Abstract
本发明属于骶韧带修复技术领域,具体涉及一种加强人体内骶韧带强度的补片及手术方法。补片由生物可吸收材料制成,形状与骶韧带外形相贴合,大小与骶韧带、尿生殖筋膜之间的Okabayashi间隙范围相适应。实际手术时,根据患者体征对补片进行裁剪,将补片置于患者骶韧带和尿生殖筋膜之间的Okabayashi间隙范围,使补片附着在原有骶韧带位置上,促进骶韧带与补片表面发生粘连、肉芽增生,从而增加其强度,能够有效预防和减少手术后的复发。由于补片的存在,不必过度向后高位牵拉骶韧带,从而使阴道顶端位于中央位置,解剖更加符合生理解剖。
Description
技术领域
本发明属于骶韧带修复技术领域,具体涉及一种加强人体内骶韧带强度的补片及手术方法。
背景技术
骶韧带又称子宫骶韧带,是子宫韧带之一,由平滑肌和结缔组织构成。参考图1和图2,骶韧带起于子宫颈后面,向后绕过直肠,附于骶骨前面,是支持盆底的重要结构,由于盆底肌肉的损伤,韧带断裂或薄弱会导致子宫脱垂等症状,现有医疗水平为解决这一问题主要采取骶棘韧带固定、骶骨阴道固定术、高位骶韧带悬吊术。骶棘韧带固定术方法可靠,但固定点偏向后外侧,如做双侧固定,则阴道顶端会向两侧过度延展,如做单侧固定则阴道会偏向一侧。骶骨阴道固定术阴道轴向较好,其使用补片完全替代原有断裂的骶韧带,其弊端在于补片较大,其性能与原有骶韧带有一定差异性,且该补片固定于骶骨一侧,存在其他并发症风险,术后严重并发症较多,可发生骨髓炎、椎间盘炎、补片侵蚀、暴露、感染等情况。高位骶韧带悬吊术最符合阴道的解剖轴线,而且该部位解剖相对安全,不管腔镜下还是经阴道操作都很方便。但已经发生脱垂的患者往往骶韧带较为薄弱,手术后随访复发的概率相对以上两种术式为高,而且过度牵拉也会使阴道的轴向偏向后方,导致前盆腔受力增大,有可能增加前盆脱垂的可能。
发明内容
为了解决上述技术问题,本发明的目的在于提供一种加强人体内骶韧带强度的补片。
为实现上述技术目的,本发明采用的技术方案为:一种加强人体内骶韧带强度的补片,由生物可吸收材料制成,其形状与骶韧带外形相贴合,类似扇形结构或类三角形结构,大小与骶韧带、尿生殖筋膜之间的Okabayashi间隙范围相适应。
作为优选的技术方案,所述补片的尾侧边长2~4cm。
作为优选的技术方案,所述补片的背侧边长4~5cm。
本发明还提供了采用以上所述的加强人体内骶韧带强度的补片的手术方法,手术时,根据患者体征对补片进行裁剪,将补片置于患者骶韧带和尿生殖筋膜之间的Okabayashi间隙范围,使补片附着在原有骶韧带位置上,促进骶韧带与补片表面发生粘连、肉芽增生,从而增加其强度。
具体地,补片的附着位置为:尾侧边从宫颈到骶四筋膜,背侧边从骶四筋膜到骶二筋膜,上斜边低于输尿管和腹下神经水平,使用缝合线将补片的尾侧边和背侧边固定在宫颈、阴道后壁及骶骨筋膜上,固定后腹膜将补片覆盖起来。
由于采用上述技术方案,本发明具有至少以下有益效果:
(1)补片附着在原有骶韧带位置上,可以促进骶韧带与补片表面发生粘连、肉芽增生,从而增加其强度,有效预防和减少手术后的复发。
(2)由于补片的存在,不必过度向后高位牵拉骶韧带,从而使阴道顶端位于中央位置,解剖更加符合生理解剖。
(3)补片位于骶韧带和尿生殖筋膜之间的Okabayashi间隙的范围,内侧与直肠之间有骶韧带间隔,外侧有骶前筋膜和泌尿生殖筋膜间隔,上方低于腹下神经水平,距离输尿管更远。侵蚀的可能很低,远离阴道残端,不存在暴露的风险。
(4)补片较小,引起异物感染的可能较小,引起排异反应的可能也较小。

具体实施方式
下面结合附图和实施例,进一步阐述本发明。在下面的详细描述中,只通过说明的方式描述了本发明的某些示范性实施例。毋庸置疑,本领域的普通技术人员可以认识到,在不偏离本发明的精神和范围的情况下,可以用各种不同的方式对所描述的实施例进行修正。因此,附图和描述在本质上是说明性的,而不是用于限制权利要求的保护范围。
参考图1和图2,骶韧带1呈扇形结构,起自子宫颈和阴道上部后方,终止于第2~4骶椎前面。图中,1为骶韧带,2为漏斗韧带,3为圆韧带,4为主韧带,5为子宫。
参考图3和图4,一种加强人体内骶韧带强度的补片,简称补片6,其由生物可吸收材料制成,形状与骶韧带1外形相贴合,类似扇形结构或类三角形结构,大小与骶韧带、尿生殖筋膜之间的Okabayashi间隙范围相适应,尾侧边长约3cm,背侧边长约4~5cm,补片6的形态较小,引起异物感染的可能较小,引起排异反应的可能也较小。
实际手术时,根据患者体征大小不同对补片进行裁剪,将补片6置于患者骶韧带和尿生殖筋膜之间的Okabayashi间隙范围,补片6的尾侧边从子宫颈7(如切除子宫则阴道残端)到第四骶椎8筋膜(即骶四筋膜),背侧边从第四骶椎8筋膜到第二骶椎9筋膜(即骶二筋膜),上斜边低于输尿管和腹下神经水平,使用缝合线将补片的尾侧边和背侧边固定在宫颈、阴道后壁及骶骨筋膜上,固定后腹膜将补片覆盖起来,使补片附着在原有骶韧带位置上,促进骶韧带与补片表面发生粘连、肉芽增生,从而增加其强度。
由于补片的存在,手术时不必过度向后高位牵拉骶韧带,从而使阴道顶端位于中央位置,解剖更加符合生理解剖。
补片位于骶韧带和尿生殖筋膜之间的Okabayashi间隙的范围,内侧与直肠之间有骶韧带间隔,外侧有骶前筋膜和泌尿生殖筋膜间隔,上方低于腹下神经水平,距离输尿管更远。侵蚀的可能很低,远离阴道残端,不存在暴露的风险。

Claims (6)
1.一种加强人体内骶韧带强度的补片,由生物可吸收材料制成,其特征在于:形状与骶韧带外形相贴合,大小与骶韧带、尿生殖筋膜之间的Okabayashi间隙范围相适应。
2.如权利要求1所述的加强人体内骶韧带强度的补片,其特征在于:所述补片的尾侧边长2~4cm。
3.如权利要求1所述的加强人体内骶韧带强度的补片,其特征在于:所述补片的背侧边长4~5cm。
4.采用如权利要求1所述的加强人体内骶韧带强度的补片的手术方法,其特征在于:根据患者体征对补片进行裁剪,将补片置于患者骶韧带和尿生殖筋膜之间的Okabayashi间隙范围,使补片附着在原有骶韧带位置上,促进骶韧带与补片表面发生粘连、肉芽增生,从而增加其强度。
5.如权利要求4所述的手术方法,其特征在于,补片的附着位置为:尾侧边从宫颈到骶四筋膜,背侧边从骶四筋膜到骶二筋膜,上斜边低于输尿管和腹下神经水平。
6.如权利要求5所述的手术方法,其特征在于:使用缝合线将补片的尾侧边和背侧边固定在宫颈、阴道后壁及骶骨筋膜上,固定后腹膜将补片覆盖起来。
