Description

Traditional Chinese medicine composition for treating myelodysplastic syndrome, preparation method and application thereof, and honeyed pills
Technical Field
The invention relates to the technical field of extraction of traditional Chinese medicine compositions, in particular to a traditional Chinese medicine composition for treating myelodysplastic syndrome, a preparation method and application thereof, and honeyed pills.
Background
Myelodysplastic syndrome (MDS) is a heterogeneous group of clonal diseases of the myeloid lineage that originate from hematopoietic stem cells and is characterized by abnormal development of myeloid cells, manifested by ineffective hematopoiesis, refractory cytopenia, and high risk of transformation into acute myeloid leukemia. According to the pathogenesis characteristics and clinical manifestations, the traditional Chinese medicine is defined as marrow toxicity strain, wherein the marrow represents the disease position, the toxicity represents the disease property, and the strain represents the disease state. The pathogenesis of the marrow toxicity is the deficiency of vital qi, repeated affection of pathogenic toxin, internal retention of the pathogenic toxin, lying in bone marrow, blocking the biochemistry of qi and blood, blood stasis caused by the toxin, toxic stasis mutual resistance, inability to generate essence and blood, resulting in essence deficiency and blood deficiency, shortness of breath and weakness, and presenting the symptoms of deficient and impaired symptoms. The pathogenesis is characterized in that the disease belongs to the syndrome of excess pathogenic factors and deficiency-excess mixed disease, toxin and stasis are excess pathogenic factors, deficiency of qi and blood and yin and yang is deficiency vital qi, the relationship of mutual elimination and growth exists between the vital qi and the pathogenic factors, the deficiency of the vital qi is aggravated by the injury of the vital qi caused by excess pathogenic factors, and the excess pathogenic factors are more predominant by the failure of the vital qi to resist the pathogenic factors.
At present, modern medicine is mainly used for treating myelodysplastic syndrome, and although some traditional medical treatment formulas are available in recent years, the treatment effect is not ideal.

Intermittent oral taking of medicine for treating hematopathy. The current symptoms are as follows: debilitation, aggravation after activity, intermittent fever at night, dry mouth, restlessness, insomnia, short breath, poor diet, general sleep, frequent urination, dry stool. A red tongue with yellow coating and a deficient and rapid pulse. Bone marrow biopsy report: myeloproliferation is approximately normal, and granulocytic, erythrocytic and megakaryocytic hyperplasia with abnormal morphology. Blood routine: neutrophil count: 0.62X 109L, red blood cells: 2.04X 1012L, hemoglobin: 63g/L, platelets: 39X 109/L。

1.2 diagnostic criteria for myelodysplastic syndrome
1.2.1 Western diagnosis:
the diagnosis of myelodysplastic syndrome requires 2 requirements and 1 determination criterion.
The requirements are as follows:
(1) continuous (more than or equal to 6 months) primary or multi-blood cell reduction; hemoglobin (HGB) <110g/L, neutrophil absolute count (ANC) < 1.5X 109/L, Platelets (PLT) < 100X 109/L;
(2) other hematopoietic and non-hematopoietic disorders that can lead to cytopenia and dysplasia are excluded.
Determination of the standard:
(1) dysplasia: the proportion of dysplastic cells in the erythroid, granulocytic and megakaryocytic cell lines in the bone marrow smear was > 10%.
(2) The proportion of the annular sideroblasts to the nucleated erythrocytes is 15 percent; or 5% of the proportion of nuclear erythrocytes occupied by the circular sideroblasts accompanied by the SF3B1 mutation.
(3) Primary cells: the content of the smear in the bone marrow reaches 5 to 19 percent.
(4) MDS is a common chromosomal abnormality.
1.3 treatment regimens
The study subjects were randomly divided into four groups according to sex, mild disease and moderate disease, and four groups according to severe disease and critical illness (1 patient in critical illness was divided into group 1), wherein group 1 took 15 doses of the Chinese medicinal composition prepared in example 1, 300ml per dose, 150ml once, twice a day, and orally; group 2 took 30 pills of the honeyed pill prepared in example 2, 1 pill/time and 2 times/day, and was orally taken; group 3 takes 15 doses of the traditional Chinese medicine composition prepared in the example 3, each dose is 300ml, 150ml is taken once, and the traditional Chinese medicine composition is taken twice a day; group 4 takes 15 doses of the Chinese medicinal composition prepared in example 4, each dose is 300ml, 150ml is taken once, twice a day and orally; group 5 took 15 doses of the Chinese medicinal composition prepared in comparative example 1, each dose was 300ml, 150ml was once, twice a day, and taken orally.
1.4 therapeutic criteria
Firstly, symptom observation: observing the improvement condition of the traditional Chinese medicine treatment on the clinical symptoms of the patients, such as shortness of breath, hypodynamia, lusterless complexion, dry mouth and throat, bleeding position and the like.
Evaluation indexes are as follows: the symptoms of traditional Chinese medicine are divided into four grades according to clinical observation:
(1) mild case of slight card
(2) Medium and high grade
(3) Serious syndrome
(4) Critical syndrome/pattern
Treatment was recorded as the appearance of symptoms.
The evaluation method comprises the following steps: comparison of Total integral of symptoms before and after treatment (before/after treatment)
The effect is shown: the clinical symptoms and physical signs of the traditional Chinese medicine are obviously improved, and the syndrome integral is reduced by 70 to 94 percent.
The method has the following advantages: the clinical symptoms and physical signs of the traditional Chinese medicine are improved, and the syndrome integral is reduced by 30-69%.
And (4) invalidation: the clinical symptoms and physical signs of the traditional Chinese medicine are not obviously improved or even aggravated, and the syndrome integral is reduced by less than 30 percent.
1.5 analysis of results
Chinese medicine symptom integration result (unit: case)
Figure GDA0003549715610000121
Figure GDA0003549715610000131
It can be seen from the results of groups 1 and 5 that the total effective rate of treatment of myelodysplastic syndrome is significantly improved by replacing fructus cannabis with poria cocos, angelica sinensis, scutellaria baicalensis, gypsum and polygala tenuifolia. Mainly because the body fluid of the myelodysplastic syndrome is slightly consumed, the fructus cannabis is removed; in addition, because the syndrome is the symptoms of essence deficiency and blood deficiency, qi deficiency and deficiency, and deficiency and damage in one group, the angelica is added to enhance the effect of enriching the blood; in addition, the syndrome is also a syndrome of deficiency of both qi and yin, and consumes qi and damages yin, the treatment principle of the gypsum and the scutellaria baicalensis in combination with the ginseng and the ophiopogon root in tonifying qi, nourishing yin and clearing heat is to nourish heart yin, clear heart and calm nerves, and the polygala tenuifolia in combination of calming heart and calming nerves, so that the medicines are cooperatively combined to play a role in treating myelodysplastic syndrome.
In summary, the application examples 1 to 3 show that the traditional Chinese medicine composition and the honeyed pills prepared by the invention can effectively improve the symptoms of myelodysplastic syndrome, have the total effective rate of more than 40 percent, and can relieve the progress of the myelodysplastic syndrome.
In addition, the Chinese medicinal composition prepared in example 1 has the best effect by comprehensively considering the treatment effect.
Although the present invention has been described in detail with reference to the above embodiments, it is only a part of the embodiments of the present invention, not all of the embodiments, and other embodiments can be obtained without inventive step according to the embodiments, and the embodiments are within the scope of the present invention.
