Description

技术领域
本发明涉及支气管的配准,并且更特别地涉及用于将三维支气管树模型与患者的真实支气管树自动地配准的装置、系统及方法。
背景技术
用于检查患者气道的常规装置是支气管镜。通常,支气管镜通过患者的鼻子或口腔而插入患者的气道中,并且可以延伸进入患者的肺内。典型的支气管镜包括细长挠性管,细长挠性管具有用于照明支气管镜尖端的远端区域的照明组件、用于从支气管镜尖端提供视频图像的成像组件、以及工作通道,器械(例如,诸如活检工具的诊断器械、治疗器械)可以插入通过所述工作通道。

EMN系统10通常包括被构造成支撑患者的操作台40;被构造成用于通过患者的口和/或鼻而插入患者气道内的支气管镜50;联接至支气管镜50以显示从支气管镜50接收的视频图像的监视设备60;包括追踪模块72、多个参考传感器74和电磁(EM)场发生器76的追踪系统70;包括用于帮助通路规划、识别目标组织、导航至目标组织和数字地标记活检位置的软件和/或硬件的工作站80。
图1还示出了两种类型的导管引导组件90、100。两种导管引导组件90、100均可用于EMN系统10并且共享大量公共部件。每个导管引导组件90、100均包括连接至延伸的工作通道(EWC)96的手柄91。EWC96的尺寸适于放置在支气管镜50的工作通道内。操作中,将包括EM传感器94的可定位引导件(LG)92插入至EWC96中并且锁定在合适的位置处,使得EM传感器94延伸超出EWC96的远端93一所需距离。通过追踪模块72和工作站80可以获得EM传感器94在由EM场发生器76产生的EM场内的位置、以及因而EWC96的远端的位置。导管引导组件90、100具有不同的操作机构,但是各个均包含手柄91,通过旋转和加压可以操作手柄以操纵LG92和EWC96的远端93。导管引导组件90目前由美敦力股份有限公司以商标名操作套件而市场化和销售。类似地,导管引导组件100目前由美敦力股份有限公司以商标名EDGETM操作套件而市场化和销售。两个套件均包括手柄91、EWC96和LG92。为了更详细地描述导管引导组件90、100,参考Ladtkow等人共同持有的、2013年3月15日提交的发明名称为“微波切除导管及其使用方法”的美国专利公开序列号No.US2014/0046315,所述文献全文并入本文中以作为参考。
如图1所示,患者示出为躺在操作台40上,其中支气管镜50通过患者口部而插入并且进入患者的气道中。支气管镜50包括照明源和视频成像系统(未明确示出)并且联接至监视设备60(例如,视频显示器),以用于显示从支气管镜50的视频成像系统接收的视频图像。
包括LG92和EWC 96的导管引导组件90、100被构造成用于通过支气管镜50的工作通道插入并且进入患者的气道内(但是可以替代性地在不具有支气管镜50的情况下使用导管插入组件90、100)。LG92和EWC 96经由锁定机构99可以选择性地相对于彼此锁定。使用六自由度的追踪系统70来进行导航,例如,该追踪系统70类似于在美国专利No.6,188,355和已公开的PCT申请No.WO 00/10456和WO 01/67035中所公开的系统,每篇专利文献全文并入本文中以作为参考,或者使用任何其它合适的定位测量系统以实施导航,但是也可以设想其它结构。追踪系统70被构造成与导管引导组件90、100一起使用,以随着EM传感器94连同EWC96移动通过患者的气道而追踪EM传感器的位置,如下文所详细描述的。
如图1中所示,电磁场发生器76定位在患者的下方。电磁场发生器76和多个参考传感器74与追踪模块72相互连接,追踪模块获得每个参考传感器74在六个自由度上的位置。一个或多个参考传感器74附接至患者胸部。参考传感器74的六自由度坐标被发送至工作站80,工作站包括应用81,所述应用使用由参考传感器74收集的数据来计算患者的参考坐标系。
图1中还示出了导管活检工具102,在导航至目标并且移除LG92之后所述导管活检工具可插入至导管引导组件90、100内。导管活检工具102用于从目标组织收集一个或多个组织样本。如下文详述的,导管活检工具102还设置成与追踪系统70一起使用,以帮助将导管活检工具102导航至目标组织、随着相对于目标组织操作导管活检工具102以获得组织样本而追踪导管活检工具的位置、和/或标记获得组织样本处的位置。
虽然上文参考被包括在LG92内的EM传感器94而详述了导航,但是还可以设想的是,EM传感器94可以嵌入或集成在导管活检工具102内,其中导管活检工具102可以替代性地用于导航,而无需LG92或者无需使用LG92所需的必须工具交换件。在以下专利文献中描述了各种可使用的活检工具:2013年11月20日提交的发明名称为“用于将活检工具导航至目标位置以及使用该活检工具获得组织样本的装置、系统和方法”的美国临时专利申请No.61/906,732,2014年9月17日提交的发明名称为“用于将活检工具导航至目标位置以及使用该活检工具获得组织样本的装置、系统和方法”的美国专利申请No.14/488,754,2014年12月9日提交的发明名称为“用于将活检工具导航至目标位置以及使用该活检工具获得组织样本的装置、系统和方法”的美国专利申请No.14/564,779,每篇文献均全文并入本文中以作为参考并且可用于本文所述的EMN系统10。
在操作计划期间,工作站80使用计算机断层扫描(CT)图像数据以产生和观察患者气道的3D模型、使得能够(自动、半自动或手动地)识别3D模型上的目标组织以及允许选择通路通过患者气道而到达目标组织。更特别地,CT扫描经处理和组装成3D体积,随后3D体积用于产生患者气道的3D模型。3D模型可以呈现在与工作站80相关联的显示屏监视器上、或者以任何其它合适的方式呈现。通过使用工作站80,3D体积的不同切片和3D模型的视图可得以呈现和/或可以由临床医生操作,以帮助识别目标以及选择合适的通路通过患者气道来接近目标。3D模型还可以示出进行先前活检之处的位置的标记,包括与获得的组织样本相关的数据、时间和其它识别信息。这些标记还可以被选择作为通路规划达到的目标。一旦被选择,则存储通路以在导航操作期间使用。在Baker于2013年3月15日提交的发明名称为“通路规划系统和方法”的美国专利申请公开No.US 2014/0281961、US 2014/0270441和US2014/0282216中描述了合适的路径规划系统和方法的示例,每篇文献均全文并入本文中以作为参考。
在导航期间,EM传感器94与追踪系统70一同使得随着EM传感器94或活检工具102被推进通过患者气道而能够追踪EM传感器94和/或活检工具102。
现在参考图2,示出了工作站80的系统图。工作站80可以包括存储器202、处理器204、显示器206、网络接口208、输入装置210和/或输出模块212。
存储器202包括非瞬时的计算机可读的存储介质,以用于存储可由处理器204执行并且控制工作站80的操作的软件和/或数据。在一个实施例中,存储器202可以包括一个或多个固态存储装置,例如闪存芯片。作为替代或者除了一个或多个固态存储装置之外,存储器202可以包括通过大容量存储控制器(未示出)和通信总线(未示出)而连接至处理器204的一个或多个大容量存储装置。虽然此处包含的对计算机可读介质的描述涉及固态存储,但本领域技术人员应当意识到的是,计算机可读的存储介质可以是能够由处理器204访问的任何可用介质。即,计算机可读的存储介质包括以任何方法或技术可实施的非瞬时的、易失的和非易失的、可移除的和不可移除的介质,以用于存储信息,例如计算机可读的指令、数据结构、程序模块或其它数据。例如,计算机可读的存储介质包括RAM、ROM、EPROM、EEPROM、闪存或其它固态存储技术、CD-ROM、DVD、蓝光或其它光学存储器、磁带盒、磁带、磁盘存储器或其它磁存储装置、或者可用于存储所需信息并且可由工作站80访问的任何其它介质。
存储器202可以存储应用81和/或CT数据214。当由处理器204执行时,应用81可以致使显示器206呈现用户界面216。网络接口208可以构造成连接至网络,例如包括有线网络和/或无线网络的局域网(LAN)、广域网(WAN)、无线移动网络、蓝牙网络和/或互联网。输入装置210可以是用户可以借助其而与工作站80交互的任何装置,诸如鼠标、键盘、脚踏板、触摸屏和/或语音接口。输出模块212可以包括任何连接端口或总线,例如并行端口、串行端口、通用串行总线(USB)或者本领域技术人员已知的任何其它类似的连接端口。
现在参考图3,示出了用于将3D模型与患者气道配准的示例性方法的流程图。如上所述,在步骤302中,在开始配准处理之前产生3D模型,在步骤304中,临床医生基于3D模型而产生导航计划,该导航计划包括一个或多个目标。之后,临床医生将导航计划从存储器202、USB装置或者从网络接口208载入应用81中。导航计划可能需要配准患者肺的所有区域或仅仅一些区域。
在步骤306中,应用81显示引导,以用于实施3D模型与患者气道的自动配准(如上所述),并且特别地如Brown等人于2015年7月2日提交的发明名称为“实时自动配准反馈”的在审美国专利申请No.14/790,581中所述,所述文献全文引入本文中以作为参考。在配准期间,追踪EM传感器94在患者气道内的位置,并且存储指示EM传感器94在由EM发生器76产生的EM场内的位置的多个点。在步骤308中,应用81确定自动配准是否已经完成。如果没有完成,则处理返回步骤306,在该步骤中显示进一步引导以完成自动配准处理。如果已经完成,则处理进行至步骤310。
在步骤310中,应用81通过显示引导以将EM传感器94导航至目标404近侧,从而开始局部配准处理。之后,在步骤312中,应用81确定目标近侧的一个或多个隆凸位置。应用81通过分析气道中的任何分支和目标近侧的3D模型的区域而确定隆凸的位置。如图4所示,3D模型的视图400包括气道树402、目标404、一个或多个隆凸406以及自隆凸406处的分支起始的气道分支408。例如,应用81可以识别在目标404附近大致均匀地间隔开的多个隆凸406。在本实施例中,临床医生通过观察由定位在EM传感器94近侧(例如在LG92或EWC 96中)的照相机反馈的实时视频可以视觉地检测隆凸。临床医生可以将视觉检测得的隆凸与3D模型所示的气道匹配。
在步骤314中,应用81显示引导,以将EM传感器94导航至从隆凸406处的分支起始的每个气道分支408中。临床医生沿着所显示的引导,以在患者气道内导航EM传感器94。例如,引导可以指示临床医生将EM传感器94导航进入至每个气道分支408内约1cm。在步骤316中,随着EM传感器94被导航进入至自隆凸406起始的气道分支408内,应用81追踪EM传感器94的位置,并且存储指示EM传感器94在由EM发生器76产生的EM场内的位置的多个点。在步骤318中,应用81使用指示EM传感器94的位置的存储点,以进行3D模型与目标近侧的患者气道的局部配准。例如,可以基于一定范围的内插法技术(诸如薄板样条函数(TPS))内插法)可以进行局部配准。在实施例中,TPS内插法可以用于非刚性地配准3D模型与在自动配置期间存储的指示EM传感器在由EM发生器76产生的EM场内的位置的点,并且该内插法可以增加在局部配准期间存储的附加点。
之后,在步骤320中,应用81确定对于当前目标是否已经完成局部配准。如果没有完成,则处理返回其中显示进一步引导的步骤314。如果已经完成,则处理进行至步骤322,在该步骤中应用81确定在导航计划中是否还存在尚未进行局部配准的任何其它目标。如果是,则处理返回步骤310,在该步骤中应用81显示引导以将EM传感器94导航至下一目标近侧。如果否,则局部配准处理完成并且处理结束。
除了使用隆凸406进行逐步配准外,隆凸406还可以用作用于定位目标404的基准标记。与植入的异体标记不同,由于隆凸406不能移动,所以隆凸406尤其有利于作为基准标记。
虽然附图中已经示出了本发明的各个实施例,但是本发明并不旨在局限于此,旨在的是本发明的范围如本领域所允许的那样宽泛并且以同样的方式理解说明书。因而,上述描述不应当理解为是限制性的,而仅仅是特定实施例的示例。本领域技术人员将预期处于随附权利要求的范围和精神内的其它修改。
Claims (20)
1.一种用于将内腔网络与内腔网络的3D模型配准的方法,所述方法包括:
基于内腔网络的图像而产生内腔网络的3D模型;
在内腔网络中确定在内腔网络内的目标近侧的多个位置;
将内腔网络的3D模型与内腔网络配准;
显示引导以用于将传感器导航到从目标近侧的多个位置中的每个位置起始的多个内腔中的每个内腔内;
在将传感器导航到从所确定的多个位置中的每个位置起始的多个内腔中的每个内腔内时追踪传感器;
基于对多个内腔中的每个内腔内的传感器的追踪而局部地配准内腔网络的3D模型与目标近侧的内腔网络;以及
通过局部配准来增强内腔网络的3D模型与内腔网络的配准。
2.根据权利要求1所述的方法,其中,内腔网络是患者的气道。
3.根据权利要求2所述的方法,其中,3D模型是患者的气道的模型。
4.根据权利要求1所述的方法,其中,所述多个位置中的至少一个位置被用作用于识别目标位置的基准标记。
5.根据权利要求1所述的方法,其中,所述方法还包括:
使用薄板样条函数内插法来非刚性地配准内腔网络的3D模型与内腔网络;以及
通过局部配准来增强内腔网络的3D模型与内腔网络的非刚性配准。
6.根据权利要求1所述的方法,其中,所述方法还包括使用薄板样条函数内插法来局部地配准内腔网络的3D模型与目标近侧的内腔网络。
7.根据权利要求1所述的方法,其中,所述方法还包括:
确定内腔网络的3D模型与目标近侧的内腔网络的局部配准是否已经完成;并且
如果确定目标近侧的局部配准尚未完成,则显示进一步引导以用于将传感器导航到从目标近侧的多个位置中的每个位置起始的多个内腔中的每个内腔内。
8.根据权利要求1所述的方法,其中,所述方法还包括:
确定内腔网络的3D模型与至少一个附加目标近侧的内腔网络的局部配准是否已经完成;并且
如果确定所述至少一个附加目标近侧的局部配准尚未完成,则显示进一步引导以用于将传感器导航到从所述至少一个附加目标近侧的多个位置中的每个位置起始的多个内腔中的每个内腔内。
9.根据权利要求1所述的方法,其中,所述方法还包括:
在计算装置的存储器中存储内腔网络的3D模型与内腔网络的配准;以及
通过局部配准来增强所存储的内腔网络的3D模型与内腔网络的配准。
10.根据权利要求1所述的方法,其中,所述多个位置是内腔网络中的分支。
11.一种用于将内腔网络与内腔网络的3D模型配准的系统,所述系统包括:
构造成在患者体内的内腔网络内被导航的传感器;以及
计算装置,所述计算装置包括处理器和存储指令的存储器,当由处理器执行时所述指令使得计算装置执行下列操作:
基于内腔网络的图像而产生内腔网络的3D模型;
在内腔网络中确定目标近侧的多个位置;
将内腔网络的3D模型与内腔网络配准;
在将传感器导航到从目标近侧的多个位置中的每个位置起始的多个内腔中的每个内腔内时追踪传感器;
基于对多个内腔中的每个内腔内的传感器的追踪而局部地配准内腔网络的3D模型与目标近侧的内腔网络;以及
通过局部配准来增强内腔网络的3D模型与内腔网络的配准。
12.根据权利要求11所述的系统,其中,内腔网络是患者的气道。
13.根据权利要求12所述的系统,其中,3D模型是患者的气道的模型。
14.根据权利要求11所述的系统,其中,所述多个位置中的至少一个位置被用作用于识别目标位置的基准标记。
15.根据权利要求11所述的系统,其中,所述多个位置是内腔网络中的分支。
16.一种非瞬时的计算机可读的存储介质,所述存储介质存储指令,当由处理器执行时所述指令使得计算装置执行下列操作:
基于内腔网络的图像而产生内腔网络的3D模型;
在内腔网络中确定目标近侧的多个分叉的位置;
将内腔网络的3D模型与内腔网络配准;
在将传感器导航到从目标近侧的多个分叉中的每个分叉起始的多个内腔中的每个内腔内时追踪传感器;
基于对多个内腔中的每个内腔内的传感器的追踪而局部地配准内腔网络的3D模型与目标近侧的内腔网络;以及
通过局部配准来增强内腔网络的3D模型与内腔网络的配准。
17.根据权利要求16所述的非瞬时的计算机可读的存储介质,其中,内腔网络是患者的气道。
18.根据权利要求16所述的非瞬时的计算机可读的存储介质,其中,3D模型是患者的气道的模型。
19.根据权利要求16所述的非瞬时的计算机可读的存储介质,其中,所述多个分叉中的至少一个分叉被用作用于识别目标位置的基准标记。
20.根据权利要求16所述的非瞬时的计算机可读的存储介质,其中,当由处理器执行时所述指令使得计算装置显示引导以用于将传感器导航到从目标近侧的多个分叉中的每个分叉起始的多个内腔中的每个内腔内。
