Abstract
本发明公开了一种治疗癌性腹水的药物组合物,本发明所述药物由锐不、比都督、锐摸速、锐毕多埋、锐地、锐借改、锐不鲁烧组方及辅料制备而成。具有清热解毒、利尿消肿、活血散瘀、通络除湿的功效,适用于患有各种癌症的癌性腹水人群,以达到治疗癌性腹水的疗效,制成的药物疗效显著,无毒副作用。

本发明涉及一种治疗癌性腹水的苗药组方及其制备方法,属于药品技术的领域。
技术背景:
癌性腹水是晚期癌症患者最常见的并发症之一,临床上,癌症腹水的发生率在15%-50%。肝癌、肺癌、胃癌、乳腺癌、胰腺癌、肠癌等癌症,发生腹水的可能性很大。癌症患者出现腹水,一般也就意味着这名患者步入了晚期的行列。腹水的产生,是癌细胞长期在体内浸润的恶性体现。民间把癌症形象的叫做体内生疮,虽然不准确,但是在发病形态上形容的还是相对精准的。癌细胞在体内肆意侵占和生根,正常组织会出现破损以及持续性的溃疡,而腹水就是在这个过程中不断积累起来的,腹水是体内正常组织被侵润破损后,一点点渗出来的液体,当然也包括血了。恶性腹水会使患者感到疲劳、呼吸困难、食欲不振、呕吐、腹部胀痛,日常生活受到严重限制,患者的活动耐受性降低,稍微的走动都会感到劳累不堪。目前临床上,对于腹水处理的方式有两种,一种是药物干预,另一种是穿刺抽取。药物通常是使用利尿药物来减少体内腹水的积存,适用于症状较轻的患者,但利尿剂副作用比较大。腹水严重者采取穿刺的方式用针筒抽取腹水或者给患者装上一个引流装置,将体内腹水引出体内。以上方式虽可以暂时减少体内腹水,却是治标不治本。苗医药被列为第二批国家级非物质文化遗产名录,苗族医对疾病的认识和立方遣药有其独特模式。苗族在长期生产和与疾病斗争的实践中,总结积累了丰富而宝贵的医药经验,逐渐形成了苗族医的“纲、经、症、疾”的理论模式,以及关于病因、疾病分类、诊断、治疗和预防等方面的具有浓郁民族特色的苗族医药理论。苗医认为,毒、亏、伤、积、菌、虫是导致人体生病的六种因素,简称六因。而六因归根结底都要用产生毒害力的方式才能导致人体生病,所以苗医素有“无毒不生病”之说。故针对癌症腹水,苗医采用清热解毒、利尿消肿、活血散瘀、通络除湿的药物,来除去体内癌细胞滋生的毒素,不仅有效的排除内体腹水,还能起到清热解毒的作用,从根本上治疗癌性腹水。其安全性大,副作用小,也无药物依赖性。本专利申请得到了2019科技部国家重点研发计划中医药现代化研究《十五个少数民族医防治常见病特色诊疗技术、方法、方药整理与示范研究》(项目编号:2019YFC1712500,课题编号:2019YFC1712504)项目支持。
发明内容:
本发明所要解决的技术问题在于提供一种治疗癌性腹水的药物及其制备方法。所述药物具有清热解毒、利尿消肿、活血散瘀、通络除湿的功效,能治疗各种癌性腹水,适用于癌性腹水人群。
为解决以上技术问题,本发明采用以下技术方案实现:
一种治疗癌性腹水的苗药组方,按照重量组份计算,由锐不50-150份、比都督50-150份、锐摸速50-150份、锐毕多埋50-150份、锐地50-150份、锐借改50-150份、锐不鲁烧50-150份及辅料制备而成。
具体地说,所述治疗癌性腹水的苗药组方,按照重量组份计算,由锐不80-120份、比都督80-120份、锐摸速80-120份、锐毕多埋80-120份、锐地80-120份、锐借改80-120份、锐不鲁烧80-120份及辅料制备而成。
更具体地说,所述所述治疗癌性腹水的苗药组方,按照重量组份计算,由锐不100份、比都督100份、锐摸速100份、锐毕多埋100份、锐地100份、锐借改100份、锐不鲁烧100份及辅料制备而成。
前述治疗癌性腹水苗药组方的制备方法为:按照比例称取方中所有药材,与药物中可以接受的药用辅料进行组合,并按照常规的制备方法进行加工,制成相应的药物制剂。
具体地说,前述治疗癌性腹水苗药组方的制备方法为:按照处方比例称取各药材,加6 倍量水浸泡0.5h后煎煮3次,每次1h,三次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩成浸膏,与药物中可以接受的药用辅料进行组合,并按照常规的制备方法进行加工,制成相应的药物制剂。
所述药物制剂包括煎膏剂、硬胶囊剂、片剂、颗粒剂、糖浆剂或口服液。所述煎膏剂这样制备:按照处方比例称取各药材,加6倍量水浸泡0.5h后煎煮3次,每次1h,三次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,另取制成量30%蔗糖,勤翻干炒至全溶,色转黄,至发泡及微有青白烟发生时即得炒糖,将炒糖慢慢加入上述浸膏中,搅拌均匀,继续用文火加热收膏,待取少许能平拉成丝,或滴于纸上不见水迹时,即得。
所述胶囊剂这样制备:按照处方比例称取各药材,加8倍量水浸泡0.5h后煎煮2次,每次2h,两次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,60℃干燥成干浸膏,粉碎成细粉,加入制成量5%的羟丙甲纤维素和1%的微晶纤维素混合均匀,用60%的乙醇制粒,干燥,整粒,填装胶囊,即得。
所述片剂这样制备:按照处方比例称取各药材,加5倍量水浸泡0.5h后煎煮3次,第一次1.5h,第二次0.5h和第三次0.5h,三次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,浸膏于60℃干燥,并粉碎成细粉,即得干浸膏,取制成量1%的硬脂酸镁混合均匀,另取制成量10%的淀粉,加水溶解,制成浓度为10%的淀粉浆,制粒,干燥,压片,薄膜包衣,即得。
所述颗粒剂这样制备:按照处方比例称取各药材,加10倍量水浸泡0.5h后煎煮2次,每次1h,两次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,浸膏于60℃干燥,并粉碎成细粉,即得干浸膏,取干浸膏1份,蔗糖2份,用90%乙醇为润湿剂制颗粒,制成颗粒后于60℃干燥,整粒,即得。
发明所述药物组合物在制成药剂时,单位剂量的药剂可含有本发明的药物活性物质0.1-1000mg,其余为药学上可接受的辅料。药学上可接受的辅料以重量计可以是制剂总重量的0.1-99.9%。
本发明所述药物由锐不(又名:腹水草)、比都督(又名:土茯苓)、锐摸速(又名:猪秧秧)、锐毕多埋(又名:水黄连)、锐地(又名:水黄花)、锐借改(又名:半边莲)、锐不鲁烧(又名:土知母)。及辅料制备而成。各成分性质及功效如下:锐不(又名:腹水草)为玄参科植物爬红岩
Veronicastrum axillare(Sieb. et Zucc.)Yamazaki或毛叶腹水草Veronicastrum villosulum(Miq.)Yamazaki的全草;味苦,性微寒;归肝、脾、肾经;具有行水,消肿,散瘀,解毒之功效;用于肝硬化腹水,肾炎水肿,跌打损伤,疮肿疔毒,烫伤,毒蛇咬伤。比都督(又名:土茯苓)为百合科植物光叶菝葜Smilax glabra Roxb.的干燥根茎;夏、秋二季采挖,除去须根,洗净,干燥;或趁鲜切成薄片,干燥;味甘、淡,性平;归肝、胃经;具有解毒,除湿,通利关节的功效;用于梅毒及汞中毒所致的肢体拘挛,筋骨疼痛;湿热淋浊,带下,痈肿,瘰疬,疥癣。锐摸速(又名:猪秧秧)为茜草科植物猪秧秧Gali xma paris eL.war.tererum(Gren.et God r.) Reich b.的全草;味甘、辛、微苦,性凉;归肝,膀胱经;具有清热解毒,利尿消肿,活血通络的功效;主治泌尿系感染,水肿,牙龈出血,崩漏,痛经,白带,癌症,白血病,阑尾炎;外用治疗急性乳腺炎,痈肿疮疖,跌打损伤等。锐毕多埋(又名:水黄连)为毛茛科唐松草属植物箭头唐松草Thalictrum simplex L. var. brevipes Hara的根,以全草入药;春末夏初采,洗净,晒干;味苦,性寒;具有清热,利尿的功效;用于黄疸、腹水,小便不利;外用治眼结膜炎。锐地(又名:水黄花)为大戟科大戟属植物水黄花Euphorbia chrysocoma Levl. et Van.的根皮和叶;分布于贵州各地。此外,云南、四川、广西、西藏、湖北亦有;味苦、辛,性寒;归心、肾经;具有逐水,利尿之功效;用于水肿,臌胀,疥疮瘙痒。锐借改(又名:半边莲)为桔梗科植物半边莲Lobelia chinensis Lour. 的干燥全草;夏季采收,除去泥沙,洗净,晒干;味辛,性平;归心、小肠、肺经;具有清热解毒,利水消肿的功效;用于痈肿疗疮,蛇虫咬伤,臌胀水肿,湿热黄疽,湿疹湿疮。锐不鲁烧(又名:土知母、川射干)为鸢尾科植物鸢尾Iris tectorum Maxim.的干燥根茎;全年均可采挖,除去须根及泥沙,干燥;味苦,性寒;归肺经;具有清热解毒,祛痰,利咽的功效;用于热毒痰火郁结,咽喉肿痛,痰涎壅盛,咳嗽气喘。

锐借改(半边莲)
功效:解毒消炎,利水消肿
主治:肝硬化腹水,肾炎水肿,疮毒疡肿
锐不鲁烧(土知母)
功效:泻热通便,利水消肿,解毒消痈
主治:热结便秘,小便不利,水肿,痛疮
中医分析:中医治疗癌性腹水,将其归于“臌胀”范畴,治疗以“行气、活血、利水”为法,本方用药兼顾发病“癌毒”基础,又重视“气、血、水”邪毒,在清热消肿、解毒抗癌的基础上,起到利水消肿、活血化瘀的作用。
苗医分析:苗医将本病归为“普噶秋”的范畴,乃多种病因侵入人体,伤及气血水。气机受阻,胃肠功能下降,水谷不化,食积腹胀,气虚血流减慢,气滞血瘀,青筋壮大,伤气必伤水(蒙笨完茵沃),同样伤气也必伤水(茵乌茵象)。气血水三者共损,日久则成本病。本病属于可分为热经水臌病及冷经水臌病两类,热经水臌病与中医臌胀实胀相似,冷经水臌病与中医臌胀虚胀相似。热经水臌病主要采用沆笨丢象(行气化瘀)、佳沃(利水)进行治疗;冷经水臌病主要采用攻下利水进行治疗。通过利水、化瘀、行气调畅人体的气血水三要素,达到气通、血安、水行的生理状态,以缓解肿瘤患者腹腔积液。本方用药兼顾气血水,考虑全面,无论冷经水臌病抑或热经水鼓病均适宜。
实验例:药效学研究
一、毒理学实验
1、实验动物:SD大鼠30只,清洁级,体质量180~220g,雌雄各半,购买于贵州医科大学实验动物中心,动物许可证:SYXK(黔)2018-001。
2、实验方法:将30只SD大鼠随机分成3组,即本发明药物实施例1低剂量组、中剂量组、高剂量组,每组10只,雌雄各半。采用本发明药物实施例1低(5g/kg)、中(10g/kg)、高(20g/kg)三个剂量,分别灌胃给药大鼠,连续给药60天,观察大鼠生长发育、行为活动、精神状态、血液即组织病理改变。
3、结果与结论:观察给药过程中和60天后各指标,结果显示未见大鼠死亡,各组大鼠体重、摄食量、行为活动、精神状态、外周血象、重要脏器系数等均正常,未见明显毒副作用,说明本发明药物在规定使用剂量下服用是安全可靠的。
二、临床试验典型病例
1、陈某,男,67岁,原发性肝癌,行肝动脉造影+TACE术,术后腹胀、隐痛、呼吸不畅,下肢浮肿,饮水难下,夜半发热,精神倦怠,上腹部CT平扫+增强显示腹腔少量积液,采用西医常规治疗症状无改善,给予本发明实施例1苗药组合煎膏剂,放入热水中化开后口服,每次20g,每日三次,或直接吞服。服用一个疗程后,患者自述腹胀、腹痛改善,呼吸功能有所好转,继续服用3个疗程,CT复查显示腹腔积液明显减少,腹胀、腹痛消失,呼吸顺畅,患者病情稳定,无其他不适,身体状况良好。
2、张某,男,52岁,患者左肺癌手术切除后,出现胸腔积液,症见咳嗽,咳痰,胸部不适,使用头孢类药物消炎、抽液后短暂控制,效果不佳,反复发作,遂使用本发明苗药实施例2胶囊剂口服治疗,一次3粒,一日3次。服用1个疗程后,咳嗽、不适症状减轻,继续服用3个疗程,上述症状基本消失,检查积液量明显减少,患者生活质量提高,病情稳定。
3、张某,女,71岁,升结肠癌,症见腹痛,神疲乏力,胸闷气短,畏寒肢冷,双下肢肿胀。给予本发明药物实施例1苗药组合煎膏剂,放入热水中化开后口服,每次20g,每日三次,服用一个疗程后,患者腹痛症状减轻,精神好转,胸闷等症状改善,继续治疗3个疗程,各项临床症状基本消失,双下肢肿胀明显好转,患者生活治疗明显改善。
4、李某,女,47岁,患乙肝20余年,患者自述腹胀、腹痛,症见皮肤暗黄,消瘦,精神倦怠。经医院确诊为肝右叶多发结节,彩超显示肝硬化、脾大,腹水,经抽水、西医常规治疗,效果不佳,易反复。给予本发明实施例2苗药胶囊剂,口服治疗,一次3粒,每日三次。服用1个疗程后,患者腹胀、腹痛较前改善,精神有所好转,继续治疗3个疗程,复查见腹腔腹水明显减少,腹痛症状明显改善,精神恢复正常,皮肤状态好转,病情稳定后出院。
与现有技术相比,本发明选用锐不(又名:腹水草)、比都督(又名:土茯苓)、锐摸速(又名:猪秧秧)、锐毕多埋(又名:水黄连)、锐地(又名:水黄花)、锐借改(又名:半边莲)、锐不鲁烧(又名:土知母)组方,具有清热解毒、利尿消肿、活血散瘀、通络除湿的功效,适用于患有各种癌症的癌性腹水人群,以达到治疗癌性腹水的疗效,制成的药物疗效显著,无毒副作用,达到了发明目的。
以下通过实施例进一步说明本发明,但不作为对本发明的限制。

实施例1:

工艺:按照处方比例称取各药材,加5倍量水浸泡0.5h后煎煮3次,第一次1.5h,第二次0.5h和第三次0.5h,三次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,浸膏于60℃干燥,并粉碎成细粉,即得干浸膏,取制成量1%的硬脂酸镁混合均匀,另取制成量10%的淀粉,加水溶解,制成浓度为10%的淀粉浆,制粒,干燥,压片,薄膜包衣,即得片剂。
用法用量:用开水冲服,一次3片,一日3次。
实施例4:
处方:锐不70g、比都督80g、锐摸速50g、锐毕多埋150g、锐地100g、锐借改50g、锐不鲁烧120g。
工艺:按照处方比例称取各药材,加10倍量水浸泡0.5h后煎煮2次,每次1h,两次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,浸膏于60℃干燥,并粉碎成细粉,即得干浸膏,取干浸膏1份,蔗糖2份,用90%乙醇为润湿剂制颗粒,制成颗粒后于60℃干燥,整粒,即得颗粒剂。
用法用量:口服,一次10克,一日3次。
实施例5:
处方:锐不55g、比都督120g、锐摸速110g、锐毕多埋125g、锐地75g、锐借改50g、锐不鲁烧90g。
工艺:按照处方比例称取各药材,加5倍量水浸泡0.5h后煎煮2次,每次2h,两次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,另取制成量30%蔗糖,勤翻干炒至全溶,色转黄,至发泡及微有青白烟发生时即得炒糖,将炒糖慢慢加入上述浸膏中,搅拌均匀,继续用文火加热收膏,待取少许能平拉成丝,或滴于纸上不见水迹时,即得。即得煎膏剂。

处方:锐不120g、比都督85g、锐摸速65g、锐毕多埋120g、锐地140g、锐借改90g、锐不鲁烧50g。
工艺:按照处方比例称取各药材,加4倍量水浸泡0.5h后煎煮3次,每次1.5h,两次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,60℃干燥成干浸膏,粉碎成细粉,加入制成量5%的羟丙甲纤维素和1%的微晶纤维素混合均匀,用60%的乙醇制粒,干燥,整粒,填装胶囊,即得胶囊剂。
用法用量:口服,一次3粒,一日3次。
实施例7:
处方:锐不150g、比都督75g、锐摸速120g、锐毕多埋145g、锐地65g、锐借改80g、锐不鲁烧80g。
工艺:按照处方比例称取各药材,加8倍量水浸泡0.5h后煎煮3次,每次2h,三次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,浸膏于60℃干燥,并粉碎成细粉,即得干浸膏,取制成量1%的硬脂酸镁混合均匀,另取制成量10%的淀粉,加水溶解,制成浓度为10%的淀粉浆,制粒,干燥,压片,薄膜包衣,即得片剂。
用法用量:用开水冲服,一次3片,一日3次。

1.一种治疗癌性腹水的苗药组方,其特征在于:按照重量组份计算,由锐不50-150份、比都督50-150份、锐摸速50-150份、锐毕多埋50-150份、锐地50-150份、锐借改50-150份、锐不鲁烧50-150份及辅料制备而成。
2.如权利要求1所述治疗癌性腹水的苗药组方,其特征在于:按照重量组份计算,由锐不80-120份、比都督80-120份、锐摸速80-120份、锐毕多埋80-120份、锐地80-120份、锐借改80-120份、锐不鲁烧80-120份及辅料制备而成。
3.如权利要求1或2所述治疗癌性腹水的苗药组方,其特征在于:按照重量组份计算,由锐不100份、比都督100份、锐摸速100份、锐毕多埋100份、锐地100份、锐借改100份、锐不鲁烧100份及辅料制备而成。
4.制备权利要求1-3中所述治疗癌性腹水苗药组方的制备方法,其特征在于:按照比例称取方中所有药材,与药物中可以接受的药用辅料进行组合,并按照常规的制备方法进行加工,制成相应的药物制剂。
5.如权利要求4所述治疗癌性腹水苗药组方的制备方法,其特征在于:按照组方比例称取各药材,加水浸泡后煎煮,煎液合并,混匀,浓缩,趁热过滤,滤液浓缩成浸膏,与药物中可以接受的药用辅料进行组合,并按照常规的制备方法进行加工,制成相应的药物制剂。
6.如权利要求5所述治疗癌性腹水苗药组方的制备方法,其特征在于:所述药物制剂包括煎膏剂、硬胶囊剂、片剂、颗粒剂、糖浆剂或口服液。
7.如权利要求6所述治疗癌性腹水苗药组方的制备方法,其特征在于:所述煎膏剂这样制备:按照处方比例称取各药材,加6倍量水浸泡0.5h后煎煮3次,每次1h,三次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,另取制成量30%蔗糖,勤翻干炒至全溶,色转黄,至发泡及微有青白烟发生时即得炒糖,将炒糖慢慢加入上述浸膏中,搅拌均匀,继续用文火加热收膏,待取少许能平拉成丝,或滴于纸上不见水迹时,即得。
8.如权利要求6所述治疗癌性腹水苗药组方的制备方法,其特征在于:所述胶囊剂这样制备:按照处方比例称取各药材,加8倍量水浸泡0.5h后煎煮2次,每次2h,两次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,60℃干燥成干浸膏,粉碎成细粉,加入制成量5%的羟丙甲纤维素和1%的微晶纤维素混合均匀,用60%的乙醇制粒,干燥,整粒,填装胶囊,即得。
9.如权利要求6所述治疗癌性腹水苗药组方的制备方法,其特征在于:所述片剂这样制备:按照处方比例称取各药材,加5倍量水浸泡0.5h后煎煮3次,第一次1.5h,第二次0.5h和第三次0.5h,三次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,浸膏于60℃干燥,并粉碎成细粉,即得干浸膏,取制成量1%的硬脂酸镁混合均匀,另取制成量10%的淀粉,加水溶解,制成浓度为10%的淀粉浆,制粒,干燥,压片,薄膜包衣,即得。
10.如权利要求6所述治疗癌性腹水苗药组方的制备方法,其特征在于:所述颗粒剂这样制备:按照处方比例称取各药材,加10倍量水浸泡0.5h后煎煮2次,每次1h,两次煎液合并,混匀,浓缩成原药液的80%时,趁热过滤,滤液浓缩至60℃时相对密度为1.20-1.30的浸膏,浸膏于60℃干燥,并粉碎成细粉,即得干浸膏,取干浸膏1份,蔗糖2份,用90%乙醇为润湿剂制颗粒,制成颗粒后于60℃干燥,整粒,即得。
