Abstract

背景技术
所述前体肽尤其包含在N端的21个氨基酸的信号序列,被称为“肾上腺髓质素前肽原”(pre-proADM)(Kitamura K,Sakata J,Kangawa K,Kojima M,Matsuo H,Eto T.Cloningand characterization of cDNA encoding a precursor for human adrenomedullin,《生物化学与生物物理研究通讯》(Biochem Biophys Res Commun)1993:194:720-725)。

败血症通常是由宿主对感染的反应失调引起的(Singer M,Deutschman CS,Seymour CW等,(2016)The Third International Consensus Definitions for Sepsisand Septic Shock(Sepsis-3).《美国医学会杂志》(Jama)315:801-81),最常由细菌引起,而真菌或病毒感染则较不常见(Eggimann P,Garbino J,Pittet D(2003)Epidemiology ofCandida species infections in critically ill non-immunosuppressed patients.《柳叶刀传染病》(Lancet Infect Dis)3:685-702)。因此,真菌血症仅存在于未选择的败血症病例的3%中(Eggimann(同上))。反之,真菌是在腹膜炎中从腹部病灶中回收的最常分离的物种之一,并且众多患者在住院期间发生真菌定殖(Eggimann(同上))。

然而,未公开肾上腺髓质素原(proADM)或其部分肽或片段,特别是中区肾上腺髓质素原(MR-proADM)对于IFI/IFD诊断的适用性,同样,也未公开与败血症或败血性休克相关的IFI/IFD的诊断。
发明内容
本发明的目的是提供用于对真菌感染,特别是与败血症或败血性休克相关的IFI/IFD进行诊断和/或风险分层的改进方法。
该目的通过用于真菌感染,特别是与败血症或败血性休克相关的IFI/IFD的体外诊断和/或风险分层的方法来实现,其中从待检查的患者测定标志物肾上腺髓质素原(proADM)或其部分肽或片段,特别是中区肾上腺髓质素原(MR-proADM),或包含在标志物组合(组、簇)中者(下文称为根据本发明的方法)。然而,中区肾上腺髓质素原(MR-proADM)是一个优选的实施方案,并显示出很大的血浆稳定性,这是特别有利的。

除非另有定义,否则本文使用的技术和科学术语具有与本发明所属领域的普通技术人员通常所理解的相同含义。
根据本发明的“诊断”包括确定、监测、确认、亚分类和预测相关疾病、病症、并发症或风险。“确定”涉及意识到疾病、病症、并发症或风险。“监测”涉及跟踪已经被诊断的疾病、病症、并发症或风险,例如分析疾病的进展或特定治疗对疾病或病症的进展的影响。“确认”涉及加强或证实已经使用其它指标或标志物进行的诊断。“亚分类”涉及根据所诊断的疾病、病症、并发症或风险的不同亚类进一步限定诊断,例如根据疾病的轻度和重度形式限定。“预测”涉及在其它症状或标志物变得明显或已经显著改变之前对疾病病症或并发症进行预后。

因此,特别有利的是,借助于根据本发明的方法可以进行可靠的诊断和/或风险分层。根据本发明的方法允许做出临床决策,从而导致对重症患者的真菌感染的更快速诊断。根据本发明的方法允许临床决策,其导致对IFI/IFD,特别是与败血症或败血性休克相关的IFI/IFD的更快速诊断。这样的临床决策还包括使用药物进一步治疗,以处理或治疗IFI/IFD,特别是与败血症或败血性休克相关的IFI/IFD。
适当的治疗要求IFI/IFD的早期诊断和鉴别,特别是与败血症或败血性休克相关的IFI/IFD,甚至在急诊室就诊时,还要结合临床决策。由于IFI/IFD的临床症状尚不明确,并且最近用于检测真菌病原体的诊断工具也存在相关弱点,因此,由例如细菌或病毒病原体引起的其它传染病的鉴别和划界以及IFI/IFD、特别是与败血症或败血性休克相关的IFI/IFD的鉴定是必不可少的。

在根据本发明的方法的另一优选实施方案中,在IFI/IFD的病程中进行诊断和/或风险分层,以作为治疗的伴随手段并调整治疗性处理,例如机械通气、肾脏替代治疗或抗真菌治疗。
治疗性处理的调整还可以包括决定是否像以前那样进一步处理受试者或是否适应处理。例如,治疗性处理的调整可以是将受试者保持在重症监护病房(ICU)或急诊室(ED)或是否被释放。

本文使用的术语“败血症”涉及败血症发展中的所有可能阶段。

在本发明的范围内,术语“IFI/IFD”也具有全身性、一般性、深层、内脏性和严重性的、威胁生命的真菌感染的含义,而不是表面的、局部的、定殖的、良性的、自限性的真菌疾病或感染。

根据本发明的用于诊断或风险分层的优选患者是重症患者,特别是由于药物或疾病或者由于诱导、增强或抑制免疫应答的手段而受到免疫调节的患者,包括免疫功能低下的患者,例如接受移植的患者,重度中性粒细胞减少症患者或癌症患者。因此,对于根据本发明的风险分层,上述患者是优选的。
根据本发明的用于诊断或风险分层的优选患者可以是被诊断患有败血症、严重败血症或败血性休克的重症患者,或具有根据传染病但无明显败血症症状的患者,具有局部感染(例如呼吸道、泌尿道、腹部、皮肤、粘膜、生殖器、中枢神经系统)的患者,接受机械通气、输液、肾脏替代治疗的患者,接受抗真菌治疗的患者,接受外科手术(尤其是肝脏外科手术)的患者,具有肝硬化的患者以及遭受创伤(多发伤)的患者。
本发明的方法可以用于患者管理,指的是:
·入住医院或重症监护病房的决定,
·将患者转移到专科医院或专科医院病房的决定,
·对从重症监护病房或医院提早出院的评估,
·资源分配(例如医师和/或护理人员、诊断、治疗方法)。
因此,本发明涉及对IFI/IFD、特别是由念珠菌属(例如白色念珠菌、光滑念珠菌)、曲霉属(例如烟曲霉)等引起的IFI/IFD的诊断和/或风险分层,所述菌种是造成IFI/IFD、特别是与败血症或败血性休克相关的IFI/IFD的最常见病原体。
在一个非常优选的实施方案中,本发明涉及与败血症和/或败血性休克相关的IFI/IFD的诊断和/或风险分层。
术语“与败血症和/或败血性休克相关的IFI/IFD”特别包括这些适应症的合并症,即,除了现有的潜在疾病(指数疾病)即IFI/IFD,还有现有的、在诊断上可区分的疾病谱即确定败血症和/或败血性休克,即存在相关的疾病谱。如果及时将抗真菌剂(药物)应用于患者,则该方法可以防止由于首次IFI/IFD引起的败血症和/或败血性休克的不良后果。
在另一个实施方案中,肾上腺髓质素原(proADM)或其部分肽或片段,特别是中区肾上腺髓质素原(MR-proADM)的测定可以另外与其它标志物一起进行,其中肾上腺髓质素原(proADM)或其部分肽或片段,特别是中区肾上腺髓质素原(MR-proADM)以标志物组合(组、簇)包含,特别优选地已经指示IFI/IFD、特别是与败血症或败血性休克相关的IFI/IFD的那些。

特别地,体液中半乳甘露聚糖的检测被用于诊断人类的侵袭性真菌(曲霉病)感染。
在本发明的另一个实施方案中,根据本发明的方法可以通过平行或同时测定标志物(例如具有96个腔以上的多孔滴定板)来进行,其中所述测定是在至少一个患者样本上进行的。
此外,根据本发明的方法及其测定可以使用自动分析设备,尤其是使用Kryptor(B.R.A.H.M.S GmbH,德国亨尼格斯多夫)来进行的。
在另一个实施方案中,根据本发明的方法及其确定可以借助于快速检验(例如侧流检验)来进行,例如使用单参数或多参数确定。
此外,本发明涉及肾上腺髓质素原(proADM)或其部分肽或片段,特别是中区肾上腺髓质素原(MR-proADM),或包含在标志物组合(组、簇)中者的用途,其用于对IFI/IFD、特别是与败血症或败血性休克相关的IFI/IFD进行体外诊断和/或风险分层。如果需要,标志物组合可以含有另一个合适的标志物。
本文中测定(或测量或检测)肾上腺髓质素原(proADM)或其部分肽或片段,特别是中区肾上腺髓质素原(MR-proADM)的水平是使用如下所述的检测方法和/或诊断测定法进行的。
另一个目的是提供相应的诊断测定法,或将这种测定法用于实施根据本发明的方法的用途。
如本文中所提及,“测定法”或“诊断测定法”可以是在诊断领域中应用的任何类型。这样的测定法可以基于待检测的分析物与一种或多种具有特定亲和力的捕获探针(捕获分子)的结合。关于捕获分子与目标分子或相关分子之间的相互作用,亲和常数优选大于108M-1。
此外,标志物可以通过基于质谱的方法来确定,例如确定感兴趣的蛋白质或其片段的相对定量或确定其绝对定量的方法。
相对定量“rSRM”可以通过以下实现:
1.通过比较样本中检测到的给定目标片段肽的SRM(选定反应监测)特征峰面积与至少第二、第三、第四或更多个生物样本中目标片段肽的相同SRM特征峰面积,来确定目标蛋白存在的增加或减少。
2.通过比较样本中检测到的给定目标肽的SRM特征峰面积与源自不同且分离的生物源的其它样本中来自其它蛋白质的片段肽形成的SRM特征峰面积,来确定目标蛋白存在的增加或减少,其中针对一个肽片段的两个样本之间的SRM特征峰面积比较例如相对于每个样本中分析的蛋白质量进行归一化。
3.通过比较给定目标肽的SRM特征峰面积与相同生物样本内源自不同蛋白质的其它片段肽的SRM特征峰面积,来确定目标蛋白存在的增加或减少,以将组蛋白水平的变化相对于在各种细胞条件下不会改变其表达水平的其它蛋白质水平进行归一化。
4.这些测定法可应用于目标蛋白的未修饰片段肽和修饰片段肽,其中修饰包括但不限于磷酸化和/或糖基化,乙酰化,甲基化(单、二、三),瓜氨酸化,泛素化,并且其中以与测定未修饰肽的相对量的相同方式来确定修饰肽的相对水平。
给定肽的绝对定量可通过以下实现:
1.将来自单个生物样本中的目标蛋白的给定片段肽的SRM/MRM特征峰面积与从生物样本掺入蛋白质裂解物中的内部片段肽标准物的SRM/MRM特征峰面积进行比较。内标可以是被询问的目标蛋白或被标记的重组蛋白的片段肽的被标记的合成形式。在消化之前(对于重组蛋白是强制的)或之后,将该标准物以已知量掺入样本中,并且可以分别确定生物样本中的内部片段肽标准物和天然片段肽的SRM/MRM特征峰面积,接着比较两个峰面积。这可以应用于未修饰的片段肽和修饰的片段肽,其中修饰包括但不限于磷酸化和/或糖基化,乙酰化,甲基化(例如单甲基化、二甲基化或三甲基化),瓜氨酸化,泛素化,并且其中修饰肽的绝对水平可以与测定未修饰肽的绝对水平相同的方式确定。
2.也可以使用外部校准曲线对肽进行定量。正态曲线法使用恒定量的重肽作为内标,并将变化量的轻质合成肽掺入样本中。需要使用与测试样本相似的代表性基质来构建标准曲线,以解决基质效应。此外,反向曲线方法可避免基质中内源分析物的问题,其中将恒定量的轻肽掺入到内源分析物的顶部以产生内标,并掺入变化量的重肽以产生一组浓度标准。将与正态或反向曲线进行比较的测试样本掺入一定量的标准肽,其量与掺入用于产生校准曲线的基质中的内标相同。

在败血症发作(T0)以及其后第1天(T1)、第2天(T2)、第7天(T3)、第14天(T4)、第21天(T5)和第28天(T6)收集血液样本。收集了相关基线数据(人口统计学数据、主要感染部位),临床数据(疾病严重程度评分,如简化急性生理评分(SAPS II)、序贯器官衰竭估计评分(SOFA)和急性生理健康评估评分(APACHE II),手术程序,抗真菌治疗,结果参数)以及常规感染参数(例如白细胞、C反应蛋白(CRP)、降钙素原(PCT)、体温)。
免疫测定法。使用试剂盒(Pyroquant Diagnostik GmbH)根据制造商的说明书来测量BG的血浆浓度。MR-proADM的血浆浓度是使用TRACE(时间分辨扩增穴状化合物发射)技术与新型夹心免疫测定法(Kryptor Compact Plus分析仪,BRAHMS,德国亨尼格斯多夫)结合测量的。在所有患者中,所有时间点的血浆样本中的GM浓度均采用酶联免疫测定法(PlateliaTM Aspergillus AG,Biorad,慕尼黑)进行测量。使用相同的技术测量支气管肺泡灌洗液(BALF)中的GM浓度,但仅在怀疑为侵袭性曲霉病(IA)的选定病例中进行。以下GM浓度用作截止值:血浆>0.5,BALF>1.0。
流式细胞术。在FACSVerse流式细胞仪(BD Biosciences,德国海德堡)上根据制造商的说明书使用多重测定法(人类Th1/Th2/Th17细胞因子试剂盒,BD Biosciences,德国海德堡)测量II-2、IL-4、IL-6、IL-10、IL-17A、TNF-α和IFN-γ的血浆浓度。
临床微生物学。
血液培养。在海德堡大学医院如其它地方所述常规进行血液培养测试(GumbingerC,Hug A,Murle B等,(2013)Early blood-based microbiological testing wasineffective in severe stroke patients.《神经科学杂志》(J Neurol Sci)325:46-50)。
使用无菌技术经由直接静脉穿刺(例如肘前静脉)获得全血样本,并且将10mL血液接种到需氧和厌氧液体培养基(BACTEC PLUS,BD Biosciences,德国海德堡)中。将培养物温育5天(BACTEC,BD Biosciences,德国海德堡),并根据批准的内部医院标准技术对阳性培养物进行分析,包括通过VITEK2(Biomerieux,德国努尔丁根)或MALDI TOF(Bruker,美国威斯康星州麦迪逊)和自动化抗微生物敏感性测试(VITEK 2)鉴定。

简言之,将气管抽吸物和引流液手动涂在Columbia(BD)、巧克力(bM)、MacConkey(bM)、Schaedler和卡那霉素-万古霉素(BD,双板)和发色念珠菌琼脂(BD)上,同时通过PREVI IsolaTM仪器在相同琼脂类型上对伤口拭子进行半自动接种。除了如所述将Schaedler-KV双板在厌氧室(GasPak;Becton,美国新泽西州富兰克林湖狄金森)中于37℃温育48小时之外,所有板都在5%CO2中于37℃温育24至48小时(Mischnik A,Mieth M,Busch CJ等,(2012)First evaluation of automated specimen inoculation for woundswab samples by use of the Previ Isola system compared to manual inoculationin a routine laboratory:finding a cost-effective and accurate approach.《临床微生物学杂志》(J Clin Microbiol)50:2732-2736)。通过MALDI-ToF质谱法鉴定细菌和真菌菌落,并在VITEKII仪器(bM)上进行自动AST。
群组定义。呼吸道或引流液中的念珠菌属被分类为定殖。在伴有肺浸润的深呼吸道标本中血液培养、术中拭子和曲霉属菌均呈阳性结果被分类为感染。
抗念珠菌抗体滴度。使用Behring ELISA处理器(BEP III,Siemens HealthcareDiagnostics,德国马尔堡)如制造商说明书中所述使用Serion ELISA classicTM白色念珠菌IgA/lgG/lgM(ESR 117A/G/M,Virion Serion,德国维尔茨堡)检测并定量血清中的白色念珠菌特异性IgM、IgA和IgG抗体(Zou M,Tang L,Zhao S等,(2012)Systematic reviewand meta-analysis of detecting galactomannan in bronchoalveolar lavage fluidfor diagnosing invasive aspergillosis.PLoS One 7:e43347)。
统计分析。将所得数据输入电子数据库(Excel 2010;Microsoft公司,美国雷德蒙德),并使用SPSS软件(21.0版;SPSS公司,美国芝加哥)进行评估。使用绝对频率和相对频率汇总分类数据。量化数据使用附有四分位数的中位数进行汇总。Kolmogorov-Smirnov检验用于检查正态分布。由于非正态分布的数据,因此使用非参数方法进行评估(对于分类数据为卡方检验,对于连续数据为Mann-Whitney U检验)。使用ROC分析计算出用于检测真菌感染的适当截止值。P值<0.05被认为具有统计学显著性。关于符号表示和更高的显著性等级:p<0.05:*,p<0.01:**,p<0.001:***。
结果
患者特征。本研究中纳入总共50名败血性休克患者。表1中呈现了患者的特征。潜在的败血症病灶是腹部(n=43;86%),其次是肺(n=6;12%)以及泌尿生殖道(n=1;2%)。28天和90天的总死亡率分别是22%(n=11)和34%(n=17)。ICU停留时长以及住院时长的中位数分别是20天和44天。

真菌病原体和感染部位。
基于培养物的微生物学诊断。通过基于培养物的微生物学诊断评估,真菌病原体存在于33名患者(66.0%)中,而17名患者(34.0%)显示阴性真菌培养物。分别在25名(75.8%)或8名(24.2%)的患者中的一个或多个位置发现了真菌分离株,它们位于以下部位:呼吸道(n=17;51.5%)、腹部(n=21;63.6%)和血液培养物(n=3;9.1%)。表1呈现了具有或不具有真菌病原体的患者的特征。在纳入研究之前,具有真菌病原体的患者接受了肝脏手术的可能性更高,并且肾脏替代疗法的需求已显著增加。关于发病率的其它标志物,真菌阳性患者显示机械通气持续时间显著延长,并且气管切开术的需求倾向于增加。此外,具有真菌病原体的患者的ICU停留时长和住院时长均显著延长。令人惊讶的是,不具有真菌病原体的患者的28天死亡率显著增加,而90天死亡率显示相当。
根据方法章节中所述的群组定义,分别在22名(44.0%)和11名(22.0%)患者中发现了定殖和感染。在定殖患者中,有8名(16.0%)参与者仅在呼吸道分泌物中显示念珠菌属(5x白色念珠菌,1x白色念珠菌和光滑念珠菌,2x白色念珠菌和念珠菌属),而在6名(12.0%)患者中仅可从引流液中培养念珠菌属(3x白色念珠菌,2x光滑念珠菌,1x白色念珠菌和光滑念珠菌)。反之,有8名(16.0%)患者在两方面定殖(4x白色念珠菌,1x白色念珠菌和念珠菌属,3x白色念珠菌和光滑念珠菌)。在感染患者中,在3名(6.0%)患者(2x白色念珠菌,1x光滑念珠菌)中发现了真菌血症,并且7名(14.0%)患者(4x白色念珠菌,1x光滑念珠菌,1x克柔念珠菌,1x白色念珠菌和光滑念珠菌)发现了阳性腹部伤口拭子。此外,在1名(2.0%)患者中,在呼吸道分泌物中分离出烟曲霉。关于风险因素,在真菌感染患者中,在纳入研究之前进行肝脏手术以及观测到肝硬化的可能性更高。此外,在遭受真菌感染的患者中,ICU停留以及机械通气持续时间显著延长,并且气管切开术的需求显著增加。尽管显示发病率增加,但感染和未感染患者在28天和90天的死亡率没有显著差异。
抗真菌治疗。在研究参与期间,50名患者中共有21名(42.0%)接受了抗真菌治疗。在不具有任何真菌分离株的17名患者中,有2名(11.8%)患者接受了经验性抗真菌治疗。在其余的33名具有真菌分离株的患者中,有19名(57.6%)接受了抗真菌治疗,其中有15名(78.9%)患者开始特异性治疗。反之,在其余4例(21.1%)中,根据经验疗法开始治疗,随后在所有这些患者中停止治疗。在7名(33.3%)患者中,最初的抗真菌治疗在疾病过程中发生了变化。
(1,3)-β-D-葡聚糖(BG)。在整个研究期间,三个亚组之间的BG血浆浓度相当,因此无法预测真菌感染的诊断价值(数据未示出)。即使在患有念珠菌血症的患者中,BG血浆浓度也没有可靠地增加。
半乳甘露聚糖(GM)。50名患者中有46名(92.0%)的GM血浆浓度仍低于<0.5的截止值。相反,有4名患者(8.0%)的GM血浆浓度偶尔升高,超过了截止值,而没有IA的任何其它(临床、放射、培养)征象或风险因素(数据未示出)。在这些情况下,GM血浆浓度升高最有可能归因于潜在的抗生素疗法(例如哌拉西林-他唑巴坦),众所周知这与GM浓度升高有关(Boonsarngsuk,V.;Niyompattama,A.;Teosirimongkol,C.;Sriwanichrak,K.False-positive serum and bronchoalveolar lavage aspergillus galactomannan assayscaused by different antibiotics.Scand.《传染病杂志》(J.Infect.Dis.)2010,42,461-468;Metan,G.The interaction between piperacillin-tazobactam and aspergillusgalactomannan antigenemia assay:Is the story over?《感染》(Infection)2013,41,293-294)。
如通过对BALF中的烟曲霉进行培养检测所评估,一名患者表现出IA诊断,这通过高分辨率计算机断层扫描技术得到证实。此外,BALF中的GM浓度增加到截止值以上,而GM血浆浓度在所有时间点都保持在截止值以下。除了败血性休克以及每名男性已有的肥胖症和胰岛素依赖性糖尿病外,患者没有患IA的经典易患风险因素(例如中性粒细胞减少症,用细胞毒剂治疗的血液肿瘤疾病,皮质类固醇摄入,先天性或后天性免疫缺陷)。所述患者用脂质体两性霉素B治疗6周,这导致BALF中的GM降至截止值以下。而且,在治疗期结束后,BALF的培养对于烟曲霉仍保持阴性。
抗念珠菌抗体滴度。在没有任何真菌发现的患者亚组(n=17)中,有4名患者(23.5%)呈现“假”阳性抗念珠菌抗体滴度(>1:320),而定殖患者(n=22)中81.8%(n=18)显示阳性测试结果。遭受真菌感染的患者(n=11)也有81.8%(n=9)显示阳性测试结果,但不幸的是,两名呈现念珠菌血症的患者(在败血症发作时)未显示出阳性抗念珠菌抗体滴度。
关于真菌免疫,应特别注意INF-γ、IL-4、IL-6、IL-10、IL-17以及MR-proADM的血浆水平。从败血症发作后的第7天(T3)开始,与两个对照组相比,真菌感染患者的促炎性细胞因子INF-γ血浆水平分别显著升高。这种INF-γ增加与感染患者中免疫抑制性细胞因子IL-10和IL-4的大量释放同时发生。与在不同时间点具有真菌定殖或没有任何真菌发现的败血症患者相比,真菌感染患者的血浆IL-6水平显著升高,尤其是在疾病的早期阶段(例如在T0、T1时)。同时,与败血症发作后前7天内具有真菌定殖或没有任何真菌发现的败血症患者相比,遭受真菌感染的败血症患者中IL-17A显著增加(图2A)。
因此,如通过ROC分析所评估,发现IL-17A是适用于早期鉴定真菌感染患者的工具(真菌感染患者相对于非感染患者的ROC-AUC(=不具有任何真菌分离株的患者+定殖患者),例如在t0:0.714;截止值14.165pg/ml→敏感性0.818;1-特异性0.323,t1:0.776;截止值:14.22pg/ml→敏感性0.818;1-特异性0.29,t2:0.865;截止值15.00pg/ml→敏感性0.818;1-特异性0.194,等等)(图2B)。
MR-proADM的血浆水平也是如此,与定殖患者以及无任何真菌发现的患者相比,感染患者中的MR-proADM血浆水平显著增加(图3A)。因此,如通过接收者操作特征(ROC)分析所评估,表明MR-proADM是适用于鉴定真菌感染患者的工具(真菌感染患者相对于非感染患者(=参见上文)的ROC曲线下面积(AUC),例如,在t0:0.738;截止值:6.99nmol/l→敏感性0.727;1-特异性0.333,t1:0.755;截止值:8.53nmol/l→敏感性0.727;1-特异性0.212,t2:0.774;截止值5.10nmol/l→敏感性0.818;1-特异性0.273,等等)(图3B)。

Claims (19)
