Abstract

本发明公开了食管癌的分子标志物及其在制备检测食管癌试剂盒中的应用,本发明中通过预先对食管癌病人组织进行RNA测序,筛选与食管癌相关的差异表达的IncRNA,新发现了四个IncRNA在食管癌患者组织中呈现异常表达,建立了四个IncRNA的检测方法,并在食管癌病人血液中验证,表明该四个IncRNA可用于食管癌诊断、治疗效果评估或预后情况评估。

技术领域
本发明属于肿瘤分子标志物技术领域,具体涉及食管癌的分子标志物及其在制备检测食管癌试剂盒中的应用。
背景技术
食管癌(esophageal cancer,EC)是最常见的恶性肿瘤之一,常发生在消化系统里,是一种原发于食管上皮的恶性肿瘤。此病在我国的发病率和死亡率居于前列,是高发癌症。虽然我国多年极力加强食管癌的预防诊治工作,但不可避免的是一旦确诊为食管癌,90%的患者均为中晚期,5年生存率不足20%,相关研究表明,诱发食管癌发病因素有很多,诸如吸烟、饮酒、微量元素缺乏、遗传等因素都与食管癌发病有关。因此,深入了解肿瘤发生发展的分子机制,补充和完善现有的细胞癌变理论体系,有助于为食管癌的防、诊、治提供重要的理论依据。
近些年由于二代测序技术RNA-seq的快速发展,使人们对非编码RNA有了更深刻的认识,其中包括长链非编码RNA(long non coding RNA,IncRNA),IncRNA是人们近期研究的热点。其特征很多,诸如长度较长>200nt,数量众多,二级结构丰富,具有特异性,但不具有蛋白编码能力的转录产物等特征。随着研究水平的提高,学者们发现IncRNA有很强的调控功能。其表达的正常与否和许多疾病密切相关,同时在癌症中发挥着重要的作用。

实施步骤一 RNA提取
待检血液和组织样本来自于食管癌癌症患者及正常人,血液RNA提取时采用了Trizol法。
待检血液和组织样本中RNA提取步骤为:
(一)血液RNA提取:
(1)3500rpm离心5min,取出上清;
(2)在上清中加1mL RNA Blood,混匀后在冰上静置20-30min;
(3)加200μL氯仿,振荡混匀,冰上静置10min,4℃(提前预冷)离心12,000rpm,10min;
(4)取出上清,加等体积的异丙醇混匀,冰上静置10min后4℃离心12,000rpm,10min;
(5)弃掉上清,加500μL冰乙醇弹洗,4℃离心10,000rpm,5min;
(6)重复(5)实验步骤;
(7)在超净工作台吹干液体,向管中间部位悬空滴加30-100μL RNase-Free H2O,室温放置2min,12,000rpm离心2min,得到RNA溶液;(注:洗脱体积不少于30μL)。
(8)混匀后去测浓度即可,不及时用的放在-80℃冰箱保存。
(二)组织中提取RNA:
(1)匀浆处理:在组织(10-20mg)加裂解液RL300μL,将组织研磨;随后向匀浆中加入RNase-Free H2O和Proteinase K,分别是590μL和10μL,混匀后56℃处理10-20min;
(2)12,000rpm离心2-5min,取上清进行一下操作;
(3)缓慢加入无水乙醇(0.5倍上清体积),混匀(可能出现沉淀),将溶液和沉淀一起转入吸附柱CR3中(吸附柱放入收集管),12,000rpm离心30-60s,弃掉收集管中的废液,将吸附柱放回收集管中;
(4)向吸附柱CR3中加入去蛋白液RW1350μL,12,000rpm离心30-60s,弃废液,将吸附柱放回收集管中;
(5)DNase I工作液配制:取10μL DNase I储存液放入新的RNase-Free离心管中,加入70μL的RDD溶液,轻柔混匀;
(6)向吸附柱CR3中加入80μL DNase I工作液,室温放置15min;
(7)向吸附柱CR3中加入350μL去蛋白液RW1,12,000rpm离心30-60s,弃废液,将吸附柱放回收集管中;
(8)向吸附柱CR3中加入500μL漂洗液RW,室温静置2min,12,000rpm离心30-60s,弃废液,将吸附柱放回收集管中;
(9)重复(8)步骤;
(10)12,000rpm离心2min,弃废液,将吸附柱CR3置于室温放置数分钟,以彻底晾干吸附材料中残余漂洗液;
(11)将吸附柱CR3转入一个新的RNase-Free离心管中,向吸附膜的中间部位悬空滴加30-100μL RNase-Free H2O,室温放置2min,12,000rpm离心2min,得到RNA溶液;(注:洗脱体积不少于30μL)。

实施步骤四 结果验证与分析
数据处理:本发明使用7500PCR仪器最佳PCR程序扩增出目的基因进行数据导出处理得到4个新基因的融解曲线图。利用GraphPad绘制基因表达情况图。所有的统计分析都是在R语言和统计计算环境下完成的(R version 2.15.1;R Foundation for StatisticalComputing)。
本发明操作方法:包括提RNA、所述的引物、逆转录和定量PCR反应条件,检测食管癌患者和正常人血液中novel IncRNA_1、2、3、4的表达情况,获得较好的融解曲线。见图1。图1中A、B、C和D分别为novel IncRNA_1、2、3和4在食管癌患者和正常人血液中的融解曲线。
荧光定量PCR结果显示,与正常组相比,novel IncRNA_1、2在食管癌患者血液中表达量显著升高(P<0.01);novel IncRNA_3在食管癌患者血液中表达量显著降低(P<0.01),novel IncRNA_4在食管癌患者血液中表达量降低(P<0.05)。见图2。
SPSS绘制ROC曲线结果显示,novel IncRNA_1、2、3和4基因AUC面积分别是0.73、0.71、0.97和0.90,具有较高的特异性和灵敏度,其中,novel IncRNA_3检测食管癌的敏感性达到97.2%,灵敏度达到97.5%。能够对病人的病情起到一定的预测和诊断。见图3。

本发明具有如下优点:测定novel IncRNA_1、2、3和4基因在食管癌患者和正常人血液中的表达情况,并进一步验证。通过荧光定量PCR最佳体系实现对novel IncRNA_1、2、3和4基因的检测,结果显示novel IncRNA_1、2、3、4具有一定的诊断意义,可用于食管癌诊断、治疗效果评估或预后情况评估。
本发明对与食管癌进展相关的IncRNA进行鉴定并探索其功能与机制,我们收集了19例食管癌患者和10例正常人血液样本,其中食管癌患者的选择标准如下:(1)通过病理检测确诊的患者(患者所有材料都是通过镜检,活组织切片检查或手术得到,并通过显微镜检查确认的);(2)患者没有其他的肿瘤病史。组织是在患者诊断过程中,且尚未接受任何治疗(手术或化疗)时采集的。本发明的实验方法为对19例食管癌患者和10例正常人血液进行RNA提取、逆转录、荧光定量PCR,并进行验证。对数据结果进行分析发现,与正常组相比,novel IncRNA_1和2在食管癌患者血液中表达量显著上调(P<0.01),见图2,novelIncRNA_3在食管癌患者血液中表达量显著降低(P<0.01),novel IncRNA_4在食管癌患者血液中表达量降低(P<0.05)。见图2。总之上述结果说明了novel IncRNA_1、2、3和4基因在食管癌患者血液中表达情况,并可作为食管癌诊断及预后的指标。
