导管设备
技术领域
本发明总体上涉及导管系统,更具体而言,涉及可从活体治疗区域移除组织的抽吸导管。
背景技术
水力抽吸导管(也被简称为“抽吸导管”)用于从治疗部位移除材料,特别是从身体脉管内移除材料。这类导管通常包括与喷嘴相邻的吸口,用于在导管远端外部产生使流体流入吸口的环境。收集的材料和加压流体通过导管沿近端方向返回,从而从脉管中移除所收集的材料。
常规抽吸导管会在远端导管末端引起一定的流体动力学影响,这在收集周围区域的目标材料方面是有利的,但会因为施加给组织的剪切应力较高而破坏并损伤组织细胞。因此,本发明的目的是保留用导管设备从身体脉管移除物质的功能,同时避免该过程中不必要的组织损伤。
发明内容
通过本发明,可以收集活生物体内治疗区域中的材料,并将其从体内取出。本发明的抽吸导管设备包括该设备远端周围的低剪切应力流体环境仍足以支撑通孔吸口收集目标组织。

图1示出了用于抽吸导管的本发明的导管,其包括具有远端12的管状本体2,远端12形成有吸口15。本体2靠近近端连接Y形件3,图2中更详细地示出了该Y形件3。
第一管6与Y形件3相连,其中,加压流体源与该第一管6相连。在图中所示的示例性实施例中,泵9从储液器18中抽出液体,并通过压力通道25输送加压液体。此外,排放管24与Y形件3相连,通往排放储液器8。
压力通道25可以是由Y形件3和本体2或由位于Y形件3和本体2内的管限定的腔(lumen),其与本体2内的压力通道相连。

加压流体被泵9驱动通过本体2内的压力通道25通向导管的远端12,如图2和图3所示。

管21可输送用于沿远端方向以流体流20射出经过吸口15的加压流体,其中,射出口31相对于吸口15位于近端,如图2和图4所示。在其它实施例中,例如如图5-图7所示,管20可输送用于沿近端方向射出经过吸口15的加压流体,其中,射出口31相对于吸口15位于远端。管21(图中示出的为直的或呈U形)用作喷嘴,沿着吸口15的内部以仔细控制的方式引导通过压力通道25输送的流体,使得以可靠且有效的方式获得喷射作用(ejector action)。通过该喷射作用,在吸口15附近的位于导管外的材料可通过该吸口15被吸入,并沿着排放通道22被带向近端。如图2所示,排放管24与排放通道22相连。通过压力通道25输送的加压流体通过管21以射流20的形式以相对高的速度射出,由此在吸口15的位置处产生抽吸。通过吸口15引入抽吸导管的材料经由排放通道22和排放管24的移除可由该抽吸来支撑。被吸入的材料与通过压力通道25输送的流体一起通过排放管24被移除至排放储液器中,例如,图3中的排放储液器8。
本发明的抽吸导管包括具有近端34和远端12的管状本体2。该管状本体2可包括两个或更多个自近端34延伸至远端12的腔。第一腔可限定加压流体通道25,供加压流体自近端34通往远端12。第二腔可限定排放通道22,供流体自远端12通往近端34。根据需要,管状本体2可包括第三和第四腔。
吸口15形成于本体2的远端12附近,用于促进周围的流体和颗粒物进入腔室13内。
具有限定射出口31的远端35的管21自加压流体通道25延伸。管21可以自加压流体通道25的内部延伸。在一些实施例中,管21可成形为钩的形式,这种形式的管21的射出口31将流体射流20朝近端导引。在另一可能的实施例中,管21可基本呈直线型延伸,其射出口31朝远端导引流体射流20。可以想到的是,管21可具有各种不同的构造来实现本发明的喷射作用。该“喷射作用”是一种施加给在吸口15附近位于导管外部的材料的流体力。
在所有实施例中,管21包括引导流体经过吸口15的射出口。
排放通道22用于朝着近端34输送流体和通过吸口15收集的材料。排放通道22可包括定向壁38,其将流体由面对远端定向重定向成面向近端定向的方向,如转向箭头39所示。通过这种方式,沿着排放通道22的流体流可除去通过吸口15被吸入腔室13内的来自于活体的血栓、栓子、组织或其它颗粒物等材料。
本体2可包括第三腔40,用于促进现有的导引和支撑装置(例如,导丝16)的通过,从而将导管1导引至所期望的身体位置。本体2还可包括第四腔,例如用于促进将医师指定的流体输送至治疗区域。
如上所述,管21包括射出口31,其使通过其的流体流朝着吸口15定向。射出口31可位于管21的末端,但也可以定位在沿着管21的某个点处。射出口31与由管21限定的一腔流体连通,其中,管21的该腔用于以相对高的速度和高流量输送流体。流体通过射出口31的相对高的出口速度是由管21的该腔的构造形成的,管21的该腔具有相对小的横截面积,而加压流体被迫使通过这样小的横截面。
本发明的一个特别的特征是导管1能够通过吸口15产生足够的吸力,可在避免对周围组织造成损伤的同时将目标材料吸入腔室3内,由此使远端栓塞的风险最小化。特别地,流体流速和管21管腔直接被配置成使得通过射出口31输送的相对高速度的流体流确保通过吸口15施加给红细胞和其它周围细胞的剪切力保持在200 N/m2的阈值应力水平(该水平估计会损伤细胞)或其以下。举例而言,通过直径为0.05 cm的射出口31的3 mL/s的潜在流速可导致施加给吸口15附近的红细胞的约200 N/m2的最大剪切应力。通过管21的射出口31流出的流体流会在吸口15与周围流体之间形成压差,从而使本体2周围区域的材料移至吸口15内。两个单独流体域的这种压差并不是基于伯努利原理。
在经过吸口15后,流体射流20可撞击排放通道22,由此在排放通道22内建立高压力区,从而促进沿着排放通道22沿近端方向朝着近端34去除收集的材料,并最终将其从患者体内除去。收集室8可与排放管24联接,其中,排放管24本身与排放通道22流体连接,用于收集移除的材料。
本体2的远端12可成形为三维弯曲部,以限定流体流重定向装置。该弯曲部可具有一个或多个轴向分量和径向分量。该弯曲部可使流出管21的射出口31且越过吸口15的流体流20沿不平行于本体2的纵轴45的斜路径流动。流体流重定向装置的示例包括重定向壁38。
管21的远端35可单独成形为三维弯曲部,形成另一流体流重定向装置。管21的该弯曲部可具有轴向和径向分量。该弯曲部使在管21的远端35流出射出口31且越过吸口15的流体沿不平行于纵轴45的斜路径流动。该斜路径相对于纵轴45的径向和轴向角度可为5º-45º。
上述两种弯曲部使越过吸口15的流体射流20沿着可相对于本体2的近端34的轴线和纵轴45都偏斜的路径,由此使周围流体产生旋转动量。在本体2周围的流体中产生的漩涡可使待去除材料变得不稳定,从而有助于将其去除。
在一些实施例中,本体2的近端34附近可包括阀52,以允许压力通道25内的加压流体的高频循环或脉动。阀52可包括产生高频纵波的高频可动构件,其产生的高频纵波通过压力通道25内及远端12的加压流体传送,以进一步破坏本体2周围待去除的材料的稳定性。阀52可以0.5-10 Hz的打开/关闭循环进行操作。
本体2的远端12可包括流出侧孔口,允许高压流体以高速度流入周围区域,以进一步破坏待去除材料的稳定性使其脱落。替代地,所述流出侧孔口可结合到第四腔中。
从图中可清楚地看出,具有压力通道与流体射流泵的压力进口的组合式压力连接的本发明的导管可以和先前已知的水力抽吸导管一样容易地使用。与已知导管相比,没有其它连接,并且本发明的导管可以制造成具有更大长度和更小直径。

Claims (7)

1.一种抽吸导管设备,其包括:
管状本体,具有近端部和远端部及位于所述远端部上用于从所述远端部外部的区域收集材料的吸口,所述管状本体至少限定自所述近端部向所述远端部延伸的第一和第二腔,不同的所述第一和第二腔中的每一个都与限定于所述管状本体的远端部内的腔室流体连通,所述腔室与所述吸口流体连通,所述管状本体的远端部限定与所述管状本体的侧壁围绕的中心轴重合或平行的纵轴;
管,其自所述管状本体的第一腔延伸至所述腔室,并限定管通道及与所述管通道流体连通的射出口,所述射出口与所述管通道被定位和定向成使得将来自所述管通道的流体流导引成从所述射出口向所述腔室发射的射出射流,所述射出射流以在所述材料上产生喷射作用的方式在近端到远端方向上流动经过所述吸口;
