Abstract
The invention discloses an anti-inflammatory, antipruritic and skin-moistening traditional Chinese medicine external preparation, and a preparation method and application thereof, and relates to the technical field of traditional Chinese medicines; the traditional Chinese medicine external preparation comprises the following components in parts by weight: and (3) a component A: 24-26 parts of a component B: 5-7 parts of component C: 25-27 parts of a lubricant; vaseline: 270-300 parts of a lubricant; the component A consists of 9-11 parts of moutan bark and 14-16 parts of radix rehmanniae; the component B is radix sophorae flavescentis; the component C consists of 5-7 parts of pepper, 9-11 parts of mint and 9-11 parts of borneol. The traditional Chinese medicine external preparation has remarkable anti-inflammatory and antipruritic effects and good skin moisturizing and blood circulation promoting effects by selecting and matching the medicinal components in a specific proportion and optimizing the dosage proportion of the medicinal components, and particularly has remarkable treatment effects on patients with itching skin diseases such as chronic eczema, contact dermatitis, prurigo, neurodermatitis, atopic dermatitis, psoriasis and the like.

Technical Field
The invention relates to the technical field of traditional Chinese medicines, in particular to an anti-inflammatory, antipruritic and skin-moistening traditional Chinese medicine external preparation, and a preparation method and application thereof.
Background
The chronic pruritus skin disease has the characteristics of repeated attack and difficult pruritus, the external traditional Chinese medicine preparation has better curative effect in clinic, and a plurality of classical formulas are also produced. "Sheng Ji Zong Lu" indicates that "the ointment is taken for moistening it to dispel pathogenic toxin, and so on, so it can dispel the pathogenic toxin and also can be used for massage. The scorpion plaster (proved recipe of the university of Chinese medicine of Heilongjiang for several decades) is one of external medicines with special effects for treating various diseases in surgery, and has definite effect and wide application; the main components of the traditional Chinese medicine are scorpion, centipede, borneol and the like, and the traditional Chinese medicine has good effects of removing blood stasis and dredging collaterals, but has weak effects of cooling blood and activating blood, eliminating dampness and relieving itching, and has adverse reactions of skin injury, swelling, itching and the like of partial allergic disease patients in the clinical use process.
Therefore, a traditional Chinese medicine preparation which has anti-inflammatory and antipruritic effects on inflammatory and pruritic skin and avoids skin irritation and sensitization in the use process is developed, and the requirements of markets and patients can be effectively met.
Disclosure of Invention
The invention aims to overcome the defects of the prior art and provide an anti-inflammatory, antipruritic and skin-moistening traditional Chinese medicine external preparation, a preparation method and application thereof.

Further, the traditional Chinese medicine external preparation comprises the following components in parts by weight: and (3) a component A: 25 parts of a component B: 6 parts of component C: 26 parts of Vaseline: 270-600 parts.
Further, the component A consists of 10 parts of moutan bark and 15 parts of radix rehmanniae.
Further, the component C consists of 6 parts of pepper, 10 parts of mint and 10 parts of borneol.
Further, the weight portion of Vaseline in the traditional Chinese medicine external preparation is 285. Namely, the ratio of the total weight of the component A, the component B and the component C to the weight of the Vaseline is 1:5.

In a second aspect, there is provided a method for preparing the anti-inflammatory, antipruritic, and skin-caring external preparation of traditional Chinese medicine according to the first aspect, comprising the steps of:
(1) Weighing the components according to a proportion;
(2) Heating vaseline to dissolve, adding cortex moutan, radix Sophorae Flavescentis and radix rehmanniae into vaseline, decocting to brown, and separating out effective components to obtain mixture;
(3) When the temperature of the mixture is reduced to 60 ℃, adding peppermint and pricklyash peel for frying, and filtering while the mixture is hot to obtain filtrate;
(4) And (3) after the temperature of the filtrate is reduced to 50 ℃, adding borneol, stirring uniformly, and cooling and solidifying to obtain the traditional Chinese medicine external preparation.

The traditional Chinese medicine external preparation provided by the invention is prepared by adjusting the composition of the medicines mentioned in the background art, eliminating toxic medicines, and screening medicines with remarkable external curative effects of clearing heat and detoxicating, cooling blood and eliminating spots through experiments.
Specifically, the invention takes moutan bark as a monarch drug. The medicine is described in the compendium of materia medica as 'nourishing yin, reducing pathogenic fire, resolving macula toxin, benefiting throat, and relieving urination and blood stagnation'. "Ben Cao Jing Shu" theory: cortex moutan, which is bitter and slightly pungent, is cold in nature and nontoxic, and is pungent in nature to dispel accumulation of pathogen, bitter and cold in nature to remove blood heat, enter blood system and cool blood heat. The invention takes radix rehmanniae as ministerial drug, and the function of radix rehmanniae recorded in 2010 edition of Chinese pharmacopoeia is "heat clearing and blood cooling, yin nourishing and body fluid production promoting". Can be used for treating heat-induced ying blood, toxic heat, speckle, hematemesis, epistaxis, yin injury due to heat disease, polydipsia, constipation due to body fluid deficiency, fever due to yin deficiency, bone steaming, internal heat, and diabetes. Modern pharmacology discovers that the radix rehmanniae pigment extracted from radix rehmanniae has certain antibacterial activity on escherichia coli, staphylococcus aureus and bacillus subtilis. Most of flavonoid active substances are flavanone and flavanol, and have anti-inflammatory and insecticidal effects; the alkaloid active substances are matrine and oxymatrine, and have antiinflammatory effect. The active ingredient of the antiallergic effect of kuh-seng is mainly oxymatrine, which has obvious inhibition effect on passive skin anaphylaxis and reversed phase skin anaphylaxis of rats, arthus reaction and delayed anaphylaxis induced by sheep red blood cells. Matrine can inhibit various stages of inflammatory process, and has various degrees of inhibition on various inflammatory mediators. The oxymatrine has strong immunoregulatory effect, and can exert anti-inflammatory effect by influencing antibody level of host, immune cell change, cytokine and other inflammatory regulatory factors. Has obvious inhibiting effect on colibacillus, staphylococcus aureus, streptococcus A, streptococcus B, dysentery pole amiable, salmonella trehalosi and proteus.
The invention further selects pepper, peppermint and borneol as guiding drugs. Wherein, the Chinese prickly ash is pungent and warm in flavor, mainly indicated for pathogenic wind and pathogenic qi, warming middle energizer, eliminating cold arthralgia, strengthening teeth, improving eyesight and expelling descending qi. Mainly has the effects of warming middle-jiao to relieve pain, killing parasites and relieving itching. Modern pharmacological researches have the anti-inflammatory activity and antibacterial activity, and simultaneously have the effect of inhibiting the hyperplasia of prostate, so that the traditional Chinese medicine composition has good effects of relieving and eliminating pain and itch. Peppermint is cool and pungent in nature, enters lung and liver meridians, and has the effects of dispelling wind-heat, clearing head and eyes, relieving sore throat and eruption, soothing liver and relieving depression, and aromatic and dirt-removing. The traditional Chinese medicine external preparation is prepared by taking vaseline as a matrix, decocting the following raw material components in parts by weight, filtering, cooling and solidifying, wherein the mass ratio of the sum of the raw material components to the vaseline is 1:5; the weight portions of the raw material components are as follows: 25 parts of a component A, 6 parts of a component B and 26 parts of a component C; wherein the component A consists of 10 parts of moutan bark and 15 parts of radix rehmanniae; the component B consists of 6 parts of kuh-seng; the component C consists of 6 parts of pepper, 10 parts of mint and 10 parts of borneol.
The preparation method of the traditional Chinese medicine external preparation comprises the following steps:
(1) Heating white vaseline to dissolve;
(2) Then adding A, B components into the vaseline in the step (1), and decocting until the vaseline turns brown, so as to separate out active ingredients and obtain a mixture;
(3) Frying herba Menthae and fructus Zanthoxyli in component C, and filtering to obtain filtrate;
(4) And (3) after the temperature of the filtrate is reduced to 50 ℃, adding the borneol in the component C, stirring uniformly, and cooling and solidifying to obtain the external traditional Chinese medicine preparation.

Microbial limit detection and clinical trials
Microbial limit detection
Microbial limit detection was performed on the Chinese medicinal external preparations prepared in examples 1 to 9:
microbial limit: the regulations 1105, 1106 and 1107 are checked according to the China pharmacopoeia 2015 edition, and the regulations should be in compliance with the regulations.
Preparing a test sample ointment: 10g of the external preparation of the traditional Chinese medicine prepared in examples 1-9 is respectively taken and placed in an empty bottle containing sterilized glass beads, sterile sodium chloride-peptone buffer solution with pH of 7.0 is added to 100ml, the mixture is shaken while being added, the mixture is stood for 1 minute, and the lower layer solution is taken as a test solution of 1:10, so that the test solutions corresponding to the external preparation of the traditional Chinese medicine prepared in examples 1-9 are respectively obtained.
Total number of aerobic bacteria: 1ml of the 1:10 test solution corresponding to examples 1 to 9 was taken and examined by the plate method.
Total number of mold and yeast: 1ml of the 1:10 test solution corresponding to examples 1 to 9 was taken and examined by the plate method.
Pseudomonas aeruginosa: 10ml of the 1:10 test solution corresponding to examples 1-9 was inoculated into not less than 100ml of trypticase soytone liquid medium, and examined according to law.
Staphylococcus aureus: 10ml of the 1:10 test solution corresponding to examples 1-9 was inoculated into not less than 100ml of trypticase soytone liquid medium, and examined according to law.
The microbial limit detection results of the external preparations of Chinese medicines prepared in examples 1 to 9 are shown in the following Table 1, and all meet the regulations:

According to the test results of Table 1, the microbial limit detection results of the external preparation of the traditional Chinese medicine prepared in examples 1-9 all meet the relevant regulations.
Clinical trial
Clinical trials evaluate the effects of the external preparation of the traditional Chinese medicine prepared in examples 1-9 on treating dermatitis eczema, prurigo, chronic actinic dermatitis, psoriasis and erythrodermic patients. Each type of patient who tried each external preparation of traditional Chinese medicine was 10 persons, and the use was continued for 7 days, and statistical patient use evaluation (percentage of patients well evaluated) was recorded, and the evaluation results of the clinical trial are shown in table 2 below.

According to the test results of Table 2, the external traditional Chinese medicine preparation has obvious treatment effects on dermatitis eczema, prurigo, chronic actinic dermatitis, psoriasis and erythrodermic patients.
In comparison with example 1 and examples 2 to 3, in example 2, the amount of the component A and the amount of the component B are increased and the amount of the component C is reduced, and on the premise that the total amount of the components A and B is kept unchanged, the external preparation of the traditional Chinese medicine of example 2 has obviously reduced curative effects on dermatitis eczema, chronic actinic dermatitis and erythroderma; example 3 the amount of the component a was reduced while the amounts of the component B and the component C were increased, and the external preparation of the traditional Chinese medicine of example 3 was significantly reduced in the treatment effect of dermatitis eczema, prurigo, chronic actinic dermatitis, psoriasis and erythroderma on the premise that the total amount remained unchanged; therefore, the external preparation of the traditional Chinese medicine has obvious anti-inflammatory and antipruritic effects, but the specific dosage among the components in the traditional Chinese medicine can influence the treatment effect of the external preparation of the traditional Chinese medicine. The invention can realize remarkable treatment effect on various itching skin diseases such as chronic eczema, contact dermatitis, prurigo, neurodermatitis, atopic dermatitis, psoriasis and the like by proportioning according to specific component proportions.
In comparative example 1 and examples 8 to 9, example 8 decreased the amount of the C component while increasing the amount of the B component, and when the total amount was smaller than that of example 1, the external preparation for a Chinese medicine of example 8 had significantly decreased therapeutic effects on dermatitis eczema, prurigo, chronic actinic dermatitis, psoriasis and erythroderma as compared with example 1; on the basis of the example 8, the example 9 reduces the amount of the component A and increases the amount of the component C on the premise of keeping the total amount unchanged, and compared with the example 8, the external traditional Chinese medicine preparation of the example 9 has obviously reduced therapeutic effects on prurigo, chronic actinic dermatitis and erythroderma, but obviously improved therapeutic effects on dermatitis eczema.
Comparative example 1 and examples 5 to 7, examples 5 to 7 increase the amounts of the A, B and C components and at the same time appropriately increase the addition amount of vaseline; however, examples 5-7 showed a different degree of decrease in the therapeutic effect of dermatitis eczema, prurigo, chronic actinic dermatitis, psoriasis and erythroderma compared to example 1; the specific dosage among the components in the invention can influence the treatment effect of the external preparation of the traditional Chinese medicine. The invention can realize remarkable treatment effect on various itching skin diseases such as chronic eczema, contact dermatitis, prurigo, neurodermatitis, atopic dermatitis, psoriasis and the like by proportioning according to specific component proportions.
Comparative example 1 and examples 4 and 8, example 4 greatly reduced the amount of C component while increasing the amount of B component, and increased the amount of petrolatum when the total amount was much smaller than that of examples 1 and 8; example 4 has significantly reduced therapeutic effects on dermatitis eczema, chronic actinic dermatitis, psoriasis and erythroderma compared to example 1; compared with example 8, example 4 has significantly improved therapeutic effects on dermatitis eczema and prurigo; therefore, in the traditional Chinese medicine preparation, the dosage of the Vaseline is properly added to improve the treatment effect of other components, the Vaseline is selected as a matrix to keep skin moist, promote the hydration degree of inflammatory skin, reduce the loss of percutaneous moisture, accelerate the self-repairing capability of the skin, effectively isolate the skin from contacting bacteria in the air, reduce the possibility of infection, and simultaneously cooperate with the monarch, minister, assistant and guide medicines to play the roles of anti-inflammatory, antipruritic and moisturizing, and improve the overall efficacy.
Meanwhile, the invention also provides a method for treating dermatitis eczema, prurigo, chronic actinic dermatitis, psoriasis and erythrodermic patients by using the traditional Chinese medicine external preparation prepared in the example 1 in a hospital, the patients are revisited in 5-year clinical use conditions as shown in the following table 3, the use evaluation of the patients is recorded and counted, and the percentage in the table is the proportion of the patients with good treatment effect evaluation of the traditional Chinese medicine external preparation.
