背景技术
血脑屏障(BBB),也称为血脑屏障或血脑膜屏障,由脑微血管中的单层内皮细胞组成。这些内皮细胞之间具有紧密连接,因此限制了血液区室和实质区室之间的细胞旁和跨细胞交换。内皮细胞被基底膜、星形细胞足和周细胞包围,从而增强BBB(Sharif等人,2018[16])。脑内皮下面的基底层主动参与BBB的动力学,由3层组成。第一层是由内皮细胞合成的,其特征在于存在层粘连蛋白4和5。第二层特征在于存在层粘连蛋白1和2,由星形胶质细胞合成。第三层特征在于胶原IV的存在,在前两层之间并由两种细胞类型形成。这三层还由不同类型的胶原、糖蛋白和蛋白多糖,特别是硫酸乙酰肝素蛋白多糖(HSPG)组成(Cardoso等人,2010[4])。

在现有技术中还确实需要找到一种化合物和/或组合物,其使得可以修复例如由于阿尔茨海默病、帕金森病、亨廷顿病、多发性硬化、脑血管意外(CVA)后、外伤性脑病(例如慢性脑感染,例如病毒性或细菌性脑膜炎引起的BBB的改变和/或损害,由于例如脑肿瘤存在和/或治疗(例如脑的化疗和/或放疗)引起的改变和/或损害。
在现有技术中还确实需要找到一种化合物和/或组合物,其允许BBB的功能恢复,例如在所述BBB的损害和/或恶化之后。

B/使用生物相容性聚合物功能修复血脑屏障
在本实例中,评价了根据本发明的生物相容性聚合物RGTA在改变后,例如在脑血管意外(CVA)后对BBB渗透性的影响。
在该实例中,使用大鼠CVA模型。它是1小时的脑缺血,通过腔内途径闭塞脑动脉然后再灌注而获得。使用的大鼠是平均体重为300-350g的雄性Sprague Dawley大鼠。每次使用的大鼠数量为每组四至六只大鼠。在该模型中,众所周知的是BBB的渗透性逐渐增加以在诱导脑缺血后24-48小时达到峰值(Garrigue等人2016[6];Sharif等人,2018[16])。图1表示在如Abdullahi等人,2018所述使用的模型中渗透性随时间的变化。
在脑缺血后1h、3h、24h、48h和7天,在注射造影剂后,通过MRI进行BBB渗透性评价。该造影剂在生理条件下不穿过BBB。通过股静脉静脉内注射造影剂。通过注射施用的造影剂的量为200μmol/kg(Dotarem(注册商标),Guerbet S.A)。
大鼠,即每组和每次四到五只动物,用生物相容性聚合物处理,即分子量为100,000到150,000Da的RGTA OTR4132。图2表示所述聚合物的结构。在脑缺血后1小时施用生物相容性聚合物OTR4132,施用的包含浓度为0.5mg/kg的OTR4132的组合物的体积为300μl/尾静脉。
大鼠,即每组和每次四到五只动物,用对照溶液,即生理血清(0.9%NaCl盐水溶液)处理。对照溶液以与包含化合物OTR4132的组合物相同的方式施用,即在脑缺血后1小时,施用的组合物的体积为250μl,通过股静脉施用。
通过在由MRI获得的图像上观察来观察造影剂的渗透性和扩散。观察到的中枢神经系统区域位于受缺血影响的大脑半球以及健康的对侧半球。使用适当的软件(Image J(商标)(Wayne Rasband,NIMH,Maryland,USA))通过MRI分析测定造影剂在获得的图像上的扩散。造影剂的扩散和/或血脑屏障的渗透性如图3所示。
如图3的图所示,给予造影剂和对照溶液的大鼠在CVA后24小时、48小时和7天表现出血脑屏障渗透性增加(黑条);这些结果与在现有技术中获得的结果一致(Garrigue等人2016[6])。该图还清楚地和出乎意料地表明,与接受对照溶液的缺血组的大鼠相比,用生物相容性聚合物OTR4132处理大鼠使得在用RGTA处理的大鼠组中在缺血后24和48小时显著降低BBB的渗透性成为可能。特别地,结果表明了用对照溶液处理的大鼠与用包含根据本发明的生物相容性聚合物的组合物处理的大鼠之间的统计学显著差异(ANOVA,随后是Tukey的事后HSD检验,p<0.05)。
图3中获得和说明的结果清楚地表明,使用根据本发明的生物相容性聚合物使得可以在CVA之后保持BBB的完整性。特别地,这些结果清楚地表明,使用根据本发明的生物相容性聚合物可以保护BBB,促进其修复,并且在BBB的生理特性改变/修饰的情况下,恢复生理特性和/或减少其变化。
除了通过MRI获得的结果之外,根据文献Hone等人,2018[7]中描述的方法,在诱导脑缺血后通过用伊文思蓝染色来测量BBB的渗透性。使用的大鼠是平均体重为300-350g的雄性Sprague Dawley大鼠,对11只大鼠进行实验,其中5只大鼠在脑缺血后72小时静脉内注射2%浓度的伊文思蓝(在生理条件下不穿过BBB),注射的体积为4ml/kg,即对于体重分别为300-350g的大鼠为1.2-1.4ml。
六只大鼠用生物相容性聚合物,即分子量为100,000至150,000Da的RGTA OTR4132处理,在脑缺血后1小时施用,通过颈内动脉动脉内施用的包含2.22μg剂量的OTR4132的组合物的体积为50μl。
五只大鼠用对照溶液处理,即生理血清(0.9%NaCl盐水溶液)以与包含化合物OTR4132的组合物相同的方式施用,即在脑缺血后1小时,通过颈内动脉动脉内施用的组合物的体积为50μl。
施用伊文思蓝后三十分钟,用生理盐水对动物进行心内灌注,取出脑并分离半球。然后将样品在磷酸盐缓冲盐水中研磨,然后在60%三氯乙酸存在下置于4℃下。然后将样品离心(1,000g,30分钟),收集上清液用于610nm下的分光光度计读数。同时,制备递增浓度范围的伊文思蓝。
收集的上清液中存在的组织伊文思蓝然后通过分光光度法在610nm下测量来定量。
图4表示根据个体获得的结果。在对照组动物中获得的结果显示,与对侧半球相比,同侧半球中的BBB渗透性显著改变(2向ANOVA(p组=0.1582;p半球=0.0933;p组*半球=0.0175),然后进行Tukey HSD事后检验p=0.0374)。出乎意料地,在用生物相容性聚合物OTR4132治疗的动物组的同侧和对侧半球之间没有观察到差异(2向ANOVA(p组=0.1582;p半球=0.0933;p组*半球=0.0175),然后进行Tukey HSD事后检验p=0.9965)。此外,该分析还表明,与对照动物相比,用生物相容性聚合物OTR4132处理的动物的同侧半球中BBB的改变在统计学上显著降低(2向ANOVA(p组=0.1582;p半球=0.0933;p组*半球=0.0175),然后进行Tukey HSD事后检验p=0.0439)。
该实施例清楚地表明,包含式AaXxYy或AaXxYyZz的聚合物的根据本发明的组合物的实例有利地使得可以保护BBB和/或恢复BBB的生理特性。特别地,该实施例清楚地表明,包含式AaXxYy或AaXxYyZz的聚合物的根据本发明的组合物的实例使得可以在CVA之后保持BBB的完整性。特别地,这些结果清楚地表明,使用根据本发明的生物相容性聚合物可以保护BBB,促进其修复,并且在BBB的生理特性改变/修饰的情况下,恢复生理特性和/或减少其变化。
实施例2:生物相容性聚合物用于治疗血脑屏障改变和血脑屏障功能恢复的用途

Claims (14)

a表示单体数,
x表示单体A被X基团的取代度,
y表示单体A被Y基团的取代度。
2.根据权利要求1所述的用途,其中所述组合物还包含透明质酸。
3.根据权利要求1或2所述的用途,其中,X表示式-CH2-COO-的基团。
4.根据权利要求1或2所述的用途,其中,Y表示-SO3-基团或-SO3-Na+。
5.根据权利要求1或2所述的用途,其中所述单体数“a”使得所述式(I)聚合物的质量大于或等于2,000道尔顿。
6.根据权利要求1或2所述的用途,其中x为10%至150%。
7.根据权利要求1或2所述的用途,其中所述取代度“y”为10%至170%。
8.根据权利要求1或2所述的用途,其中所述生物相容性聚合物还包含不同于X和Y的化学Z官能团,其能够赋予所述聚合物额外的生物或物理化学特性。
9.根据权利要求8所述的用途,其中由“z”表示的所有单体A被Z基团的取代度为1%至50%。
10.根据权利要求8所述的用途,其中所述Z基团是能够赋予所述聚合物更好的溶解性或亲脂性的物质。
11.根据权利要求8所述的用途,其特征在于所述Z基团相同或不同并且选自氨基酸或脂肪酸。
12.根据权利要求11所述的用途,其中,所述Z基团选自苯丙氨酸或乙酸。
13.根据权利要求1或2所述的用途,其中所述生物聚合物通过以下方式施用以保护和/或修复/恢复血脑屏障:以0.1至5mg/kg体重的剂量胃肠外施用,和/或以0.1至5mg/kg体重的剂量口服施用,和/或以0.1至100μg.ml-1的剂量颅内施用。
14.根据权利要求2所述的用途,其中透明质酸的浓度为1至10mg/ml。
