Description

希氏束检测圈套导管
技术领域
本发明涉及希氏束检测圈套导管(snare catheter),并且更详细地,涉及将圈套和EP导管插入到形成两个腔的双腔导管以检测心肌的电信号从而查找希氏束的位置并且用圈套捕获通过希氏束的环扎线并将其稳定地引导至体内的希氏束检测圈套导管。
背景技术
心脏的传导系统从心房内的窦房结(Sinoatial Node)经过房室结(Atrioventricular Node)并且在心室的希氏束(His bundle)中分成右束(Right bundle)和左束(Left bundle),从而通过浦肯野纤维(PurkinjeFibers)构成。
在心电图中,QRS波通过心室肌的去极化过程而生成,紧接于P波的最初的下行波称为Q波,最初的上行波称为R波,并且紧接于R波的下行波称为S波。QRS的宽度意味着在希氏束中电传到至心室整体的时间,在正常情况下,QRS波的宽度为约0.12秒(约90ms内外)以内,并且在0.12秒(120ms)以上的情况下,暗示着心室内传导障碍。当导电时间长时,QRS宽度变宽,并且当导电时间短时,QRS宽度变窄。宽QRS引发心室的运动无法被统一的心室运动的不协调(Ventricular dessynchonization),由此存在着导致心室功能丧失的副作用。
当心脏的电传输系统本身发生功能障碍或者脱离该系统并且非正常地发生电力且电力传输到其它位置时,则发生心律不齐。
在这种心脏心律不齐疾病患者的诊断方法中,存在有心电图检查、动态心电检查(holter monitoring)、电生理学检查等。
其中,电生理学检查在通过胳膊或腿的血管将安装有电极的细导管(catheter)插入到心脏内部的状态下,在心脏的多种部位处测量电活动并记录或者向心脏的多种部位直接施加电刺激并观察心脏的反应。通过这种电生理学检查,医生也可查找到发生心律不齐的准确的原因部位,并且实施去除发现原因部位的施术或者设置人工心脏起搏器(Pacemaker)来治疗心律不齐。
作为这种研究的一环,本发明人在韩国公开专利第10-2016-0011530号和韩国公开专利第10-2016-0020887号中提出并申请了将通过冠状静脉窦的起搏器导线末端定位在心室间隔内的方法、装置和导管。所述公开专利是通过改良本发明人所提出的韩国注册专利第10-1116867号的环扎线来发明的,其涉及能够比传统技术更加简便地将人工心脏起搏器固定到心脏的装置。
所述公开专利包括在使施术线通过上腔静脉和冠状静脉窦流入并且通过中间或接部的心室间隔之后,将其再次引导至上腔静脉或下腔静脉的步骤、以及沿着所述施术线插入起搏器的导线并将导线的末端定位在心室间隔内部的步骤。
此时,在将起搏器的导线末端固定在靠近定位在心室间隔处的希氏束(His)的位置处以施加电刺激时,导电时间可变短并且可获得窄QRS。
因此,需要进行对于将起搏器导线引导至希氏束以使得起搏器导线可向靠近希氏束(His)的位置施加电刺激的装置的研究。
发明内容
要解决的技术问题
本发明的目的在于提供能够将人工心脏起搏器设置在靠近希氏束的位置处,并且能够将对用于检测心肌的电信号以确认希氏束的位置的电信号进行检测的EP导管插入到体内以提高心律不齐治疗的效果的希氏束检测圈套导管、以及同时将用于捕获环扎线的圈套插入到体内以捕获通过希氏束的环扎线并将其移动到体外的希氏束检测圈套导管。
本发明的另一目的在于提供能够将导管移动至期望的位置以便更加容易地检测希氏束并且捕获通过希氏束的环扎线的希氏束检测圈套导管。
本发明的目的并不限制于以上提及的目的,并且通过下面的记载,本发明所属技术领域的普通技术人员能够明确地理解未提及的其它目的。
解决技术问题的手段
为了实现所述目的,根据本发明的希氏束检测圈套导管包括内部形成有第一腔(lumen)和第二腔(lumen)的双腔导管、插入到所述第一腔(lumen)或所述第二腔并且检测心肌的电信号的电信号检测导管、以及插入到所述第一腔或所述第二腔(lumen)并且在远端部处设置有一个以上的环形线的圈套,其中,所述电信号检测导管和所述圈套插入到彼此不同的所述腔。
此时,其特征在于,所述双腔导管在远端部处设置有以预设角度弯曲的弯曲部,并且其中,所述双腔导管包括设置在近端部处的位置调节装置,从而在调节所述位置调节装置时,所述双腔导管的远端部的位置被调节。

发明效果
根据本发明的希氏束检测圈套导管可将电信号检测导管和圈套同时插入到双腔导管以检测心肌的电信号,从而使环扎线通过用户期望的心肌的位置,并且可容易捕获通过希氏束的环扎线以使其移动到下腔静脉。
此外,根据本发明的希氏束检测圈套导管可利用位置调节装置将双腔导管的远端部移动到期望的位置,从而可使电信号检测导管和圈套伸出得更靠近心肌。

根据本发明的希氏束检测圈套导管是用于使比起搏器导线更早插入到体内的环扎线(施术线)贯穿希氏束(His)并且使其容易引导至下腔静脉的装置,以便使起搏器导线的末端固定在希氏束(His)中以减少导电时间并且提高心律不齐治疗效果。
图1是根据本发明的优选的第一实施例的希氏束检测圈套导管的立体图。参照图1,根据本发明的优选的第一实施例的希氏束检测圈套导管包括双腔导管10、圈套20、电信号检测导管30。
所述双腔导管10为圆筒形,其具有通常的导管(catheter)的外形,并且包括第一腔12和第二腔14从而形成可使器械插入到内部的两个腔(lumen),因此将其称为双腔导管(dual lumen catheter)。

所述圈套20包括包含一个以上的环形线的捕获部22、以及一端与所述捕获部22结合并且另一端插入到所述第一腔12或所述第二腔14的本体部24。所述圈套20可使用能够插入到体内的金属材质(SUS304、镍钛诺、钛等)等。
所述捕获部22包括两端结合到所述本体部24的一端处的一个以上的环形线,并且优选地包括三个至四个环形线。所述捕获部22的特征在于,其由形状记忆合金、弹性体或者自膨式支架构成,因此在插入到所述第一腔12或所述第二腔14时收缩并且在伸出时展开。
更详细地,所述圈套20随着推动和拉动所述本体部24的一侧而移动,并且通过推动所述本体部24,所述圈套20插入到所述第一腔12或所述第二腔14,并且伸出到所述双腔导管10的外部并展开。当通过希氏束(His)的环扎线50定位在所述圈套20处时,拉动所述本体部24以捕获所述环扎线50并将其插入到所述双腔导管10内部。
所述电信号检测导管30的特征在于,其可包括多个电极32并且可插入到所述第一腔12或所述第二腔14,并且所述圈套20和所述电信号检测导管30插入到彼此不同的腔。
因此,所述双腔导管10可同时插入有所述圈套20和所述电信号检测导管30,可由所述电信号检测导管30确认希氏束的位置,并且可利用所述圈套20来捕获通过希氏束的环扎线并将其由下腔静脉取出。
所述电极32用于检测心肌的电信号,所述电信号检测导管30的外周面处可结合有一个以上的所述电极32,并且更详细地,形成为带形状的多个所述电极32以彼此恒定间距间隔开地设置在所述电信号检测导管30的外周面处。可利用多个所述电极32检测多种位置的电信号并且查找希氏束(His)的位置。
所述双腔导管10可包括设置在近端部处的位置调节装置40,所述位置调节装置40可在一方向上旋转,并且随着调节所述位置调节装置40,所述双腔导管10的远端部可向左或向右移动以移动到施术者期望的心肌的位置,以使得所述圈套20和所述电信号检测导管30可伸出到靠近心肌的位置处。
所述位置调节装置40在近端部末端处结合有止血阀42以阻挡所述位置调节装置40的近端部末端,并且防止从所述双腔导管10流入到所述位置调节装置40的血液由所述位置调节装置40的近端部末端漏出。所述止血阀42优选地由硅材质构成,但是不限定于此。
图2是根据本发明的优选的第二实施例的希氏束检测圈套导管的立体图。
在根据本发明的优选的第二实施例的希氏束检测圈套导管中,双腔导管10包括形成在远端部侧面处的孔18,并且所述孔18与第一腔12的远端部末端结合并连通。
换言之,所述第一腔12的远端部末端定位在所述双腔导管10的远端部侧面处,并且第二腔14的远端部末端定位在所述双腔导管10的远端部末端处。
因此,插入到所述第一腔12的圈套20或电信号检测导管30由所述双腔导管10的远端部侧面伸出。
图3是根据本发明的优选的第三实施例的希氏束检测圈套导管的立体图。
在根据本发明的优选的第三实施例的希氏束检测圈套导管中,双腔导管10包括形成在远端部侧面处的孔18,并且所述孔18与第二腔14的远端部末端结合并连通。
换言之,所述第二腔14的远端部末端定位在所述双腔导管10的远端部侧面处,并且第一腔12的远端部末端定位在所述双腔导管10的远端部末端处。
因此,插入到所述第二腔14的圈套20或电信号检测导管30由所述双腔导管10的远端部侧面伸出。
图4是根据本发明的优选的第四实施例的希氏束检测圈套导管的立体图。
根据本发明的优选的第四实施例的希氏束检测圈套导管在双腔导管10的远端部处包括弯曲部16。所述弯曲部16弯曲成使所述双腔导管10的远端部具有预定的角度,并且所述双腔导管10包括所述弯曲部16,从而弯曲成预定角度并进入到体内。
虽然根据本发明的优选的第四实施例的所述双腔导管10包括所述弯曲部16,但是为了使所述双腔导管10的远端部进一步朝向心肌移动,在近端部处包括位置调节装置40。所述位置调节装置40与图1中上述的相同,并且可调节所述位置调节装置40来使所述双腔导管10的远端部向左或向右移动。
因此,根据本发明的希氏束检测圈套导管可包括双腔导管10,从而可使圈套20和电信号检测导管30移动到施术者期望的心肌的位置,可检测心肌的电信号来确认希氏束的位置,并且可捕获贯穿希氏束的环扎线并将其引导至下腔静脉。
图5是利用根据本发明的希氏束检测圈套导管来施术的方法的顺序图。
首先,将引导线(未示出)插入到股静脉-下腔静脉-右心房-右心室。随后,使双腔导管10沿着所述引导线(未示出)进入到体内,并且此时,在将所述引导线(未示出)插入到所述双腔导管10内的同时,所述引导线(未示出)沿着插入到体内的路径进入到体内。所述引导线(未示出)插入到所述双腔导管10的第一腔12或第二腔14,并且插入到第一腔12和第二腔14中的任一个均无妨。所述双腔导管10插入到作为与所述引导线(未示出)相同的路径的股静脉-下腔静脉-右心房-右心室并且使远端部定位在右心室。
此时,为了使所述双腔导管10的远端部移动至施术者期望的位置,可利用位置调节装置40来使所述双腔导管10的远端部左右移动。
随后是将电信号检测导管30插入到已插入到体内的双腔导管10的步骤。所述电信号检测导管30可插入到所述第一腔12或第二腔14,并且所述电信号检测导管30插入到体内并且检测心肌的电信号。此时,所述引导线(未示出)可去除到体外。所述电信号检测导管30可包括多个电极以检测心肌的多种位置的电信号,并且可根据检测到的电信号来确认希氏束(His)的位置。环扎线50贯穿到通过所述电信号检测导管30检测到的希氏束的位置。
所述环扎线50通过上腔静脉-冠状静脉窦移动到体内并且贯穿希氏束以起到引导起搏器导线的末端以使其可固定在希氏束处的作用。因此,在所述环扎线50贯穿希氏束时,沿着所述环扎线50的路径插入到体内的起搏器导线的末端可固定在希氏束处,并且可进一步增加心律不齐的治疗效果。
为了捕获贯穿希氏束的所述环扎线50,将圈套20插入到所述双腔导管10内。在所述圈套20被推动以从所述双腔导管10的远端部伸出并且将所述环扎线50定位在所述圈套20的所述捕获部22处之后,在所述圈套20被拉动并且插入到所述双腔导管10时,所述环扎线50与所述圈套20一同插入到所述双腔导管10。
最后,在所述双腔导管10、所述圈套20、所述电信号检测导管30通过下腔静脉去除到体外时,所述环扎线50可与所述圈套2一同移动至下腔静脉并且使末端定位在体外。
因此,根据本发明的希氏束检测圈套导管可将电信号检测导管和圈套同时插入到双腔导管以检测心肌的电信号,从而使环扎线通过用户期望的心肌的位置,并且可容易捕获通过希氏束的环扎线以使其移动到下腔静脉。
虽然参照上述内容和附图对本发明的实施例进行了说明,但是本发明所属技术领域的普通技术人员可理解,在不变更本发明的技术思想或必要特征的情况下能够将本发明实施为其它具体形态。因此,以上记载的实施例在所有方面均为示例性的,而不应理解为限定性的。

Claims (7)

1.一种希氏束检测圈套导管,包括:
双腔导管,所述双腔导管内部形成有第一腔和第二腔;
电信号检测导管,所述电信号检测导管插入到所述第一腔或所述第二腔并且检测心肌的电信号;以及
圈套,所述圈套插入到所述第一腔或所述第二腔并且在远端部处设置有一个以上的环形线,
其中,所述电信号检测导管和所述圈套插入到彼此不同的所述腔。
2.如权利要求1所述的希氏束检测圈套导管,其中,所述双腔导管在远端部处设置有以预设角度弯曲的弯曲部。
3.如权利要求1所述的希氏束检测圈套导管,其中,所述双腔导管包括设置在近端部处的位置调节装置,从而在调节所述位置调节装置时,所述双腔导管的远端部的位置被调节。
4.如权利要求1所述的希氏束检测圈套导管,其中,所述双腔导管包括形成在远端部侧面处的孔,所述孔与所述第一腔或所述第二腔的远端部末端结合并连通,并且所述电信号检测导管或所述圈套由所述孔伸出。
5.如权利要求1所述的希氏束检测圈套导管,其中,所述圈套包括:
捕获部,所述捕获部包括一个以上的环形线;以及
本体部,所述本体部的一端与所述环形线结合,并且另一端通过所述圈套的腔并定位在体外,
其中,所述本体部的另一端被推动或拉动以使得所述圈套的捕获部插入到所述双腔导管内或者由所述双腔导管伸出。
6.如权利要求1所述的希氏束检测圈套导管,其中,所述第一腔或所述第二腔中插入有引导线,并且所述引导线比所述希氏束检测圈套导管更早插入到体内以使得所述希氏束检测圈套导管容易移动到体内。
7.如权利要求3所述的希氏束检测圈套导管,其中,所述位置调节装置的近端部末端处包括止血阀,从而在将所述希氏束检测圈套导管插入到体内时,防止流入到所述双腔导管内的血液流出到外部。
