学和高通量筛选等新技术为DN理想标志物的筛选提供更多的思路。蛋白质组学是一种高通量的蛋白质组分检测技术。它可以大规模、高通量、高灵敏度地检测组织、尿液、血浆和血清等多种生物样品中的蛋白质组分及其变化。有效地研究复杂疾病的分子机制和筛选生物标志物是一种很有前途的手段。数据非依赖性扫描模式(Data-independent acquisition,DIA)是基于静电场轨道阱Orbitrap带来的一项全新的、全息式数据非依赖性采集定量技术,是近几年来发展的一种新的质谱数据采集方式。该技术将传统蛋白质组的高通量和质谱绝对定量MRM/PRM的精确定量及高重复性等优点集于一体。与传统的DDA(data dependentacquisition,数据依赖性扫描模式)质谱技术相比,DIA采用了不同的数据扫描模式,将质谱的整个扫描范围划分为几个窗口,然后对每个窗口中的所有离子进行检测和破坏,从而获得样品中所有离子的信息,而不存在遗漏和差异,对低丰度蛋白鉴定方面更具优势。
果糖-1,6-双磷酸酶(fructose-1,6-bisphosphatase,FBP1)是糖异生的关键酶,其在多种类型肿瘤组织中的表达均下调。FBP1低表达与癌症低生存率和高复发率相关。FBP1抑制Warburg效应以及Wnt/β-Catenin、缺氧诱导因子(hypoxia-inducible factor,HIF)及RAS/MAK等信号通路,从而抑制肿瘤进展。同时,FBP1在NK细胞的免疫杀伤过程中也发挥作用。在肾脏中,已有研究表明其主要表达于近端肾小管细胞。病理过程扰乱肾近端小管功能,增加尿液中FBP1的活性[1],测定尿液FBP1的酶活性和含量可以反应肾小管的损伤程度。比如,尿液FBP1酶活性作为特发性肾病综合征(INS)患儿肾近端小管损伤的指标[2]和作为监测睾丸癌患者肾毒性治疗期间近端肾单位损伤的指标[3],以及尿液FBP1蛋白水平作为肾脏移植前缺血导致肾小管损伤的标志物[4]。这些结果提示FBP1可作为肾小管损伤标志物。但是,已有的几篇有关尿液FBP1报道针对的均是急性肾小管损伤,在慢性肾脏疾病情况下,比如糖尿病肾病、膜性肾病、狼疮肾炎、IgA等,FBP1能否能作为肾小管损伤标志以及不同疾病之间是否也存在差异性均不清楚。

本发明的目的可通过以下技术方案实现:
尿液FBP1作为检测靶点在制备判定糖尿病肾病进展的诊断试剂中的应用。
检测尿液FBP1的试剂作为检测靶点在制备判定糖尿病肾病进展的诊断试剂中的应用。
检测尿液FBP1的试剂作为检测靶点在制备区分糖尿病非肾病、糖尿病肾病和非糖尿病肾病的辅助诊断试剂中的应用。
作为本发明的一种优选,所述的非糖尿病肾病选自膜性肾病、狼疮肾炎或IgA肾炎。
有益效果:
本专利用DIA质谱技术对不同病理分期的糖尿病肾病患者(I-IV期)尿液进行蛋白质组学分析,筛选出7个在糖尿病肾病早期(DN I期和II期)患者尿液中特异性富集,而健康对照者尿液中不存在的蛋白:血栓反应蛋白-4(Thrombospondin-4,THSP4)、碳酸酐酶II(Carbonic Anhydrase II,CA II)、半胱氨酸蛋白酶抑制剂SN(Cystatin SN,CST1)、磷脂酶B样蛋白2(Phospholipase B Domain Containing 2,PLBD2)、果糖-1,6-双磷酸酶(fructose-1,6-bisphosphatase,FBP1)、前列腺干细胞抗原(Prostate stem cellantigen,PSCA)和脯氨酰羧肽酶(Prolylcarboxypeptidase,PRCP)。
随后利用ELISA方法对初筛的蛋白进行验证,筛选出尿液FBP1水平在早期DN患者(DN-II期)尿液中显著升高,并随着疾病的进展呈增高趋势。最后,利用ELISA方法进一步扩大样本对健康对照、糖尿病非肾病、糖尿病肾病和非糖尿病肾病(膜性肾病、狼疮肾炎和IgA肾炎)患者尿液中FBP的量进行比较,发现DN患者尿液FBP1水平显著高于糖尿病非肾病(DM)、膜性肾病(MN)、狼疮肾炎(LN)和IgA肾炎(IgAN)患者。FBP1作为检测靶标用于糖尿病肾病的诊断,根据尿液中FBP1的含量鉴别糖尿病肾病与糖尿病非肾病的ROC曲线显示其敏感性和特异性分别为80%和85%,AUC为0.889。相关性分析结果显示,DN患者尿液FBP1水平与e-GFR呈负相关,与尿素氮、间质炎症评分、间质纤维化和肾小管萎缩(IFTA)评分呈正相关。
最后,利用免疫荧光和免疫组化技术追溯了尿液FBP1的来源,发现FBP1蛋白在早期DN患者近端肾小管上皮细胞减少,并且随着DN的进展,近端肾小管上皮细胞的FBP1降低,而在MN、LN和IgAN患者肾组织不存在显著变化,这一方面提示了尿液FBP1来源于损伤的近端肾小管上皮细胞的释放,也进一步支持了尿液FBP1可作为糖尿病肾病进展标志物的观点。
综上所述,本专利表明,与健康对照和DM相比,尿液FBP1在早期DN患者显著增高,并随着DN的进展,DN患者肾小管上皮细胞FBP1表达显著降低,尿液FBP1呈现增高趋势;而与健康对照和DM相比,MN、IgAN和LN患者尿液中的FBP1与无显著差异,且与对健康照相比,MN、IgAN和LN患者肾组织FBP1的表达并无显著降低。这些数据表明尿液FBP1可作为DN早期和疾病进展的特异性标志物。

具体实施方式
实施例1糖尿病非肾病患者(DM)和糖尿病肾病患者(DN)尿液蛋白组分析
(1)DM患者入组条件:a)年龄>=18周岁;b)2型糖尿病(WHO诊断标准),尿微量白蛋白阴性,eGFR>90,肝肾功能正常,C)首治的糖尿病,病程大于2年,无眼底及其他病变。排除条件:a)年龄≥65岁;b)合并其他并发症及肾脏疾病。糖尿病肾病患者入组条件:a)年龄>=18周岁;b)2型糖尿病(T2DM)患者活检证实为DN。排除条件:a)年龄≥65岁;b)在肾活检时有非糖尿病肾病合并症或急性肾损伤。
(2)收集糖尿病患者和经肾脏穿刺明确DN分型(DN-I、II、III和IV)的患者晨尿各3例,每例5ml。3000rpm离心5分钟,取上清;
(3)对尿液进行蛋白组分析,同时经过生物信息学手段对尿液差异蛋白的功能等情况分析可以用于发现有诊断标志物潜力的蛋白。热图显示蛋白质组分析结果,在不同病理阶段的DN和健康对照组之间,共发现15种差异蛋白(变化倍数>2或<0.5;p<0.05)(图1A)。在DN-I和健康对照组之间共发现30种差异蛋白(变化倍数>2或<0.5;p<0.05)(图1B)。与健康对照组相比,所有阶段的DN患者都有自己的特定尿蛋白谱。重要的是,包括CAII、CST1、PSCA、PRCP、PLBD2、FBP1和THSP4在内的七种蛋白在早期DN患者(DN-E,包括DN-I和DN-II,统称为DN早期)中被筛选为特异性尿蛋白(图1C),但在健康对照组中未被筛选,这表明这七种蛋白可能是DN早期诊断标记物的候选蛋白。
实施例2初筛蛋白在不同病理阶段DN患者尿液中的验证
按上述样本收集标准,进一步收集糖尿病非肾病患者20例和经肾脏穿刺明确是糖尿病肾病患者的晨尿70例,每例200ul,3000rpm离心5分钟,取上清;用ELISA方法对蛋白组结果进行验证:
(1)试剂盒从冷藏环境中取出,置于室温环境下平衡15-30min后方可使用。
(2)建立标准曲线:分别空白孔1孔(不加样品及酶标试剂,其余试剂与各孔相同),设立标准孔6孔。在酶标包被板第一至六孔上准确加样对应标号的标准品50ul,将第七孔作为空白对照。在第八十九至九十三孔重复此操作,做复孔。
(3)加样:待测样品孔每孔均加入待测样品稀释液40ul,然后再加待测样品10ul,加样将样品加于酶标板底部,尽量不触及孔壁,轻轻晃动混匀。加样时间不超过5min
(4)温育:用封板膜封板,置于37℃温育30min。
(5)洗板:小心揭掉封板膜,弃去每孔中液体,甩干,将每孔加满洗涤液,静置30s后弃去,拍干,重复5次,充分洗涤后在滤纸上印干。
(6)加酶:每孔加酶标试剂50ul,空白孔除外,用封板膜封板,置37℃ 60分钟。
(7)洗板
(8)显色:每孔先加入显色剂A 50ul,再加入显色剂B 50u,轻轻振荡混匀,37℃避光显色10min。
(9)终止:每孔加终止液50ul,终止反应,此时蓝色立传黄色,在终止15min内进行测定。
(10)测定:以空白孔调零,450nm波长依序测量各孔的吸光度(OD值),统计数据。结果如图2所示,DN-II患者尿液中FBP1的量即开始显著高于糖尿病非肾病患者,表明尿液FBP1可作为DN的早期诊断标志物。
实施例3尿液FBP1作为糖尿病肾病特异性标志物的验证和分析。
(1)进一步按实施例1和2中的样本收集标准收集60例糖尿病非肾病患者和60例糖尿病肾病患者晨尿各200ul,3000rpm离心5分钟,取上清;
(2)用ELISA对尿液FBP1的表达量,步骤同实施例2;
(3)整合实施例2和4中的检测结果(糖尿病非肾病患者87例,糖尿病肾病患者96例),绘制ROC曲线进行分析。
(4)我们继续将已鉴定的FBP1应用于新受试者,以测试其作为DN特异性诊断标志物的可能性。纳入DN(n=60)、DM(n=60)、IgAN(n=20)、MN(n=20)和LN(n=20)。如图3A所示,DN患者尿中FBP1水平显著高于DM、IgAN、MN和LN,表明FBP1可能是DN患者尿中的特异性蛋白。
(5)ROC曲线分析结果显示,DN和DM的ROC曲线下面积为0.896(p<0.001),敏感性为80%,特异性为85%,临界值为1.94(图3B)。为了评估尿中FBP1水平与疾病活动性之间的关系,我们分析了FBP1水平与实验室参数的相关性,包括e-GFR、尿素氮、间质炎症评分、间质纤维化和肾小管萎缩(IFTA)评分。结果表明,FBP1水平与e-GFR呈负相关(r=-0.56,p<0.001;图3C),与尿素氮呈正相关(r=0.55,p<0.001;图3D)。此外,通过多因素分析,尿FBP1水平也与间质炎症评分和IFTA评分相关。如图4E和F所示,在DN患者的活检标本中,尿FBP1水平升高与更高程度的间质炎症和IFTA显著相关
实施例4组织FBP1表达与DN疾病进展之间的关系分析。
收集肾癌癌旁肾组织作为正常对照和实施例1和2中的糖尿病肾病患者及其他肾脏病患者纳入标准收集肾穿刺组织;
(1)分别进行冰冻切片;
(2)由于肾脏是尿蛋白的主要来源之一,我们检测了疾病条件下FBP1在肾脏中的定位和表达。首先,采用免疫荧光染色法检测正常对照组(正常,n=2)、IgAN(n=2)、DN早期(DN-E,包括DN-I和DN-II,n=2)、DN晚期(DN-L,包括DN III和IV,n=2)、MN(n=2)和LN(n=2)受试者中FBP1的肾脏表达分布和变化。图4A显示,与对照组相比,糖尿病肾病早期肾小管上皮细胞中的FBP1的表达水平即显著降低,而在MN、IgAN和LN患者肾小管的表达保持不变。这与尿液FBP1在健康对照和MN、IgAN和LN之间无显著差异的结果是相一致的。这同时也表明不同慢性肾脏病患者肾损伤标志物和急性肾损伤标志物之间的差异性。
此外,与DN早期相比,DN晚期的FBP1进一步降低,表明肾小管上皮细胞中FBP1的表达变化可能与DN的进展有关。
(3)在此基础上,我们进一步证实了肾小管上皮中FBP1在糖尿病肾病进展过程中的表达变化,我们使用免疫组织化学染色扩大样本进行验证。肾组织收集情况如下:正常对照组10例,早期DN组10例,晚期DN组10例。结果表明,在扩大样本中,肾小管上皮细胞FBP的表达随着DN的进展而降低(图4B和C)。

1.尿液FBP1作为检测靶点在制备判定糖尿病肾病进展的诊断试剂中的应用。
2.检测尿液FBP1的试剂作为检测靶点在制备判定糖尿病肾病进展的诊断试剂中的应用。
3.检测尿液FBP1的试剂作为检测靶点在制备区分糖尿病非肾病、糖尿病肾病和非糖尿病肾病的辅助诊断试剂中的应用。
4.根据权利要求3所述的应用,其特征在于所述的非糖尿病肾病选自膜性肾病、狼疮肾炎或IgA肾炎。
