Description

Application of flavored honeycomb pills in preparation of medicines for treating lymphadenitis
Technical Field
The invention relates to the technical field of traditional Chinese medicine preparations, in particular to application of Jiawei honeycomb pills in preparation of medicines for treating lymphadenitis.
Background
Lymphadenitis is commonly seen in clinic and is divided into acute lymphadenitis and chronic lymphadenitis, a large number of statistics of morbidity are not available for reference at present, about 10% of people who visit departments are involved in diagnosis, and etiology and pathogenesis are still medical problems, so that students consider that the lymphadenitis is related to virus infection and autoimmunity.
Lymphadenitis is a self-limiting disease with good prognosis, but chronic lymphadenitis often recurs for a plurality of times and is not cured for a long time, and the chronic lymphadenitis is easily developed into tissue cell necrotizing lymphadenitis. The surgery mostly adopts antibiotics or operation treatment for acute lymphadenitis, and the antibiotics treatment is easy to generate drug resistance, so that the treatment effect is affected, and the operation treatment can cause damage to the body of a patient. In addition, most patients with chronic lymphadenitis do not seek medical treatment or do not know that the patients can take medicine, the patients wait for self-healing, and few patients repeatedly attack or progress to tissue cell necrotizing lymphadenitis, so that the treatment difficulty is increased.
The traditional Chinese medicine preparation has the advantages of small toxic and side effects, no drug resistance and capability of avoiding the damage caused by the operation to the body of a patient, is an ideal treatment method at present, but has no traditional Chinese medicine preparation with good treatment effect on both acute lymphadenitis and chronic lymphadenitis at present.

Analysis of the curative effects of the modified nidus Vespae pills prepared in examples 4 to 6
The diagnosis standards of the lymphadenitis in the traditional Chinese medicine and the western medicine are respectively as follows:
1. diagnostic criteria for traditional Chinese medicine
Wind-heat evil toxin type: the enlargement of submaxillary and cervical lymph nodes, obvious tenderness, removable pushing, unchanged skin color, fever, headache, soreness of limbs, sore throat and the like, thin and yellow tongue coating and superficial and rapid pulse.
Internal accumulation of phlegm-fire: the lymph nodes are obviously enlarged, have hard texture and are pressed for pain; the tumor is fixed, the push is not moved, and the feeling of fluctuation of the push is accompanied by high fever, polydipsia, anorexia, constipation, red tongue, yellow and greasy coating and rapid pulse.
2. Western diagnostic criteria
Acute lymphadenitis: the red swelling of human body is caused by sudden bacterial infection, and is characterized in that the illness is urgent, most patients suffer from swelling pain, reduced appetite, debilitation, headache and the like, few patients also have uncomfortable reactions such as fever, nausea and vomiting and the like, and the lymph parts also have swelling conditions and pain during compression.
Chronic lymphadenitis: the chronic inflammation of adjacent organs can cause symptoms such as repeated pain, repeated swelling and the like of the lymph nodes, the disease period is longer, the symptoms are not obvious due to acute lymphadenitis, but the swollen parts of the lymph nodes are harder, the pain feeling is not obvious when the lymph nodes are pressed, and then the condition that the lymph nodes gradually become smaller and even fade exists.
Nonspecific lymphadenitis: usually the onset is urgent, but the symptoms are numerous and mild, with only mild lymphadenectasis and pain. The symptoms of the traditional Chinese medicine composition are heavy, local redness and fever can occur, and obvious tenderness exists. In severe cases, the lymph nodes can continuously grow and even fuse into clusters, and sometimes abscess can also be formed to have fluctuation sensation, and even cause ulcer and pus. Nonspecific lymphadenitis is regional lymphadenitis caused by common bacterial or viral infection.
Specific lymphadenitis: pain is not obvious for patients, but the lymph nodes are continuously enlarged, and symptoms of fever, hypodynamia and local slight pain are often caused, and even symptoms of lymph node crumbling and purulence are caused. Specific lymphadenitis is generally associated with fungal infection, tubercle bacillus infection, and the like.
Diagnostic typing:
the Chinese medicine is typed: (1) pathogenic wind-heat toxin type, (2) phlegm-fire internal accumulation type.
Western medicine typing is comprehensively judged and typed according to four diagnostic standards, such as: the acute lymphadenitis is met, and meanwhile, whether the acute lymphadenitis is acute specific or acute nonspecific is determined according to specific and nonspecific diagnostic standards, so that the diagnosis and the typing are finally determined, wherein the specific typing is as follows: (1) acute specific lymphadenitis, (2) acute nonspecific lymphadenitis, (3) chronic specific lymphadenitis, and (4) chronic nonspecific lymphadenitis.
Efficacy assessment criteria: (1) healing: the swelling and pain of the lymph nodes and the systemic symptoms disappear, and the blood is normal; (2) effective: the swelling and pain of the lymph nodes are improved, the general symptoms gradually disappear, and the blood is normal; (3) ineffective: exacerbating local and systemic symptoms, even abscess formation, and increased blood normal white cell count.
The administration method comprises the following steps:
two pills are taken once a day, 2 times a day, each time in the morning and evening, and 30 days is 1 course of treatment.
The total number of cases taken and the evaluation results of the curative effects of the flavored honeyed pills prepared in the different examples after 1-6 treatment courses are shown in table 1, wherein the effective rate=the effective rate/the total rate×100%; cure rate = number of cure cases/total number of cases x 100%; inefficiency = number of invalid cases/total number of cases x 100%; total effective rate= (effective number + recovery number)/total number x 100%.

First, materials and methods
1. Experimental animal
30 rats, male and female, were not limited, and randomly aliquoted into 2 groups. (1) model set: preparing 10% saline solution from fresh egg yolk, fully emulsifying in a homogenizer to form egg yolk emulsion (Egg Yolk Emulsion), and injecting 0.5ml egg yolk emulsion into one side of hind leg (muscle) and flank (subcutaneous); (2) blank group: no treatment is done.
2. MRI examination
1.5T superconducting magnetic resonance imager, using head coil, conventional SE sequence, scanning field 150mm×150mm. Axial and coronal scans are mainly, sometimes complementary sagittal scans, T1WI: TR 510ms,TE 14ms,T2WI: TR 630 ms, TE 70ms; layer thickness 3mm,2 acquisitions. (1) The MRI examination time of the model group is 7 days after the first injection of the yolk emulsion; (2) blank group for any selected time.
3. Pathological examination
After MRI plain scan was completed, animals were sacrificed, upper popliteal fossa and internal iliac lymph nodes were exposed, lymph nodes were removed and the size of upper popliteal fossa lymph nodes was measured, 10% neutral formalin-immersed, paraffin-embedded, HE stained and visualized under light.
4. Evaluation method
The size (minor diameter) of the popliteal lymph node on T1WI was measured and the average calculated and the results are shown in Table 2; the signal intensities of the popliteal lymph nodes and surrounding muscles on T1WI, T2WI were measured using the region of interest method, and the signal intensity of the muscle was used as a reference, and the lymph node signal intensity divided by the muscle signal intensity was used as a normalized signal intensity, and the results are shown in Table 3 and FIGS. 1 to 4.
5. Statistical method
The size and signal intensity differences of the 2 groups of popliteal lymph nodes were evaluated by using SPSS10.0 statistical software package and fully random design analysis of variance method, and the differences of the 2 groups of average numbers were subjected to multiple comparison by using SNK-q test.
(II) results
1. Animal performance after molding
Model group: the tolerance to the egg yolk emulsion is good, the phenomena of local reaction, whole body condition damage and the like are avoided, and the enlarged popliteal lymph node can be reached within 6-7 days;
blank group: no obvious change was observed.
2. MRI performance
Lymph node size:
(1) Model group: multiple groups of lymph nodes are enlarged and MRI can show popliteal fossa, intra-iliac, and inguinal superficial lymph nodes; the anterior group and the posterior group of the intra-iliac lymph nodes are all shown at the bifurcation of the abdominal aorta, and 3-4 lymph nodes are all shown in this area; (2) blank group: the popliteal lymph node is smaller, but is clearly shown on both T1WI and T2 WI. Only 3 mice showed clear internal iliac lymph nodes on T1WI and were not readily identifiable on T2 WI. From table 2 it can be seen that the model group is larger than the popliteal lymph node in the blank group.
Table 2 shows the size of each group popliteal fossa lymph node (unit: mm;)
