intestinal canal obstruction: patients with crohn's disease have transmural inflammation of the intestinal tract, intestinal stenosis, and often develop intestinal flatulence after eating;
intestinal inflammation: can affect the peristalsis of the intestinal tract, and when the peristalsis of the intestinal tract is slow, the patient can have flatulence.
Intestinal microecological changes: the abdominal distension of patients with Crohn's disease is a common manifestation because the patients with Crohn's disease have severe intestinal inflammation, intestinal microecological disorder, decreased intestinal beneficial bacteria and increased harmful bacteria, which often include aerogenic bacilli, and the aerogenic bacilli cause abdominal distension of the patients.
All patients with the above symptoms can be used as the criteria for selecting cases.
3. Diagnostic criteria (established with reference to the criteria for crohn's disease recommended by the world health organization in 2014): (1) typical clinical symptoms. (2) And X-ray shows gastrointestinal tract fissure-shaped ulcer, cobblestone sign, pseudopolyp, multiple stenosis, fistula formation and the like. The lesions are distributed in segments. CT may show loops with thickened intestinal walls, pelvic or abdominal abscesses. (3) And longitudinal or creeping ulcers with jumping distribution are seen under the endoscope, and the surrounding mucous membrane is normal or proliferated like pebbles. (4) Non-caseous necrotic granuloma, or massive lymphocyte accumulation in lesion biopsies.
4. Inclusion criteria
(1) The standard for the diagnosis of Crohn's disease is met and the clinical severity is mild to moderate patients. (according to international CDAI scores, scores were mild from 150-275, moderate from 275-450, and scores greater than 450 were severe. Mild and moderate patients were included in this study, and severe patients were excluded), the CDAI scoring system included 8 dimensions: the number of bowel movements per day in the past 1 week, daily abdominal pain conditions (none =0,2, weight = 3), daily general health (health =0 requires the administration of phencyclidine/opioid (none =0 product, (standard body mass-actual body mass)/standard body mass.1 week observation was made for the above 8 dimensions, multiplied by the corresponding weights, and the sum of the 8 scores was the total score of the CDAI.
(2) The age is 18-65 years old, and the nature is not limited.
5. Exclusion criteria
(1) The disease condition is severe.
(2) Infectious colitis such as bacillary dysentery, amebic dysentery, chronic schistosomiasis, and intestinal tuberculosis, and patients with ulcerative colitis, ischemic enteritis, and radiation enteritis.
(3) Patients with severe complications such as local stenosis, intestinal obstruction, intestinal perforation, rectal polyp, toxic colonic dilatation, colon cancer, rectal cancer and anal disease
(4) Pregnant and lactating women.
(5) Those with severe primary heart, liver, lung, kidney, blood or severe disease affecting their survival.
(6) Disabled patients as prescribed by law (blind, deaf, dumb, intellectual disability, mental disorder, physical disability).
(7) Suspected or definite history of alcohol, drug abuse.
(8) According to the judgment of researchers, other diseases which reduce the possibility of entering the group or complicate the group exist, such as the condition that the working environment is frequently changed and the like, which is easy to cause missed visits.
(9) Allergic constitution, e.g. a history of allergy to two or more drugs or food; or one known to be allergic to the present drug ingredients.
6. The treatment scheme comprises the following steps:
the traditional Chinese medicine is prepared by 400mL of the traditional Chinese medicine enema (taking the example 1 as an example), and the enema is kept for 1 time every night, and 14 days are a treatment course. The curative effect is judged by observing two courses of treatment.
The mesalazine group is orally taken 3 times a day by 4 mesalazine tablets. The treatment course is the same as the traditional Chinese medicine.
6.1 Grading standard of abdominal distension:
degree I (mild): a swollen abdomen, slightly higher than the chest, is soft. Percussion presents a bass drum sound. Bowel sounds may be attenuated, slightly accentuated or normal. Slight abdominal distension may occur.
II degree (moderate degree), the abdomen is raised obviously higher than the chest, and has certain tension. Percussion presents the middle-tone drum sound, and the range and intensity are both increased and obvious. Bowel sounds are often attenuated, and voluntary abdominal distension is obvious, affecting food intake or other metabolism.
Degree III (severe): the whole abdomen is obviously raised and spherical, and the pressure is hard and uncomfortable without rebound pain. The percussion is a high-pitched drum sound, and can generate metal tapping sound without any change along with the body position. The bowel sound is obviously weakened or disappeared, the intestinal tract reaction is obvious when the intestinal tract sound is heard and smells over water.
And the efficacy was judged by the following formula:
the therapeutic effect determination value = pre-treatment integral-post-treatment integral/pre-treatment integral × 100%
The therapeutic criteria are as follows: the effect is shown: decision value >2/3, valid: 1/3< decision value <2/3, invalid: the decision value is <1/3.

The total effective rate of the traditional Chinese medicine is 93.9 percent, is obviously superior to the total effective rate of the mesalazine group (68.7 percent), and has obvious difference (P is less than 0.05) after statistical treatment.
The same or similar results were obtained in the examples 1 and other examples, which are not listed here.
Experiments show that the traditional Chinese medicine enema prepared by the invention has stable and reliable curative effect on the abdominal distension caused by the Crohn disease, no obvious toxic or side effect, safe medication, stable and reliable curative effect, and the curative effect of 93.9 percent, has obvious advantages in treatment compared with the existing medicines, has very strong clinical use value, is innovation on the medicines for treating the abdominal distension caused by the Crohn disease, and has huge economic and social benefits.

Claims (4)

1. A traditional Chinese medicine enema for treating abdominal distension caused by Crohn's disease is characterized by being prepared from the following raw material medicines in parts by weight: 10-20g of elecampane, 15-20g of rhizoma cyperi, 10-20g of agastache rugosus, 9-15g of radix puerariae, 12-15g of scutellaria baicalensis, 12-15g of coptis chinensis, 20-30g of purslane, 20-30g of houttuynia cordata, 6-9g of lithospermum and 15-20g of astragalus membranaceus, 400-600mL of water is added into the raw materials, the raw materials are soaked for 15-25 minutes at the temperature of 18-25 ℃, boiled by strong fire, decocted for 20-30 minutes by slow fire and filtered to obtain first filtrate; adding 400-600mL of water into the residue, boiling with strong fire, decocting with slow fire for 20-30min, filtering to obtain the second filtrate, mixing the two filtrates, and mixing.

The traditional Chinese medicine enema for treating Crohn's disease abdominal distension according to claim 1, which is prepared from the following raw materials in parts by weight: 15g of costustoot, 18g of rhizoma cyperi, 15g of wrinkled gianthyssop herb, 12g of kudzuvine root, 13g of baical skullcap root, 14g of golden thread, 25g of purslane, 25g of heartleaf houttuynia herb, 7g of lithospermum and 18g of astragalus root, 500mL of water is added into the raw materials, the raw materials are soaked for 15 to 25 minutes at the temperature of between 18 and 25 ℃, boiled with strong fire, decocted with slow fire for 25 minutes and filtered to obtain first filtrate; adding 500mL of water into the residue, boiling with strong fire, decocting with slow fire for 25min, filtering to obtain the second filtrate, mixing the two filtrates, and mixing.

The traditional Chinese medicine enema for treating Crohn's disease abdominal distension according to claim 1, which is prepared from the following raw materials in parts by weight: 12g of costustoot, 19g of nutgrass galingale rhizome, 12g of wrinkled gianthyssop herb, 14g of kudzuvine root, 14g of baical skullcap root, 13g of golden thread, 22g of purslane, 28g of heartleaf houttuynia herb, 8g of sinkiang arnebia root and 16g of membranous milkvetch root, adding 550mL of water into the raw materials, soaking for 15-25 minutes at 18-25 ℃, boiling with strong fire, decocting with slow fire for 20min, and filtering to obtain first filtrate; adding 450mL of water into the residue, boiling with strong fire, decocting with slow fire for 30min, filtering to obtain the second filtrate, mixing the two filtrates, and mixing.

The traditional Chinese medicine enema for treating Crohn's disease abdominal distension according to claim 1, which is prepared from the following raw materials in parts by weight: 18g of costustoot, 16g of nutgrass galingale rhizome, 18g of wrinkled gianthyssop herb, 10g of kudzuvine root, 13g of baical skullcap root, 14g of golden thread, 28g of purslane, 22g of heartleaf houttuynia herb, 9g of sinkiang arnebia root and 19g of membranous milkvetch root, adding 600mL of water into the raw materials, soaking for 15-25 minutes at 18-25 ℃, boiling with strong fire, decocting with slow fire for 30min, and filtering to obtain first filtrate; adding 400mL of water into the medicine residues, boiling with strong fire, decocting with slow fire for 20min, filtering to obtain a second filtrate, combining the two filtrates, and mixing uniformly to obtain the traditional Chinese medicine composition.
