背景技术
胶质瘤是儿童中最常见的癌症之一。由于其异质性,胶质瘤不呈现单一突出的抗原。因此,针对小儿胶质瘤的嵌合抗原受体(chimeric antigen receptor,CAR)T细胞治疗将需要鉴定和靶向泛癌抗原(pan-cancer antigen)。最近,报道了一种通用的抗FITC(异硫氰酸荧光素)CAR-T细胞,其中施用具有与肿瘤特异性配体连接的荧光素的双特异性衔接子分子促进CAR-T细胞与肿瘤细胞的桥接,并且随后CAR-T细胞活化杀伤癌细胞。已经报道了特异性地靶向癌细胞膜的某些合成磷脂醚(phospholipid ether,PLE)。因为它们靶向癌细胞膜本身,所以它们有成为泛癌靶向配体的潜能。已知这些PLE会被正常细胞快速分解代谢,但会在肿瘤细胞膜上持续很长时间。此外,这些PLE需要冗长的合成(12至17个步骤)。
含有与异硫氰酸荧光素缀合的PLE(称为FL-PLE)的双特异性衔接子分子与抗FITCCAR-T细胞组合使用,以实现针对胶质瘤的治疗(图1)。聚乙二醇(PEG3)接头用于连接FITC和脂质组分。FL-PLE是在12个步骤的合成中产生的。为了提高对癌症的特异性,以在荧光素的芳环上的-OH上存在保护基(其被存在于癌症中的活性氧物质选择性地裂解)的形式(称为Pro-FL-PLE)引入额外水平的特异性(图2)。基于Pro-FL-PLE的显像剂在胶质瘤中显示出约1.5的肿瘤与背景比,这表明这种受保护的形式在CAR-T细胞治疗剂靶向癌组织时可显示出对癌组织的特异性。Pro-FL-PLE的合成以17个步骤完成。

药物组合物可通过将一种或更多种化合物与可药用载体和任选地一种或更多种另外的药物活性剂组合来制备。

通常,对于人对象,化合物的每日经口剂量为约0.01毫克/kg/天至1000毫克/kg/天。在每天一次或更多次施用中的0.5至50毫克/kg的经口剂量可产生治疗结果。根据施用方式,可以适当调整剂量以达到局部或全身的期望的药物水平。例如,静脉内施用可以从每天一个数量级到数个数量级的较低剂量变化。如果在这样的剂量下对象的响应不足,则可以采用甚至更高的剂量(或通过不同的、更局部的递送途径实现的有效更高的剂量)直至患者耐受性允许的程度。考虑每天多个剂量以达到化合物的适当全身水平。
对于本文中所述的任何化合物,治疗有效量可以最初从动物模型中确定。治疗有效剂量也可以从已经在人中测试过的化合物和已知表现出类似药理学活性的化合物例如其他相关活性剂的人数据中确定。对于肠胃外施用可能需要更高的剂量。可以基于所施用化合物的相对生物可利用度和效力来调整施加的剂量。基于上述方法和其他方法调整剂量以达到最大效力是本领域公知的并且在普通技术人员的能力范围内。
对于临床使用,本公开内容的任何化合物可以以等于或等效于0.2至2000毫克(mg)化合物/千克(kg)对象体重/天的量施用。本公开内容的化合物可以以等于或等效于2至2000mg化合物/kg对象体重/天的剂量施用。本公开内容的化合物可以以等于或等效于20至2000mg化合物/kg对象体重/天的剂量施用。本公开内容的化合物可以以等于或等效于50至2000mg化合物/kg对象体重/天的剂量施用。本公开内容的化合物可以以等于或等效于100至2000mg化合物/kg对象体重/天的剂量施用。本公开内容的化合物可以以等于或等效于200至2000mg化合物/kg对象体重/天的剂量施用。当本公开内容的化合物的前体或前药待施用而不是化合物本身时,其以等效于(即足以递送)上述量的本发明化合物的量施用。
本公开内容的化合物的制剂可以以治疗有效量施用于人对象。典型的剂量范围为约0.01微克/kg体重/天至约2mg/kg体重/天。待施用的药物剂量可能取决于这样的变量,例如病症的类型和程度、特定对象的整体健康状况、施用的特定化合物、用于配制该化合物的赋形剂以及其施用途径。常规实验可用于优化任何特定化合物的剂量和给药频率。

对于治疗用途,可通过将化合物递送至期望表面的任何方式向对象施用有效量的化合物。可通过技术人员已知的任何手段来实现施用药物组合物。施用途径包括但不限于静脉内、肌内、腹膜内、膀胱内(膀胱)、经口、皮下、直接注射(例如,注射到肿瘤或脓肿中)、经黏膜(例如,眼表面)、吸入和表面。
对于静脉内和其他肠胃外施用途径,本公开内容的化合物可配制成冻干制剂、脂质体嵌入或包封的活性化合物的冻干制剂、水性混悬剂中的脂质复合物或盐复合物。冻干制剂通常在施用之前不久在合适的水溶液中重构,例如在无菌水或盐水中重构。

释放本公开内容化合物的位置可以是胃、小肠(十二指肠、空肠或回肠)或大肠。本领域的技术人员可获得不会在胃中溶解但会在十二指肠或肠中的其他地方释放材料的制剂。释放可通过保护本公开内容化合物或通过在胃环境外(例如在肠中)释放化合物来避免胃环境的有害影响。
为了确保完全的胃抗性,在至少pH 5.0下不可渗透的包衣是必要的。用作肠溶包衣的较常见惰性成分的一些实例是乙酸纤维素偏苯三酸酯(cellulose acetatetrimellitate,CAT)、羟丙基甲基纤维素邻苯二甲酸酯(hydroxypropylmethylcellulosephthalate,HPMCP)、HPMCP 50、HPMCP 55、聚乙酸乙烯酯邻苯二甲酸酯(polyvinyl acetatephthalate,PVAP)、Eudragit L30D、Aquateric、乙酸纤维素邻苯二甲酸酯(celluloseacetate phthalate,CAP)、Eudragit L、Eudragit S和虫胶(shellac)。这些包衣可用作混合膜。
也可在片剂上使用包衣或包衣混合物,其并非旨在针对胃提供保护。这可包括糖包衣或使片剂更容易吞咽的包衣。胶囊剂可由用于递送干的治疗剂(例如粉末)的硬壳(例如明胶)组成;对于液体形式,可使用软明胶壳。扁囊剂(cachet)的壳材料可以是厚的淀粉纸或其他可食用的纸。对于丸剂、锭剂、模制片剂或片剂研磨物(tablet triturate),可使用湿法聚集(moist massing)技术。
治疗剂可作为细碎多颗粒以约1mm粒度的颗粒剂或丸粒剂(pellet)的形式包含在制剂中。用于胶囊剂施用的材料的制剂也可以是散剂、轻微压缩的可咀嚼物(lightlycompressed plug)或甚至是片剂。治疗剂可通过压缩来制备。
着色剂和矫味剂可均包含在内。例如,本公开内容化合物可被配制(例如通过脂质体或微球包封)并随后进一步包含在可食用产品中,例如含有着色剂和矫味剂的冷藏饮料中。
可用惰性材料稀释治疗剂或提高治疗剂的体积。这些稀释剂可包括碳水化合物,尤其是甘露糖醇、a-乳糖、无水乳糖、纤维素、蔗糖、经修饰右旋糖酐和淀粉。某些无机盐也可用作填充剂,包括三磷酸钙、碳酸镁和氯化钠。一些市售稀释剂是Fast-Flo、Emdex、STA-Rx 1500、Emcompress和Avicell。
崩解剂可包含在将治疗剂配制成固体剂型中。用作崩解剂的材料包括但不限于淀粉,包括基于淀粉的商业崩解剂Explotab。羟基乙酸淀粉钠、安伯来特(Amberlite)、羧甲基纤维素钠、超支链淀粉(ultramylopectin)、藻酸钠、明胶、橘皮、酸性羧甲基纤维素、天然海绵和膨润土均可被使用。崩解剂的另一种形式是不溶性阳离子交换树脂。粉末胶(powderedgum)可用作崩解剂和黏合剂,并且这些可包括粉末胶,例如琼脂、刺梧桐胶(Karaya)或西黄蓍胶。藻酸及其钠盐也可用作崩解剂。
黏合剂可用于将治疗剂保持在一起以形成硬片剂,并且包括来自天然产物的材料,例如阿拉伯胶、西黄蓍胶、淀粉和明胶。另一些包括甲基纤维素(methyl cellulose,MC)、乙基纤维素(ethyl cellulose,EC)和羧甲基纤维素(carboxymethyl cellulose,CMC)。聚乙烯吡咯烷酮(PVP)和羟丙基甲基纤维素(hydroxypropylmethyl cellulose,HPMC)二者均可在醇溶液中用于使治疗剂粒化。
抗摩擦剂(anti-frictional agent)可包含在治疗剂的制剂中,以阻止在配制过程中黏冲(Sticking)。润滑剂可用作治疗剂与模壁之间的层,并且这些可包括但不限于:硬脂酸(包括其镁盐和钙盐)、聚四氟乙烯(polytetrafluoroethylene,PTFE)、液体石蜡、植物油和蜡。也可使用可溶性润滑剂,例如十二烷基硫酸钠、十二烷基硫酸镁、多种分子量的聚乙二醇、Carbowax 4000和6000。
可添加在配制期间可改善药物流动特性并在压缩期间帮助重排的助流剂。助流剂可包括淀粉、滑石、致热二氧化硅和水合硅铝酸盐。
为了帮助治疗剂溶解到水性环境中,可添加表面活性剂作为润湿剂。表面活性剂可包括阴离子型洗涤剂,例如十二烷基硫酸钠、二辛基钠磺基琥珀酸盐和二辛基钠磺酸盐。可使用的阳离子型洗涤剂包括苯扎氯铵和苄索氯铵。可包含在制剂中作为表面活性剂的潜在非离子型洗涤剂包括聚桂醇400;聚氧乙烯40硬脂酸酯(polyoxyl 40 stearate);聚氧乙烯氢化蓖麻油(polyoxyethylene hydrogenated castor oil)10、50和60;单硬脂酸甘油酯;聚山梨酸酯40、60、65和80;蔗糖脂肪酸酯;甲基纤维素和羧甲基纤维素。这些表面活性剂可单独或作为混合物以不同比例存在于本公开内容化合物或衍生物的制剂中。
可经口使用的药物制剂包括由明胶制成的推入-配合胶囊剂(push-fitcapsule),以及由明胶和增塑剂(例如甘油或山梨糖醇)制成的软密封胶囊剂。推入-配合胶囊剂可包含活性成分与填充剂(例如乳糖)、黏合剂(例如淀粉)和/或润滑剂(例如滑石或硬脂酸镁)以及任选地稳定剂的混合。在软胶囊剂中,可将活性化合物溶解或悬浮在合适的液体中,例如脂肪油、液体石蜡或液体聚乙二醇中。另外,可添加稳定剂。也可使用配制用于经口施用的微球。这样的微球在本领域中已被很好地定义。用于经口施用的所有制剂应是适合于这样的施用的剂量。
对于经颊施用,组合物可采用以常规方式配制的片剂或锭剂的形式。
对于表面施用,化合物可配制成如本领域公知的溶液剂、凝胶剂、软膏剂、乳膏剂、混悬剂等。全身性制剂包括被设计用于通过注射(例如皮下、静脉内、肌内、鞘内或腹膜内注射)施用的那些,以及被设计用于经皮、经黏膜、经口或经肺施用的那些。
对于通过吸入施用,根据本公开内容使用的化合物可伴有使用合适的抛射剂(例如二氯二氟甲烷、三氯氟甲烷、二氯四氟乙烷、二氧化碳或其他合适的气体)从加压包装或喷雾器以气雾剂喷射呈现的形式方便的递送。在加压气雾剂的情况下,剂量单位可通过提供阀门以递送计量的量来确定。用于吸入器或吹入器的胶囊和药筒(cartridge)(例如,明胶)可配制为包含化合物与合适的粉末基质(例如乳糖或淀粉)的粉末混合物。
当期望全身性地递送化合物时,可将其配制成用于通过注射(例如,通过推注注射或持续输注)进行肠胃外施用。用于注射的制剂可以在添加防腐剂的情况下以单位剂型(例如在安瓿中或在多剂量容器中)存在。组合物可采取例如在油性或水性载剂中的混悬剂、溶液剂或乳剂的这样的形式,并且可包含配制剂,例如助悬剂、稳定剂和/或分散剂。
用于肠胃外施用的药物制剂包括水溶性形式的活性化合物的水溶液。此外,可将活性化合物的混悬剂制备为合适的油性注射混悬剂。合适的亲脂性溶剂或载剂包括脂肪油(例如芝麻油)、或合成脂肪酸酯(例如油酸乙酯或甘油三酯)、或脂质体。水性注射混悬剂可包含提高混悬剂黏度的物质,例如羧甲基纤维素钠、山梨糖醇或右旋糖酐。任选地,混悬剂还可包含合适的稳定剂或提高化合物的溶解性以允许高度浓缩溶液剂制备的药剂。
或者,活性化合物可以是粉末形式,以用于在使用之前用合适的载剂(例如无菌无热原水)构建。
化合物也可以配制成经直肠或经阴道组合物,例如栓剂或保留灌肠剂(retentionenema),例如包含常规栓剂基质,例如可可脂或其他甘油酯。
除上述制剂之外,还可将化合物配制成储存制剂(depot preparation)。这样的长效制剂可使用合适的聚合物或疏水性材料(例如作为可接受油中的乳剂)或离子交换树脂来配制,或配制为微溶性(sparingly soluble)衍生物,例如微溶性盐。
药物组合物还可包含合适的固相或凝胶相载体或赋形剂。这样的载体或赋形剂的一些实例包括但不限于碳酸钙、磷酸钙、多种糖、淀粉、纤维素衍生物、明胶和聚合物,例如聚乙二醇。
合适的液体或固体药物制剂形式为,例如,用于吸入的水性或盐水溶液;微囊化的、包封的(encochleated)、包衣于微观金颗粒(microscopic gold particle)上的、包含在脂质体中的、雾化的(nebulized)气雾剂;用于植入到皮肤中的、或干燥至尖锐物体上以被划进皮肤中的丸粒剂。药物组合物还包括颗粒剂、散剂、片剂、包衣片剂、(微)胶囊剂、栓剂、糖浆剂、乳剂、混悬剂、乳膏剂、滴剂或具有活性化合物延长性释放的制剂,在其制剂中,习惯上使用上述赋形剂和添加剂和/或辅料,例如崩解剂、黏合剂、包衣剂、溶胀剂、润滑剂、矫味剂、甜味剂或增溶剂。药物组合物适用于多种药物递送系统。对于药物递送方法的简要综述,参见Langer,R.,Science 249:1527-33(1990)。
本公开内容的化合物和任选地其他治疗剂可以以本身(纯的(neat))或以可药用盐的形式施用。当在药物中使用时,盐应是可药用的,但可方便地使用非可药用的盐以制备其可药用盐。这样的盐包括但不限于由以下酸制备的那些:盐酸、氢溴酸、硫酸、硝酸、磷酸、马来酸、乙酸、水杨酸、对甲苯磺酸、酒石酸、柠檬酸、甲烷磺酸、甲酸、丙二酸、琥珀酸、萘-2-磺酸和苯磺酸。同样,这样的盐可制备为碱金属或碱土盐,例如羧酸基团的钠盐、钾盐或钙盐。

治疗剂(具体地包括但不限于本公开内容的化合物)可以以颗粒提供。本文中使用的颗粒意指可以全部或部分由本公开内容化合物或如本文中所述的其他治疗剂组成的纳米粒或微粒(或在一些情况下更大的颗粒)。颗粒可以在被包衣(包括但不限于肠溶包衣)包围的核芯中包含治疗剂。治疗剂也可以分散在整个颗粒中。治疗剂也可以被吸附到颗粒中。颗粒可以具有任何级别的释放动力学,包括零级释放、一级释放、二级释放、延迟释放、持续释放、立即释放及其任意组合等。除了治疗剂之外,颗粒还可包含药学和医学领域中常规使用的那些材料中的任一种,包括但不限于可侵蚀的、不可侵蚀的、可生物降解的或不可生物降解的材料或其组合。颗粒可以是包含为溶液或半固体状态的本公开内容化合物的微胶囊。颗粒实际上可以是任何形状。
不可生物降解的和可生物降解的聚合物材料二者均可用于制造用于递送治疗剂的颗粒。这样的聚合物可以是天然或合成聚合物。聚合物基于期望释放的时间段来选择。特别令人感兴趣的生物黏附聚合物包括在Sawhney et al.,Macromolecules 26:581-587(1993)中描述的可生物侵蚀的水凝胶,其的教导并入本文。这些包括聚透明质酸、酪蛋白、明胶、明胶蛋白、聚酐、聚丙烯酸、藻酸盐、壳聚糖、聚(甲基丙烯酸甲酯)、聚(甲基丙烯酸乙酯)、聚(甲基丙烯酸丁酯)、聚(甲基丙烯酸异丁酯)、聚(甲基丙烯酸己酯)、聚(甲基丙烯酸异癸酯)、聚(甲基丙烯酸月桂酯)、聚(甲基丙烯酸苯酯)、聚(丙烯酸甲酯)、聚(丙烯酸异丙酯)、聚(丙烯酸异丁酯)和聚(丙烯酸十八酯)。
治疗剂可包含在控制释放系统中。术语“控制释放”旨在指任何含有药物的制剂,其中药物从制剂中释放的方式和谱受到控制。这指的是立即释放制剂和非立即释放制剂,其中非立即释放制剂包括但不限于持续释放和延迟释放制剂。术语“持续释放”(也称为“延长释放”)以其常规意义使用,是指在延长的时间段内提供药物的逐渐释放并且可以导致延长时间段内基本上恒定的药物血液水平的药物制剂。术语“延迟释放”以其常规意义使用,是指其中在制剂施用与药物从其中释放之间存在时间延迟的药物制剂。“延迟释放”可以涉及或可以不涉及药物在延长时间段内的逐渐释放,并因此可以是或可以不是“持续释放”。
长期持续释放植入物(implant)的使用可特别适合于治疗慢性病症。本文中使用的“长期”释放意指构建和布置植入物以递送治疗水平的活性成分持续至少7天,并且多至30至60天。长期持续释放植入物是本领域普通技术人员公知的并且包括一些上述释放系统。
相关领域的普通技术人员将理解,鉴于普通技术人员已知的信息,对本文中所述组合物和方法的其他合适的修改和改编从本文中所包含的公开内容的描述中容易地显现,并且可以在不脱离本公开内容的范围或其任何实施方案的情况下做出。
实施例
现在一般性地描述本公开内容,其通过参考以下实施例将更容易地被理解,所述实施例仅出于举例说明本公开内容的多个方面和实施方案的目而包括在内并且不旨在限制本公开内容。
PLE衍生物的合成
基于二甲苯的FL-PLE(图3)和Pro-FL-PLE(图4)的合成以2至3个步骤(图5)完成。
可被ROS、酶和低pH或肿瘤微环境中的任何其他触发因素裂解的脂质-荧光素缀合物的受保护形式(图6)也可以以类似方式合成(图6)。该合成也适用于制备包含用于双特异性的两个不同保护基的Pro-FL-PLE(逻辑门或双门方法,图7)。
合成和表征
用于脂质-接头(MS-100)、脂质-FITC(MS-109)和掩蔽的脂质-FITC(MS-115)的合成程序
脂质MS-100的制备。将N,N-二甲基脑磷脂十八烷基酯(5mg,0.011mmol,1当量)和4-(溴甲基)苄胺.HBr盐(3.4mg,0.012mmol,1.1当量)溶解在0.3mL氯仿(或甲苯)中。在添加碳酸钾(1.5mg,0.011mmol,1当量)之后,将所得混合物加热至80℃并搅拌12小时。反应的完成由LC-MS监测(流动相为A=20mM碳酸氢铵缓冲液,pH 7;B=乙腈;梯度为7分钟内0至100%B,0.75mL分钟-1,λ=220nm)。将粗制产物用于下一步。
脂质-FITC MS-109的制备。将来自上述反应的粗制脂质MS-100(0.011mmol)与异硫氰酸荧光素(4.3mg,0.011mmol,1.0当量)和碳酸钾(4.5mg,0.033mmol,3.0当量)在0.3mL氯仿中反应。将混合物在室温下搅拌2小时。反应完成通过LC-MS监测(保留时间=4.2分钟,流动相为A=20mM碳酸氢铵缓冲液,pH 7;B=乙腈;梯度为7分钟内0至100%B,0.75mL分钟-1,λ=280nm)。通过使干燥的氮气经过来蒸发溶剂。将所得混合物溶解在DMSO中(注意:添加足量的DMSO以制备黏度较低的溶液。将溶液通过0.45微米过滤器过滤,然后加载到HPLC上。应避免任何剧烈摇动或移液,以防止形成乳液)。然后通过HPLC对溶液进行纯化(流动相为A=20mM乙酸铵缓冲液,pH 7;B=乙腈;梯度为35分钟内0至100%B,12mL分钟-1,λ=280nm)。将具有纯产物的级分合并并冻干以提供6mg(59%产率)的MS-109。
掩蔽的脂质-FITC MS-115的制备。将脂质-FITC MS-109(4mg,0.004mmol)与1H-苯并[d][1,2,3]三唑-1-基(2-(吡啶-2-基二硫烷基)乙基)碳酸酯(3.5mg,0.010mmol,2.2当量)和DIEA(3.0当量)在0.15mL氯仿-DMSO(2∶1)中反应。将混合物在室温下搅拌12小时。反应的完成由LC-MS监测(保留时间=5.8分钟,流动相为A=20mM碳酸氢铵缓冲液,pH 7;B=乙腈;梯度为7分钟内0至100%B,0.75mL分钟-1,λ=280nm)。通过使干燥的氮气经过来蒸发溶剂。将所得混合物溶解在DMSO中(注意:添加足量的DMSO以制备黏度较低的溶液。将溶液通过0.45微米过滤器过滤,然后加载到HPLC上。应避免任何剧烈摇动或移液,以防止形成乳液)。然后通过HPLC对溶液进行纯化。(保留时间=流动相为A=20mM乙酸铵缓冲液,pH 7;B=乙腈;梯度为35分钟内0至100%B,12mL分钟-1,λ=280nm)。将具有纯产物的级分合并并冻干以提供1mg(18%)MS-115。
1H-苯并[d][1,2,3]三唑-1-基(2-(吡啶-2-基二硫烷基)乙基)碳酸酯根据Journal of Medicinal Chemistry,53(21),7767-7777;2010中报道的方案制备。
