Abstract
本发明提供一种用于修复女性盆底器官脱垂的悬吊网片,所述悬吊网片整体呈丫形,分为前侧上部网片、后侧上部网片、下部网片三者一体连接,所述前侧上部网片末端对称设有双长臂吊带结构。所述悬吊网片由聚丙烯单丝与聚对二氧环己酮单丝加入无毒性增塑剂进行纺丝编织而成。本发明通过在盆底网片的前侧上部网片上增加两个长臂网片,长臂网片可以通过耻骨固定网片,从而解决了盆底补片在手术过程中在松弛的筋膜处难以固定的问题。同时本发明通过优化网片本身的材料,有效地解决了可吸收类盆底补片在临床使用中降解速度快的问题,使手术的效果持续稳固。
Description
技术领域
本发明涉及治疗盆腔器官脱垂(POP)的外科手术设备,具体涉及一种用于修复女性盆底器官脱垂的植入体内的悬吊网片。
背景技术
盆底器官脱垂主要指盆腔内器官:膀胱、子宫、阴道及直肠因盆腔组织退化、创伤、先天发育不良或产后因素,引起的子宫、阴道和/或相邻器官发生病理性下降。盆腔器官脱垂包括阴道前壁膨出、子宫脱垂、阴道穹窿脱垂、阴道后壁膨出、直肠膨出等。手术途径包括经阴道、经腹及腹腔镜等,用网片完成的各种悬吊及全盆底重建术是目前治疗盆底脱垂的安全、微创的治疗。
随着社会人口的老龄化,POP已成为中老年妇女的常见病,严重地影响妇女的健康和生活质量,特别是妨碍妇女的工作和社会活动,被称为“社交癌”。在经济发达的国家,尿失禁比高血压、抑郁症和糖尿病等更常见,医疗费用已远高于冠心病、骨质疏松症和乳腺癌等,成为威胁妇女健康的5种最常见慢性疾病之一。目前中国广大女性盆底功能障碍的(PFD)患者中,有就诊意识的不到1/3,患者对于本病的认知差,或者认为是老年的必然表现,或由于羞怯等原因,没有主动就诊,就诊率很低。
许多中老年妇女随着年龄的增大,常常出现下腹不适,甚至外阴下坠有块物掉出。其实这些症状就是盆腔器官脱垂的临床表现。据统计,大约75%的女性会患有不同程度的盆腔器官脱垂。那盆腔器官脱垂是怎么发病的呢?我们可以把女性的盆底比作一张由肌肉、筋膜和韧带等构成的“吊床”,并且根据环境变化自我调节张力,吊床上就是盆腔内的膀胱,子宫以及直肠等各种脏器。随着年龄增大,受到妊娠、分娩以及雌激素水平下降等诸多因素的影响,吊床的支持作用逐渐减弱,以至于出现松弛和塌陷,从而吊床上的膀胱、子宫或者直肠等盆底脏器出现下坠。除去轻度脱垂,中重度脱垂以上脱垂的病人出现症状,会影响到患者的生活质量,所以需要进行手术治疗。脱垂的程度按照POP-Q分法分脱垂程度(I至IV度)。传统的盆底器官脱垂的治疗方法是将脱垂的子宫和膨出的阴道壁切除,把损伤的筋膜进行缝合修补,或者做膀胱悬吊固定。这种手术不仅将没有病变的子宫切除,缩短了阴道长度,创伤大,而且悬吊的结构仍然是缝合在已经松弛的盆底结构上,因此复发率很高。
近来随着外科网片材料的发展,各种网片盆底修复系统应运而生,盆底器官脱垂的治疗进入了全新的微创网片植入时代。使用网片行盆底重建术,具有无需开腹,创伤小,术后恢复快,且不易复发等优点。
尽管如此,目前盆底重建术中的外科网片材料仍存在不尽如人意之处。随着女性年龄的增长,身体机能退化,部分筋膜和肌肉出现松弛状况,导致市面上现有的盆底补片在手术过程中难以在松弛的筋膜处固定,且固定不牢固,常规术式固定点易于脱落。同时,可吸收类盆底补片在临床使用中降解速度快,使得手术效果难以持续。
发明内容
有鉴于此,为克服现有技术的不足,本发明提供一种治疗盆底脱垂的悬吊网片,该悬吊网片在手术中易于固定且固定牢固,不易降解,使手术的效果持续稳固。
本发明所述的悬吊网片整体呈丫形,分为前侧上部网片、后侧上部网片、下部网片三者一体连接构成,所述前侧上部网片末端对称设有双长臂吊带结构。
进一步,所述悬吊网片由聚丙烯单丝与聚对二氧环己酮单丝加入无毒性增塑剂纺丝编织而成。
进一步,所述无毒性增塑剂为邻苯二甲酸酯。
或者,所述悬吊网片由聚二恶烷酮PDO纺丝编织而成。
进一步,所述前侧上部网片长8.0~9.0厘米、宽1.5~2.5厘米,后侧上部网片长8.0~9.0厘米、宽1.5~2.5厘米,下部网片长10.0~11.0厘米、宽1.5~2.5厘米,所述双长臂吊带结构长10.5厘米,宽0.5厘米~1.0厘米。
进一步,所述前侧上部网片长8.5厘米、宽2.0厘米,后侧上部网片长8.5厘米、宽2.0厘米,下部网片长10.5厘米、宽2.0厘米,所述双长臂吊带结构长10.5厘米,宽0.75厘米。
本发明的有益效果在于:
1.本发明通过在盆底网片的前侧上部网片上增加两个长臂网片吊带结构,长臂网片通过耻骨固定网片,从而解决了盆底补片在手术过程中在松弛的筋膜处难以固定的问题。
2.本发明通过采用慢降解可吸收材料制作悬吊网片,利用材料本身降解速度慢的特定,有效地解决了可吸收类盆底补片在临床使用中降解速度快的问题,使手术的效果持续稳固。
3.本发明提供的悬吊网片能够帮助临床医生在手术过程中根据病人的情况,高效地选择固定方式。

如图2、3、4所示,本发明所述悬吊网片整体8呈丫形,由前侧上部网片1、后侧上部网片2、下部网片3一体连接构成,所述前侧上部网片1末端对称设有双长臂吊带结构4。
所述悬吊网片由聚丙烯单丝与聚对二氧环己酮单丝加入无毒性增塑剂纺丝编织而成。
所述无毒性增塑剂为邻苯二甲酸酯。
所述前侧上部网片1长8.5厘米、宽2.0厘米,所述后侧上部网片2长8.5厘米、宽2.0厘米,所述下部网片3长10.5厘米、宽2.0厘米,所述双长臂吊带结构长10.5厘米、宽0.75厘米。
如图5所示体内大肠、子宫及膀胱的位置,所述悬吊网片的上部分网片分别固定在子宫两侧,下部分网片固定在筋膜位置。
实施例2
与实施例1基本相同,所不同之处在于,所述悬吊网片由聚二恶烷酮PDO纺丝编织而成。
所述前侧上部网片1长9.0厘米、宽2.5厘米,所述后侧上部网片2长8.0厘米、宽1.5厘米,所述下部网片3长11.0厘米、宽2.5厘米,所述双长臂吊带结构长10厘米、宽1.0厘米。

2.按照权利要求1所述的悬吊网片,其特征在于,所述悬吊网片由聚丙烯单丝与聚对二氧环己酮单丝加入无毒性增塑剂纺丝编织而成。
3.按照权利要求2所述的悬吊网片,其特征在于,所述无毒性增塑剂为邻苯二甲酸酯。
4.按照权利要求1所述的悬吊网片,其特征在于,所述悬吊网片由聚二恶烷酮PDO纺丝编织而成。
5.按照权利要求1所述的悬吊网片,其特征在于,所述前侧上部网片长8.0~9.0厘米、宽1.5~2.5厘米,后侧上部网片长8.0~9.0厘米、宽1.5~2.5厘米,下部网片长10.0~11.0厘米、宽1.5~2.5厘米,所述双长臂吊带结构长10.5厘米,宽0.5厘米~1.0厘米。
