Abstract

本发明公开一种骨科手术术中止血用AB胶,其中A胶为含水溶性钙盐的壳聚糖‑淀粉膏体,B胶为含β‑甘油磷酸纳、水溶性磷酸盐及羟甲基纤维素的膏体。当AB胶的物质混合后可原位反应形成可任意填充骨缺损并在植入后20分钟内成型的胶体,具有止血成骨功能,可替代骨蜡应用于骨科手术术中止血。与传统骨蜡相比,该材料除保留了可任意填充及止血的特点外,还具有生物相容性好、可降解、促成骨的优点,同时AB管的设计保证了材料的长期稳定性。

Description

一种骨科手术术中止血用AB胶
技术领域
本发明属于骨科生物材料技术领域,具体地涉及一种骨科手术术中止血用AB胶。
背景技术
骨髓腔内毛细血管损伤造成的骨组织出血是脑外科开颅手术、胸外科开胸手术、骨科开放性手术、及车祸、高空坠落等骨折创伤中的常见症状。由于骨髓处松质骨具有结构松散、血运丰富的特点,使得该部位一旦出现骨组织出血则既难以靠血管收缩来自行止血,也无法通过电凝、钳夹、止血纱布、明胶海绵填塞等临床常规止血方法来有效止血。
针对这一问题,当前临床中普遍是通过使用骨蜡来填充创面的裂隙和血窦,堵住骨髓部毛细血管渗血,进而激活机体凝血机制来实现骨渗血创面快速止血的。骨蜡以蜂蜡为主体成分,辅以石蜡、棕榈酸异丙酯等材料构成,一般为单独无菌包装,需要使用时用手搓揉变软后能任意塑形,通过填充于骨上渗血处实现止血。遗憾的是尽管骨蜡具有能任意塑形进行物理填充止血的优点,但存在术中阻挡不住渗血、术后阻碍骨愈合、材料移位及引发组织感染等一系列问题。
为此,开发既能保留传统骨蜡可填充骨缺损的优点,又具有良好生物相容性、可降解、促成骨特点的新型材料具有现实的临床意义。
发明内容
针对上述存在的问题,本发明旨在提供一种易操作、易保存,并具有填充、止血、成骨功能的骨科手术术中止血用AB胶。
为了实现上述目的,本发明所采用的技术方案如下:一种骨科手术术中止血用AB胶,A胶和B胶分开保存并紫外消毒,A胶为含水溶性钙盐的壳聚糖-淀粉膏体,B胶为含β-甘油磷酸纳,水溶性磷酸盐及羟基纤维素的膏体。
进一步地,A胶膏体中壳聚糖的浓度为1%-4%,钙离子的浓度为2.5mM-250 mM,糊化淀粉的浓度为5%-20%。其中糊化淀粉指将淀粉加热到80度以上形成的糊化物降温后再与含钙离子的壳聚糖水溶液混合后形成。
进一步地,B胶膏体中β-甘油磷酸纳的浓度为0.5%-4%,羟甲基纤维素的浓度为2%-20%,磷酸根离子的浓度为2 .5mM-250 mM。
进一步地,A胶中的水溶性钙盐的选择范围包括氯化钙、硝酸钙、葡萄糖酸钙等在内的钙盐。
进一步地,A胶中的淀粉可以为玉米淀粉、红薯淀粉等。
进一步地,B胶中的水溶性磷酸盐可以为磷酸钠、磷酸钾、磷酸氢钠、磷酸氢钾、磷酸二氢钠、磷酸二氢钾等。
A、B胶在不使用状态下分开保存,避免反应;使用时,将A胶与B胶于注射器或容器中按1:1的比例进行混合即可制成适用于渗血部位填充的止血材料,当A、B胶相遇后,壳聚糖与β-甘油磷酸纳形成温敏性凝胶,调控材料植入遇血部位后的成胶行为,钙离子与磷酸根离子原位反应形成磷酸钙用于促成骨,淀粉与羟甲基纤维素用于增加膏体的稳定性和粘性,并提高止血性能。
本发明的有益效果是:
将本发明公开的AB胶按比例混合后可原位反应形成可任意填充骨缺损并在植入后20分钟内成型的胶体,适用于渗血部位填充,凝血速度快,止血效果好,可替代骨蜡应用于骨科手术术中止血,其在保留传统骨蜡良好填充与止血效果同时,具有制备操作简单,生物相容性好,可降解,可促成骨的优点,适于推广使用,具有现实的临床意义;且AB胶分开保存的设计保证了材料的长期稳定性。

具体实施方式
为了使本领域的普通技术人员能更好的理解本发明的技术方案,下面结合附图1-4对本发明的技术方案做进一步的描述。
实施例1:
A胶为含氯化钙的壳聚糖-淀粉膏体,B胶为含β-甘油磷酸纳,磷酸钠及羟基纤维素的膏体;A胶膏体中壳聚糖的浓度为3%,钙离子的浓度为100 mM,糊化淀粉的浓度为10%,所用水为去离子水。B胶膏体中β-甘油磷酸纳的浓度为2%,羟甲基纤维素的浓度为10%,磷酸根离子的浓度为60 mM。AB胶分开包装并紫外消毒。
使用时将A胶与B胶于注射器中按1:1的比例进行混合,混合效果图如图1所示;将混合胶进一步置于去离子水中,效果如图2所示,从图中看出AB胶在液体中依旧保持稳定成型的状态,说明该胶可以在体液环境中保持结构稳定不溃散。
将混合后的AB胶使用于渗血部位填充,体外实验材料凝血时间为30秒。
实施例2:
A胶为含葡萄糖酸钙的壳聚糖-淀粉膏体,B胶为含β-甘油磷酸纳,磷酸氢钠及羟基纤维素的膏体;A胶膏体中壳聚糖的浓度为4%,钙离子的浓度为200 mM,糊化淀粉的浓度为10%,所用水为去离子水。B胶膏体中β-甘油磷酸纳的浓度为2%,羟甲基纤维素的浓度为15%,磷酸根离子的浓度为200 mM。AB胶分开包装并紫外消毒。
使用时将A胶与B胶于注射器中按1:1的比例进行混合后使用于渗血部位填充,体外实验材料凝血时间为18秒,封堵效果如图3所示,从中可以看出,AB胶应用在出血模型上时可以有效封堵各类出血创面,止血效果佳。
对比例1:
将壳聚糖和β-甘油磷酸纳混合成胶,壳聚糖的浓度为3%、β-甘油磷酸纳的浓度为2%,混合后使用于渗血部位填充。
对比例2:
将壳聚糖和β-甘油磷酸纳混合成胶,壳聚糖的浓度为3%、β-甘油磷酸纳的浓度为2%
糊化淀粉的浓度为10%,混合后使用于渗血部位填充。
统计实施例1-2和对比例1-2中混合胶的成胶时间,结果如图4所示,从图中可以看出,实施例1和2这种公开的混合胶的成胶时间明显低于对比例中公开的混合胶,可用于渗血部位的快速填充止血。

Claims (6)

1.一种骨科手术术中止血用AB胶,其特征在于,A胶和B胶分开保存并紫外消毒,A胶为含水溶性钙盐的壳聚糖-淀粉膏体,B胶为含β-甘油磷酸纳,水溶性磷酸盐及羟基纤维素的膏体。
2.如权利要求1所述的一种骨科手术术中止血用AB胶,其特征在于,A胶膏体中壳聚糖的浓度为1%-4%,钙离子的浓度为2.5 mM-250 mM,糊化淀粉的浓度为5%-20%。其中糊化淀粉指将淀粉加热到80度以上形成的糊化物降温后再与含钙离子的壳聚糖水溶液混合后形成。
3. 如权利要求1所述的一种骨科手术术中止血用AB胶,其特征在于,B胶膏体中β-甘油磷酸纳的浓度为0.5%-4%,羟甲基纤维素的浓度为2%-20%,磷酸根离子的浓度为2 .5mM-250mM。
4.如权利要求1所述的一种骨科手术术中止血用AB胶,其特征在于,A胶中的水溶性钙盐的选择范围包括氯化钙、硝酸钙和葡萄糖酸钙。
5.如权利要求1所述的一种骨科手术术中止血用AB胶,其特征在于,A胶中的淀粉的选择范围包括玉米淀粉和红薯淀粉。
6.如权利要求1所述的一种骨科手术术中止血用AB胶,其特征在于,B胶中的水溶性磷酸盐的选择范围包括磷酸钠、磷酸钾、磷酸氢钠、磷酸氢钾、磷酸二氢钠和磷酸二氢钾。
