Abstract

发明背景
胶质母细胞瘤(GBM)是人类最常见的恶性脑肿瘤。胶质母细胞瘤也称为多形性胶质母细胞瘤,是一组被称为星形细胞瘤的肿瘤中的一种。典型地,起始于星形胶质细胞,滋养并支持大脑中神经细胞的一种星形的细胞。胶质母细胞瘤周围有大量喂养该肿瘤的血管,使其在大脑内部迅速生长。胶质母细胞瘤是成人中被诊断的最常见的恶性原发性脑肿瘤,在美国每年预计有12000-13000个新病例发生。
胶质母细胞瘤预后性较差。目前只有肿瘤切除,放疗和替莫唑胺化疗可能对胶质细胞瘤患者表现一定程度的临床益处。然而,存活时间通常只有约14-18个月的范围,并且5年的存活率低于10%。
因此,迫切需要开发能够促进生存的新疗法。
嵌合抗原受体(CAR)T疗法是近年来美国临床肿瘤学会提名的肿瘤领域最重要的进展(见临床癌症进展2018:美国临床肿瘤学会抗癌进展年度报告。临床肿瘤期刊2018:JCO2017770446)。迄今为止,美国食品药品监督管理局已经批准了两种商业化的CAR T产品,用于治疗急性淋巴白血病,包括诺华公司的Tisagenlecleucelel(也称为Kymariah)和凯特医药(Kite Pharma)公司的Axicabtageneciloleucel(也称为Yescarta)。鉴于其在血液系统恶性肿瘤中卓越功效,人们努力将CAR T疗法应用于实体瘤。但是,该领域仍处于起步阶段,需要更积极的临床结果来验证该方法。另一方面,与其他实体瘤不同,胶质母细胞瘤通常对开发有效的疗法提出了独特的一系列挑战。
发明概要
本发明提供了一种用于治疗胶质母细胞瘤的以CAR T疗法为基础的方法。在研究期间,该方法出乎意料地表现出对胶质母细胞瘤的临床受试者明显的益处。
本发明的一个方面涉及治疗胶质母细胞瘤或其他脑肿瘤的方法,该方法包括步骤:(i)制备包含嵌合抗原受体(CAR)分子的细胞;和(ii)对有此需要的哺乳动物施用有效量的所制备的细胞。CAR分子含有可与胶质母细胞瘤或其他脑肿瘤相关的肿瘤抗原结合的抗原结合域,并且该肿瘤抗原可以是碳酸酐酶IX。
特别地,施用所制备的细胞是通过颅内注射,颅内注射直接靶向胶质母细胞瘤或其他脑肿瘤的边界内。
优选立体定向进行颅内注射。
在上述方法中,所制备的细胞进一步包括与CAR分子结合使用的药剂(agent),以增加治疗效果。
在一个实施方式中,该药剂可以是刺激淋巴细胞增殖的分子。这种分子的例子包括白介素7。
在另一个实施方式中,该药剂可以是通过趋化性内源性免疫细胞募集的以消除胶质母细胞瘤或其他脑肿瘤的分子。该分子的例子包括趋化因子(C-C基序)配体19。
本发明的方法可进一步包括额外的步骤。该步骤的一个例子是,在施用所制备的细胞前,先用抑制血管内皮生长因子的疗法治疗胶质母细胞瘤或其他脑肿瘤。特别地,抑制血管内皮生长因子的疗法可以是施用有效量的贝伐珠单抗(Bevacizumab)(阿瓦斯汀(Avastin))。
本发明的方法可以应用于哺乳动物,例如人或老鼠。

图9显示了用于肿瘤浸润淋巴细胞的流式细胞术分析的设门策略。我们首先使用SSC-FSC门来排除没有细胞碎片,然后通过FSC-H-FSA-A门来排除二倍体。使用活-死染色排除死细胞。然后基于GFP+肿瘤细胞标志物的表达对活细胞进行门控。GFP-细胞被认为是包括白细胞在内的无肿瘤细胞。然后进一步基于CD3、CD4和CD8表达对GFP细胞进行表型分析。CD3+CD4+细胞被门控为CD4+淋巴细胞,而CD3+CD8+细胞被门控为CD8+淋巴细胞。

发明详述
本发明提供了用于治疗患有胶质母细胞瘤或其他类型脑肿瘤的个体的方法。所述方法的方面包括向对碳酸酐酶IX(CAIX)具有特异性的单独的CAR-T细胞施以可有效破坏肿瘤的量。还提供了包括生物工程产品的可用于实践本主题方法的试剂。
在描述本方法之前,应当理解,本发明不限于所述的特定方法,因此当然会有所不同。还应理解,本文中使用的术语仅出于描述特定实施方式的目的,而不旨在进行限制,因为本发明的范围仅受所附权利要求书的限制。
主题方法主要用于治疗目的。因此,如本文所用,术语“治疗”用于指疾病的预防以及对预先存在情况的治疗。本发明提供的特别重要的益处是为了稳定或改善患者临床症状,对持续疾病进行治疗。该治疗期望在包括中枢神经系统及其周围组织在内的受影响组织失去功能之前进行。例如,对癌症患者的治疗可以是减小肿瘤大小、消除恶性细胞或防止病情已经缓解的患者复发。
当在权利要求和/或说明书中使用术语“抑制”、“减少”或“预防”或这些术语的任何变形形式均包括任何可测量的减少或完全抑制,以达到期望的结果。

免疫效应细胞是在免疫反应中保护机体的瞬时激活细胞。一旦触发的抗原/病原体被清除,免疫效应细胞最终停止增殖并死亡。效应B细胞被称为浆细胞并分泌抗体,活化的T细胞包括细胞毒性T细胞和辅助性T细胞。
“免疫疗法”是指通过调节对疾病抗原的免疫应答来治疗疾病(例如,癌症)。在本发明的上下文中,免疫疗法是指通过施用抗体(例如,单克隆抗体)和/或通过施用引发抗肿瘤抗原免疫应答的抗原的方式提供在受试者中的抗肿瘤免疫应答。
通过上述方法制备的CAR-T细胞在应用于受试者之前会被充分富集或被充分分离。
如本文所用,术语“充分富集”或“充分分离”是指细胞群体从包含所需细胞群的原始混合细胞群中富集至少约20倍,更好的是至少约500倍,甚至更好地至少约5000倍或更多。
术语“抗体”与“免疫球蛋白”可互换使用。它涵盖了多克隆抗体和单克隆抗体药剂,其中抗体可能是任何感兴趣的类别(例如IgG,IgM及其亚类),以及包括杂交抗体、变异抗体、F(ab').sub.2片段、F(ab)分子,Fv片段、在噬菌体上显示的单链可变片段(scFv)、单链抗体、单域抗体、双抗体、嵌合抗体、人源化抗体及其功能片段,该功能片段表现亲代抗体分子的免疫结合特性。
术语“单克隆抗体”是指具有同质抗体群体的抗体组合物。该术语不受其合成方式的限制。该术语包括完整的免疫球蛋白分子,以及Fab分子、F(ab')2片段、Fv片段,噬菌体上显示的单链可变片段(scFv)、包含抗体和非抗体蛋白二者的抗原结合部分的融合蛋白,以及表现亲代单克隆抗体分子的免疫结合特性的其他分子。
本领域技术人员理解如何制备和筛选多克隆和单克隆抗体。
术语“抗原”和“表位”在本领域众所周知,是指大分子(例如,多肽)的一部分,其被免疫系统的组成部分,例如抗体或T细胞抗原受体特异性识别。如本文所用,术语“抗原”包括抗原表位,例如作为抗原表位的抗原片段。表位可以被溶液中的抗体识别,例如从其他分子中游离出来的。当表位与I类或II类主要组织相容性复合物分子缔合时,表位可以被T细胞抗原受体识别。
在抗体特征的上下文中,术语“抗体的特异性结合”或“抗原特异性抗体”是指抗体优先结合存在于不同抗原的均质混合物中的特定抗原的能力。在某些实施方式中,特异性结合作用可以区分样本中的目标和非目标抗原(或“靶向”和“非靶向”抗原),在一些实施方式中,超过约10至100倍或更多(例如,超过约1000或10000倍)。在某些实施方式中,当抗体和抗原特异性结合为抗体-抗原复合物时,二者的亲和力以KD(解离常数)低于10-6M,低于10-7M,低于10-8M,低于10-9M,低于10-9M,低于10-10M,低于10-11M,低于10-12M或更少来表征。
“有效量”是足以产生有益或期望的临床结果的量。可以一次或多次施用来施用有效量。为了本发明的目的,试剂抗体的有效量是足以诊断、减轻、改善、稳定、逆转、减慢或延迟疾病状态发展的量。
通常来说,血脑屏障(BBB)包括对大脑组织输送营养和氧气的血管。由于血脑屏障,脑肿瘤无法转移到中枢神经系统外的器官。这种现象通过限制全身CAR-T细胞的不良反应而帮助了CAR-T疗法的颅内应用。尽管一些研究人员已经证明,通过外周静脉注射CAR-T细胞可以在GBM中发现,并且其具有细胞毒性,但全身使用CAR-T细胞的剂量比颅内应用的剂量高几个数量级。即使使用多次注射策略,患者颅内注射所需的CAR-T细胞数量也很容易满足。GBM扩散的唯一常见方法是脊髓转移。幸运的是,脑室内注射CAR-T细胞能够控制脊髓转移(10.1158/1078-0432.CCR-15-0428)。本发明中,尽管总数很小,但60%的完全康复率已经超出了预期,并证明了CAIX CAR-T本身是用于GBM治疗的非常有前景的一种方法。
临床上,手术仍然是GBM的首选。然而,总可以预料到会残留肿瘤细胞,因为GBM细胞可以被高度浸润,并且进行扩大切除以清除所有恶性细胞是不现实的。在这种情况下,可以在手术中或之后容易地进行CAR-T细胞颅内注射,以尽可能多地移除残留的肿瘤细胞。同时,可以一起颅内注射CAR-T细胞,以帮助降低肿瘤细胞在中枢神经系统中扩散的风险。
另一方面,CAIX是一种位于膜上的蛋白质,通过维持细胞内pH来发挥作用。CAIX在正常细胞中温和表达,可通过低氧诱导因子1α由低氧条件诱导。由于肿瘤细胞的糖酵解活性增强和肿瘤微环境中的缺氧,CAIX过度表达,并且其对于在各种类型的肿瘤中的肿瘤细胞的生存至关重要。在许多癌症中,也发现CAIX过表达会促进肿瘤进展,并与不良预后有关。
在肾细胞癌中,其特征是由于VHL突变的频繁功能丧失而导致缺氧信号增强,靶向CAIX的CAR T治疗在小鼠模型中显示出抗肿瘤作用。由于患者产生了抗CAR T细胞的体液和细胞免疫应答,因此靶向CAIX的CAR T治疗转移性肾细胞癌的I/II期试验失败。
然而,我们进行了概念验证研究以证明CAIX可能作为GBM的CAR-T靶标。由于缺氧或假性缺氧,CAIX是一种可诱导的膜定位蛋白。使用CAIX作为靶标利用了GBM的快速增殖。正常脑组织和I至III级的胶质母细胞瘤似乎CAIX表达的概率低。本发明中可检测到CAIX的GBM为60-70%,这与先前的研究相似(10.1093/neuonc/nos216)。常氧状态下的GBM细胞系显示出CAIX低表达,但其中一些仍可被CAR-T细胞杀死。这可能是由于CAR-T细胞耗氧导致的相对局部缺氧,该现象在人类患者中也可以发生。当与T细胞共培养时(数据未显示),幼稚U251细胞中CAIX的表达增加。因此,在向人GBM中注射CAR-T细胞时,除了微血管受损的影响之外,T细胞还可以进一步诱导CAIX表达。
对其他CAR-T的分析显示树突状细胞和T细胞向肿瘤组织的渗透增加。施用CAR-T细胞之前受体T细胞的消耗会对CAR-T细胞的治疗效果产生负面影响,表明CAR-T细胞和受体免疫细胞协同发挥抗肿瘤活性。
此外,与其他药剂组合可增益CAR-T细胞疗法,例如刺激淋巴细胞增殖或通过趋化性内源性免疫细胞募集以消除肿瘤的药剂。在本发明的方法中,制备的CAR-T细胞还可以包括用于刺激淋巴细胞增殖的白介素7或用于通过趋化性内源性免疫细胞募集的趋化因子(C-C基序)配体19。
CAR-T疗法与其他疗法相结合已经发现可以更好地控制肿瘤。据信CAIX CAR-T与抗血管生成剂如阿瓦斯汀(Avastin)或索拉非尼(sorafenib)的组合应表现出更好的疗效。一方面,抗血管生成可使肿瘤微环境缺氧并诱导CAIX表达。另一方面,据报道,缺氧可增强细胞毒性T细胞的功能。
如本文所用,术语“与……结合”可用于,例如在二次治疗的整个给药过程中进行一次治疗;一次治疗的施用与二次治疗的施用存在一定时间的重叠,例如在施用二次治疗之前施用一次治疗并且在结束二次治疗之前结束一次治疗;在施用一次治疗之前先施用二次治疗并且在结束一次治疗结束前结束二次治疗;在施用二次治疗之前施用一次治疗并且在一次治疗结束前结束施用二次治疗;在施用一次治疗开始之前先施用二次治疗并且再二次治疗结束前先结束一次治疗。这样“与组合”也可以指涉及两种或更多种疗法的给药方案。如本文所用,“与...组合”也指以相同或不同的药剂,通过相同或不同的途径,以相同或不同的剂型施用两种或更多种疗法。
但是,CAR-T治疗和抗血管生成剂的顺序可能很重要,需要进一步的实验确定。如果理由明确,也可以结合其他临床使用的疗法。例如,由于检查点抑药剂可减少CAR-T细胞的消耗,因此CAR-T疗法有望与检查点抑药剂一起使用。
实施发明的最佳模式
以下实施例可以更具体地阐述本发明,但不限制本发明的范围。
实施例1
细胞培养和试剂
HEK293T细胞和胶质母细胞瘤细胞系包括U251、LN 229、T98G和A172均来自美国典型培养物保藏中心(ATCC;马纳萨斯,弗吉尼亚州)。所有细胞均在辅以10%胎牛血清(FBS;Gibco)和1%青霉素和链霉素(Gibco)的杜氏改良鹰(Dulbecco's Modified Eagle)培养基(DMEM;Gibco)中培养。U251-luc细胞是通过将含荧光素酶的慢病毒(EF1a-ffLuc2-eGFP)稳定转染到幼稚U251细胞中合成的。
人体样本采集
冷冻的胶质母细胞瘤组织从国立卫生研究院(NIH;贝塞斯达,马里兰州)中国家神经疾病和中风学会(NINDS)的神经外科的组织库里得到。福尔马林固定且石蜡包埋的胶质母细胞瘤组织取自复旦大学华山医院(中国上海)。
免疫印迹和免疫组化
免疫印迹实验操作如下。简而言之,从冷冻的组织或细胞系中收集蛋白质。使用共40μg蛋白质进行电泳,然后转移至硝酸纤维素膜上。用5%脱脂乳封闭后,将膜与一抗(1:1000稀释)在4℃孵育过夜,然后孵育二抗(1:3000稀释;来自细胞信号科技)。抗CAIX抗体购自挪沃斯生物(Novus Biologicals)(利特尔顿,科罗拉多州)。
流式细胞术
如下所示处理并收集细胞。根据制造商的规程,使用APC偶联的抗CAIX抗体(研究和发展系统,明尼阿波利斯市,明尼苏达州)在黑暗中将细胞(1μg)染色1小时。使用BD FACSCanto II流式细胞仪(BD生物科技(BD Biosciences),圣河西,加利福尼亚州)对细胞使用流式细胞仪检测之前加入4',6-二脒基-2-苯基吲哚(DAPI)。使用FlowJo软件(FlowJo,阿什兰,俄勒冈州)分析数据。
CAIX CAR表达载体的产生
使用pLenti-EF1a-C-mGFP标记克隆载体(奥利基因科技(OriGeneTechnologies),洛克维尔,马里兰州)生成CAIX CAR表达载体(Lenti-EF1a-CAIX-3rd-CAR)。简而言之,将原始载体上的mGFP序列替换为CAR盒,包括信号肽、抗CAIX单链可变片段(scFv)、CD8铰链、CD28跨膜细胞内结构域、4-1BB和CD3zeta。通过酶切和桑格定序(Sangersequencing)实验确定最终的载体。
慢病毒的产生和转导
慢病毒包膜表达质粒pMD2.G和包装质粒psPAX2分别是Addgene质粒#12259和12260。将pMD2.G、psPAX2和Lenti-EF1a-CAIX-3rd-CAR质粒以2:4:5的比例转染到在DMEM中培养的不含抗生素的HEK293T细胞中。每天更换培养基,并在接下来的两天收集上清液。慢病毒通过HIV-1p24抗原酶联免疫吸附实验(泽普洛马特科斯(ZeptoMetrix),布法罗市,纽约州)定量检测,并使用Lenti-X浓缩器(克隆技术实验室(Clontech Laboratories),山景城,加利福尼亚州)进行浓缩。
外周血单核细胞(PBMC)取自国家国立卫生研究院临床中心的血液库招募的健康捐献者,并保存在液氮中直至使用。将PBMC在RPMI 1640中融化过夜,并用Dynabeads人体T激活剂CD3/CD28(赛默飞科技)以1:1的比例在补充了5%人血清(吉布科(Gibco))的AIM V培养基(吉布科(Gibco))中活化24小时。使用淋巴细胞分离介质富集活细胞,并用磷酸盐缓冲液(PBS;Gibco)洗涤两次。然后在32℃的V型底96孔板(康宁,康宁市,纽约州)中以1200g将T细胞与含有CAIX CAR载体或空载体的慢病毒转导2小时。每个孔中含有0.25百万个细胞和病毒,其MOI为40,携带8μg/ml聚丙烯(西格玛奥德里奇)和300个国际单位(IU)的人白介素2(hIL-2;派普泰克,落基山,新泽西州)。3小时后将转导的细胞重悬,并转移至6孔板中,在100IU hIL-2存在情况下扩增2-3天。
酶联免疫吸附测定(ELISA)
如下所示处理细胞48小时,并收集上清液。以5000g离心5分钟从样品中移除细胞和细胞碎片,然后样品保持在-80℃直至使用。将来自小鼠的眼眶窦中的血样收集至含EDTA的试管中,然后以10000g离心10分钟去除血细胞,血浆保存在-80℃直至使用。
根据制造商的说明,分别使用人TNF酶联免疫吸附试剂盒II(BD生物科技(BDBiosciences),圣河西,加利福尼亚州)和人IFN gamma ELISA Read-SET-Go!试剂盒(昂飞,圣地亚哥,加利福尼亚州)测定TNF-α和IFN-γ的浓度。
异种移植小鼠模型
小鼠实验已获得NINDS和国家癌症研究所(NCI)动物使用与护理委员会的批准。向NOD-PrkdcscidIl2rgtmiWjl(NSG)小鼠(来自NCI-弗莱德里克(NCI-Frederick)动物机构的6-8周大)颅内接种100000个悬浮于2μL汉克平衡盐溶液(HBSS;科晶,康马克,纽约州)中的U251-luc细胞。一周后,检测荧光素信号以确认小鼠中肿瘤细胞的生存情况。根据信号强度将小鼠分为指定组,以保持基线平衡。总共将2-2.5μL HBSS中的200万抗CAIX CAR-T细胞,或空载体转导的T细胞,或模拟(对照T细胞)中注射入肿瘤。未经治疗的小鼠注射了相同体积的HBSS。每周两次腹膜内注射阿瓦斯汀(Avastin),剂量为10mg/kg体重,持续30天。每三天监测一次肿瘤的生存情况。所有动物研究的生存终点均定义为达到以下标准的任何一个:1)体重损失超过15%;2)颅骨突出;3)头枕;4)驼背;5)共济失调,6)毛发粗糙或7)行动不便。
肿瘤浸润淋巴细胞(TIL)的分离
小鼠颅内接种悬浮于2μL HBSS中的100,000个U251-luc细胞,并按上述方法治疗一周。治疗3周后,杀死小鼠并切除肿瘤。在使用肿瘤分离试剂盒(美天旎生物技术(Miltenyi Biotec))进行酶消化处理的存在下,使用Gentle MACS Dissociator(美天旎生物技术(Miltenyi Biotec),贝吉施格拉德巴赫,德国)对肿瘤进行机械破坏。短暂旋转后收集上清液。10000g离心10分钟进一步从上清液中除去细胞和细胞碎片,并将样品保存在-80℃直到进行酶联免疫吸附分析。
将含有T细胞的悬浮液在FACS缓冲液中用抗人CD3抗体(#317332)、CD4抗体(#300514)、CD8抗体(#301032)(生物传说(Biolegend),圣地亚哥,加利福尼亚州)染色,然后通过BD FACS Canto II流式细胞仪进行分析(BD生物科技(BD Biosciences),圣河西,加利福尼亚州)。使用FlowJo软件(FlowJo,阿什兰,俄勒冈州)进行数据分析。
统计分析
如本发明所示,数据表示为平均值和标准偏差(SD)或平均值的标准误差(SEM)。使用卡普兰-米儿(Kaplan-Meier)评估得出生存曲线。使用Prism 6(格拉夫帕软件(GraphPadSoftware),圣地亚哥,加利福尼亚)进行统计分析。使用对数秩检验比较生存曲线。使用未配对的学生t检验可分析其他变量。Ap<0.05被认为具有统计学意义。
结果
GBM中CAIX的过表达
为了证明CAIX是GBM的潜在靶标,研究测试CAIX在3个III级和5级IV胶质瘤样本中的表达。五分之三的GBM(IV级神经胶质瘤)样本检测到了CAIX的过度表达,而在III级神经胶质瘤中未检测到CAIX(图1A)。进一步研究测试确认其他27个切除的GBM样品的结果,其中18个CAIX染色呈阳性(图2A)。同样地,来自TCGA数据库的大量样本说明GBM组织中的CAIX转录相比相对正常的组织有明显增加(图3A和3B)。高频的CAIX过度表达和高CAIX转录患者的预后明显较差表明CAIX可能成为有前景的GBM治疗靶标。
但是,体外培养的GBM细胞系通常表达低水平的CAIX,但可通过缺氧条件显著诱导(图1B和1C)。在一项测试CAIX CAR-T细胞体外功效的研究中,我们使用幼稚的和内源性CAIX转染的GBM细胞系(图4A和4B),它们被证实CAIX在细胞膜上高表达(图4A和4B)。
CAIX CAR-T的产生
将第三代CAR-T载体(图5A)转导至供体来源的T细胞。流式细胞分析显示约30%T细胞表达了CAR(图5B)。

首先,使用幼稚的和经CAIX转染的U251细胞测试CAIX CAR-T细胞的特异性细胞毒性,注意到CAR-T细胞在CAIX+细胞上表现出强的细胞毒性,但对幼稚U251细胞几乎没有影响(图5C和5D)。然后,使用不同数量的CAIX CAR-T细胞和未转染的对照T细胞与CAIX转染的U251细胞以恒定的效应子/肿瘤比(E/T)为4进行共培养。大量的细胞会表现出更显著的CAR-T细胞细胞毒性(图5C和5D),表明抗原密度在CAIX CAR-T功效中具有重要作用。此外,我们使用缺氧诱导模型以测试CAIX CAR-T细胞的功效,其可以更生理化地模拟CAIX表达的体内诱导。幼稚U251细胞上的CAR-T细胞暴露于低氧条件下时可以观察到细胞毒性显著增强(图5D)。在LN229和T98G细胞中也观察到了相似的结果(图6A和6B)。
为了验证CAIX介导了该效果,我们敲除了U251细胞中的CAIX基因,其中缺氧不能诱导CAIX表达(图7A-7C)。正如预期,即使抗CAIX CAR-T细胞预先暴露于缺氧状态,他们也无法杀死CAIX缺陷细胞(图7A-7C)。同样地,对CAIX表达诱导不敏感的GSC827几乎没有对抗CAIX CAR-T细胞的响应,而在缺氧状态下过表达CAIX的GSC923表明其对于我们的CAR-T治疗良好的响应(图7A-7C)。总之,这些结果表明,在存在CAIX抗原的情况下,CAR-T细胞的功能性激活与其细胞毒性有关。
与CAR-T细胞的细胞毒性一致,在存在CAIX CAR-T细胞而非对照T细胞(无转染载体)或模拟T细胞(骨干载体转染)的情况下,可以观察到IFN-γ,TNF-α和IL-2的水平升高(图5E)。此外,在转染CAIX(图5E)或缺氧暴露的GBM细胞(图6B)中,这些细胞的细胞因子的分泌显著上调。这些结果表明,在存在CAIX抗原的情况下,CAR-T细胞的功能激活与其细胞毒性有关。
CAIX CAR-T在体内具有抗肿瘤作用
为了进一步验证CAIX CAR-T在体内的功效,生成了颅内小鼠模型(图8A)。在接种U251-luc细胞约一周后,原位注射CAR-T。与使用媒介注射小鼠相比,CAIX CAR-T细胞显著限制了肿瘤的生长并延长了这些小鼠的存活时间(图8B和8C)。为了增强功效,在小鼠模型中尝试了多次注射策略,CAR-T组的肿瘤生长明显延迟(图8B和8C)。令人惊讶的是,CAIXCAR-T细胞治愈了十只最初治疗的小鼠中的两只,而对照T细胞没有显示出这种功效(图8D)。
肿瘤细胞病理学分析显示了注射CAR-T后免疫细胞的显著浸润以及肿瘤内细胞因子的释放。治疗两周后采集脑肿瘤并用流式细胞术分析人类T细胞标记物(CD3、CD4和CD8),然后设门策略如图9所示。使用肿瘤浸润淋巴细胞(TILs)分析评价CAR-T细胞对CAIX抗原的响应。在脑肿瘤中,我们观察到用抗CAIX CAR-T细胞治疗的小鼠中CD3+T细胞增加(图10A)。特别地,使用抗CAIX CAR-T细胞治疗可导致CD8+T细胞和CD4+T细胞的丰度明显增加(图10B-10D)。这说明CAIX抗原刺激后CAR-T细胞具有更强的存活和/或增殖能力。值得注意的是,与注射前的CD4+T细胞相比,细胞毒性CD8+T细胞在存活和/或增殖方面具有更好的优势(图10D)。此外,我们还观察到肿瘤上清液和血液中IFN-γ、TNF-α和IL-2的分泌也有类似的趋势,即使用抗CAIX CAR-T细胞治疗使细胞因子分泌增多(图10E和10F)。
此外,为利用胶质母细胞瘤发展而引起的自然缺氧,我们使用抗血管生成药剂如阿瓦斯丁(Avastin)和索拉非尼(sorafenib),在肿瘤微环境中药理上诱导缺氧条件,可能进一步提高抗CAIX CAR-T细胞的疗效。我们发现,阿瓦斯丁(Avastin)和抗CAIX CAR-T的组合协同抗肿瘤效果优于单独使用任何一种疗法(图11A和11B)。

Claims (10)
1.一种治疗胶质母细胞瘤或其他脑肿瘤的方法,该方法包括:
制备包含嵌合抗原受体(CAR)分子的细胞,该CAR分子具有结合与胶质母细胞瘤或其他脑肿瘤相关的肿瘤抗原的抗原结合域,以及向有此需要的哺乳动物施用有效量的所制备的细胞,
其中:
所述肿瘤抗原包括碳酸酐酶IX;
所述施用制备的细胞是通过颅内注射;和
所述颅内注射直接靶向胶质母细胞瘤或其他脑肿瘤的边界内。
2.如权利要求1所述的方法,其特征在于,所述颅内注射是立体定向进行。
3.如权利要求1所述的方法,其特征在于,所述制备的细胞进一步包括与CAR分子结合使用的药剂,以增加治疗效果。
4.如权利要求3所述的方法,其特征在于,所述药剂包括刺激淋巴细胞增殖的分子。
5.如权利要求4所述的方法,其特征在于,所述刺激淋巴细胞增殖的分子包括白介素7。

8.如权利要求1所述的方法,其特征在于,所述方法进一步包括:在施用所制备的细胞前,用抑制血管内皮生长因子的疗法治疗胶质母细胞瘤或其他脑肿瘤。
9.如权利要求8所述的方法,其特征在于,所述抑制血管内皮生长因子的疗法包括施用有效量的阿瓦斯汀(Avastin)。
10.如权利要求9所述的方法,其特征在于,所述哺乳动物是人。
