本发明涉及一种敷料组合物及其制备方法和应用,具体涉及一种以透明质酸(钠)、交联透明质酸(钠)和聚乳酸‑羟基乙酸共聚物为骨架制成的敷料组合物及其制备方法和应用,本发明的敷料组合物还含有甘油、油酸、维生素E棕榈酸酯、丙二醇、尼泊金甲酯。

技术领域
本发明属于生物医学工程技术领域,具体涉及一种敷料组合物及其制备方法和应用,尤其是一种以透明质酸(钠)、交联透明质酸(钠)和PLGA为骨架制成的敷料组合物及其制备方法和应用。
背景技术
传统的医用敷料,包括棉纱布、无纺布等,在用于创面时,虽然具有材料容易获得、制作简单、价格低廉等优点,但是也具有显而易见的缺点:吸收量较小,即当创面渗出液较多时需频繁更换;通透性太高,容易使创面脱水或受到外界灰尘污染;敷料粘连创面带来换药痛苦及潜在的二次创伤以及对溃疡等难愈性创面治疗上的不适用。传统的医用敷料用于创面的愈合,其理论基础是干性愈合理论,传统的医学观点认为,伤口愈合需要干燥环境和氧气的作用,创面的干净清洁有利于伤口的愈合,因此使用传统的纱布敷料等或直接暴露伤口,均是基于干性愈合理论。但是,干性愈合疗法在治疗慢性创面中面临多种难以解决的问题,如坏死组织易形成干硬厚痂、痂下可能积脓积液、潜行和窦道引流不畅、外露骨和肌腱坏死等。另外,与干性愈合理论相左的是:人类对氧气的利用需要血红蛋白的氧和作用,大气中的氧是不能直接被伤口利用的。
湿性愈合理论,是在无菌条件下,运用密闭性敷料和/或药液,保持伤口适宜湿润的环境和适宜的温度,有利于创面上皮细胞的形成,促进肉芽组织生长和创面愈合。医用水凝胶或水胶体敷料用于创面的愈合,主要是基于湿性愈合理论,即在提供湿性环境、覆盖并保护创面、吸收创面渗液的同时,加速创面愈合,但一般不适用于渗液较多、周围皮肤脆弱或感染伤口。
目前,基于湿性愈合理论并已有产品上市的敷料包括:1、水凝胶敷料,是一种水活性胶质软膏或凝胶,主要适用于部分皮层及全皮层损伤伤口、有黄色腐肉或黑色坏死伤口,水凝胶敷料能使伤口产生水合作用,提供理想的湿润环境,促进坏死组织自溶,加速伤口愈合,同时也具有填充伤口的作用。其缺点是无阻隔作用,不能阻止周围细菌的侵入;涂抹时易造成伤口周围皮肤浸渍。2、水胶体敷料,适用于表浅和部分皮层损伤的伤口,小到中量渗液的伤口。产品特点主要是,有亲水性颗粒和疏水性聚合物组成,可吸收少到中量的伤口渗液,形成潮湿的伤口愈合环境,不撕裂新生肉芽组织,促进上皮细胞胶原蛋白的合成,加速微血管增长,防止外界细菌的侵犯。其缺点包括:不适合用于渗液较多的伤口、因揭除时的潜在周围皮肤损伤而不适用于周围皮肤脆弱的伤口、不主张用于深部潜行的伤口等。3、藻酸盐类敷料,以海洋中藻酸纤维为主要原料,主要适用于中到大量渗液的伤口、轻度出血伤口。产品特点是可以吸收产品自身重量17倍-20倍的渗液量,胶体与创面之间的钙离子交换可使伤口快速止血,吸收血小板促进血凝块生成,从而起到凝血作用。其缺点是不适用于干性创面,且一般需要和其它敷料配合使用。4、泡沫类敷料,适用于大部分的伤口,产品特点是可以吸收中到大量的渗液,透性低并由此使创面长期保持湿润,防止肉芽组织水肿、不浸渍周围皮肤,使用方便且顺应性好。但其也有一些缺点:在用于低渗出的创面时不利于自身的清创过程、因不透明而无法观察创面、使用成本相对较高。5、离子敷料,适用于感染有脓性分泌物的伤口,产品特点主要是敷料中含有银或银复合物,而银离子具有抗菌抗炎能力,持续释放出的银离子具有消炎杀菌作用。但离子敷料的缺点是:已上市品基本未见有吸收渗液的能力、容易在伤口造成着色、可能引起银离子过敏、长期使用有潜在的肝肾损害风险等。
生物相容性高分子具有很多重要的生理功能,常见的生物相容性高分子包括:多糖、蛋白质以及合成型高分子等。天然透明质酸是由D-葡糖醛酸和N-乙酰基-D-葡糖胺的交替单元组成的天然杂多糖,由Karl Mayer教授在1934年首次从牛眼玻璃体中提取获得,其后人们发现其在人和其他脊椎动物的结缔组织中广泛存在,例如细胞间隙、运动关节组织、脐带、皮肤、软骨、血管壁、滑液以及鸡冠等组织和器官中都含有透明质酸。在人体生理状态下,透明质酸通常以钠盐的形式存在。其中,透明质酸钠及其凝胶在骨科、妇科及整形科等领域应用广泛,还可用于作为眼用制剂载体或直接作为眼用制剂应用于眼科手术,即透明质酸钠类产品在眼科手术中也有重要应用。透明质酸钠还是关节滑液及软骨的重要组成成分。天然的透明质酸或其钠盐虽然应用领域广泛且具有明确的应用优势,但天然的透明质酸或其钠盐也有明确的缺点。包括:在体内的半衰期短;稳定性较差,易发生降解的缺点;具有亲水性过强的缺点。为了克服天然透明质酸或其钠盐的上述缺点,现有技术中采用的常见手段为化学改性和分子间交联,即形成化学改性透明质酸和交联透明质酸。目前市售的透明质酸主要是通过从动物组织提取制得,也可由微生物发酵方式制得;而化学改性透明质酸和交联透明质酸,则一般是在现有透明质酸的来源基础上通过化学合成修饰制得。
聚乳酸-羟基乙酸共聚物,英文缩写为PLGA,是一种重要的可生物降解的高分子材料,可被用于医疗或制药领域,由于具有无毒安全、生物相容性好和降解性能可控等优点,在制药领域是制备微球的理想材料,并且已经被美国FDA批准用于制备注射用微球的载体,目前已成功上市或研发成熟的产品包括注射用奥曲肽缓释微球、注射用亮丙瑞林缓释微球、注射用曲普瑞林缓释微球等。PLGA虽然作为制药用辅料在注射用微球等领域获得较好应用,但应用PLGA制备医疗器械,尤其是制备医用创伤敷料,在现有技术中还未见到有研发比较成熟的例子。PLGA的生物可降解性能优势体现在:降解产物是乳酸和羟基乙酸,这两种降解产物也是人体代谢途径的副产物,因此在PLGA被应用于组成医药材料或生物材料中时也不会产生对人体的毒副作用。
在中国按照医疗器械管理的外用敷料,其应用范围较广。慢性静脉性溃疡是下肢慢性静脉功能不全(chronic venous insufficiency,CVI)最严重和最难治的并发症,在人群中总发病率约为0.4%-1.3%。据报道约有12%-14%的CVI病人存在静脉性溃疡,静脉性溃疡治疗最重要的方面是预防溃疡形成。敷料治疗可作为临床上治疗静脉性溃疡的优势性或替代性疗法。
目前市场上的医用水胶体或水凝胶敷料主要使用壳聚糖、海藻酸钠、银离子等功能性成分,这些产品存在促进伤口愈合能力较差,消炎、止血、止痛效果不明显,安全性有待再评价,尤其是对于各种慢性体表伤口包括溃疡、糖尿病足、下肢静脉/动脉溃疡、压疮、烧伤及烫伤等伤口治疗见效时间不能使患者达到满意,治疗过程可能会使患者感受痛苦,影响了患者的顺应性。
发明内容
有鉴于以上情况,本发明旨在提供一种医用敷料组合物及其制备方法和应用,针对现有技术中各种水凝胶敷料或水胶体敷料等基于湿性愈合理论的敷料所存在的不足,该敷料组合物具有持久保湿,快速、持久、有效吸收伤口渗液,抗菌消炎,加快各种急慢性伤口的愈合等作用,防止粘连,不易形成干痂,避免换药时的再次损伤,减少换药频次,并在以上基础上减少瘢痕的产生。适于各种伤口,特别是不多于中等量渗液的伤口,包括但不限于体表溃疡、糖尿病足、下肢静脉/动脉溃疡、压疮、烧伤及烫伤等。

本领域技术人员可以理解,必要时,本发明的敷料组合物中可进一步含有适量的pH调节剂。优选的pH调节剂例如三乙醇胺、醋酸等。优选地,本发明敷料组合物的pH值为6.0-7.0,最优选6.5±0.1。
根据本发明,透明质酸和/或透明质酸钠可以是市售的,也可以是自制的,优选,其重均分子量范围可以在10k Da-400k Da之间;进一步优选,其重均分子量范围在200k Da-400k Da之间;更优选,其重均分子量范围在250k Da-350k Da之间;最优选,其重均分子量范围在290k Da-310k Da的之间。
根据本发明,交联透明质酸和/或交联透明质酸钠可以是市售的,也可以是自制的,优选,其重均分子量范围可以在1000k Da-3000k Da之间;进一步优选,其重均分子量范围在1500k Da-2500Da之间;更优选,其重均分子量范围在1800kDa-2200k Da之间;最优选,其重均分子量范围可以在1900k Da-2100k Da的之间。
根据本发明,优选,聚乳酸-羟基乙酸共聚物中乳酸和羟基乙酸的比例为50:50;优选,聚乳酸-羟基乙酸共聚物的重均分子量范围可以在10000Da-20000Da之间。本领域技术人员可以理解,其中可以混入少量例如不超过20%其他型号的聚乳酸-羟基乙酸共聚物,例如乳酸和羟基乙酸的比例为25:75或75:25的聚乳酸-羟基乙酸共聚物。
在本发明的敷料组合物中,透明质酸和/或透明质酸钠、交联透明质酸和/或交联透明质酸钠、聚乳酸-羟基乙酸共聚物为其骨架成分,共同支撑整个敷料组合物,从而可以避免使用天然纤维、人造纤维、生物可降解纤维等材料,具有优异的生物相容性,特别是由于使用了上述骨架成分,与其他成分配合使得本发明的敷料组合物能够实现持久保湿,快速、持久、有效吸收伤口渗液,抗菌消炎,加快各种急慢性伤口的愈合等作用,并能够减少瘢痕的产生。
本领域技术人员可以理解,本发明的敷料组合物中也可以加入适量的具有抗炎活性的物质,包括但不限于化学合成药物和/或植物提取物等,也可以加入适量本领域已知的各种有益于伤口愈合的活性物质。
根据本发明的另一个方面,本发明提供上述敷料组合物的制备方法,包括如下步骤:
(1)将聚乳酸-羟基乙酸共聚物溶于溶剂例如乙酸乙酯中,得其溶液;将透明质酸和/或透明质酸钠、交联透明质酸和/或交联透明质酸钠置于水中,搅拌直至匀化;将透明质酸钠和/或透明质酸钠、以及交联透明质酸和/或交联透明质酸钠的匀化液与聚乳酸-羟基乙酸共聚物的溶液混合,搅拌直至匀化;调节pH至6.5-7.5,优选7±0.1;去除有机溶剂和部分水例如含水量不超过10%,例如冷冻干燥直至含水量不超过10%;研磨至粒径为1000μm以下,优选10μm-300μm,得骨架材料;
本领域技术人员可以理解,在上述步骤(1)中,在制备透明质酸和/或透明质酸钠、以及交联透明质酸和/或交联透明质酸钠的匀化液时,可以适当加热,例如加热至50-70℃,优选55-65℃;在将透明质酸钠和/或透明质酸钠、以及交联透明质酸和/或交联透明质酸钠的匀化液,与聚乳酸-羟基乙酸共聚物的溶液混合时,也可以适当加热,例如加热至50-70℃,优选55-65℃。
(2)将甘油、油酸、维生素E棕榈酸酯混合,搅拌直至混溶,得混合液I;
本领域技术人员可以理解,在上述步骤(2)中,可以适当加热,例如加热至60-80℃。
(3)将丙二醇、尼泊金甲酯、骨架材料和水混合,搅拌直至匀化,调节pH至6.0-7.0,优选6.5±0.1,得膏状物II;
本领域技术人员可以理解,在上述步骤(3)中,可以适当加热,例如加热至80-90℃。
(4)在搅拌的条件下,将混合液I缓慢加入到膏状物II中,搅拌或研磨直至匀化。
本发明的敷料组合物,可以单独使用,也可以涂布在其他医用材料例如医用纱布、医用塑料材质薄膜上使用。例如,本发明的敷料组合物可以直接涂抹在患者创口处使用,或者涂布在其他医用材料上使用;本发明的敷料组合物还可以被加工成各种形状,例如片状、塞状等单独使用,或者与其他医用材料复合在一起使用。

1、本发明首先提出了以透明质酸和/或透明质酸钠、交联透明质酸和/或交联透明质酸钠混合聚乳酸-羟基乙酸共聚物制成医用敷料的骨架成分,具有优异的吸水性,可有效吸收渗液,在吸收水分或液体后可联结形成具有粘性的薄膜状,附着在伤口的表面,有利于对无定型伤口或表面小、而伤口内部较深伤口的治疗。
2、本发明的敷料能够在有效吸收渗液的基础上,保持伤口湿性愈合环境,促进肉芽和上皮组织有序生长,加速伤口愈合,减少疤痕形成,且安全性高、无刺激性。
具体实施方式
下面结合具体实施例,进一步阐述本发明。应理解,这些实施例仅用于说明本发明而不用于限制本发明的范围。此外,应理解,在阅读了本发明所记载的内容之后,本领域技术人员可以对本发明作各种改动或修改,这些等价形式同样落于本发明所限定的范围。
制备例1医用敷料骨架材料的制备
取PLGA(型号50-50,重均分子量15000Da)2.2g,溶于适量乙酸乙酯中得PLGA溶液;
取交联透明质酸钠(重均分子量2000k Da)9.3g、透明质酸钠(重均分子量300kDa)0.95g,置于适量水中,在60±5℃下搅拌直至匀化;
在60℃±5℃、搅拌的条件下向交联透明质酸钠和透明质酸钠的匀化液中加入PLGA溶液,搅拌直至匀化,用三乙醇胺和/或醋酸调节pH约为7,冷冻干燥至含水量在10%以内,将所得冻干品研磨为过60目筛的细粉,即得医用敷料骨架材料A 13.5g。其中,交联透明质酸钠的含量约为68.9%,透明质酸钠的含量约为7.0%,PLGA的含量约为16.3%,含水量约为7.8%。
制备例2医用敷料骨架材料的制备
取PLGA(型号50-50,重均分子量15000Da)2.45g,溶于适量乙酸乙酯中得PLGA溶液;
取交联透明质酸钠(重均分子量2000k Da)11.75g、透明质酸钠(重均分子量300kDa)0.96g,置于适量水中,在60±5℃下搅拌直至匀化;
在60℃±5℃、搅拌的条件下向交联透明质酸钠和透明质酸钠的匀化液中加入PLGA溶液,搅拌直至匀化,用三乙醇胺和/或醋酸调节pH约为7,冷冻干燥至含水量在10%以内,将所得冻干品研磨为过80目筛的细粉,即得医用敷料骨架材料B 16.5g。其中,交联透明质酸钠的含量约为71.2%,透明质酸钠的含量约为5.8%,PLGA的含量约为14.8%,含水量约为8.2%。
实施例1以制备例1的骨架材料A制备医用水凝胶敷料1
取12.5g甘油、0.15g油酸、0.25g维生素E棕榈酸酯混合,在70±5℃下搅拌直至混溶,得混合液I;
取2.0g丙二醇、0.1g尼泊金甲酯、10.0g制备例1所制得的骨架材料A粉末和8.0g水混合,在85±5℃下搅拌直至匀化,用三乙醇胺和/或醋酸调节pH至6.5±0.1,得膏状物II;
在搅拌的条件下,将混合液I缓慢加入到膏状物II中,搅拌或研磨直至匀化,即得医用水凝胶敷料约33.0g。其中,骨架材料A中的交联透明质酸钠的含量约为20.9%,透明质酸钠的含量约为2.1%,PLGA的含量约为4.9%,甘油的含量约为37.9%,油酸的含量约为0.5%,维生素E棕榈酸酯的含量约为0.8%,丙二醇的含量约为6.1%,尼泊金甲酯的含量约为0.3%,含水量约为26.5%。
实施例2以制备例2的骨架材料B制备医用水凝胶敷料2
取8.5g甘油、0.17g油酸、0.25g维生素E棕榈酸酯混合,在70±5℃下搅拌直至混溶,得混合液I;
取2.0g丙二醇、0.1g尼泊金甲酯、15.0g制备例2所制得的骨架材料B粉末和6.0g水混合,在85±5℃下搅拌直至匀化,用三乙醇胺和/或醋酸调节pH至6.5±0.1,得膏状物II;
在搅拌的条件下,将混合液I缓慢加入到膏状物II中,搅拌或研磨直至匀化,即得医用水凝胶敷料约32.0g。其中,骨架材料B中的交联透明质酸钠的含量约为33.4%,透明质酸钠的含量约为2.7%,PLGA的含量约为6.9%,甘油的含量约为26.6%,油酸的含量约为0.5%,维生素E棕榈酸酯的含量约为0.8%,丙二醇的含量约为6.3%,尼泊金甲酯的含量约为0.3%,含水量约为22.5%。
实施例3实施例1-2医用敷料的抑菌效果
参考GB/T15979-2002《一次性使用卫生用品卫生标准》附录C产品杀菌性能、抑菌性能及稳定性测试方法中的规定,对实施例1-2制备的敷料、市售敷料产品(对照例1,商品名:康惠尔清创胶,康乐保中国医疗用品有限公司)、由实施例1中除骨架材料A外其它组分混合制成的敷料(对照例2)、阴性对照(PBS溶液)进行了体外抑菌试验。
试验用菌:金黄色葡萄球菌、大肠埃希氏菌、铜绿假单胞菌、白色念珠菌。
试验方法为:分别制备以上试验用菌浓度为104-105cfu/g的菌悬液。取0.1mL的菌悬液均匀涂布在片状(2.0cm×3.0cm,厚0.1cm)实施例1-2制备的敷料、对照例1-2的敷料上作用20min后,将样片投入5.0ml的PBS溶液中,充分混匀,阴性对照组取0.1mL的菌悬液加入5.6ml的PBS溶液中,充分混匀,各组分别取0.5mL置于无菌平皿中,用营养琼脂培养基(金黄色葡萄球菌、大肠埃希氏菌、铜绿假单胞菌)或改良马丁琼脂培养基(白色念珠菌)30mL作倾注,转动平皿,充分混匀,置37℃±2℃恒温箱(金黄色葡萄球菌、大肠杆菌、铜绿假单胞菌)培养24h或2℃±2℃恒温箱(白色念珠菌)培养72h,观察结果,计算菌落数,按照如下公式计算抑菌率:
抑菌率=(阴性对照组菌落数-各组菌落数)/阴性对照组菌落数
当抑菌率≥50%并<90%时,表明测试敷料有抑菌作用;当抑菌率≥90%,表明测试敷料有较强抑菌作用。
试验结果如表1所示,由表1中数据可以得出,本发明实施例1-2的敷料具有较好的抑菌活性,优于市售敷料(对照例1),也优于实施例1中除骨架材料A外其它组分混合制成的敷料(对照例2)。

实施例4实施例1-2医用敷料的效果
通过创面愈合试验,考察实施例1-2制备的医用敷料促进伤口愈合的效果。
具体试验方法如下:
试验动物:选用体重在1.8Kg~2.5Kg的新西兰白兔,随机分组,观察时间点分别为3d、5d、10d、15d,每个时间点选用13只新西兰白兔。对新西兰白兔于试验前24h~72h进行背部剪毛后,用脱毛膏做脱毛处理。用2.0%的戊巴比妥钠溶液,按1.0mL/Kg的量以耳缘静脉注射将白兔麻醉,用80℃热水烫伤10s,在白兔背部致为I度至浅II度烫伤,尺寸2.0cm×2.0cm,左右两侧各3处呈对称状态,6处烫伤的间隔必须达到2.0cm或大于2.0cm。
烫伤1天后分别将实施例1-2所制备的医用敷料分别覆盖每只白兔背部的一个烫伤部位,分别作为实施例组1和实施例组2;将市售敷料产品(商品名:康惠尔清创胶,康乐保中国医疗用品有限公司)分别覆盖每只白兔背部的一个烫伤部位,作为对照组1;将实施例1中除骨架材料A外其它组分且将水量减少至50%后混合制成的敷料分别覆盖每只白兔背部的一个烫伤部位,作为对照组2;普通医用纱布分别覆盖每只白兔背部的一个烫伤部位,作为对照组3;每只白兔背部的最后一个烫伤部位不用敷料,仅做消毒处理,作为对照组4。于各取样时间点计算创面的愈合率。
创面愈合率的计算方法如下:先将创面描记在半透明纸上,再以此为模板,将质地均匀的硬纸片剪成同样大小,然后用天平称量,以硬纸片的质量间接地表示创面面积的大小。按下式计算创面愈合率,并计算13只兔子的创面平均愈合率:
创面愈合率(%)=(原始创面面积-未愈合创面面积)/原始创面面积。
试验结果如表2所示,从此表中创面平均愈合率的结果得出:实施例组1-2对新西兰白兔造模的烫伤创面,在创面愈合的有效性上,明显优于对照组1,有显著性差异,即本发明所制得的医用敷料,比类似的市售品有明显优势;实施例组1-2对新西兰白兔造模的烫伤创面,在创面愈合的有效性上,明显优于对照组2、3,有显著性差异,即本发明所制得的医用敷料,比不含有骨架材料的其他组分制成的敷料以及普通医用纱布敷料,具有明显优势;实施例组1-2在创面愈合的有效性上,与对照组4相比较,也具有明显的优势。

Claims (9)
3.如权利要求1或2所述的敷料组合物,其特征在于,所述敷料组合物的pH值为6.0-7.0,最优选6.5±0.1。
4.如权利要求1-3任一项所述的敷料组合物,其特征在于,透明质酸和/或透明质酸钠的重均分子量在10k Da-400k Da之间;优选在200k Da-400k Da之间;更优选在250k Da-350k Da之间;最优选在290k Da-310k Da的之间。
5.如权利要求1-4任一项所述的敷料组合物,其特征在于,交联透明质酸和/或交联透明质酸钠的重均分子量在1000k Da-3000k Da之间;优选在1500k Da-2500Da之间;更优选在1800k Da-2200k Da之间;最优选在1900k Da-2100k Da的之间。
6.如权利要求1-5任一项所述的敷料组合物,其特征在于,聚乳酸-羟基乙酸共聚物中乳酸和羟基乙酸的比例为50:50;优选,聚乳酸-羟基乙酸共聚物的重均分子量在10000Da-20000Da之间。
7.如权利要求1-6任一项所述的敷料组合物的制备方法,包括如下步骤:
(1)将聚乳酸-羟基乙酸共聚物溶于溶剂例如乙酸乙酯中,得其溶液;将透明质酸和/或透明质酸钠、交联透明质酸和/或交联透明质酸钠置于水中,搅拌直至匀化;将透明质酸钠和/或透明质酸钠、以及交联透明质酸和/或交联透明质酸钠的匀化液与聚乳酸-羟基乙酸共聚物的溶液混合,搅拌直至匀化;调节pH至6.5-7.5,优选7±0.1;去除有机溶剂和部分水例如含水量不超过10%,例如冷冻干燥直至含水量不超过10%;研磨至粒径为1000μm以下,优选10μm-300μm,得骨架材料;
(2)将甘油、油酸、维生素E棕榈酸酯混合,搅拌直至混溶,得混合液I;
(3)将丙二醇、尼泊金甲酯、骨架材料和水混合,搅拌直至匀化,调节pH至6.0-7.0,优选6.5±0.1,得膏状物II;
(4)在搅拌的条件下,将混合液I缓慢加入到膏状物II中,搅拌或研磨直至匀化。
8.一种敷料,其特征在于,含有如权利要求1-6任一项所述的敷料组合物。
9.如权利要求1-6任一项所述的敷料组合物在制备敷料中的用途。
