Abstract

Description
背景技术
(醋酸甲基强的松龙)和/>(醋酸甲羟孕酮)被配制成其中包括含有聚乙二醇(PEG)3350的溶媒的胃肠外水性混悬液。PEG 3350被添加到溶媒中以在空间上稳定混悬液,并且它已经成为胃肠外水性混悬液的成分超过20年。稳定的机理解释如下。PEG是一种非离子型水溶性表面活性剂,其具有HO(CH2CH2O)nH化学式。PEG聚合物的节段,称为“锚定”链,其吸附到活性药物成分(API)颗粒的表面以形成吸附层。该层的厚度取决于几个参数,如聚合物浓度、介质的溶解性、温度和聚合物的分子量。被称为稳定链或“尾”的其他节段延伸到溶液中[T.Tadros.Interaction forces between particlescontaining grafted or adsorbed polymer layers.Advances in Colloidal InterfaceScience,104,2003]。这些尾部相互连接成颗粒之间的桥接,导致受控的絮凝。因此,API沉淀为松散桥接的颗粒,其易于再分散。然而,如果没有向溶媒中添加表面活性剂,颗粒将以缓慢的速率独立沉降,形成难以再分散的致密沉积物[T.Tadros.Control of stability/flocculation and rheology of concentrated suspensions.Pure&Appl.Chem.,64(11),1992]。如图1所示,未絮凝的(deflocculated)颗粒具有与颗粒直径平方成比例的慢的自由沉降速度(根据斯托克斯定律),并形成致密的饼状物。另一方面,絮凝的(flocculated)颗粒沉降得更快,形成更少堆积的饼状物,如更高的沉积物高度所代表,其可以容易地重新悬浮[L.Wu;J.Zhang;W.Watanabe,Adv.Drug Delivery Rev.63(2011)pp.456-469]。需要注意的是,广泛的聚集或不受控制的絮凝是不希望的,因为它导致颗粒非常快速的沉降,使得抽吸准确剂量具有挑战性,并且可导致用于向患者给药的针头堵塞。
然而,尽管有上述益处,使用PEG还是有一些已知的问题和顾虑。例如,已知PEG和类似的表面活性剂如聚山梨醇酯(PS),其易于自动氧化形成氢过氧化物(hydroperoxide),随后链降解形成副产物如甲酸,导致pH持续降低,直到顶部空间中的氧耗尽[M.Danbrow,E.Azaz,A.Pillersdorf.Autoxidation of polysorbates.J.Pharm.Sci.,67(12),1978]。除了pH下降之外,表面活性剂的降解可能导致混悬液增稠,从而导致含量均匀性问题。为了解决PEG氧化降解引起的pH下降并延长产品(其以小瓶形式提供)的保质期,小瓶顶部空间中的空气被氮气(氮气覆盖层)取代,或者在制剂中加入缓冲剂。然而,即使使用氮气,随着时间的推移,氧气仍然会降解这些产品。PEG的另一个问题是,在长时间储存后,混悬液有时会有较差的再分散性(结块和致密的沉积物)。
也有一些出版物讨论了PEG对患者的不良安全性影响。Nelson讨论了由于PEG的存在而在椎管内注射中使用的相关风险[D.A.Nelson.Dangers frommethylprednisoloneacetate therapy by intraspinal injection.Arch Neurol,45,1988]。他的结论是,使用乙二醇,当椎管内注射时,可能会导致严重的影响,如无菌性脑膜炎、蛛网膜炎或硬脑膜炎。Honorio等人研究了PEG对哺乳动物神经冲动的影响[T.Honorio,A.J.Gissen,G.R.Strichartz,M.J.Avaram,B.G.Covino.The effect of polyethylene glycol onmammalian nerve impulses.Anesth Analg.,66,1987]。PEG浓度在20%或以上会抑制神经细胞的复合动作电位,降低A、B和C神经纤维的传导速度。Herold等人使用动物模型研究了对烧伤患者重复局部应用含有PEG的抗菌乳膏的潜在毒性(D.A.Herold.Toxicity oftopical polyethylene glycol.Toxicology and Applied Pharmacology,65,1982)。将这种乳膏应用于兔子的开放性伤口导致总钙升高、渗透压间隙升高、高阴离子间隙代谢性酸中毒和肾衰竭。PEG的吸收、随后代谢为肾毒性化合物以及一元酸和二元酸被认为是这些症状的原因。Herold等人提出,在吸收之后,通过醇脱氢酶和醛脱氢酶将PEG顺序氧化成有机酸是其毒性机制[D.A.Herold,K.Keil,D.E.Bruns.Oxidation of polyethylene glycolby alcohol dehydrogenase.Biochemical Pharmacology,38,1989]。
为了解决上述技术问题和所述的PEG安全问题,开发了一种基本上无PEG的水性混悬液制剂,其中溶媒中唯一的赋形剂是张度剂如氯化钠(NaCl)和季铵化合物如肉豆蔻基-γ-甲基吡啶鎓氯化物(MGPC)。令人惊讶的是,发现与那些市售制剂相比,完全去除PEG并且减少市售皮质类固醇制剂中发现的MGPC的量导致了意想不到的长期稳定性。
此外,还令人惊讶地发现,通过去除PEG,通过成比例地增加MGPC的量,醋酸甲基强的松龙(MRA)混悬液中更高的药物载量是可能的。以前,高于80mg/mL的MRA浓度不能够在市场上获得,部分原因是较高的浓度带来了再悬浮性的挑战。通过完全去除PEG,令人惊讶地发现,尽管现有技术教导认为需要PEG来稳定混悬液,但是稳定的、更高浓度的MRA是可能的。更高浓度剂量的MRA现在是可实现的,例如单次注射120mg/mL的MRA,这是FDA批准的最大日剂量。对于这样的肌肉注射,它将有助于减少与大体积剂量和多次注射相关的疼痛和不适等。
发明概述

根据本发明的第一方面,提供了一种用于胃肠外使用的药物水性混悬液制剂,其包含皮质类固醇、季铵化合物、等张剂(isotonicity agent)和水;并且其中所述制剂基本上不含聚乙二醇和聚山梨醇酯中的每一种。
在本发明的另一方面,提供了一种用于胃肠外使用的药物水性混悬液制剂,其包含皮质类固醇、季铵化合物、张度剂(tonicity agent)和水;并且其中所述制剂基本上不含聚乙二醇和聚山梨醇酯中的每一种。

定义
本文使用的术语“ppm”是指百万分之一重量份。
术语“基本上不含聚乙二醇和聚山梨醇酯”是指不有意添加聚乙二醇和聚山梨醇酯以改善制剂的性质(例如物理稳定性),并且如果存在的话,其不超过痕量,并且优选小于100ppm。
本文使用的术语“PEG”表示“聚乙二醇”。用于药物用途的PEG的典型优选分子量范围为PEG 200至PEG 8000。PEG 3350,其在分析证书中具有分子量的范围,即3015-3685,并且用于所有或大多数甲羟孕酮和甲羟孕酮的市售制剂中。PEG 3350包含了PEG的定义。

本文使用的术语“等张剂(isotonicity agent)”是指溶液中控制等张性的添加剂。等张剂的非限制性实例包括氯化钠、氯化钾。
本文使用的术语“张度剂(tonicity agent)”是指当溶解在溶液中时可影响该溶液渗透压的添加剂。张度剂的非限制性实例包括氯化钠和氯化钾。例如,9mg/mL的NaCl(张度剂)提供等张溶液。为清楚起见,所有等张剂都是张度剂。
已经发现,通过消除PEG 3350并降低市售剂型中发现的肉豆蔻基-γ-甲基吡啶鎓氯化物(MGPC)的浓度,在pH下降和再悬浮性方面显著改善了稳定性。MGPC不仅作为防腐剂和润湿剂,而且作为阳离子表面活性剂用于API颗粒的受控絮凝,从而导致优异的再分散性。类似地,对于/>已经发现通过去除PEG和PS,并且在溶媒中仅使用MGPC和氯化钠,提高了混悬液在保质期内的稳定性。
还发现,通过消除PEG,有可能获得新的稳定的高浓度甲基强的松龙制剂,其允许施用更高剂量,以最小化与更高剂量体积和多次注射相关的疼痛和不适。
混悬液的配制遵循标准的两步过程,其中制备溶媒,随后加入醋酸甲基强的松龙(MRA)粉末以制备混悬液,润湿,搅拌,高剪切混合,最后再次搅拌。当温度控制在23-30℃时,将所需量的非API成分依次添加到Milli-Q水中。溶媒通过0.22μm过滤器过滤,其pH值重新调整至6.8-7.0。随后称出MRA粉末并加入到溶媒中,然后搅拌10分钟。将得到的浆料进行1分钟的高剪切混合,然后再搅拌10分钟。最后,如果需要,使用稀释的NaOH或HCl将混悬液的pH调节至6.8-7.0。监控混悬液温度,以确保其保持在23-30℃范围内。
制备了两种40mg/mL混悬液,以比较使用当前溶媒与使用无PEG溶媒配制的产品的特性。在第一个实验(T1)中,使用上述方法制备包含PEG 3350(30mg/mL)、MGPC(0.233mg/mL)和NaCl(9.0mg/mL)的对照溶媒。在第二个实验(T2)中,溶媒包含0.1165mg/mL MGPC和9.0mg/mL NaCl。对于T1和T2制剂,使用相同的MRA批次,以提供40mg/mL的API浓度。沉淀1天后,对照混悬液和无PEG混悬液的沉积药物高度(SDH)分别为26%和45%。两种混悬液都只需要2-3次倒置就能完全再悬浮API。
进行了稳定性研究,其中用每种制剂填充1mL小瓶,并用塞子盖住小瓶。小瓶以直立位置储存在40℃的实验室烘箱中。以不同的时间间隔取出小瓶,并测量pH值。如表1和图2所示,与对照(T1)的pH值为2.7相比,无PEG的稳定性样品(T2)的pH值在40℃储存514天后稳定在约4.8。对照(T1)需要4次倒置才能完全再悬浮API,而无PEG制剂(T2)需要2次倒置。

制备了两种制剂,以比较使用当前溶媒配制的混悬液与仅使用MGPC和NaCl制备的溶媒配制的混悬液。对照溶媒含有1.554mg/mL对羟基苯甲酸甲酯、0.17mg/mL对羟基苯甲酸丙酯、9.9mg/mL NaCl、32.85mg/mL PEG 3350和2.735mg/mL PS80。新溶媒包含9.9mg/mL NaCl和0.223mg/mL MGPC。通过0.22μ过滤器过滤溶媒,并将滤液pH调节至6.4-6.6。随后称出醋酸甲羟孕酮(MPA)粉末,加入到溶媒中以获得150mg/mL的API浓度,并搅拌10分钟。将得到的浆料进行一分钟的高剪切混合,然后进行另一个30分钟的搅拌。最后,如果需要,使用稀释的NaOH或HCl将混悬液的pH调节至6.4-6.6。监控混悬液温度,以确保其保持在23-30℃范围内。
沉淀1天后,对照和无PEG的混悬液的沉积药物高度(SDH)分别为69%和95%。对照需要5次倒置才能完全再悬浮API,而无PEG混悬液需要2次倒置。

实施例3:高浓度的无PEG的120mg/mL MRA混悬液的制备
高浓度混悬液的配制遵循实施例1的无PEG组合物概述的标准两步方法,其中制备溶媒,随后加入醋酸甲基强的松龙(MRA)粉末以制备混悬液,润湿,搅拌,高剪切混合,最后再次搅拌。当温度控制在23-30℃时,将所需量的非API成分依次添加到纯净水中。溶媒通过0.22μm过滤器过滤,并调节至pH 6.0-7.0。随后称出MRA粉末并加入到溶媒中,然后搅拌10分钟。将得到的浆料进行1分钟的高剪切混合,然后再搅拌10分钟。最后,如果需要,使用稀释的NaOH或HCl将混悬液的pH调节至6.5-7.0。监控混悬液温度,以确保其保持在23-30℃范围内。
实施例3A:无PEG的120mg/mLMRA混悬液的沉淀
使用上述方法配制120mg/mL MRA混悬液HC-1至HC-6,其中MGPC浓度不同(0.3mg/mL至0.699mg/mL)。将这些混悬液填充到闪烁小瓶中,密封,静置但不搅动,以检查固体沉降(沉积药物高度%,SDH)。在未受扰动的混悬液上测定沉淀长达30天。MGPC浓度对沉积速率和最终稳定的沉积物高度都有影响。更高水平的MGPC与SDH的更快下降直接相关,如图4所示。同时,MGPC水平的增加也与最终沉积高度成反比,其中最高水平的MGPC提供了最致密的沉积物。在这两种情况中,HC1至HC5相对于MGPC水平具有剂量依赖性的响应。另一方面,具有最高MGPC浓度的HC6在1天后显示出最大的SDH高度,但达到了与HC5相似的最终状态。图4显示这些高浓度的无PEG制剂不发生结块或未絮凝。
实施例3B:无PEG的120mg/mLMRA混悬液的再悬浮性
对HC1-HC5进行了再悬浮性实验。填充并密封单独的闪烁小瓶。在不同的时间点,通过轻柔的倒置对体系进行温和的混合,以获得完全均匀的分散液。图5描绘了在各种MGPC浓度下完全再悬浮120mg/mL MRA的平均倒置次数(n=2)。在每次实验之间,观察到无可见颗粒的透明无色上清液。这种混悬液在持续的再悬浮实验后证明了可重复的沉降,并恢复到一致的固体体积。
实施例3C:无PEG的120mg/mLMRA混悬液的稳定性
使用上述方法测试使用包含MRA(120mg/mL)、NaCl(9mg/mL)和肉豆蔻基-γ-甲基吡啶鎓氯化物(MGPC,范围为0.3-0.4mg/mL)的混合混悬液HC7-HC10的高浓度制剂的稳定性。将这些混悬液填充到1mL玻璃小瓶中,加塞,密封并放置在稳定室(倒置)中,评估在加速老化(40℃)、中速老化(30℃)和长期老化(25℃)条件下储存后的性能。用低于0.3mg/mL的MGPC水平制备的混悬液是不可行的,而高于0.4mg/mL的MGPC浓度在该MRA水平下是不利的。这些混悬液的pH遵循与实施例1的无PEG制剂观察到的相似的趋势,如表3所示。储存在40℃的样品在6个月后表现为在约pH 5.0处的水平,而在较低温度条件下表现为约pH 5.5。

实施例4:160mg/mL和200mg/mL无PEG的MRA混悬液的制备
使用上述方法,按照表4中概述的矩阵制备一系列批次。本研究旨在确定使用报道的MRA/MGPC比例可实现的混悬液固体限值。对于该实验,“稳定的混悬液”被定义为满足所有预先建立的标准:1)分散体必须形成絮凝的混悬液,在静置时形成透明的上清液,2)分散体必须仅以轻柔的倒置重新悬浮,从而证明容易再悬浮,以及3)必须在再悬浮之后重新沉淀,提供透明的上清液,从而证明稳定的沉降/再悬浮)。将各批次放入闪烁小瓶中,并在两周内进行监测。160mg/mLMRA可以以MRA/MGPC的这种重量比w/w配制,然而,想要超过例如200mg/mL,可能需要额外的MGPC来稳定悬浮颗粒。

尝试用PEG制备稳定的160mg/mL MRA混悬液。如上所述制备各批次,包含PEG3350。使用0.233mg/mL和0.466mg/mL的MGPC浓度不能成功获得稳定的MRA絮凝混悬液。这些制剂在静置时不会沉淀,并且不能振摇来提供均匀的分散液。
实施例5:盐浓度对120mg/mL MRA混悬液再悬浮性的影响
为了检查盐在MRA高浓度混悬液中的作用,使用上述方法制备混悬液,用NaCl(4.5和9mg/mL)和KCl(4.5和9mg/mL)制备120mg/mL MRA和0.35mg/mL MGPC,结果见表6。

钠盐和钾盐能够以9mg/mL的浓度将含有0.35mg/mL MGPC的120mg/mL MRA混悬液重新悬浮。然而,两种盐媒介在整个研究窗口中都很容易分散。所有溶液在4.5mg/mL盐水平下都未能再悬浮,表明MRA的稳定分散体需要一个下限。在胃肠外药物产品的药物制剂中,与氯化钾相比,氯化钠优选作为张度剂。制备了一系列制剂,从5mg/mL到13mg/mL,并在未受扰动情况下追踪SDH%28天,见图6。降低盐浓度导致120mg/mL MRA混悬液的相伴的压实。有趣的是,从5到13mg/mL的所有氯化钠水平都很容易地重新悬浮,大多数是一次倒置就实现(注:5mg/mL在第21天和第28天需要2次倒置,13mg/mL在第28天需要2次倒置)。这证明了对离子组成和5至13mg/mL盐范围的要求,优选氯化钠。
Claims (24)
1.一种用于胃肠外使用的药物水性混悬液制剂,其包含皮质类固醇、季铵化合物、张度剂和水;并且其中所述制剂基本上不含聚乙二醇和聚山梨醇酯中的每一种。
2.根据权利要求1所述的制剂,其中所述皮质类固醇是醋酸甲基强的松龙或醋酸甲羟孕酮。
3.根据权利要求2所述的制剂,其中所述季铵化合物是浓度小于0.5mg/mL的肉豆蔻基-γ-甲基吡啶鎓氯化物。
4.根据权利要求3所述的制剂,其中肉豆蔻基-γ-甲基吡啶鎓氯化物的浓度为0.07-0.3mg/mL。
5.根据权利要求4所述的制剂,其中肉豆蔻基-γ-甲基吡啶鎓氯化物的浓度为0.07-0.23mg/mL。
6.根据权利要求3所述的制剂,其中所述张度剂是氯化钠。
7.根据权利要求6所述的制剂,其中所述氯化钠的浓度为9mg/mL。
8.根据权利要求6所述的制剂,其中所述皮质类固醇是醋酸甲基强的松龙。
9.根据权利要求8所述的制剂,其中所述醋酸甲基强的松龙的浓度范围为20-160mg/mL。
10.根据权利要求8所述的制剂,其中所述醋酸甲基强的松龙的浓度范围为20-80mg/mL。
11.根据权利要求6所述的制剂,其中所述皮质类固醇是醋酸甲羟孕酮。
12.根据权利要求11所述的制剂,其中所述醋酸甲羟孕酮的浓度范围为135-165mg/mL。
13.根据权利要求1所述的药物水性混悬液制剂,其基本上由醋酸甲基强的松龙或醋酸甲羟孕酮、浓度小于0.5mg/mL的肉豆蔻基-γ-甲基吡啶鎓氯化物、氯化钠和水组成。
14.根据权利要求1所述的药物水性混悬液制剂,其中所述皮质类固醇是醋酸甲基强的松龙或醋酸甲羟孕酮,所述季铵化合物是浓度小于0.5mg/mL的肉豆蔻基-γ-甲基吡啶鎓氯化物,并且其中所述制剂在40℃在pH 4-7下保持至少100天的时间。
15.根据权利要求14所述的制剂,其中所述时间为至少300天。
16.根据权利要求15所述的制剂,其中所述时间为至少500天。
17.一种含有根据权利要求2所述的制剂的具有顶部空间的小瓶,其中所述小瓶在所述顶部空间中填充环境空气。
18.根据权利要求1所述的制剂,其中所述皮质类固醇是浓度为80-180mg/mL的醋酸甲基强的松龙,所述季铵化合物是浓度等于醋酸甲基强的松龙浓度的0.25%-0.33%的肉豆蔻基-γ-甲基吡啶鎓氯化物。
19.根据权利要求18所述的制剂,其中张度剂是氯化钠或氯化钾。
20.根据权利要求19所述的制剂,其中所述醋酸甲基强的松龙的浓度为80-160mg/mL。
21.根据权利要求20所述的制剂,其中肉豆蔻基-γ-甲基吡啶鎓氯化物的浓度是醋酸甲基强的松龙浓度的0.3%。
22.根据权利要求1所述的制剂,其中所述皮质类固醇是120mg/mL醋酸甲基强的松龙,所述季铵化合物是0.35mg/mL肉豆蔻基-γ-甲基吡啶鎓氯化物,所述张度剂是9mg/mL氯化钠。
23.根据权利要求1所述的制剂,其中所述皮质类固醇是80mg/mL醋酸甲基强的松龙,所述季铵化合物是0.12至0.23mg/mL肉豆蔻基-γ-甲基吡啶鎓氯化物,所述张度剂是9mg/mL氯化钠。
24.根据权利要求1所述的制剂,其中所述皮质类固醇是40mg/mL醋酸甲基强的松龙,所述季铵化合物是0.12mg/mL肉豆蔻基-γ-甲基吡啶鎓氯化物,所述张度剂是9mg/mL氯化钠。
