具体实施方式
T细胞活化通过广泛的机制,包括负共刺激被严格调节,但是单独分子介质在功能上相互作用的程度仍不清楚。CTLA4和PD-1是利用独特的分子和细胞机制的T细胞活化的关键负调节物。CTLA4和PD-1均是T细胞负共刺激分子,其功能是减弱T细胞活性。这些分子利用独特的分子机制来进行这些功能。这些分子机制是由很大程度上独特的细胞信号传导途径介导的(Wei et al.,2018)。CTLA4的主要功能是竞争B7配体的结合(B7-1/CD80,B7-2/CD86),这导致CD28正共刺激和下游PI3K/AKT信号传导的降低。PD-1的主要功能是在与其配体PD-L1/PD-L2结合时抑制近端T细胞受体信号传导;然而,据报道CD28信号传导的抑制也为PD-1的重要功能(Hui et al.,2017)。抗体介导的阻断通过阻止配体结合而抑制这些功能。这种信号传导能力丧失和随之发生的下游生物事件可通过遗传手段通过Ctla4和Pdcd1的功能丧失等位基因的组合来建模。
本发明人试图了解由CTLA4和PD-1施加的调节机制在功能上是独立的还是依赖的,并找到小鼠中Ctla4与Pdcd1(编码PD-1)之间遗传相互作用的证据。在完全遗传不存在Pdcd1的情况下,Ctla4的单等位基因丧失导致约一半的小鼠死亡。死亡率由多种组织,包括胰腺和心脏中的自身免疫引起(参见,例如,图3E)。相比之下,在同窝出生Ctla4+/+Pdcd1-/-或Ctla4+/-Pdcd1+/-中未观察到死亡或严重的表型。这些数据揭示了在Pdcd1空背景(nullbackground)的情况下,Ctla4表现出条件性单倍不足。总之,这些发现表明CTLA4和PD-1在功能上相互作用并支持这样的阈值模型,其中负共刺激分子以剂量依赖性和加和的方式减弱T细胞活化。
该动物模型将使得能够调查疾病病因并鉴定调节免疫相关不良事件的产生的因素。该动物模型会在心脏和胰腺(以及其他器官)中发生自身免疫,这一点很重要,因为致命性心肌炎(心脏的炎症)和I型糖尿病(胰腺的自身免疫破坏)是在人患者中与组合抗CTLA4和抗PD-1治疗相关的两种罕见但非常严重的并发症类型。该模型还显示出能够对与检查点阻断相关的其他类型自身免疫不良事件(例如,胃肠)建模。所描述的转基因小鼠模型可用作组合抗CTLA4加抗PD-1免疫检查点阻断(即,靶向T细胞共刺激受体CTLA4和PD-1的单克隆抗体的治疗)的模型。PD-1的遗传丧失和CTLA4的单拷贝丧失在其中对负共刺激活性进行降低和建模的类似情况下为该治疗建模。因此,调节CTLA4和PD-1的该遗传模型概括了由于检查点阻断治疗而引起的免疫相关不良事件的现象。这一点是特别值得注意的,因为目前尚无忠实地概括与检查点阻断治疗相关的不良事件的动物模型。组合抗CTLA4和抗PD-1检查点阻断治疗目前已被批准用于治疗黑素瘤和肾细胞癌(以及超过250项的正在进行的临床试验)。
另外,该小鼠品系背景为C57BL6/J,这是值得注意的,因为该背景广泛用于肿瘤免疫学研究,并且还因为该背景通常很难诱导自身免疫,表明该表型满足高生物学棒(biological bar)。另外,不引入外源性抗原(例如转基因、病毒),并因此被识别为驱动该自身免疫的抗原是自身抗原,推测在接受检查点阻断治疗的患者中就是这种情况。此外,由于在该动物模型中出现了多种类型的自身免疫和免疫相关疾病病因,因此这使得能够研究这些疾病之间的关系。
因此,该小鼠模型可用于理解这些不良事件是如何引起的以及测试旨在减轻这样的不良事件和自身免疫的治疗的效力。这样的研究可能是设计保持治疗效力并降低不良事件(特别是罕见的非常严重的自身免疫的诱导)的下一代免疫治疗所必需的。
I.本发明的一些方面
本文中鉴定了T细胞负共刺激基因Ctla4与Pdcd1(编码PD-1)之间的遗传相互作用。在完全不存在Pdcd1的情况下,该遗传相互作用表现为Ctla4的条件性单倍不足,其导致致命性全身性自身免疫。从基本角度来看,该观察结果支持T细胞活化的阈值模型,其中多种来源的T细胞受体(TCR)信号扰动可复合在一起,导致异常的T细胞活化。这表明CTLA4和PD-1调节信号在功能上整合,并一起提供了关键的缓冲系统来抑制T细胞活化。在分子水平上,Pdcd1和Ctla4的组合基因剂量的降低可限制以细胞固有方式减弱T细胞活化的总体能力。
除对T细胞活化的基本机制有重要见解之外,这些发现在癌症免疫检查点阻断的情况下具有显著的临床意义。组合抗CTLA4加抗PD-1治疗在多种肿瘤类型中有效,所述肿瘤类型包括晚期黑素瘤和肾细胞癌。已知抗CTLA4和抗PD-1免疫检查点阻断利用独特的细胞机制(Das et al.,2015;Wei et al.,2017)。在考虑本发现的情况下,这些独特的细胞机制可能在功能上相互作用,这可以部分地解释组合治疗相对于单治疗的效力的增强(Curranet al.,2010;Postow et al.,2015;Wolchok et al.,2013)。与本发现甚至更为相关的是,在极少数情况下,免疫检查点阻断治疗可诱导严重的免疫相关不良事件以及善意的自身免疫,例如心肌炎和I型糖尿病。Ctla4+/-Pdcd1-/-小鼠提供了临床前动物模型,其用于研究由CTLA4和PD-1信号传导丧失诱导的自身免疫。这一点是特别值得注意的,因为在该模型中观察到的自身免疫密切反映在人患者中组合抗CTLA4加抗PD-1检查点阻断治疗之后可引起的心肌炎和I型糖尿病。进一步的机制理解为区分和特异性调节介导免疫检查点阻断治疗的效力和不良事件的免疫应答方面提供了潜力。
令人感兴趣的是,在Ctla4+/-Pdcd1-/-小鼠中发生的自身免疫反复出现在特定的解剖部位。为什么特定的组织部位对由PD-1和CTLA4信号传导的丧失诱导的自身免疫更加敏感仍然是一个关键的开放性问题。来自这些部位的组织特异性抗原在不存在负共刺激的情况下可能使它们特别易于被自身免疫识别。或者,组织敏感性可能是由于先前已观察到的组织特异性Treg群之间的功能差异所致(Legoux et al.,2015)。
值得注意地,CTLA4的杂合种系功能丧失等位基因导致免疫失调,临床表现高度可变(Kuehn et al.,2014;Schubert et al.,2014)。这表明人中CTLA4的单拷贝丧失是致病性的并且此外强烈暗示与其他遗传和/或环境因素的遗传相互作用。在人中或小鼠中(在不存在PD-1的情况下)CTLA4的单拷贝丧失也有可能将T细胞活化阈值降低至滋养型TCR(tonic TCR)信号传导可达到的水平,并因此随机过程可解释CTLA4缺陷型人和小鼠中临床表现的差异。一个突出的问题是其他T细胞共刺激分子或涉及其功能的分子与Pdcd1和Ctla4遗传相互作用的程度。例如,具有LRBA(CTLA4运输的重要调节物)功能丧失等位基因的患者表现出与具有功能丧失CTLA4的患者相似的自身免疫表型(Besnard et al.,2018;Hou et al.,2017)。
除CTLA4与PD-1之间的遗传相互作用的关键发现之外,本发现还表明Ctla4和Pdcd1的同时遗传缺陷是胚胎致命性的。鉴于αβ T细胞直至出生之后才出现,这一点是出人意料的(Havran和Allison,1988)。这种情况发生的机制尚不清楚,然而有两种主要的可能性,这二者都非常令人感兴趣。第一种可能性是CTLA4和PD-1限制了早在胚胎发育期间的E14出现的γδ T细胞的活化。第二种可能性是CTLA4和PD-1在发育期间可能具有尚未确定的非免疫功能。
总之,本发现揭示了Ctla4与Pdcd1之间的遗传相互作用。这为CTLA4和PD-1的调节机制之间的功能相互作用提供了明确的证据。此外,这提供了稳健的动物来对由组合抗CTLA4加抗PD-1免疫检查点阻断治疗诱导的免疫相关不良事件建模。
II.实施例
以下实施例被包括在内以示出本发明的一些优选实施方案。本领域技术人员应理解,以下实施例中公开的技术表示本发明人发现的在本发明的实践中良好发挥作用的技术,并因此可认为构成用于其实践的优选方式。然而,根据本公开内容,本领域技术人员应理解,在不脱离本发明的精神和范围的情况下,可在所公开的具体实施方案中做出许多改变并且仍然获得相同或相似的结果。
材料和方法
小鼠。将Ctla4tm1All小鼠(Chambers et al.,1997)与Pdcd1敲除小鼠(Pdcdltml.lShr)(Keir et al.,2007)(其购自杰克逊实验室(Jackson Laboratory)(021157))育种。在该杂交之前,使Pdcd1敲除小鼠与C57BL/6J回交一次。使所得F1 Ctla4+/-Pdcdl+/-小鼠互交,以产生两种基因的野生型和突变体等位基因的所有可能组合。该第一育种方案特定地利用了来源于Ctla4+/-(对于Pdcd1是野生型)和Pdcdl-/-小鼠(对于Ctla4是野生型)杂交的F1小鼠。该育种方案确保了F1小鼠中Ctla4和Pdcd1的突变体等位基因是反式的,并因此可计算重组频率(图9A)。Ctla4与Pdcd1之间的遗传距离通过评估该杂交中的重组和总事件的数目来计算。观察到的重组频率17.03cM与由小鼠表型组数据库(Mouse Phenome Database)(MPD,RRID:SCR_003212)(Bogue et al.,2018)报道的Ctla4与Pdcd1之间的16.89厘摩遗传距离接近匹配。该估计的遗传距离与在该育种方案中观察到的重组频率一致。
为了验证来自第一育种方案的发现,利用了第二相关育种方案。重要的是,该育种方法利用了不同的基因型,突变体等位基因可以是顺式或反式的,并且该方法可以以1∶1比例产生Ctla4+/-Pdcd1-/-(实验)和Ctla4+/+Pdcd1-/-(对照同窝出生)。与初始育种方法相比,这允许产生多得多的Ctla4+/-Pdcd1-/-小鼠。具体地,育种了雄性Ctla4+/-Pdcd1-/-和雌性Ctla4+/+Pdcd1-/-。使用雌性Ctla4+/+Pdcd1-/-以消除在Ctla4+/-Pdcd1-/-中观察到的自身免疫可能影响胎儿-母体耐受性或产生活产仔(viable litter)的能力的可能性。
为了生成存活曲线,将事件限定为死亡(即,发现死亡的小鼠)或由兽医人员将小鼠确定为需要安乐死(例如,由于昏睡、垂死、呼吸困难)。对于确定为需要安乐死的小鼠,将死亡日期限定为兽医人员将小鼠标记出来的日期。与死亡率相关的动物表型由兽医人员确定并报道。用于育种的小鼠在其用于该目的时从存活分析中删减。
所有小鼠均饲养在德克萨斯大学MD安德森癌症中心南校区动物饲养所(TheUniversity of Texas MD Anderson Cancer Center South Campus Vivarium),其是AAALAC认可的特定的无病原体动物设施。所有实验均根据德克萨斯大学MD安德森癌症中心机构动物护理和使用委员会(Institutional Animal Care and Use Committee,IACUC)指南进行。
基因型分型。使用Direct-to-PCR消化混合物分离基因组DNA,并对Ctla4和Pdcd1敲除小鼠进行基于聚合酶链式反应(PCR)的基因型分型。引物提供于表1中。如前所述(Chambers et al.,1997)对Ctla4tm1All小鼠进行基因型分型。Ctla4野生型和突变体等位基因的预期带尺寸分别为约75和约150bp。如前所述(Keir et al.,2007)对Pdcd1敲除小鼠进行基因型分型。Pdcd1野生型和突变体等位基因的预期带尺寸分别为418和350bp。

SNP分型。将粗制基因组DNA裂解物提交至德克萨斯大学MD安德森癌症中心实验室动物遗传学服务中心核心设施,以使用100个标志物组进行SNP分型。出于确定遗传变体是否与自身免疫相关的目的,将“未受影响”的小鼠限定为在6月龄内未表现出任何症状或死亡的小鼠,并且将“受影响”的小鼠限定为表现出症状并死于疾病的I类小鼠。
病理状况分析。动物尸检由德克萨斯大学MD安德森癌症中心兽医医学组织学实验室中或艾利森实验室(Allison laboratory)中的人员进行。对在安乐死时收集的血液样品进行使用Cobas Integra 400Plus(Roche Diagnostics,Risch-Rotkreuz,Switzerland)进行的自动血清化学分析。将经福尔马林固定的组织常规处理成石蜡块,以5微米切片,并用苏木精和伊红染色。使用另外的切片以进行特定目的组织的免疫组织化学(IHC)染色,使用针对CD3的抗体(ab16669,Abcam,Cambridge,MA),随后使用第二试剂进行显色检测(BondPolymer Refine Detection系统,DS9800,Leica,Buffalo Grove,IL)。经染色的切片由兽医病理学家使用带有Leica DFC495照相机和Leica Application Suite v4.12软件的Leica DM2500显微镜进行检查。使用半定量量表对组织学变化进行评分,其中0=无病变至4=严重病变。
流式细胞术。通过使用塑料注射器的背面通过70um滤器将合并的腹股沟、腋和肱淋巴结磨碎到补充有10%FBS和1%青霉素链霉素的RPMI-1640中来制备来自淋巴结的单细胞悬液。在前一天晚上于4℃下,将96孔平底板以200ul/孔的PBS中的1ug/ml抗CD3ε和2ug/ml抗CD28进行包被过夜。然后使用CellTrace Violet增殖试剂盒按照制造商方案(Invitrogen,C34557)对细胞进行染色。将每个样品一式三份以106个细胞/mL/孔平板接种在200ul的补充有10%FBS、丙酮酸钠、0.1%b-ME和P/S的RPMI-1640中,并将其在37℃下孵育46小时。然后将细胞转移至U底96孔板,并用FACS缓冲液洗涤两次,并在4℃下用具有25mg/mL2.4G2抗体的2%的每种牛、鼠、大鼠、仓鼠和兔血清PBS孵育10分钟,然后以50mL体积于4℃下用抗体混合物表面染色30分钟。将细胞用FACS缓冲液洗涤两次,然后使用FoxP3固定和透化试剂盒根据制造商方案(eBioscience)进行固定和透化。随后在室温下用胞内抗体混合物将细胞染色30分钟。然后将细胞用Foxp3透化缓冲液洗涤两次,然后用FACS缓冲液洗涤两次,并在LSRII(BD)上进行分析。

Luminex细胞因子和趋化因子评估。从来自上述两种育种方案的Ctla4+/-Pdcd1-/-和对照同窝出生小鼠(包括Ctla4+/+Pdcd1-/-小鼠和同时具有CTLA4和PD-1二者的小鼠(例如Ctla4+/-Pdcd1+/-小鼠)二者)收集血清。简言之,通过终末心脏穿刺收集血液,使其在室温下凝结,以8,000g离心10分钟,收集上清液血清并将其在液氮中速冻,然后在-80摄氏度下储存。使用Cytokine&Chemokine 36-plex Mouse ProcartPlex luminex测定(ThermoFisherScientific)根据制造商方案对抗体细胞因子和趋化因子的血清水平进行评估。对于每个相应的分析,所有样品均在单一批次中并行分析。将血清样品以1∶10,000进行稀释以用于分析血清抗体水平。
反相蛋白质组阵列分析。将来自16日龄Ctla4敲除小鼠和同窝出生对照小鼠的淋巴结速冻以进行后续分析。将组织样品在具有新鲜添加的蛋白酶抑制剂(Roche,05056489001)和磷酸酶抑制剂(Roche,04906837001)的1%Triton X-100、50mM HEPES(pH7.4)、150mM NaCl、1.5mM MgCl2、1mM EGTA、100mM NaF、10mM焦磷酸Na、1mM Na3VO4、10%甘油中裂解,并使用Precellys均化器(Bertin Instruments)进行均浆。将样品在样品缓冲液(10%甘油、2%SDS、62.5mM Tris-HCl、BME,pH 6.8)中稀释,然后进行阵列打印和分析。使用Array-Pro Analyzer软件(MediaCybernetics)对信号进行量化,并使用MD安德森癌症中心为RPPA核心设施开发的“超曲线拟合(Supercurve fitting)”方法进行归一化。对经归一化的线性值使用双尾T检验假设不等方差在Excel(Microsoft)中进行分析,并使用Prism6.0(GraphPad)进行绘制。
Nanostring mRNA分析。从Ctla4敲除小鼠和同窝出生对照小鼠中解剖淋巴结。使用(Qiagen)从淋巴结中提取RNA。使用nCounter平台(Nanostring)上的Mouse ImmunologyCode集合组对100ng RNA进行分析。使用该组内的持家基因在每个样品基础上将值归一化。利用皮尔逊距离矩阵(Pearson distance matrix)和Ward最小方差方法(Ward’sminimumvariance method),在R中生成Nanostring基因表达和RPPA蛋白质组数据的热图。仅绘制基因型之间在采用单因素ANOVA比较的情况下由错误发现率为5%限定的差异表达基因。
统计学。除非另外说明,否则统计学分析均在Prism 7.0或8.0(GraphPadSoftware,San Diego,CA)中进行。使用双尾T检验假设不等方差在Excel(Microsoft)中对RPPA数据的经归一化线性值进行分析,并绘制。
实施例1-在遗传不存在Pdcd1的情况下Ctla4的致命性单倍不足
为了测试Ctla4与Pdcd1之间是否存在遗传相互作用,使Ctla4和Pdcd1(编码PD-1)敲除的转基因小鼠杂交。基于来自小鼠表型组数据库(Mouse Phenome Database)(Bogueet al.,2018)的估计,鼠Ctla4和Pdcd1在遗传上以16.89cM的遗传距离连锁。使用杂合互交-育种方案(参见材料和方法),该方案允许从单杂交中产生突变体等位基因的所有可能排列,并且允许所观察到的Ctla4与Pdcd1之间重组频率的估计值在17.03cM处。出人意料地,约50%的Ctla4+/-Pdcd1-/-在3月龄内自发死亡(图1A)。相比之下,相关对照同窝出生(例如Ctla4+/+Pdcd1-/-和Ctla4+/-Pdcd1+/-)未表现出外显表型,并且在这些组中也未观察到死亡。Ctla4+/+Pdcd1-/-小鼠中外显表型缺乏与先前的观察结果(Nishimura et al.,1999)一致。另外,所有Ctla4-/-Pdcd1+/+小鼠的死亡均与CTLA4缺陷型小鼠品系的初始特征一致(Chambers et al.,1997;Tivol et al.,1995;Waterhouse et al.,1995)。然而,令人感兴趣的是,Pdcd1的单等位基因丧失加速了由CTLA4丧失诱导的致命性淋巴细胞增殖(图1A)。这表明Pdcd1基因剂量改变了CTLA4缺陷型小鼠的表型和淋巴细胞增生性疾病。这些观察结果与Ctla4+/+Pdcd1+/-或Ctla4+/-Pdcd1+/-小鼠中任何PD-1单倍不足的缺乏形成对比。
特别令人感兴趣的是Ctla4+/-Pdcd1-/-小鼠的出人意料的自发死亡,因为这提供了遗传相互作用的有力证据。为了进一步确定这一观察结果,使用不同的育种方案(参见材料和方法)产生了Ctla4+/-Pdcd1-/-和Ctla4+/+Pdcd1-/-同窝出生小鼠。该第二方法产生了显著相似的发现,其中约50%的Ctla4+/-Pdcd1-/-小鼠自发死亡,而同窝出生Ctla4+/+Pdcd1-/-小鼠中未观察到死亡(图1B)。小鼠在2至6月龄时自发死亡或成为垂死的,具有多种观察到的临床症状(表2,参见材料和方法)。然而,鉴于有或无先前这些症状表现的小鼠具有类似的死亡潜伏期,就观察到的致命性自身免疫而言,这些特定表型的发生不是必需的(图9B)。然而,令人感兴趣的是,雄性和雌性Ctla4+/-Pdcd1-/-小鼠以显著不同的频率死亡(图1C)。这表明观察到的Ctla4的条件性单倍不足是性别依赖性的,其中雌性小鼠以高于雄性小鼠的频率死亡。这种性别失衡与接受抗CTLA-4ICI的女性患者中irAE的总体风险提高一致(Valpione et al.,2018)。在死亡之前,在约三分之二的死亡小鼠中在早在1月龄时就开始出现外显的非特异性临床体征(例如体重增加减少、共济失调、呼吸困难)。反映该表型的严重程度,显示出临床体征的Ctla4+/-Pdcd1-/-小鼠的总体重显著低于未显示出临床体征的Ctla4+/-Pdcd1-/-小鼠的总体重(图2B)。
总之,这些观察结果表明,Pdcd1和Ctla4表现出强遗传相互作用。具体地,存在显著的Ctla4条件性单倍不足,其仅在Pdcd1遗传缺失的情况下出现并导致自发死亡(图1A至B)。

鉴于仅50%的Ctla4+/-Pdcd1-/-小鼠死亡,很可能另外的遗传或环境因素调节了Ctla4条件性单倍不足的外显率。对细微的遗传差异是否可成为该二分法的基础进行了研究。基于100个标志物的单核苷酸多态性分型组,所有受试小鼠均为97至100%的C57BL6/J,其中未观察到具有表型表现的任何分离的等位基因之间的关联(图7和图8)。
然后确定未自发死亡并达到存活平台年龄(约6个月)的Ctla4+/-Pdcd1-/-小鼠是否具有在生物学上相关但不足以引起死亡的细微的自身免疫。相比之下,与表型正常的Ctla4+/-Pdcdl-/-或Ctla4+/+Pdcdl-/-小鼠相比,受影响的Ctla4+/-Pdcd1-/-小鼠的总体重显著降低(图2A)。相比之下,表型正常的Ctla4+/-Pdcd1-/-和对照同窝出生Ctla4+/+Pdcd1-/-小鼠的总体重无显著差异(图2B)。进一步研究是否对来自衰老的([4+]个月)未受影响的Ctla4+/-Pdcdl-/-和Ctla4+/+Pdcdl-/-小鼠的样品进行生物学的基础血清化学。令人感兴趣的是,未观察到基因型之间的显著差异(图2B)。这表明未受影响的小鼠(限定为不存在导致死亡的表型衰退)不会显著地发生至少可通过全身性组织损伤的标志物(例如LDH)或目视检查而检出的自身免疫提高。然而,值得注意地,对表型受影响的Ctla4+/-Pdcd1-/-小鼠进行的血清化学分析揭示了组织损伤的证据,其中ALT、AST和LDH的血清水平显著升高,以及葡萄糖水平降低(图2C)。这些发现表明在Ctla4+/-Pdcd1-/-小鼠中组织破坏,并且其在外周血中可检测到。
这些数据支持了这样的模型,其中环境因素在Ctla4+/-Pdcd1-/-小鼠中调节致命性表型的外显率和发生。从基本角度来看,该观察结果支持T细胞活化的阈值模型,其中整合了多种来源的TCR信号扰动以调节T细胞活化。在Ctla4+/-Pdcdl-/-小鼠的情况下,活化阈值显著降低,以使得通常经过缓冲的另外的细微输入足以诱导异常T细胞活化和自身免疫。该模型的预测是,与来源于Ctla4+/+Pdcd1-/-小鼠的T细胞相比,来源于Ctla4+/-Pdcd1-/-小鼠的T细胞的CTLA-4水平降低。为了确认Ctla4的单拷贝丧失会导致可用CTLA-4蛋白的降低,我们评估了来自Ctla4+/-Pdcd1-/-和同窝出生对照Ctla4+/+Pdcd1-/-小鼠的活化T细胞中总CTLA-4蛋白表达。值得注意地,体外经刺激T细胞的流式细胞术分析表明,与来源于Ctla4+/+Pdcd1-/-小鼠的T细胞相比,来源于Ctla4+/-Pdcd1-/-小鼠的T细胞中CTLA-4蛋白水平较低(图2D)。
在Ctla4+/-小鼠在生物体或细胞水平上均未显示任何单倍不足的先前发现的背景下,这些发现表明PD-1负共刺激足以在功能上缓冲Ctla4的单等位基因丧失。与该观念一致,在来自Ctla4+/+和Ctla4+/-小鼠的淋巴结的转录和蛋白质组分析中观察到很小差异,其中在Ctla4-/-小鼠中观察到显著变化(图5和6)。
令人感兴趣的是,这些结果与来自Ctla4+/+和Ctla4+/-小鼠(其中在Pdcdl中无扰动)的相似组织的质谱细胞术(mass cytometry)谱分析的发现形成对比,其中未检测到细胞表型或频率的差异。这表明至少在同质近交鼠品系中,另外的调节分子机制可缓冲由Ctla4单拷贝丧失引起的信号传导的扰动。然而,在另外的扰动(例如PD-1的遗传丧失)的情况下,由于单等位基因Ctla4引起的T细胞调节的功能缺陷可能由于缓冲能力的丧失而显现。
实施例2-Ctla4+/-Pdcd1-/-小鼠发生多组织自身免疫
接下来寻求了解Ctla4+/-Pdcd1-/-小鼠的死亡原因,并研究特定组织是否受到影响。另外,寻求了解特定细胞类型是否介导疾病病因。为了解决这些问题,对来自Ctla4+/+Pdcd1-/-和Ctla4+/-Pdcd1-/-小鼠的42种组织进行组织学分析(参见材料和方法)。在多种组织,包括心脏、胰、肺、肝和胃肠道中观察到炎症。具体的病理观察结果包括淋巴细胞性心肌炎、动脉内膜炎、肺血管炎和淋巴细胞性胰腺炎。在最显著的组织学发现中,在Ctla4+/-Pdcd1-/-小鼠的心脏和胰腺组织中观察到显著的免疫浸润(图3A至D;图10和图11A)。本质上更次要但也值得注意的,另外的病理观察结果包括唾液腺、泪腺、哈氏腺(harderiangland)和肾中的淋巴样浸润。另一些次要观察结果包括肝变性/坏死、淋巴细胞性胃炎和滑膜炎,但是这些发现与基因型相关的程度仍未完全确定。令人感兴趣的是,在非淋巴外周组织而不在淋巴器官(例如脾或淋巴结)中观察到更显著的组织学发现。这表明在Ctla4+/-Pdcd1-/-小鼠中特异性地破坏了外周免疫耐受性。
为了更好地探询心肌浸润的性质,进行了对更大组群小鼠的详细的H&E组织学分析,以及针对T细胞的相似组织样品的免疫组织化学染色(利用CD3作为泛T细胞(pan-Tcell)标志物)。心肌炎由Ctla4+/-Pdcd1-/-小鼠中显著的T细胞浸润组成。组织学分析也表明其他免疫群例如巨噬细胞的浸润。这些数据表明,在Ctla4+/-Pdcd1-/-小鼠中看到的心肌炎在组织学上类似于患有ICI相关心肌炎的患者。一系列组织学分析的结果总结于下表3至4中。

鉴于C57BL6/J近交小鼠品系对自身免疫的发生具有高度抗性,这些发现是更加值得注意的。例如,与此处的发现一致,与C57BL6/J背景相比,针对PD-1呈缺陷的小鼠在Balb/c背景下发生更严重的自身免疫(Nishimura et al.,1999;Nishimura et al.,2001)。还要着重注意的是,由于衰老小鼠中PD-1的遗传丧失而发生的自身免疫主要是由自身抗体介导的。然而,在Ctla4+/-Pdcd1-/-小鼠中抗体水平未升高(图11B)。这与在Ctla4+/-Pdcd1-/-小鼠中观察到的外周组织的淋巴细胞性浸润形成对比。
值得注意的是,这些发现具有与组合抗CTLA4加抗PD-1免疫检查点阻断治疗(例如,伊匹单抗(ipilimumab)加纳武单抗(nivolumab))相关的自身免疫和其他免疫相关不良事件(immune related adverse event,irAE)的极大相似性(Sznol et al.,2017)。特别地,在Ctla4+/-Pdcd1-/-小鼠中观察到的胰腺和心脏组织中观察到的严重自身免疫显示出与作为与CTLA-4和PD-1的治疗性阻断相关的罕见但非常严重的不良事件的暴发性心肌炎和胰岛素依赖性糖尿病类似(Barroso-Sousa et al.,2018;Johnson et al.,2016;Moslehiet al.,2018)。支持Ctla4+/-Pdcd1-/-小鼠紧密地概括了该生物学这一观念,治疗相关心肌炎和糖尿病显示出直接由T细胞介导。
然后寻求获得对以下的见解:作为由Ctla4的条件性单倍不足诱导的全身性自身免疫之基础的T细胞的抗原特异性,以及特别地,特定组织抗原是否被反复识别。为了探究这种可能性,对来自Ctla4+/+Pdcd1-/-和Ctla4+/-Pdcdl-/-小鼠的淋巴结、心脏和胰腺组织进行TCR测序(参见材料和方法)。令人感兴趣的是,在Ctla4+/+Pdcd1-/-与Ctla4+/-Pdcd1-/-小鼠之间未观察到T细胞克隆性的显著变化(图4)。与先前表征的野生型或Ctla4+/-小鼠相比,在这两种品系中,T细胞克隆性均提高。这表明PD-1的丧失足以诱导T细胞增殖和克隆型的扩增,但是这些克隆的致病活性受到另外的机制的限制。这与Pdcd1-/-小鼠中自身免疫表型的缺乏或长潜伏期是一致的。然而,在这种情况下,Ctla4的单拷贝丧失显示出足以消除另外的调节限制,这从而允许通过扩增的T细胞克隆表现出致病性活性。
为了评估在不存在PD-1的情况下由于Ctla4的条件性单倍不足引起的自身免疫是仅由于外周耐受缺陷引起还是还由于中枢耐受缺陷引起,对Ctla4+/-Pdcdl-/-和同窝出生Ctla4+/+Pdcd1-/-小鼠中的胸腺发育进行表征。Ctla4的单等位基因丧失不会影响胸腺细胞组成,这与CTLA-4在胸腺发育期间不发挥关键作用的先前报道(Chambers et al.,1997;Wei et al.,2019)一致。与在淋巴结来源的T细胞中的这些观察结果一致,来源于Ctla4+/-Pdcd1-/-小鼠的胸腺来源Treg(新产生和再循环)表达的CTLA-4蛋白降低。总之,这些数据表明,Ctla4单倍不足导致外周耐受缺陷而不是中枢耐受缺陷。
最后,对Ctla4与Pdcd1之间遗传相互作用的分子基础进行研究。假设Ctla4的单拷贝丧失会导致信号传导和转录输出的细微变化,在其他野生型条件的情况下,由于T细胞活化信号传导途径内的稳健缓冲作用,所述细微变化不会调节T细胞活性或表型。然而,在另外不存在PD-1或可能其他负共刺激分子的情况下,由于Ctla4单倍不足引起的细微分子缺陷可外显出现。为了探究这种可能性,我们利用反相蛋白质组分析(reverse phaseproteomic analysis RPPA)来探测在来源于野生型、杂合和纯合Ctla4敲除小鼠的淋巴结组织中238种蛋白质靶标的表达。该RPPA组包括信号传导分子和磷酸化表位的阵列,并因此非常适合检测典型信号传导途径的变化。
如所期望的,与同窝出生对照相比,在Ctla4-/-小鼠中,与增殖途径相关的蛋白质高度上调,这与Ctla4敲除小鼠的淋巴细胞增生性表型一致。这包括CDK1、p-Rb、p-S6、p-CHK1和p-STAT3的显著提高,伴有p21的下调(图5F)。最值得注意的是,主成分分析(PCA)和无监督层次聚类表明Ctla4+/-和Ctla4+/+小鼠的蛋白质组谱是独特的。这些组之间的差异很大程度上是由Ctla4+/-小鼠中多种蛋白质(例如p21、DUSP4和B7-H3)的下调驱动的。同样,基因表达分析检测到Ctla4+/-与Ctla4+/+小鼠之间的差异(尽管程度较小),以及Ctla4-/-小鼠中的显著的转录变化。虽然鉴于对整个淋巴结组织进行了分析,蛋白质组和转录谱的所观察到差异可以反映细胞固有信号传导的变化以及相对细胞组成的变化,但这些发现仍然表明,Ctla4的单拷贝丧失会导致细微的分子单倍不足表型。蛋白质组水平上的这些细微变化(图5)显示出不足以调节免疫应答,然而,这与在转录水平上不存在这样的差异一致(图6)。总之,这些数据表明,PD-1负共刺激足以在小鼠中在功能上缓冲Ctla4的单等位基因丧失。这种功能缓冲在Ctla4+/-Pdcd1-/-小鼠中丧失,因此这允许自发性自身免疫的发生和发作。
