背景技术
近年来,人们更好地了解了与疾病相关的酶和其他生物分子的结构,这极大地帮助了寻找新的治疗剂。已成为广泛研究的主题的一类重要的酶是蛋白激酶。
蛋白激酶构成了一个大的负责控制细胞内的各种信号转导过程的结构相关酶家族。蛋白激酶由于其结构和催化功能的保守性而被认为是从一个共同祖先基因进化而来的。几乎所有的激酶都含有相似的250-300个氨基酸的催化结构域。激酶可以通过它们施以磷酸化的底物(例如,蛋白质-酪氨酸、蛋白质-丝氨酸/苏氨酸、脂质等)分类为多个家族。

发明内容
本公开提供了治疗一种或多种骨髓增生性病症、使其稳定或减轻其严重性或进展的方法。在某些实施方案中,提供的方法包括缓和与骨髓增生性病症的治疗相关的一种或多种不良事件。在一些这样的实施方案中,所述一种或多种不良事件是硫胺素缺乏症。

定义

具体实施方式
骨髓纤维化
骨髓增生性赘生物(MPN)相关的骨髓纤维化(MF)是一种严重且危及生命的疾病,其可作为从头发生(de novo)或原发性骨髓纤维化(PMF)存在或者从先前的真性红细胞增多症(PV后MF)或原发性血小板增多症(ET后MF)演化而来(Swerdlow SH,Campo E,HarrisNL,Jafie ES,Pileri SA,Stein H等人,World Health Organization classification oftumors of haematopoietic and lymphoid tissues.Lyon:IARC Press 2008)。所述疾病的特征为克隆性骨髓增生、无效性红细胞生成、骨髓基质改变、肝脾髓外造血,以及细胞因子表达异常(Tefferi A,Pardanani A.JAK inhibitors in myeloproliferativeneoplasms:rationale,current data and perspective.Blood Rev.2011 Sep;25(5):229-37)。患者通常表现为脾脏肿大、全身症状、中度至重度贫血、血小板减少和白细胞增多。

化合物I

实施例

目的.主要研究目的是评价在使用菲卓替尼的情况下脾脏体积减小至少35%的受试者的百分比。次要目的是:
·评价菲卓替尼的安全性

研究群体.该研究将录入大约110名患有中度或高风险的原发性骨髓纤维化(PMF)、真性红细胞增多症后骨髓纤维化(PV后MF)或原发性血小板增多症后骨髓纤维化(ET后MF)的受试者。
出于任何原因停用协议规定的疗法的所有受试者每3个月都将被随访以了解生存期、后续疗法以及骨髓纤维化向急性髓系白血病(AML)的进展,直到死亡或一直到治疗结束(EOT)之后12个月、失访、撤回针对进一步收集数据的同意书,或研究结束时为止,以先到者为准。

菲卓替尼在门诊连续每天一次自行口服施用,优选每天在相同时间与晚上的晚餐中的食物一起施用。如果漏服一剂,则应在第二天的与先前在错过剂量之前服用的时间相同的时间服用下一剂。
对于前3个周期,将在第1天和第15天进行站点访问,此后在后续周期的第1天进行站点访问。在第1周期的第8天,站点将通过电话与受试者联系,以评估恶心、呕吐和腹泻的发生并讨论其管理。
受试者可以用菲卓替尼继续治疗,直到出现不可接受的毒性、缺乏治疗效果、疾病进展或直到撤回同意书为止。
在研究过程中将监测所有受试者的不良事件。
出于任何原因停用协议-规定的疗法的所有受试者将在最后一剂菲卓替尼后的至少30天内被随访。
与菲卓替尼相关的最常见不良事件是血液学和胃肠道的不良事件。与JAK抑制剂相关的血液学不良事件是剂量依赖性的、是基于机制的,并且通过剂量降低、剂量中断和输血来管理。
本研究中的菲卓替尼剂量为400mg/天。如果受试者经历了如剂量修正时间表的表格(表1)中指定的药物毒性,则应当中断给药,并且可能需要修正剂量。
如果从起始剂量降低2个剂量水平之后受试者仍不耐受菲卓替尼治疗,则必须将他/她从研究治疗中撤出。如果毒性在如“剂量修正时间表(Dose Modification Scheduletable)”的表格(表1)中指定的时间段内仍未消退,则必须将受试者从研究治疗中撤出。如剂量修正时间表的表格(表1)中所定义,在某些情况下可再度逐步升级剂量。菲卓替尼的每日剂量不能超过400mg/天。
受试者可以用菲卓替尼继续治疗,直到出现不可接受的毒性、缺乏治疗效果或直到受试者不遵守治疗或撤回同意书时为止。
剂量修正时间表
可以采用灵活的剂量修正方案使个体受试者的药物毒性降低至最低限度,可能的每日剂量为200mg、300mg或400mg。
与菲卓替尼相关的最常见不良事件是血液学和胃肠道事件。与JAK抑制剂相关的血液学不良事件是剂量依赖性的、是基于机制的,并且通过剂量降低、剂量中断和输血来管理。
如果受试者经历表1(下文)中指定的药物毒性,则必须中断给药;在一些情况下(即,当它不是肝功能测试(LFT)异常时),剂量可以根据研究者的判断在研究过程中以100mg/天递减来滴定,降低到200mg/天的最小剂量。对于肾功能严重损伤并且共同施用强或中度CYP3A4抑制剂的受试者,将调整菲卓替尼的剂量。
如果从起始剂量降低2个剂量水平之后受试者仍不耐受菲卓替尼治疗,则必须将他/她从研究治疗中撤出。如果毒性在表1中指定的时间段内仍未消退,则必须将受试者从研究治疗中撤出。在某些情况下,可再度逐步升级剂量。菲卓替尼的每日剂量不能超过400mg/天(基于游离碱重量)。

与强和中度CYP3A4抑制剂共同施用的剂量调整
菲卓替尼与强或中度CYP3A4抑制剂伴随施用可增加菲卓替尼的暴露。菲卓替尼暴露增加可增加暴露相关AE的风险,需要仔细考虑。
对于采用与强CYP3A4抑制剂的共同施用的受试者,建议菲卓替尼起始剂量的剂量降低—从400mg剂量降低至200mg。如果菲卓替尼治疗期间需要引入强CYP3A4抑制剂,考虑以2个剂量水平递减的剂量降低(例如,300mg减少至100mg)。强CYP3A4抑制剂包括但不限于波普瑞韦、可比司他(cobicistat)、考尼伐坦、达诺瑞韦和利托那韦、埃替拉韦和利托那韦、葡萄柚汁、茚地那韦和利托那韦、伊曲康唑、酮康唑、洛匹那韦和利托那韦、帕利瑞韦和利托那韦和(奥比他韦和/或达沙布韦)、泊沙康唑、利托那韦、沙奎那韦和利托那韦、特拉匹韦、替拉那韦和利托那韦、醋竹桃霉素和伏立康唑。
对于采用与中度CYP3A4抑制剂的共同施用的受试者,建议菲卓替尼起始剂量的剂量降低—从400mg剂量降低至300mg。如果菲卓替尼治疗期间需要引入中度CYP3A4抑制剂,考虑以1个剂量水平递减的剂量降低(例如,300mg降低至200mg)。中度CYP3A4抑制剂包括但不限于阿瑞吡坦、西咪替丁、环丙沙星、克霉唑、克唑替尼、环孢菌素、决奈达隆、红霉素、氟康唑、氟伏沙明、伊马替尼、托非索泮和维拉帕米。
如果由于菲卓替尼血浆浓度可能增加,需要基于任何菲卓替尼相关AE将菲卓替尼的剂量降低至每天每日100mg以下,则考虑通过每隔一天施用例如100mg菲卓替尼来降低平均每日剂量,这等同于50mg的平均每日剂量。
如果在降低菲卓替尼剂量之后仍不能使AE消退,则基于患者的总体获益/风险考虑中断菲卓替尼或强CYP3A4抑制剂的给药。
如果停止与CYP3A4抑制剂的共同施用,则应当相应地再度逐步升级菲卓替尼的剂量。
针对肾功能损害的剂量调整。对于发生轻度和中度肾功能损害的受试者,建议不调整剂量。在研究过程中发生严重肾功能损害的受试者中,应当按照一个剂量递减水平(例如,从400mg到300mg每天一次[QD])调整菲卓替尼的剂量。对于计划剂量为200mg QD的受试者,允许减少至100mg。
潜在的韦尼克脑病(WE)的管理
潜在的WE病例为医疗紧急情况。在用菲卓替尼治疗期间按照以下步骤进行WE的筛选和潜在WE病例的管理:
临床和认知评估.间隔史:包括审查受试者的意识错乱、记忆力问题、视力问题(例如,复视)以及营养不良的病史,吸收不良的体征和症状以及酒精使用
·体格检查:包括在筛选期间以及在每个治疗周期的第1天、在治疗结束(EOT)时和30天随访期间评估异常眼球运动、小脑异常和体重(与先前检查或患者病史相比的体重减轻)
·小型精神状态检查(MMSE):在筛选期间、在第2和第3周期以及此后每第3周期的第1天、在EOT时以及根据临床指示更频繁地客观地评估脑病的体征/症状
潜在WE的管理.如果出现可能指示WE的体征或症状:
·保持菲卓替尼直到WE被排除
·获得针对硫胺素水平的样品
·根据经验开始施用硫胺素补充剂
·将事件以AESI形式报告给发起者
·进行神经学咨询
·进行脑部MRI
·如果证实WE,则永久停用菲卓替尼
硫胺素监测和纠正。将监测硫胺素水平(针对全血),并且将向所有硫胺素水平低于正常范围的受试者施用硫胺素补充剂。
·硫胺素水平是在筛选时评估的,并且需要在开始菲卓替尼治疗之前加以纠正和重新测试
·在用菲卓替尼治疗期间,在第2、3周期和此后每第3周期的开始时或者根据以下临床指示评估硫胺素水平:
-如果受试者正在施用硫胺素补充剂,则应当在针对硫胺素补充剂的禁食状态下评估硫胺素水平,并在抽血后给予硫胺素
-如果硫胺素水平结果低于正常值,则该站点将尽快与受试者联系,以便开始施用硫胺素补充剂
-在硫胺素水平低于正常范围但≥30nM/L并且无WE体征或症状的情况下:
ο必须开始补充100mg口服硫胺素
ο如果结果是通过当地实验室获得的,则将所述事件以特别关注的不良事件(AESI)的形式报告给发起者
-在硫胺素水平<30nM/L并且有或无WE体征或症状的情况下:
ο立即用硫胺素(最好是静脉内(IV))以治疗剂量(例如,经30分钟输注500mg,每天3次,持续2到3天),或者可替代地根据当地护理标准以等效剂量肌肉内(IM)输注来进行治疗;
ο将所述事件以特别关注的不良事件(AESI)的形式报告给发起者
ο在这之后接着IV输注施用250mg到500mg硫胺素,每天一次,持续3到5天,或者可替代地根据当地护理标准以等效剂量IM输注施用
ο然后继续使用100mg硫胺素的每日口服剂量,持续至少90天

-硫胺素补充剂应当以仅硫胺素制剂的形式来施用
-如果硫胺素水平低,则要确保镁水平正常,或者如果镁水平低,则予以纠正
*特别关注的不良事件(AESI)是特定于对研究产品的了解的科学和医学关注的事件,并且可能需要研究者密切监控并与发起者快速沟通。
以下被视为特殊关注的不良事件(AESI):
·与硫胺素水平低于正常范围相关的韦尼克脑病(WE)或疑似WE病例。
·硫胺素水平低于正常范围伴有或不伴有WE体征或症状
·在开始研究治疗之后的新的恶性肿瘤
·骨髓纤维化进展为急性髓系白血病(AML)
·心脏衰竭或心脏肿大
·3级和4级高脂肪酶血症
·或根据CTCAE标准v 5.0的3级和4级高淀粉酶血症,或胰腺炎事件
·3或4级丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)或总胆红素升高或肝脏毒性事件
恶心和呕吐的管理.在用菲卓替尼治疗期间按照以下步骤进行恶心和呕吐的管理:
·在开始治疗之前,将为受试者提供管理说明(包括何时联系研究站点)
·为了缓和恶心和呕吐事件,推荐在晚餐期间将菲卓替尼与食物一起服用。
·强烈推荐在治疗的前8周根据当地实践进行预防性的抗恶心/呕吐治疗(例如,昂丹司琼)。如果使用茶苯海明或其他毒蕈碱型受体拮抗剂治疗恶心和呕吐,则要在晚上施用这些剂,以最大限度地减少嗜睡和其他潜在的神经系统AE
·根据表1保持/降低菲卓替尼的剂量
·对于3级或更高级别的恶心或呕吐或持续的事件,可能需要住院
·对于为了预防恶心和呕吐而施用的药物,如果在菲卓替尼治疗的前8周期间没有发生临床上显著的恶心和呕吐,则考虑使受试者断掉这些用药
腹泻的管理.在用菲卓替尼治疗期间按照以下步骤进行腹泻的管理:
·在开始治疗之前,受试者应当具有在家中可用的洛哌丁胺,并且将为受试者提供腹泻管理说明(包括何时联系研究站点)
·如果受试者没有经历腹泻,则不应当给予洛哌丁胺作为预防
·腹泻开始时按照当地实践用洛哌丁胺治疗。考虑洛哌丁胺以4mg的负荷剂量开始,每次腹泻排便后2mg,不超过16mg/24小时
·饮食调整,包括充分的水合作用,避免食用含乳糖的食物和酒精,少量食用米饭、香蕉、面包等。
·根据表1保持/降低菲卓替尼的剂量
·对于3级或更高级别的持续腹泻,可能需要住院
·在每个随后的28天周期的第1天、在前三个周期的第15天的受试者随访期间以及在第一个周期的第8天通过强制电话联系,评估恶心、呕吐和腹泻的管理。
关键功效评估概述
除非另有说明,否则将对功效可评价的群体执行脾脏体积反应分析,将对MFSAF群体执行骨髓纤维化症状反应分析,并且将对安全性群体执行脾脏尺寸反应分析。
根据MRI/CT的脾脏体积反应率(RR).将脾脏体积减小的反应率定义为,与基线相比,在第6周期结束时脾脏体积减小≥35%的受试者的比例。将提供反应率和95%置信区间。另外,将提供脾脏体积测量值和与基线相比的变化百分比的描述性概括。在第6周期结束时MRI/CT脾脏体积缺失的受试者(包括那些在第6周期结束之前符合脾脏肿大进展标准的受试者)将被视为非反应者。
将针对与基线相比在第6周期结束时脾脏体积减小≥25%的受试者的反应率进行敏感性分析。
根据触诊的脾脏反应率(RRP).根据触诊的脾脏反应率为在第6周期结束时与基线相比具有根据IWG-MRT 2013的脾脏反应的受试者的比例。这将针对在基线时具有脾脏增大(在LCM下方≥5cm)的受试者进行计算。在第6周期结束时脾脏尺寸评估缺失的受试者(包括那些在第6周期结束之前符合脾脏肿大进展标准的受试者)将不被视为反应者。将提供反应率和95%置信区间。
症状反应率(SRR).症状反应率(SRR)被定义为在从基线到第6周期结束时通过MFSAF 4.0版测量的总症状评分(TSS)降低≥50%的受试者的比例。将提供SRR和95%置信区间。TSS将被定义为7个症状评分中的每一个的总和(Gwaltney C,Paty J,KwitkowskiVE,Mesa RA,Dueck AC,Papadopoulos EJ,et al.Development of a harmonizedpatient-reported outcome questionnaire to assess myelofibrosis symptoms inclinical trials.Leuk Res.2017 Aug;59:26-31)。为了与先前的MF研究进行间接比较,还将从所考虑的6种症状(盗汗、瘙痒、腹部不适、早饱、左侧肋骨下疼痛、骨痛或肌肉痛)推导出修正的TSS(Mesa RA,Gotlib J,Gupta V,Catalano JV,Deininger MW,Shields AL,etal.Effect of ruxolitinib therapy on myelofibrosis-related symptoms and otherpatient-reported outcomes in COMFORT-I:a randomized,double-blind,placebo-controlled trial.J Clin Oncol.2013 Apr 1;31(10):1285-92),并且还将对SRR进行分析。

Claims (40)
