本发明涉及糖尿病的治疗方法。更具体地说,本发明涉及防止糖尿病患者血糖水平剧烈波动的治疗方法和组合物。
低血糖主要有两种。迅速的肾上腺释放引起出汗,震颤,心动过速,焦虑和饥饿。中枢神经系统的症状包括头晕,头痛,精神迟钝,意识模糊,行为不正常,抽搐以及丧失意识。低血糖经常发生或严重时,神经系统的症状更突出,而肾上腺期可能识别不出来。随着血浆葡萄糖的迅速降低或大波动(如在胰岛素反应中一样),肾上腺素能的症状就会明显(Harrison’sPrinciples of Internal Medicine,11th Ed.,McGraw-Hill BookCompany,New York,1987,p.1800)。

需要开发各种方法以便在连续强化治疗糖尿病过程中减少低血糖的发生。已经用玉米淀粉有效地防止与I型糖原贮存有关的低血糖,这种病在肝,肾和肠中有遗传性葡萄糖-6-磷酸酶活性缺失或缺陷,从而导致糖原在这些器官中积累和禁食期间的低血糖。在·摄取未烹调的玉米淀粉后6-  8小时都不会产生低血糖(J.I.Wolfsdorf,etal.,Am.J.Clin.Nutr.,51:1051-7,1990)。但是,用于这种治疗的玉米淀粉的剂量为每kg体重1.75g。该剂量远远高于糖尿病患者可以耐受的剂量。
另一项研究在在糖尿病患者中完成的,在患者住院期间供给玉米淀粉,结果血液中的葡萄糖水平的最低点降低。使儿童禁食,然后用未烹调的玉米淀粉作为标准就寝时间小吃的完全碳水化合物含量(30g的碳水化合物)给他们(M.T..Ververs et al.,Eur.J.Clin.Nutr.,47:268-73,1983)。然而,在该项研究中,玉米淀粉并未能防止低血糖,而且研究人员也未对不同剂量进行评价以确定最大效力。
因此,还需要一种更好的方法来治疗I型和II型糖尿病。具体地说,需要治疗或保持的方法来避免严重的低血糖发作,同时又不会引发高血糖。
通过摄取治疗食品组合物来调节和稳定糖尿病患者中的血液葡萄糖水平,所述组合物含糊缓慢代谢的复合碳水化合物,优选未烹调的玉米淀粉,更迅速代谢的复合碳水化合物,蛋白质和脂肪。所述食品组合物从胃肠道缓慢吸收,然后在糖尿病患者中可保持高达9小时的相对稳定的血糖水平。
优选在就寝时间前将所述水平组合物(可以是小吃形式的)施用给糖尿病患者,而且可基本有效地防止低血糖的夜间发作,同时又不会引起高血糖。
本发明涉及施用给患I型或II型糖尿病患者的治疗水平组合物,以帮助保持适当的血液葡萄糖调节并防止其大的波动,即低血糖和高血糖发作。将治疗组合物作为治疗全过程中的一部分施用,所述全过程包括控制饮食,施用胰岛素和/或在适当情况下的其他药疗法。
新食品组合物含有其必要组分:
a)  从人胃肠道缓慢吸收的复合碳水化合物(下文称为“缓慢吸收的碳水化合物”),即消化缓慢,而且甚至在3-4小时后均不能完全代谢;
b)  从消化道更迅速吸收的复合碳水化合物(下文称为“迅速吸收的碳水化合物”);
c)  蛋白质;和
d)  脂肪。
如本文所用的,术语“复合碳水化合物”指包括淀粉,多聚葡萄糖和其他多糖的大分子碳水化合物。
含前述组分的治疗组合物可以是任何传统“小吃”形式,如,棒,布丁,小饼,薄饼,牛奶和冰激淋等的混合饮料等。模仿糖果或燕麦花卷棒的小吃型棒最便于贮存,运送和施用目的,而用划线,打孔或刻槽于其上生产的时,在适当时很容易用于分开达到施用部分棒的目的。
新食品组合物优选每份或单位,如棒含有约20到约50g的营养物质,包含:
·  约17-24g的总碳水化合物(约5-15g缓慢吸收的碳水化合物和约7-19g迅速吸收的碳水化合物);
·  约5-20g的蛋白质;和
·  约3-7g的脂肪,优选至少三分之一的单不饱和脂肪。

本发明的治疗食品组合物优选每份或单位提供约115-230卡,其中:
·  约50-70%来自缓慢吸收的和迅速吸收的复合碳水化合物;
·  约15-25%来自蛋白质;和
·  约15-25%来自脂肪。
在本发明优选的实施方案中,新食品组合物是棒形式,包含17-24g的总碳水化合物,或在标准糖尿病食谱中,一对一和一半的“面包交换”当量。所述棒含有约5-15g缓慢吸收的未烹调玉米淀粉形式的碳水化合物,所述玉米淀粉通常含有约27%重的直链淀粉和73%重的支链淀粉。优选的实施方案还含有约7-19g迅速吸收的复合碳水化合物,但基本上不含单糖;约5-20g蛋白质;和约3-7g脂肪,至少三分之一是单不饱和脂肪。

由于未烹调的玉米淀粉的碳水化合物含量和其代谢率是已知的,而且相当均一,而且它容易配制到各种美味的食品组合物中,所以未烹调的玉米淀粉是本发明优选的缓慢吸收的碳水化合物。

在较小的儿科患者或夜间几乎不发作低血糖的患者中,可以施用比一单位少的剂量。
在用糖尿病儿童和青少年进行的临床研究中已经发现根据本发明制备的水平组合物,并按本文所述施用,在摄取后8-9小时或更长的时间,能够有效地将血糖水平保持在60mg/dl一250 mg/dl在“正常”范围内。
还可以在白天,用预先测量的新食品组合物剂量治疗施用胰岛素以促进葡萄糖食后吸收的患者,所述新食品组合物将在6-8小时被缓慢地代谢成单糖葡萄糖,这样来代替接受易引起血液葡萄糖水平迅速产生快速下降的峰的简单碳水化合物,如橙汁或其他糖源。在醒着的时间里,患者的葡萄糖利用和要求不同,而且取决于活动的水平和类型,如剧烈练习。因此确切的量和实际剂量的频率将因患者的不同而不同,而且对于每个患者来说,每天与每天也不同。可以用血液葡萄糖试验(通常用刺指以得到血样)来监测每天的葡萄糖水平,也用患者自己与低血糖发作有关的症状的主观经验来监测。因此,在本发明的实践中,以新食品组合物的形式施用了足够的复合碳水化合物以使血液葡萄糖水平保持在平均患者中正常约60mg/dl这个水平上一点。

根据本发明以小吃型香草坚果棒形式制备治疗食谱组合物。棒共重31g,含山梨醇,玉米淀粉,大豆蛋白质分离物,花生酱,水,多聚葡萄糖,花生,乳清蛋白浓缩物,天然香精,卵磷脂和柠檬酸。
在营养学上,棒提供了120卡路里(一个面包的当量),其中约54%来自碳水化合物,23%来自蛋白质,23%来自脂肪。所述棒饱和7g蛋白质,约3g总脂肪;约0.5g饱和脂肪,1g多不饱和脂肪和1g单不饱和脂肪。
所述棒的总碳水化合物含量为17g,其中5g是玉米淀粉(未烹调的),约12g是主要由多聚葡萄糖和花生提供的迅速吸收的碳水化合物。
所述棒含95mg钠和40mg钾。实施例2治疗糖尿病患者
将实施例1中的棒施用给以前经历了夜间低血糖发作的8名糖尿病儿童。每名儿童消费一个棒作为他或她常规夜间小吃的一部分,连续3-5个晚上,在夜间和在醒来后的早晨对每名患者的血液葡萄糖水平测量数次。
如在下文表1所列的数据,只有一名患者在消费了治疗棒后有低血糖,而且在从那个患者取的12个样品中,只有一个血糖读数超出正常,具有定义为血液葡萄糖水平<60mg/dl的低血糖。相反,所与患者在其消费了不含玉米淀粉的常规夜间小吃后,以前都曾经历过中度到严重的夜间低血糖发作。

Claims (28)

1    用于治疗糖尿病患者以消除血糖水平的波动并防止低血糖发作的治疗食品组合物,每份或单位含有约20-50g的营养物质,包括:
a)  约5-15g缓慢吸收的复合碳水化合物;
b)  约7-19g迅速吸收的复合碳水化合物;
c)  约5-20g蛋白质;和
d)  约3-7g脂肪。
2  根据权利要求1的组合物,总碳水化合物含量为约17-24g。
3  根据权利要求1的组合物,其中所述缓慢吸收的碳水化合物是玉米淀粉。
4  根据权利要求3的组合物其中所述玉米淀粉是未烹调的。
5  根据权利要求3的组合物,每单位含有约5 g的玉米淀粉。
6  根据权利要求1的组合物,其中至少所述脂肪的三分之一是单不饱和脂肪。
7  用于治疗糖尿病患者以消除血糖水平的波动并防止低血糖发作的治疗食品组合物,所述组合物每份或单位提供约115-230卡路里,其中:
a)约50-70%来自缓慢吸收和迅速吸收的复合碳水化合物;
b)约15-25%来自蛋白质;和
c)约15-20%来自脂肪。
8  根据权利要求1或7的组合物是小吃棒,布丁,饼,薄饼和牛奶和冰激淋等的混合饮料形式。
9  根据权利要求8的组合物是小吃棒形式。
10  根据权利要求9的组合物,其中所述棒用在其上划线,打孔和划槽以便于分成部分单位而生产的。
11  根据权利要求1或7的组合物,基本上没有单糖。
12  根据权利要求1或7的组合物,其中所述迅速吸收的碳水化合物,蛋白质,脂肪由多聚葡萄糖,花生,花生衍生物或其他坚果或坚果衍生物提供。
13  根据权利要求1或7的组合物,其中所述蛋白质有大豆蛋白质,乳清蛋白或酪氨酸水解产物提供。
14  治疗糖尿病患者以消除血糖水平波动并防止低血糖发作的方法,所述方法包括给患者施用每份或单位含约20-50g营养物质的治疗食品组合物,包括:
a)  缓慢吸收的复合碳水化合物;
b)  迅速吸收的复合碳水化合物;
c)  蛋白质;和
d)  脂肪。
15  根据权利要求14的方法,其中所述组合物每单位含有:
a)  约5-15g缓慢吸收的复合碳水化合物;
b)  约7-19g迅速吸收的复合碳水化合物;
c)  约5-20g蛋白质;和
d)  约3-7g脂肪。
16  根据权利要求15的方法,其中所述组合物每单位含约共17-24g的碳水化合物。
17  根据权利要求14的方法,其中所述缓慢吸收的碳水化合物是玉米淀粉。
18  根据权利要求17的方法,其中所述玉米淀粉是未烹调的。
19  根据权利要求17的方法,其中所述组合物每单位含约5g玉米淀粉。
20  根据权利要求14的方法,其中至少三分之一的所述脂肪是单不饱和的。
21  根据权利要求14的方法,其中所述组合物是小吃棒,布丁,饼,薄饼和牛奶和冰激淋等的混合饮料形式。
22  根据权利要求21的方法,其中所述组合物是小吃棒形式。
23  根据权利要求22的方法,其中所述棒用在其上划线,打孔和划槽以便于分成部分单位而生产的。
24  根据权利要求14的方法,其中将约1-2单位的所述组合物施用给患者。
25  根据权利要求14的方法,其中将半单位的所述组合物施用给患者。
26  根据权利要求14的方法,其中将所述组合物作为晚上或就寝时间前的小吃施用给患者。
27  根据权利要求14的方法,其中将所述组合物在白天施用给接受胰岛素治疗或忙于锻练的患者。
28  根据权利要求14的方法,其中所述患者是儿童或青少年。
