本发明提供一种可改善睑板腺功能的中药组合物,属于中药领域,由如下原料制成:桑叶,薄荷叶,赤芍,皂角刺,地肤子,蛇床子,丹参;对所述中药组合物进行加热水提后分离得到药汤和药渣,所述药汤可用于熏蒸,所述药渣可用于温敷;本发明对于由睑板腺功能异常导致的如:MGD引起的干眼、视频显示终端综合征(VDT)引起的干眼症状、睑板腺囊肿等疾病具有确切的临床效果,无明显使用禁忌、并发症或副作用,适应症范围广,安全性高。

技术领域
本发明涉及中药领域,具体涉及一种改善睑板腺功能的中药组合物及其应用。
背景技术
干眼是一种常见的外眼疾病,是指多种原因引起泪液质和量或动力学异常导致泪膜不稳定,从而发生以眼表组织病变为特征的一大类疾病的总称,又称角结膜干燥症。常见之症状包括眼睛干涩、容易疲倦、眼痒、有异物感、痛灼热感、分泌物黏稠、怕风、畏光、对外界刺激很敏感;有时眼睛太干,基本泪液不足,反而刺激反射性泪液分泌,而造成常常流泪;较严重者眼睛会红肿、充血、角质化、角膜上皮破损而有丝状物黏附,这种损伤日久则可造成角结膜病变,并会影响视力。
近年来干眼病患者年轻化趋势异常明显,主要是由于随着社会和科技的发展,电脑、电视、尤其智能手机越来越普及,长时间使用这些设备,面对屏幕,使眨眼减少,加快了泪液的蒸发;再加上很多人感到眼部不适,长时间使用眼药水等,进一步导致泪液分泌减少及成分异常,引发干眼症。
干眼症发病机制及病理过程复杂,眼表面的病理性改变、免疫炎症反应、性激素水平的降低以及外界环境的影响是干眼的主要因素。近年来发现,引发干眼症的一大原因是睑板腺功能障碍(Meibomian Gland Dysfunction,MGD),MGD在亚洲人群的患病率可达60%以上,对患者生活质量造成极大影响,甚至造成家庭和社会经济负担。
睑板腺位于人体的上下睑结膜,开口于睑缘,睑板腺通过分泌睑脂组成泪膜脂质层,防止泪液过度蒸发,促进泪膜稳定性。泪膜由脂质层、水液层和粘蛋白层组成,最外层是脂质层,其脂质主要由睑板腺分泌至睑缘并通过瞬目运动分布于眼球表面。若睑板腺功能出现了障碍,引起泪液分泌的质量异常,影响了泪膜的稳定性,使泪膜破裂的时间变短、泪液蒸发过快,从而对眼表造成损害,最终就导致了干眼症的发生。也就是说,MGD是一种慢性、弥漫性睑板腺异常,它通常以睑板腺终末导管的阻塞和(或)睑板腺分泌物质或量改变为特征。
目前,临床上治疗MGD及其引起的干眼症主要是疏通睑板腺、改善睑板腺功能为主;通常采用物理清洁眼睑、眼部局部使用人工泪液和皮质激素眼水等并配合口服抗生素以及热敷按摩的方法进行治疗;主要方法分为:(1)药物疗法(抗生素、激素类+人工泪液);(2)睑缘清洁、热敷、睑板腺按摩;(3)热脉动治疗仪、强脉冲光睑板腺治疗仪等医疗器械治疗。但上述方法要么治标不治本,效果不佳;要么价格昂贵,使用困难。虽然,当前也有以熏蒸方式治疗MGD及其引起的干眼症的尝试,但仍存在诸多不足,如(1)仅用生理盐水或蒸馏水熏蒸,无药物作用,仅能临时缓解症状;(2)大量退翳中药汤汁熏蒸或热敷,对于MGD等患者并未切入病机,效果不能持久;(3)用鱼腥草滴眼液等眼药水等成药进行熏蒸,因未行辩证,效果不能持久。
发明内容
针对上述问题,发明人通过长时间的临床研究和验证,根据MGD的病变特点,结合临床实践提出一种专门针对MGD病机、能够有效治疗MGD及其引发的干眼症的中药熏蒸方剂。
本发明的目的采用以下技术方案来实现:
一种改善睑板腺功能的中药组合物,由如下原料制成:
桑叶,薄荷叶,赤芍,皂角刺,地肤子,蛇床子,丹参。
优选的,由如下重量份原料制成:
桑叶1重量份,薄荷叶1重量份,赤芍1重量份,皂角刺1重量份,地肤子1重量份,蛇床子1重量份,丹参1重量份。
本发明所述中药组合物中,桑叶疏散风热,清肝明目;薄荷叶疏肝、散热、止痒;赤芍清热凉血、活血祛瘀;皂角刺杀虫排脓;地肤子、蛇床子祛风止痒;丹参活血祛瘀、凉血消痈;所述中药组合物以活血通络、疏风止痒、清肝明目为治疗原则来组方,组方治则符合MGD患者睑板腺充血、堵塞等状态,对于由MGD而引起的患者干眼及其他眼部不适、改善患者的睑板腺功能具有确切的临床效果。
本发明还提供一种用于改善睑板腺功能的中药药剂的制备方法,对所述中药组合物进行加热水提,再过滤分离得到药汤和药渣。
所述药汤可用于熏蒸,所述药渣可用于温敷。
优选的,所述中药组合物加水蒸煮,加水量没过药材,武火煮沸后转文火煮制40-45min,再过滤分离药汤和药渣。
优选的,所述加水量为每克中药组合物7-8ml。
本发明还提供一种可以配合所述药汤使用的熏蒸仪,包括雾化装置和眼罩,所述雾化装置与所述眼罩间通过连接导管连接。
优选的,所述熏蒸仪还包括加热装置,所述加热装置连接设置于所述雾化装置与所述眼罩间。
优选的,所述雾化装置为超声波雾化器。
以超声波为动力,通过超声波雾化器针对破坏药汤的表面张力,将其雾化为极微的雾滴充于密闭的眼罩内,使其均匀、持续、全面地作用于患者的角膜和结膜;加热装置可以持续对管路中的雾气进行加热,使进入眼罩的雾气温度恒温在40-43℃,刺激睑缘温度升高,利于睑板腺脂质流动。
本发明还提供一种所述药汤的熏蒸方法,包括以下步骤:
将所述药汤加入熏蒸仪,以导出的雾气对眼部进行熏蒸,所述雾气与眼周处的温度在40-43℃,熏蒸时间15-30min/次,熏蒸频率3-4次/周。
本发明的有益效果为:
(1)本发明根据MGD患者睑板腺病变特点,不同于传统治疗干眼方剂的“滋阴增液、益肝明目”的治疗原则,本方以“活血通络、疏风止痒、清肝明目”为治疗原则组方,可显著改善改善睑板腺功能,对于由睑板腺功能异常导致的如:MGD及其引起的患者干眼、视频显示终端综合征(VDT)引起的干眼症状、睑板腺囊肿等疾病具有确切的临床效果,提高了患者生活质量。
(2)本发明所述组合物无明显使用禁忌,无并发症或副作用,人群普遍适宜,适应症范围广,且价格低廉,不会对患者造成经济负担;采用无设备直接接触眼部的熏蒸方式,安全性更高。

具体实施方式
结合以下实施例对本发明作进一步描述。
实施例1
按照如下组成配制方剂:
桑叶10g、薄荷叶10g、赤芍10g、皂角刺10g、地肤子10g、蛇床子10g、丹参10g;
按照如下方法制备熏蒸药剂,并对患者进行临床治疗:
(1)将所述组方中药组合物加水蒸煮,加水量没过药材,约500ml,先武火煮沸后转文火煮制40-45min,保留药汤,得到熏蒸药剂,药渣单独保存;
(2)药汤倒入熏蒸仪进行眼部熏蒸,每次15-30min,每周3-7次,根据症状可连续或间断治疗;具体的,在超声雾化器水槽中加入灭菌注射用水至刻度线以上,将提前煮制好的药汤20-30ml加入一次性药杯中放入超声雾化器水槽上方,熏蒸治疗过程中雾气温度逐渐上升,眼周温度最终维持在40-43℃,嘱患者在治疗过程中可闭目休息或正常睁眼、眨眼;
(3)药渣保留作温敷,每次5-20min。
实施例2
某病例:罗XX,女,31岁。诊断双眼MGD,双眼干眼症。
初诊时:OSDI(眼表疾病指数问卷):75分;双眼BUT(泪膜破裂时间):右眼:2秒,左眼3秒;双眼TMH(泪河高度):右眼:0.19mm,左眼:0.23mm;双眼Shirmer(泪液分泌试验):右眼:2mm,左眼:4mm。
药物熏蒸2周后复诊:OSDI(眼表疾病指数问卷):31分;双眼BUT(泪膜破裂时间):右眼:6.68秒,左眼6.54秒;双眼TMH(泪河高度):右眼:0.22mm,左眼:0.21mm;双眼Shirmer(泪液分泌试验):右眼:7mm,左眼:8mm。
实施例3
某病例:赵XX,女,26岁。诊断双眼MGD,双眼干眼症。
初诊时:OSDI(眼表疾病指数问卷):45分;双眼BUT(泪膜破裂时间):右眼:4秒,左眼4秒;双眼TMH(泪河高度):右眼:0.19mm,左眼:0.17mm;双眼Shirmer(泪液分泌试验):右眼:3mm,左眼:4mm。
药物熏蒸2周后复诊:OSDI(眼表疾病指数问卷):23分;双眼BUT(泪膜破裂时间):右眼:7.19秒,左眼7.38秒;双眼TMH(泪河高度):右眼:0.22mm,左眼:0.27mm;双眼Shirmer(泪液分泌试验):右眼:9mm,左眼:9mm。
为更好的说明本发明熏蒸药剂的治疗效果,对本发明所述中药组合物在患者中的临床应用效果进行统计;
临床试验对象为2019年8月~2020年7月就诊于北京同仁医院的干眼(MGD)患者,共30例,其中,男13例、女17例,平均年龄41岁;
诊断标准为:(1)主观症状:干燥感、异物感、烧灼感、疲劳感、不适感、视力波动等主观症状之一和泪膜破裂时间(BUT)≤5s,或Schirmer试验(SIT)≤5mm/5min,可诊断干眼;(2)上述主观症状之一和5s<BUT≤10s,或5mm/5min<SIT≤10mm/5min时,同时有角结膜荧光染色阳性可诊断干眼。
观察指标:以自觉症状积分、SIT、BUT及角膜荧光素染色(FL)评分。比较治疗前及治疗后2周的变化。自觉症状积采取眼部症状问卷调查的形式进行评分,评分项目包括视疲劳、干涩感、异物感、烧灼感、畏光疼痛、流泪、眼红等,积分越高者程度越重。FL评分标准:无染色,记0分;染色面积占角膜1/3以下,计1分;大于1/3小于2/3面积,计2分;大于2/3面积,计3分。将角膜分为4个象限,每个象限0~3分,共0~12分。
结果分析:用药2周后,30(60眼)例患者中自觉症状积分:治疗前8.35±1.37分,治疗后5.93±1.15分,两组比较差异有统计学意义(P<0.05);泪膜破裂时间(BUT):治疗前:3.40±1.57秒,治疗后5.95±1.37秒,两组比较差异有统计学意义(P<0.05);泪液分泌试验I(shirmmer TEST I):治疗前5.31±2.07,治疗后7.65±2.07,两组比较差异有统计学意义(P<0.05);角膜荧光染色(FL评分):治疗前4.52±1.64,治疗后3.31±1.21,两组比较差异有统计学意义(P<0.05);用药2周后自觉明显改善18人,自觉症状部分改善7人,自觉无明显变化5人。
由上述统计结果可以看出,本发明提供的中药组合物及其应用方法对干眼症患者的自觉症状有明显的改善作用,对睑板腺功能有显著的改善效果,无并发症或副作用。
最后应当说明的是,以上实施例仅用以说明本发明的技术方案,而非对本发明保护范围的限制,尽管参照较佳实施例对本发明作了详细地说明,本领域的普通技术人员应当理解,可以对本发明的技术方案进行修改或者等同替换,而不脱离本发明技术方案的实质和范围。

Claims (7)

1.一种改善睑板腺功能的中药组合物,其特征在于,由如下原料制成:桑叶1重量份,薄荷叶1重量份,赤芍1重量份,皂角刺1重量份,地肤子1重量份,蛇床子1重量份,丹参1重量份。
2.一种用于改善睑板腺功能的中药药剂的制备方法,其特征在于,对权利要求1所述中药组合物进行加热水提,再过滤分离得到药汤和药渣。
3.根据权利要求2所述的一种用于改善睑板腺功能的中药药剂的制备方法,其特征在于,所述中药组合物加水蒸煮,加水量没过药材,武火煮沸后转文火煮制40-45min,再过滤分离药汤和药渣。
4.根据权利要求3所述的一种用于改善睑板腺功能的中药药剂的制备方法,其特征在于,所述加水量为每克中药组合物7-8mL。
5.一种用于改善睑板腺功能的熏蒸药剂,其特征在于,包括权利要求2所述药汤。
6.一种用于改善睑板腺功能的温敷药剂,其特征在于,包括权利要求2所述药渣。
7.权利要求1所述中药组合物在制备权利要求5所述改善睑板腺功能的熏蒸药剂中的应用,其特征在于,包括以下步骤:将所述药汤加入熏蒸仪,以导出的雾气对眼部进行熏蒸,所述雾气于眼周处的温度在40-43℃,熏蒸时间15-30min/次,熏蒸频率3-4次/周。
