Abstract
本申请属于生物医用材料技术领域,提供了一种骨内植物,包括金属螺棒以及可固化的填充材料,金属螺棒沿其中心轴线方向开设有贯通的容纳通腔;填充材料填充于容纳通腔中,填充材料包括成骨诱导矿物因子,成骨诱导矿物因子能够与金属螺棒溶解的金属离子发生沉淀骨内植物植入人体后,填充材料能够从容纳通腔的腔口渗出,并释放成骨诱导矿物因子,当遇到金属在人体内降解而形成的碱性环境时,成骨诱导矿物因子与金属离子反应形成致密层,沉积于金属螺棒的表面。实现了提高器械支撑强度、强化金属螺棒与骨界面固定效果等功能。
Description
技术领域
本申请属于生物医用材料技术领域,更具体地说,是涉及一种骨内植物。
背景技术
在骨科内植入物领域,镁及其合金内植入物材料有希望替代传统医学中的不锈钢、钛及钛合金等骨科内植入材料。与其它医用金属材料相比,镁及其合金具有如下的优势:1)可降解性,镁合金具有较低的腐蚀电位,在含有氯离子的体内环境下易发生腐蚀,并以缓慢腐蚀的方式在体内完全降解,可实现镁在人体中的降解吸收;2)生物安全性高,Mg作为人体必需的营养元素,在人体内含量仅次于Ca、K、Na排第四;3)生物力学相容性好。镁及其合金的密度约为1.7g/cm3,与人体骨组织的密度(1.75g/cm3)接近。同时,纯镁的弹性模量44.1GPa,稍高于一般骨12-20GPa,同齿骨40-80GPa水平相当,抗拉强度为120-160MPa。由此可见,镁的物理机械性能更接近于生物骨,可有效缓解植入体内骨钉骨后的应力遮挡效应。
但是镁及其合金应用到临床上作为骨科内植入物材料尚存在一些技术难题,主要是镁及其合金在体内降解太快,与其所期待的维持一定时间力学强度有落差,在修复器官恢复功能前难以维持其必要的强韧性。在预防镁及其合金体内降解的领域,通常采用涂层技术来降低镁及其合金的体内降解。然而,手术固定的操作过程,导致涂层受周围骨组织刮蹭,涂层脱落,从而无法达到预期抗腐蚀效果。另一方面如镁及其合金降解速率加快,并不利于周围骨再生。此外,涂层材料并不具有支撑骨组织的作用,随着镁及其合金的降解,整个固定系统将迅速失效,不利于提供稳定的力学支撑。
发明内容
基于此,本申请的一个目的是提供一种骨内植物金属,以解决现有技术中存在的金属在体内降解太快,在修复器官恢复功能前难以维持其必要的强韧性,不利于周围骨再生的技术问题。
为实现上述目的,本申请采用的技术方案是:
一种骨内植物,包括:
金属螺棒,金属螺棒沿其中心轴线方向开设有贯通的容纳通腔;以及
可固化的填充材料,填充材料填充于容纳通腔中,填充材料包括成骨诱导矿物因子,成骨诱导矿物因子能够与金属螺棒溶解的金属离子发生沉淀。
可选地,金属螺棒采用镁或镁合金的生物可降解活性金属制成;和/或,
填充材料为可注射性材料,填充材料包括固体和溶液混合形成的可注射物,固体分散在溶液中;成骨诱导矿物因子包含于固体中。
可选地,溶液与固体的混合比例为0.06ml/g-0.9ml/g。
可选地,固体包括以下质量百分含量的组分:磷酸盐5%-82%、氧化镁7%-65%、钙磷灰石0-15%、硅酸钙1%-15%、硅酸镁0-15%;成骨诱导矿物因子包括镁离子、钙离子、磷酸根、硅酸根;
磷酸盐、氧化镁、钙磷灰石、硅酸钙、硅酸镁电离出来的磷酸根、镁离子、钙离子、硅酸根构成成骨诱导矿物因子;和/或,
溶液包括水、天然聚合水凝胶和高分子水凝胶中的至少一种。
可选地,金属螺棒的侧壁上开设有通孔,通孔与容纳通腔连通;容纳通腔的中心轴线与通孔的中心轴线之间具有夹角。
可选地,容纳通腔包括第一腔体与第二腔体,第一腔体与第二腔体沿容纳通腔的中心轴线呈阶梯式连通,第一腔体的直径大于第二腔体的直径。
可选地,金属螺棒包括棒身与连接棒身的连接部,连接部的外周设有螺纹,第一腔体形成于连接部内;一部分第二腔体形成于棒身,另一部分第二腔体形成于连接部。
可选地,通孔的数量为多个,多个通孔沿环绕金属螺棒的方向形成多列孔阵分布。
可选地,多列孔阵中至少有两列孔阵呈对称设置。
可选地,每列孔阵包括至少两个通孔,沿环绕金属螺棒的方向,相邻两列孔阵中的通孔呈错位分布。
本申请提供的骨内植物,以金属螺棒作为基体,内嵌可固化的填充材料,填充材料填入容纳通腔后,在预设时间内固化形成固体,一方面,固化的填充材料加强了金属螺棒的支撑强度,提高骨内植物的整体力学强度;另一方面,骨内植物植入人体后,具有促进成骨的填充材料从容纳通腔的腔口中渗出,渗出的填充材料漫延至金属螺棒的外周及周围邻近的骨组织,当金属螺棒的金属在人体内降解,金属螺棒的外周环境将会变为碱性,填充材料的成骨诱导矿物因子遇到碱性环境时,与金属离子生成沉淀,形成致密层沉积于金属螺棒的表面,从而减少金属螺棒与体液的接触面积,减缓金属螺棒的电化学腐蚀,达到减缓金属螺棒降解的作用;实现了提高器械支撑强度、减缓金属降解、强化金属螺棒与骨界面固定效果等功能。

如图1至图3,本申请实施例的一种骨内植物1,包括金属螺棒10和可固化的填充材料。
其中,骨内植物1所含的金属螺棒10沿其中心轴线方向开设有贯通的容纳通腔;
可固化的填充材料,填充材料填充于容纳通腔中,填充材料包括成骨诱导矿物因子,成骨诱导矿物因子能够与金属螺棒10溶解的金属离子发生沉淀。

可选地,金属螺棒10的侧壁上开设有通孔11,通孔11与容纳通腔连通;容纳通腔的中心轴线与通孔11的中心轴线之间具有夹角。填充材料可通孔11及容纳通腔的腔口从多个方向渗出,分散至金属螺棒10外周的多处,提高全面包围金属螺棒10的可能,并为骨组织的骨再生快速补充成骨诱导矿物因子,稳定生长。
可选地,容纳通腔包括第一腔体12与第二腔体13,第一腔体12与第二腔体13沿容纳通腔的中心轴线呈阶梯式连通,第一腔体12的直径大于第二腔体13的直径。可以理解地,容纳通腔的两个腔口分别位于第一腔体12和第二腔体13的端口,第一腔体12的腔口大于第二腔体13的腔口,第一腔体12的腔口可用于注入填充材料,便于操作,并通过两端不同大小的腔口调节填充材料的渗出速度,达到长期稳定释放的效果。
可选地,金属螺棒10包括棒身14与连接棒身14的连接部15,连接部15的外周设有螺纹,第一腔体12形成于连接部15内,一部分第二腔体13形成于棒身14内,另一部分第二腔体13形成于连接部15,并与第一腔体12连接,第二腔体13的长度大于第一腔体12的长度。连接部15设外螺纹可方便将骨内植物1植入人体内,植入时,将棒身14先装入人体内,然后通过连接部15的螺纹将金属螺棒10固定于人体内。
可选地,通孔11的数量为多个,多个通孔11沿环绕金属螺棒10的方向形成多列孔阵分布,则填充材料可从通孔11与容纳通腔的腔口多处渗出,调节填充材料的漫延分布位置,达到强化金属螺棒10与骨组织固定界面的吻合紧贴。
可选地,多列孔阵中至少有两列孔阵呈对称设置,以平衡填充材料的渗出压力。
可选地,每列孔阵包括至少两个通孔11,沿环绕金属螺棒10的方向,相邻两列孔阵中的通孔11呈错位分布,填充材料错位渗出,增加填充材料的漫延面积,并减小从相邻两列孔阵渗出的填充材料的相互挤压,提高漫延速度。
本申请实施例的金属螺棒10采用镁或镁合金的生物可降解活性金属制成,镁合金例如包括但不限于镁钙合金、镁铜合金、镁锌合金、镁稀土合金等。镁及镁合金的生物安全性高,生物力学相容性好,物理机械性能更接近于生物骨,是优良的骨科内植入材料。
填充材料为可注射性材料,常温状态下,填充材料呈流体状,填充材料注入容纳通腔后30分钟内形成固态。填充时,将填充材料注入注射系统,然后将填充材料从容纳通腔的其中一个腔口注入,操作便捷。
可选地,填充材料包括固体和溶液,固体分散在溶液中;成骨诱导矿物因子包含于固体中。
可选地,固体包括以下质量百分含量的组分:磷酸盐5%-82%、氧化镁7%-65%、钙磷灰石0-15%、硅酸钙1%-15%、硅酸镁0-15%。
磷酸盐、氧化镁、钙磷灰石、硅酸钙、硅酸镁将部分电离形成镁离子、钙离子、磷酸根、硅酸根,磷酸盐、氧化镁、钙磷灰石、硅酸钙、硅酸镁电离出来的磷酸根、镁离子、钙离子、硅酸根构成成骨诱导矿物因子,镁离子、钙离子、磷酸根、硅酸根具有成骨诱导的作用,可促进骨组织再生,强化骨内植物1与骨组织之间的贴合,当镁离子、钙离子、磷酸根、硅酸根遇到金属体内降解而形成的碱性环境时,镁离子、钙离子均与磷酸根、硅酸根反应,生成镁钙为晶体的磷酸盐和硅酸盐的致密层,沉积金属螺棒10的表面。
可以理解地,当固体不含钙磷灰石时,成骨诱导矿物因子中的钙离子为磷酸盐和/或硅酸钙电离出来的钙离子;固体不含硅酸镁时,成骨诱导矿物因子中的镁离子为氧化镁电离出来镁离子。
可选地,溶液与固体的混合比例为0.06ml/g-0.9ml/g。
可选地,溶液包括水、天然聚合水凝胶和高分子水凝胶中的至少一种,天然聚合水凝胶包括壳聚糖、藻酸盐、胶原、纤维素、透明质酸钠中的至少一种,高分子水凝胶包括聚乙烯醇、聚乙烯吡咯烷酮、聚氨酯、聚乙二醇、聚丙烯酰胺的至少一种。
以下通过多个实施例来举例说明骨内植物的结构及性能等方面。
实施例1
本实施例的骨内植物包括金属螺棒及填充材料。
使用纯度为99.99%的纯镁,通过机械加工方式,制备金属螺棒。金属螺棒包括棒身与连接棒身的连接部,连接部的外周设有螺纹,金属螺棒开设有容纳通腔,容纳通腔贯通连接部和棒身,容纳通腔包括第一腔体与第二腔体,第一腔体形成于连接部内,第一腔体与第二腔体沿容纳通腔的中心轴线呈阶梯式连通,第一腔体的直径大于第二腔体的直径。
棒身开设有贯通的通孔,通孔与容纳通腔连通,容纳通腔的中心轴线与通孔的中心轴线之间相互垂直。通孔的数量为十六个,十六个通孔沿环绕金属螺棒的方向形成四列孔阵分布,每列孔阵包括四个通孔,四列孔阵呈两两对称设置,沿环绕金属螺棒的方向,相邻两列孔阵中的通孔呈错位分布。
填充材料包括固体及溶液,固体包括以下质量百分含量的组分:磷酸二氢钠24.6%、磷酸氢钙47%、硅酸钙2%、氧化镁26.4%,磷酸二氢钠、磷酸氢钙、硅酸钙、氧化镁均为直径100微米以内的粉末,按0.6ml/g比例将透明质酸钠凝胶与固体混合,搅拌成膏体,将填充材料膏体吸入注射系统,从金属螺棒的第一腔体的腔口注入容纳通腔。
按实施例1制备的骨内植物依据GBT16886进行体外细胞毒性实验,溶血实验,实验结果如表1所示,证明材料无毒,实验条件合格,可以进行动物研究。

实施例2
本实施例的骨内植物包括金属螺棒及填充材料。
使用纯度为99.99%的纯镁,通过机械加工方式,制备金属螺棒。金属螺棒包括棒身与连接棒身的连接部,连接部的外周设有螺纹,金属螺棒开设有容纳通腔,容纳通腔贯通连接部和棒身,容纳通腔包括第一腔体与第二腔体,第一腔体形成于连接部内,第一腔体与第二腔体沿容纳通腔的中心轴线呈阶梯式连通,第一腔体的直径大于第二腔体的直径。
棒身开设有贯通的通孔,通孔与容纳通腔连通,容纳通腔的中心轴线与通孔的中心轴线之间相互垂直。通孔的数量为十六个,十六个通孔沿环绕金属螺棒的方向形成四列孔阵分布,每列孔阵包括四个通孔,四列孔阵呈两两对称设置,沿环绕金属螺棒的方向,相邻两列孔阵中的通孔呈错位分布。
填充材料包括固体及溶液,固体包括以下质量百分含量的组分:磷酸二氢钠12.3%、磷酸氢钙70%、硅酸镁2.5%、硅酸钙2%、氧化镁13.2%,磷酸二氢钠、磷酸氢钙、硅酸镁、硅酸钙、氧化镁均为直径100微米以内的粉末,按0.3ml/g比例将水与固体混合,搅拌成膏体,将填充材料膏体吸入注射系统,从金属螺棒的第一腔体的腔口注入容纳通腔。
实施例3
本实施例的骨内植物包括金属螺棒及填充材料。
使用镁钙合金,通过机械加工方式,制备金属螺棒。金属螺棒包括棒身与连接棒身的连接部,连接部的外周设有螺纹,金属螺棒开设有容纳通腔,容纳通腔贯通连接部和棒身,容纳通腔包括第一腔体与第二腔体,第一腔体形成于连接部内,第一腔体与第二腔体沿容纳通腔的中心轴线呈阶梯式连通,第一腔体的直径大于第二腔体的直径。
棒身开设有贯通的通孔,通孔与容纳通腔连通,容纳通腔的中心轴线与通孔的中心轴线之间相互垂直。通孔的数量为十六个,十六个通孔沿环绕金属螺棒的方向形成四列孔阵分布,每列孔阵包括四个通孔,四列孔阵呈两两对称设置,沿环绕金属螺棒的方向,相邻两列孔阵中的通孔呈错位分布。
填充材料包括固体及胶原,固体包括以下质量百分含量的组分:磷酸二氢钠25%、磷酸氢钙43%、钙磷灰石0.1%、硅酸镁5%、硅酸钙10%、氧化镁8%,磷酸二氢钠、磷酸氢钙、钙磷灰石、硅酸镁、硅酸钙、氧化镁均为直径100微米以内的粉末,按0.15ml/g比例将胶原与固体混合,搅拌成膏体,将填充材料膏体吸入注射系统,从金属螺棒的第一腔体的腔口注入容纳通腔。
以不含填充材料的单纯镁螺钉为对比例,检测单纯镁螺钉和实施例1的骨内植物的镁离子释放效果,释放曲线如图4所示。相比单纯镁螺钉持续的升高,实施例1骨内植物中镁离子稳定释放,在60天的浸泡时间内,单纯镁螺钉的镁离子浓度成线性增加,最终达到47mM,而在实施例1骨内植物中的镁离子浓度缓慢增加,60天累积浓度维持在25mM水平上下波动,具有长期稳定释放镁离子的效果。
动物体内实验
以不含填充材料的单纯镁螺钉为对比例,将单纯镁螺钉和实施例2骨内植物植入鸸鹋体内实验。
提供成年健康双足鸸鹋20只,雄性,于鸸鹋肌肉注射舒泰15mg/kg,静脉维持剂量舒泰3mg/kg/h,侧卧位固定,无菌操作,于股骨外侧依次切开皮肤、肌肉、分离深筋膜,暴露股骨大转子,在大转子下2cm处延颈干角方向,约与平台成110°方向植入一枚K氏针,X线透视下,钻入深度约为45mm,钻头尖端至股骨头内,用直径5mm的空心钻将隧道扩大,而后用M8丝攻钻得尾端钉道,将实施例2的金属螺棒拧入。调和实施例2中的填充材料,将填充材料从金属螺棒的第一腔体腔口注射填充容纳通腔。
阳性对照仅放入单纯镁螺钉,不进行注射填充材料。碘伏冲洗缝合切口,术后鸸鹋自由活动。随机选择术后3个月,6个月进行观察取材,观察指标有步态,CT,大体观察。如图5所示,实施例2骨内植物的周围骨组织与金属螺棒贴合好,股骨头圆润光滑,力学性能好,固定牢靠,复合组件内植入物降解缓慢。阳性对照组单纯镁螺钉周围形成空腔和结缔组织,股骨头骨小梁紊乱,出现塌陷,钉(棒)尾部出现溶骨反应,失去固定作用。
这些结果表明,实施例2骨内植物的镁降解稳定,生物相容性好,材料与骨组织结合部位固定牢固,从而充分说明本发明改进金属螺钉降解性、生物相容性和固定稳定性方案的可行性和有效性。

1.一种骨内植物,其特征在于:包括:
金属螺棒,所述金属螺棒沿其中心轴线方向开设有贯通的容纳通腔;以及
可固化的填充材料,所述填充材料填充于所述容纳通腔中,所述填充材料包括成骨诱导矿物因子,所述成骨诱导矿物因子能够与所述金属螺棒溶解的金属离子发生沉淀;
所述金属螺棒的侧壁上开设有通孔,所述通孔与所述容纳通腔连通;所述容纳通腔的中心轴线与所述通孔的中心轴线之间具有夹角;
所述填充材料填入所述容纳通腔后,在预设时间内固化形成固体;
所述金属螺棒采用镁或镁合金的生物可降解活性金属制成;
所述填充材料为可注射性材料,所述填充材料包括固体和溶液,所述固体分散在所述溶液中;所述成骨诱导矿物因子包含于所述固体中;
所述固体混合物包括以下质量百分含量的组分:磷酸盐5%-82%、氧化镁7%-65%、钙磷灰石0-15%、硅酸钙1%-15%、硅酸镁0-15%;
当镁离子、钙离子、磷酸根、硅酸根遇到金属体内降解而形成的碱性环境时,镁离子、钙离子均与磷酸根、硅酸根反应,生成镁钙为晶体的磷酸盐和硅酸盐的致密层,沉积在所述金属螺棒的表面。
2.权利要求1所述的骨内植物,其特征在于:所述溶液与所述固体的混合比例为0.06ml/g-0.9ml/g。
3.权利要求1所述的骨内植物,其特征在于:所述磷酸盐、氧化镁、钙磷灰石、硅酸钙、硅酸镁电离出来的磷酸根、镁离子、钙离子、硅酸根构成所述成骨诱导矿物因子;和/或,
所述溶液包括水、天然聚合水凝胶和高分子水凝胶中的至少一种。
4.如权利要求1所述的骨内植物,其特征在于:所述容纳通腔包括第一腔体与第二腔体,所述第一腔体与所述第二腔体沿所述容纳通腔的中心轴线呈阶梯式连通,且所述第一腔体的直径大于所述第二腔体的直径。
5.如权利要求4所述的骨内植物,其特征在于:所述金属螺棒包括棒身与连接所述棒身的连接部,所述连接部的外周设有螺纹,所述第一腔体形成于所述连接部内;一部分所述第二腔体形成于所述棒身,另一部分所述第二腔体形成于所述连接部。
6.如权利要求1所述的骨内植物,其特征在于:所述通孔的数量为多个,多个所述通孔沿环绕所述金属螺棒的方向形成多列孔阵分布。
7.如权利要求6所述的骨内植物,其特征在于:多列所述孔阵中至少有两列所述孔阵呈对称设置。
8.如权利要求6所述的骨内植物,其特征在于:每列所述孔阵包括至少两个所述通孔,沿环绕所述金属螺棒的方向,相邻两列所述孔阵中的通孔呈错位分布。
